Factors Related to Long-term Hospital Length of Stay and Opinions on Discharge-related Community-based Medical and Welfare Service on Elderly Patients with Chronic Diseases in Korean Veterans Hospitals

  • Purpose: This study aims to investigate factors related to long-term length of stay (LOS) of patients with chronic diseases in Korean veterans hospitals. Methods: The subjects were 196 elderly patients with chronic disease staying in the hospital for more than 10 days, Data were collected by the survey of patients with structured questionnaires and medical records review by nurses from July 15 to August 10, 2019. Collected data were analyzed using t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. Results: The present and desired LOS were 37.78±32.66 days and 60.87±45.95 days, respectively. Factors affecting hospital LOS were found to be main disease (genitourinary) (p<.001), assistance in activities of daily living (p<.001), area of hospital (p<.001), payment of medical fees (p=.026), hospital satisfaction (p=.036) and the explanatory power of these variables was 26.4%. The most common health problems that need to be solved after discharge were symptom alleviation and health promotion. These problems can be solved using community-based facility services or visiting medical-welfare services (especially home care nursing). Conclusion: In order to reduce hospital LOS, the following measures are required: personalized self-management education, provision of transportation services for dialysis therapy of inactive patients, linking patients with visiting medical-welfare services including home care nursing and mobile healthcare services, operation of the case management system including the notice of the discharge date at admission, interim check of patient status, and connecting the patient with community resources or transferring the patient to long-term care facilities at discharge.

The Effects of Personal, Familial, School Environmental Variables on Mobile Phone Addiction by Adolescent (청소년의 휴대폰 중독성에 영향을 미치는 개인, 가족, 학교환경 변인)

  • The purposes of this study were to examine effective variables influencing on adolescents' mobile phone addiction. The subjects were the 666 middle or high school students who had their own mobile phone in Gwangju. Data were analyzed with frequency, percentage, cronbach's ${\alpha}$, mean, SD, pearson's correlation and multiple regression using SPSS/PC WIN 14.0 program. The major findings were as follows: 1. The most frequently used function of student mobile phones was text message and they used text message more than 41 times a day. Mostly they were talking to the same sex friends on the phone and the monthly charges ranged from 20,000 to 30,000 won. In general, they called to their friends after school. Their parents' attitudes toward their mobile phone using showed that the most of their parents did not care about their children's mobile phone use. The restriction of using their mobile phone at school was normal. 2. The average scores of mobile phone addiction were lower than median(3.0) and self esteem, self control, family strengths, peer conformity and school life adaptation were higher than median. 3. The adolescence's mobile phone additions were influenced by peer conformity, school life adaptation, school levels, sex and self control. And these variables explained to the adolescence's mobile phone additions about 28%.

  • The information relating to the health of person has been increasing. The information is such as medical information and personal health record and the information collected by utilization and dissemination of mobile devices. Therefore, the interest and demand for systems that can integrate and manage the Personal Health Record(PHR) is increasing. Quantity and quality of information that is collected from the patient can have a major impact on the diagnosis and treatment of Korean Medicine(KM) in clinical practice. Because closely observe the usual clinical symptoms of patients to utilize the treatment. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. Therefore, the personal health care services that can record and manage and own is necessary based on KM. In this paper, we propose Korean Medicine Personal Health Record Platform(KM PHR Platform). We have selected the significant symptoms that mean to the personal records from symptom information required for diagnosis in KM. And classifying and scoring as the symptoms were used as personal health care indicators. And significant symptoms were easily configure a screen that can be recorded. simple operation is recorded as a symptom. It was designed to reflect these functions. So KM PHR Platform helps to Personal health care. Doctor may be able to help in the diagnosis and prognosis observation by reference to shared symptom. We look forward to a variety of health services based on KM using a symptom, a medical record, personal health device information.

Study on Energy and Nutrient Intake and Food Preference of the Elderly in Care Facilities (요양시설 거주 노인의 에너지와 영양소 섭취 실태 및 식품선호도 조사)

  • Objectives: The purpose of this study was to assess energy and nutritional intake and investigate the preference for food and cooking methods of the residents in elderly care facilities. Methods: Data were collected from 72 residents (10 males and 62 females) aged ${\geq}70$ years in elderly care facilities using questionnaires, food photographs for estimating dietary intake and records for daily physical activity. Results: Average age of the study participants was 85.0 years and 41, 36 and 8 had dementia, hypertension and diabetes mellitus, respectively. 15%, 65% and 19% of subjects were physically mobile, enervated, and immobile, respectively. Daily energy intake was 1360.2 kcal in men and 1378.0 kcal in women, which were 68.0% and 86.1% of the estimated energy requirement (EER) of dietary reference intake for Koreans (KDRI) for ${\geq}75$ year old individuals, respectively. Estimated energy expenditure (EEE) of subjects calculated using formula from KDRI was 1361.9 kcal and EER calculated using estimated daily physical activity (EDPA) was 1232.9 kcal. Energy intake and EEE from KDRI were higher than EER from EDPA. Dietary intake of dietary fiber, calcium, potassium, zinc, vitamin $B_2$, niacin, vitamin C were lower, and protein, phosphorous, iron, sodium, vitamin A, vitamin $B_1$, vitamin $B_6$, vitamin E were higher than the corresponding ones of KDRI. Subjects liked meats, fishes and shellfish, and fruits, while subjects disliked milk, seaweeds and salted fish and salted vegetables. Cooked rice, soybean paste soup, beef, cooked sliced radish strip, and yogurt were favorite foods, with steam being a favorite cooking method. Subjects considered nutrition as the most important factor for improving food service quality. Conclusions: Results of this study could be utilized for improving food-service for the residents in elderly care facilities, and provide a basis for setting reference intake of energy and nutrients of the elderly having very low activity levels.

Development of Mental Health Self-Care App for University Student (대학생을 위한 정신건강 자가관리 어플리케이션 개발)

  • The purpose of this study is to develop a mobile app for mental health self care of university student. User centered design is a research design that applies the subject's needs assessment, analysis, design, development, evaluation, modification and supplement to suit the subjects. In order to manage the mental health of university students, they consisted of four main areas of mental health problems: drinking, sleeping, depression, and stress. It is designed to enable self test content, analysis and notification of inspection results, and management plan for current status of each area. Based on this, I developed an Android based mental health self-care Application. The subject can enter his or her mental health status data to explain the normal or risk level for each result, and the subject can then select the appropriate intervention method that he or she can perform. In addition, we developed a mental health self care calendar that can display the present status of each of the four areas on a day by day basis, and the current status can be expressed in an integrated manner through animations and status bars. The purpose of this study was to develop a mental health self-care app that can be improved by continuous and improved programs.

  • This study aims to develop a pattern identification system (PIS) for general users to check up their body condition. We collected previous 3 PI questionnaires, with internal consistency reliability or validity of the diagnosis by experts, through a field test. For defining weights of pattern indices, we applied the analytic hierarchy process (AHP) method with 11 experts. PIS receive two kinds of symptoms of users : body region based symptoms and core symptoms for PIS. PIS suggest possible patterns and health information on the basis of selected symptoms with analysis by AHP. This study showed PIS could be easily used for general user who wants to access Korean Medicine compared to conventional PI system. Furthermore, it could be utilized with mobile environment or as remote medicine care.

  • This paper deals with Energy Harvesting using a part of physical energy which is generated by pulldown actions. From a point of energy harvesting view, though it is difficult to regenerate constant energy during pulldown exercise unlike cyclic exercise and running machine exercise, it is possible to regenerate some energies to charge batteries for mobile phones. To investigate possibility of energy harvesting during pulldown exercise, a voltage regulation experiment was performed with a converter circuit and a motor which is attached to the exercise equipment. And we conclude that energy harvesting is possible although its small quantity during pulldown exercises.

  • In 2011, There were about 25,000 people died because of heart disease. The aim of this paper is to design a heart attack situation monitoring and spreading system for patients. Wearable computer with a sensor is used to monitor heart rate. Heart rate is transffered to smartphone with bluetooth. After analyzing heart rate, smartphone spread out the emergency situation by various service including emergency call, SNS and SMS.

  • 논문은 기존의 헬스케어 서비스의 한계를 극복하기 위해 최근 대두되고 있는 스마트 헬스케어서비스(smart healthcare service)에 관한 연구이다. 스마트 헬스케어는 스마트폰의 기능에 맞추어 기초체력 운동 지수, 섭취 칼로리, 심박 체크 등의 다양한 서비스를 제공할 수 있지만, 최근에는 보건의료서비스에 특화시켜 환자와 의사가 직 간접적으로 진료를 할 수 있게끔 하는 스마트폰 환경에서의 원격진료 서비스가 부각되고 있다. 이에 본 논문에서는 게이트웨이 기능의 스마트폰을 기반으로 스마트 헬스케어 시스템을 설계 구현하였다.

  • 최근 유비쿼터스 컴퓨팅 환경이 발전함에 따라 교통(u-Transport), 복지(u-Care), 문화(u-Fun), 환경(u-Green), 산업(u-Business), 행정(u-Government), 도시(u-City) 뿐만 아니라 사용자의 위치와 다양한 공간 정보를 제공하는 u-GIS가 유비쿼터스 컴퓨팅 환경의 핵심 요소 기술로 대두되고 있다. 이에 본 논문에서는 기존의 PC용 MMDBMS인 HS QLDB를 확장하여 모바일 장치에서 시공간 데이타를 효율적으로 처리 및 관리할 수 있는 내장형 시공간 DBMS를 설계 및 구현하였다. 내장형 시공간 DBMS는 OpenGIS "Simple Features Specification for SQL"에서 명시하는 공간 데이타 타입과 공간 연산자를 확장하여 시공간 데이타 타입과 시공간 연산자를 제공하며, 시공간 데이타 특성들 고려한 산술 연산 코딩 압축 기법을 제공하고, 모바일 저장 장치인 플래쉬 메모리에서 효율적인 시공간 데이타 검색을 위한 시공간 인덱스를 지원한다. 그리고, 내장형 시공간 DBMS와 U-GIS 서버 사이에서 시공간 데이타 수입/수출의 성능 향상을 위한 데이타 캐슁 기능과 DBMS의 안정성을 위한 백업/복구 기능을 지원한다.
