Implications of abnormal abdominal wall computed tomographic angiography findings on postmastectomy free flap breast reconstruction

  • Background Preoperative computed tomography angiography (CTA) of the abdominal wall vessels is used when planning free flap breast reconstruction (FFBR) because it provides a surgical road map which facilitates flap harvest. However, there are few reports on the effect of abnormal findings on the operative plan. Methods We conducted a retrospective study of all FFBRs performed at a tertiary referral center over a 6-year period (November 2011 to June 2017). One consultant radiologist reported on the findings. Details on patient demographics, CTA reports, and intraoperative details were collected. Results Two hundred patients received preoperative CTAs. Fourteen percent of patients (n=28) had abnormal findings. Of these findings, 18% were vascular anomalies; 36% tumorrelated and 46% were "miscellaneous." In four patients, findings subsequently prevented surgery; they comprised a mesenteric artery aneurysm, absent deep inferior epigastric (DIE) vessels, bilateral occluded DIE arteries, and significant bone metastases. Another patient had no suitable vessels for a free flap and the surgical plan converted to a pedicled transverse rectus abdominis musculocutaneous flap. The remaining incidental findings had no impact on the surgical plan or appropriateness of FFBR. More than one in 10 of those with abnormal findings went on to have further imaging before their operation. Conclusions CTA in FFBR can have a wider impact than facilitating surgical planning and reducing operative times. Incidental findings can influence the surgical plan, and in some instances, avoid doomed-to-fail and unsafe surgery. It is therefore important that these scans are reported by an experienced radiologist.

A Study on the Traffic Stream and Navigational Characteristics at the Adjacent Sea Area of Busan Central Wharf (부산 중앙부두 주변해역의 교통흐름 및 통항특성에 관한 연구)

  • At the adjacent sea area of Busan Central Wharf, a variety of vessels, such as middle-large passenger ships, small hugh speed crafts, container ships, cargo ships and working boats as well as small miscellaneous vessels are freely sailing comparatively without special rules and marine traffic control. In this research, we analyzed traffic stream and navigational characteristics cf main traffic route based on statistics and distribution of tracks by ship's type and tonnage cf the passing vessels after conducting marine traffic survey twice using exclusive software. We examined the traffic safety of the passing vessels by classifying the sea area by each function based on the analysis about this traffic situation, and analyzing the effect by designating 'Inner passage'. We also studied the plan for the effective rearrangement cf Central Wharf considering basically the traffic safety of arrival and departure in a point of view of navigators.

We also studied the plan for the effective rearrangement of Central Wharf considering basically the traffic safety oif arrival and departure in a point if view of navigators.

A Study on A Proper Application for The Navigational Rules in The Public Order in Open Port Act (개항질서법상 항법적용)

  • When sea going vessel approaches her destination port, marine traffic density is increased, especially, the marine traffic is concentrated in the entrance channel and the fairway in the inner harbour is more narrow. Therefore, special navigational rules for preventing collision at sea is needed in the inner harbour. Hence there are the rules in The Public Order in Open Port Act in Korea But, in the case of conflict of navigational rule concerned, it is possible to be raised a question that which rule shall be applied preferentially. Also, it is necessary to understand the exact meaning of the navigational rules in The Public Order in Open Port Act. Because the misunderstanding of the rule is caused the collision Therefore, this paper aims to discuss the exact meaning of the navigational rule concerned in the view of legal basis and suggest the principles for a proper application of it.
