A case of mild CADASIL patient with a novel heterozygous NOTCH3 variant

  • Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is a single-gene disease caused by mutations in the neurogenic locus notch homolog protein 3 (NOTCH3) gene. The spectrum of clinical manifestations is broad, ranging from asymptomatic to typical ischemic stroke, and mainly depends on the location of the mutations. We describe the case of a 76-year-old female without apparent neurological deficits. However, brain magnetic resonance imaging revealed confluent lesions in the white matter. Direct sequencing of the NOTCH3 gene revealed a novel pathogenic mutation, c.811T>A, which results in a mild phenotype. Therefore, this report will expand the current knowledge in regards to the mutations that can cause CADASIL.

  • Cerebrovascular Accident(stroke) is that a sudden, nonconvulsive loss of neurologic function due to an ischemic or hemorrhagic intracranial vascular event. If stroke happens at the portion of trigeminal motor nucleus or its control part of cerebral cortex, masticatory muscles will be atrophy or paralyzed. So it is possible that dental occlusion changes after stroke. A 74-aged woman recurred mild stroke 2 month ago, who had experienced severe stroke 2 years ago. After recurrence, suddenly her upper full denture was dropped when lower denture contacted upper one. According to the her occlusion exam, her lower jaw moved back slightly compared with the occlusion of old denture. And her face had asymmetry and lower jaw dislocated to paralyzed side. A 50-aged man was treated because many cervical caries, which would occur because of an aftereffect of stroke, long-herm hospitalization and limits of self oral-care. 6 years ago he had cerebral hemorrhage and he claimed that he cannot bite exactly. Just two pairs of teeth was contact on biting, his lower jaw was located back, too. This two case suggests that dental occlusion can change after stroke.

중풍환자의 혈압 상승에 대한 대추혈 사혈의 혈압 강하 효과 (Decreasing Effect of Wet-Cupping at Daechu (대추, Dazhui:GVl4)-point on the Increase of Blood Pressure in Patients with Stroke)

  • Objectives : Daechu (Dazhui:GV14 (Governor Vessel 14))-point is located between the spinous process of the 7th cervical vertebra and that of the 1st thoracic vertebra. GV14 has been used to treat hypertension, high fever, neck pain, common cold, headache and so on. Hypertension often negatively affects the improvement of stroke patients. We investigated whether wet-cupping at GV14 had any effect of decreasing blood pressure (BP). Methods : In this study, 81 stroke patients were studied, from Oct. 2000 to Sept. 2001. They were composed of a Sample group (n=36) and Control group (n=45). The Sample group (n=36) was divided into Sample-Normal (n=9), Sample-Mild (n=7), Sample-Moderate (n=l1), and Sample-Severe group (n=9). The Control group (n=45) was divided into Control-Moderate (n=34) and Control-Severe group (n=ll). We checked blood pressure 5 times (just before treatment, then after 30,60, 90, and 120min. (2hrs.)) in each sample group and 3 times (baseline, after 30, and after 120min. (2hrs)) in the control group. Results : In a comparison of before-treatment BP and after-treatment BP in the sample group, SBP significantly decreased as time passed, while DBP slightly decreased but not significantly. In comparison of BP differences in each sample subgroup, BP slightly increased in the Sample-Normal group, but significantly decreased in the Sample-Mild, Sample-Moderate, and Sample-Severe groups. In comparison of BP differences between the sample and control groups, SBP and DBP of the sample group definitely decreased more than those of the control group, but not significantly. Conclusions : This study suggests that wet-cupping at GV14 has significant decreasing effectson blood pressure in stroke patients. Accordingly, we hope that this measure will be used more widely as an emergent treatment for increased blood pressure.

The Effect of Adding Kami-guibi-tang to Acetylcholinesterase Inhibitor Treatment on the Cognitive Function of Mild Alzheimer's Disease Patients: Study Protocol of a Randomized, Placebo-Controlled, Double-Blind Pilot Trial

  • Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment. Brain MRA상 stenosis정도와 중풍변증모형을 비교한 18례 (Relationship between the Severity of Stenosis and Stroke Pattern-identification in 18 Acute Stroke Patients)

  • To assess the relationship between the severity of stenosis in MCA territory and the differentiation of syndromes in oriental medical aspects, the general characteristics, the scores of stroke-pattern identification, and the findings of MRA were compared in 18 acute cerebral infarction patientshospitalized in Dept. of Internal Medicine, Pundang CHA Oriental Medicine Hospital from 1998 sep. 1 to 1999 sep. 31. We compared the scores of stroke-pattern identification with the severity of stenosis by Kruskall-Wallis test, and analyzed the relationship by Pearson correlation test. The P value under 0.05 was regarded as significant. The results are as follows: The incidence of stenosis(mild to complete occlusion) was 83.3%. There were significant difference of the mean scores according to the severity of stenosis in Yin deficiency pattern. We could also observe a strong relationship between the severity of stenosis and Yin deficiency pattern, whose Pearson correlation coefficient was 0.655 (P<0.05). These results showed that Yin deficiency pattern could be a major cause of cerebral infarction.

뇌졸중으로 인한 편마비 중년 남성과 배우자의 성지식, 성행동, 성만족, 부부친밀감, 우울감의 관계 (Relationship among Sexual Knowledge, Frequency, Satisfaction, Marital Intimacy and Levels of Depression in Stroke Survivors and Their Spouses)

  • Purpose: The purpose of this research was to examine the relationship between sexual knowledge, frequency, satisfaction, marital intimacy, and depression levels in stroke survivors and their spouses. Methods: This study was a convenience sample of Korean men who had experienced a stroke and their spouses. The subjects were discharged from the hospital and received physical therapy. Data such as sexual knowledge, frequency, satisfaction, marital intimacy, and depression levels were collected from November 2005 to April 2006. Results: In a comparison of stroke survivors and their spouses, the survivors showed higher mean levels of sexual knowledge and marital intimacy. The levels of knowledge, frequency, satisfaction and marital intimacy were low and depression was mild in the subjects. There were significant correlations between sexual frequency, sexual satisfaction and marital intimacy. Also, subjects with lower levels of sexual satisfaction were more depressed. The spouses who possessed greater levels of sexual knowledge reported a higher frequency of sex and greater marital intimacy. In addition, spouses who reported feeling greater levels of marital intimacy experienced fewer depressive symptoms. Conclusion: These findings demonstrate a need for the development of sexual adaptation programs which could be effective in improving sexual knowledge, frequency, satisfaction, marital intimacy, and depression reported by stroke patients and their spouses. Ideally, these corrective programs should be administered before patients are discharged from the hospital.

뇌졸중 환자의 우울이 재활동기에 미치는 영향에 대한 극복력의 매개효과 (The Mediating Effect of Resilience on Depression and Rehabilitation Motivation in Stroke Patients)

  • Purpose: This study is a descriptive correlational study to identify the mediating effect of resilience on depression and rehabilitation motivation. Methods: The study was conducted on 206 stroke patients that visited the D University Hospital in B City. Data was collected from July 1 to September 11, 2015. Data were collected for depression, rehabilitation motivation, and resilience. The data were analyzed by the t-test, ANOVA, Pearson's correlation coefficient, and hierarchical multiple regression using the SPSS/WIN 21.0 statistical program. Results: 33.7% of subjects showed beyond mild depression. The level of rehabilitation motivation was higher in male stroke patients, and when the level of education and monthly income was higher. Moreover the levels of rehabilitation motivation were significantly higher when stroke patients were participating in exercise continuously. Resilience was found to partially mediate the relationship between depression and rehabilitation motivation. Conclusion: The finding of this study demonstrates that intervention on resilience, in addition to direct intervention on depression, can improve rehabilitation motivation in stroke patients.

  • Number of elderly patients requiring nerve blocks have been increasing in recent years. We had two elderly patients who suffered stroke one day and three days after lumbar facet joint block and lumbar single epidural block respectively. Both patients due to their advanced age had potential risk factor to suffer one or more of the following; stroke, hypertension, and diabetes mellitus. Due to our experience with these patients, we suggested the following: (1) Nerve blocks should be reconsidered for elderly patient who posesses a potential risk factor to suffer a stroke. (2) Prior to invasive block administration of mild sedatives or analgesics may provide beneficial effects for patients with hypertension. (3) Adequately informed consent must be fully discussed time of consultation with patient scheduled for nerve block especially for elderly and risky patient.

손 재활 로봇의 적용이 만성 뇌졸중 환자의 손 기능 향상에 미치는 영향 (Effect of Robot-Assisted Hand Rehabilitation on Hand Function in Chronic Stroke Patients)

  • The purpose of this study was to investigate effect of robot-assisted hand rehabilitation(Amadeo(R)) on hand motor function in chronic stroke patients. This study used a single-subject experimental design with multiple baselines across individuals. Three chronic stroke survivors with mild to sever motor impairment took part in study. Each participants had 2 weeks interval of starting intervention. Participants received robot-assisted therapy(45min/session. 3session/wk for 6wks). Finger active range of motion(AROM) was assessed by Range of Assessment program in Amadeo(R), and test-retest reliability was verified using Pearson correlation analysis. To investigate effect of Amadeo(R), finger AROM was measured immediately after each sessions and Fugl-Meyer Assessment of Upper extremity, Motor Activity Log, Nine hole peg board test and Jebsen-Taylor hand motor function test were assessed at pre-post intervention. Results were analyzed by visual analysis and comparison of pre-post tests. The test-retest reliability of Range of Assessment was good(r=.99). After robot-assisted therapy, finger AROM of participant 1, 2, and 3 was respectively improved by 18%, 3.6%, and 6% each. Hand motor function of participant 1, 3 was improved on all four tests, but not effect in participant 2. Robot-assisted hand rehabilitation could improve finger AROM and effect on hand motor function in chronic stroke patients.

  • Objective : The aim of this study was to examine Differences of overall characteristics depending on Symptom Severity in Acute Stroke Patient Method : We studied hospitalized patients within 10days after their ictus who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2011. We compared the general characteristics of acute stroke patient according to Scandinavian Stroke Scale score Result : The patient who had mild severity show significant difference for FBS, PP2, Homocysteine, Exercise, Tongue color, Mouth dryness, Chest discomfort, Constipation, Stool hardness. Conclusion : The above result contribute to predict severity of stroke symtoms according to risk factor and general condition of patients. Also, After occurrence of stroke, We will can block worsening of symptoms progression. Further studies will be needed to observation of follow up studies about progression of stroke among acute stroke patients with a serious disability.
