The Effects of EEG Power and Coherence on Cognitive Function in Normal Elderly, Non-Demented Elderly With Mild Cognitive Impairment, and Demented Elderly During Working Cognition Task

  • The purpose of this study was to find out the effects of electroencephalograph (EEG) power and coherence on cognitive function in normal elderly, non-demented elderly with mild cognitive impairment, and demented elderly during working cognition tasks. Forty elderly women (19 demented elderly, 10 non-demented elderly with mild cognitive impairment, 11 norma1 elderly) participated in this study, All subjects performed working cognition tasks with Raven's CPM while EEG signal was recorded, EEGs were measured continuously at rest and during the working cognition task. EEG power and coherence was computed over 21 channels: right and left frontal, central, parietal, temporal and occipital region. We found that there were more correct answers among normal elderly women than in other groups Owing the working cognition task, ${\Theta}$ wave at Fp1, Fp2 and F8, a wave at Fp2, ${\beta}$ wave at Fp1, Fp2. F4 and F8 of the frontal region was increased significantly in the demented elderly group. On the other hand. ${\Theta}$ wave at Fp1, Fp2 and F7, ${\beta}$ wave at Fp1, Fp2, F3 and F7 of the frontal region was increased significantly in the group of non-demented elderly with mild cognitive impairment. In contrast. in the normal elderly group, all of the ${\Theta}$ wave and ${\beta}$ wave at Fp1, Fp2, F3, F4, F7 and F8 of the frontal region (except ${\beta}$ wave at F3) was increased significantly, These results suggest that the nerves in prefrontal and right hemisphere regions were most active in the demented elderly group during problem solving, and the nerves in the prefrontal and left hemisphere lobe were most active in the group of non-demented elderly with mild cognitive impairment. In contrast, me majority of nerves in the frontal region were active in the normal elderly group.

맞춤형 인지프로그램이 치매와 경도인지손상노인의 LOTCA-G 및 일상생활동작에 미치는 영향 (The Effects of Individualized Cognitive Program on LOTCA-G and ADL in Elderly with Dementia and Mild Cognitive Impairment)

  • Purpose : The purpose of this study was to evaluate the influence of LOTCA-G and ADL by individualized cognitive program in elderly with vascular dementia, alzheimer dementia and mild cognitive impairment. Method : The subjects of this study, old man and woman diagnosed with vascular dementia, alzheimer dementia and mild cognitive impairment, 24 patients were picked up, who were agreed with this research and were having hospital care for 3weeks at nursing care centers. Individualized cognitive program was applied to 8 patients of vascular dementia, 8 patients of alzheimer dementia and 8 patients of mild cognitive impairment. Cognitive function measured by LOTCA-G and performance measured by FIM. The SPSS Ver. 22.0 statistical program was used for data processing. The significance level for statistical inspection was set as 0.05. Result : In comparison of LOTCA-G was significant increased after intervention and among three groups were significant difference. But FIM was no significant difference after intervention and among three groups were no significant difference. Conclusion : Therefore, the individualized cognitive program is useful to improve the cognitive function in elderly with dementia and mild cognitive impairment.

인지프로그램 참여에 따른 경도인지장애 노인들의 일상생활활동 수행과 우울 수준의 변화 (Changes in the Level of Performance of Activities of Daily Living and Depression of the Elderly with Mild Cognitive Impairment through the Participation in the Cognitive Program)

  • 본 연구는 인지프로그램 참여에 따른 경도인지장애 노인들의 일상생활활동 수행과 우울 수준의 변화를 분석하기 위한 단일집단 사전-사후설계 유사실험 연구이다. 연구대상은 J시 거주 W 복지센터를 이용하고 있는 경도인지장애 노인 16명이며, 주당 2회씩 총 8주간의 인지프로그램에 참여하였으며, 프로그램 참여에 따른 대상자들의 일상생활활동 수행수준과 우울의 변화를 평가하였다. 그 결과, 인지프로그램 참여 후 대상자들의 일상생활활동 수행수준이 유의하게 향상되었으며, 우울 수준이 통계적으로 유의미하게 감소되었다. 따라서, 경도인지장애 노인들의 일상생활활동 수행수준을 향상시키고, 우울 수준을 감소시키기 위해서 인지프로그램을 중재프로그램으로 활용하여야 할 것이며, 이 과정에서, 경도인지장애 노인들의 참여를 적극적으로 유도할 필요가 있다.

  • 노인 운전자가 스스로 운전능력을 점검하는 자가-보고식 평가는 운전 중 사고 또는 사고위험성을 사전에 대비할 목적으로 사용된다. 지역사회에 거주하는 정상노인의 다수가 경도인지장애로 인해 인지기능 저하를 경험할 수 있으며, 이들의 저하된 인지기능은 노인 운전자용 자가-보고식 평가 수행에 영향을 미치는 변수가 될 수 있다. 따라서 본 연구를 통해 지역사회에 거주하는 경도인지장애 노인 운전자가 자가-보고식 평가를 수행하는 데 있어 영향을 미치는 인지기능을 확인하였다. 지역사회에 거주하는 노인 운전자 103명의 Korean Version of the Montreal Cognitive Assessment 평가결과를 바탕으로 경도인지장애와 정상 노인을 분류하였다. 그리고 두 집단의 자가-보고식 평가인 Korean-Drivers 65 plus 점수를 비교하였고, 이 평가 결과에 영향을 미치는 인지기능을 분석하였다. 그 결과 경도인지장애 그룹이 정상 그룹과 비교하여 유의하게 낮은 평가수행 결과를 보였으며, 경도인지장애 노인 운전자의 자가-보고식 평가 결과는 시공간 실행력 및 지연 회상력과 유의한 상관성이 확인되었다. 회귀분석을 통해 자가-보고식 평가 수행에 영향을 미치는 인지기능을 확인한 결과 시공간 실행력이 가장 영향력인 높은 변수로 확인되었다. 지연 회상력 또한 부분적 영향이 확인되었지만 경도인지장애 노인 운전자의 자가-보고식 평가 결과를 변질 시키는 수준은 아닌 것으로 확인되었다.

협응이동훈련이 경도인지장애노인의 낙상물리요인에 미치는 영향 (The Effect of Coordinative Locomotor Training on Physical Factors for Falls in the Elderly with Mild Cognitive Impairment)

  • PURPOSE: This study examined the effects of coordinative locomotor training on the physical factors for falls in the elderly with mild cognitive impairment. METHODS: This study examined thirty subjects diagnosed with mild cognitive impairment by the radiologic findings, history, and physical examination. The subjects were assigned to a control group (n = 15) or experimental group (n = 15, coordinative locomotor training). The experimental group underwent coordinative locomotor training for four weeks, with training sessions two times per week. The control group was given a fall-prevention education for 60 minutes without coordinative locomotor training. To evaluate the physical factors for falls, the lower extremity strength and the Korean version of the Fullerton advanced balance scale and biorescue were measured for balance. These tests were conducted before and after training. RESULTS: Significant differences were observed between the two groups after the four weeks of coordinative locomotor training for the elderly with mild cognitive impairment the experimental group had a greater degree of improvement in the physical factors for falls. CONCLUSION: These findings suggest that coordinative locomotor training may have a functional effect on fall-prevention and the mobility of the elderly with mild cognitive impairment. In addition, it is expected to provide systematic and effective data that can be used as a fall prevention program for the elderly with mild cognitive impairment in each institution.

국내 경도인지장애 노인의 비약물적 중재에 대한 통합적 문헌고찰 (An Integrative Review of Non-pharmacological Intervention in Elderly Patients with Mild Cognitive Impairment)

  • 본 연구의 목적은 경도인지장애 노인을 대상으로 비약물적 중재를 수행한 문헌을 검색하고, 이를 분석함으로써 추후, 경도인지장애 노인을 위한 간호학에서의 비약물적 중재프로그램 개발을 위한 기초자료를 제공하기 위함이다. 연구방법으로 Whittemore & Knafl가 제시한 통합적 문헌고찰을 수행하였다. 본 연구결과 경도인지장애 노인을 대상으로 비약물적 중재를 수행한 연구는 최종 11편 이었으며, 연구의 질적 수준은 두 집단 비무작위 연구가 9편, 단일 집단 비무작위 연구가 2편으로 나타났다. 또한 2010년 이후의 연구가 10편이었고, 다양한 분야에서 경도인지장애 노인을 위한 비약물적 중재가 수행되었다. 경도인지장애노인을 위한 비약물적 중재는 인지강화 프로그램 4편, 운동프로그램 6편, 음악프로그램 1편이었다. 경도인지장애 노인을 대상으로 수행한 비약물적 중재의 효과는 각 연구마다 다양한 결과를 나타냈으며, 이러한 연구결과를 바탕으로 추후 지속적인 반복 연구가 필요할 것으로 생각된다.

스텝을 활용한 인지-운동프로그램이 경도인지장애 노인의 인지기능, 보행, 우울에 미치는 효과 (The Effects of Cognition-Exercise Program Using Step on Cognitive Function, Gait, and Depression in Elderly with Mild Cognitive Impairment)

  • Purpose : The purpose of this study was to determine the effects of a cognitive-exercise program using step on the cognitive function, gait, and depression of elderly with mild cognitive impairment. Methods : The subjects comprised 30 elderly people with mild cognitive impairment who used elderly welfare centers in the provinces A, B, and C between March 21 and June 7, 2019. They were divided into an experimental group (n=15) and a control group (n=15) before undergoing an experiment based on a pretest-posttest control group design. The lowenstein occupational therapy cognitive assessment-geriatric population was used to assess the subjects' cognitive function, while the 4-stage balance test, 30-second chair-stand test, timed up & go test, and functional reach test were employed to evaluate their gait. In addition, the beck depression inventory was used to measure their levels of depression. Results : The cognitive-exercise program using step enhanced the subjects' cognitive function and gait and reduced their levels of depression. Furthermore, statistically significant differences were found between the experimental group and the control group. Conclusion : The cognitive-exercise program using step promoted the process of sensorimotor and cognition through the learning process of various steps taking left and right steps and cognitive activities. It improves gait by activating the coordination of the musculoskeletal and nervous systems of the body and positively affecting posture control, balance, flexibility, and lower extremity muscles. It also relieved depression by performing successful step learning and cognitive activities with forward-looking (doing) that leads to pleasure and achievement. The present study confirmed the value of a cognitive-exercise program using step to treat multiple domains of functional decline in elderly patients with mild cognitive impairment. This is therefore proposed as an intervention program for this patient group.

The Effect of IADL on life satisfaction in the elderly with dementia and mild cognitive impairment: Multiple Mediating Effects of Subjective Expectation and Depression

  • 본 연구에서는 제7차 고령화연구패널조사(KLoSA) 데이터를 활용하여 치매 및 경도인지장애 노인의 IADL이 삶의 만족에 영향을 미치는데 주관적 기대감과 우울이 다중매개효과를 가지는지를 검증하였다. 분석결과, 첫째, 치매 및 경도인지장애 노인의 IADL이 높을수록 주관적 기대감은 낮아지고, 우울은 높아지며, 삶의 만족은 낮아지는 것으로 분석되었다. 둘째, 치매 및 경도인지장애 노인의 주관적 기대감이 높아질수록 우울은 낮아지고, 삶의 만족은 높아지는 것으로 분석되었다. 셋째, 치매 및 경도인지장애 노인의 IADL은 삶의 만족에 직접적인 영향을 미치며, 주관적 기대감과 우울을 통해 삶의 만족에 간접적인 영향을 미치고 있어 다중매개효과가 있는 것으로 분석되었다. 따라서 치매 및 경도인지장애 노인의 일상생활제한을 지원할 수 있는 서비스 제공기관의 확대와 우울증 감소를 위한 전문가의 주기적인 프로그램 제공, 부양자에 대한 국민연금의 부과급여 신설을 통한 주관적 기대감의 확대가 필요하다.

경도인지장애 노인의 인지향상 프로그램 중재효과 (The Intervention Effect of Cognitive Improvement Program for Elderly with Mild Cognitive Impairment)

  • Purpose: This study was conducted to identify the effects of a group cognitive improvement program on cognitive function, depression and self-esteem in elderly individuals with mild cognitive impairment. Methods: This was an experimental study that employed a pre-post design of a non-equivalence control group. The subjects were 52 elderly people with mild cognitive impairment, 25 of whom were assigned to the experimental group and 27 to the control group. The program was conducted for a total of 12 sessions for 60 minutes each. Data were analyzed using the ${\chi}2-test$, Fisher's exact test, and Independent t-test with the SPSS 20.0 program. Results: After the intervention, the group who participated showed improvement in all areas of cognitive function based on MMSE-KC (F=26.37, p.<0.001), the Rey Complex Figure Test: copy (F=20.66, p.<0.001), Immediate memory of Seoul Verbal Learning Test-Elderly's version (F=29.68, p.<0.001), delayed memory (F=45.79 p.<0.001), memory recall (F=28.97, p.<0.001), Forward of Digit Span Test (F=9.25, p=.004), backward (F=8.33, p.=0.006), language comprehension (F=13.42, p.<0.001), and digit symbol coding (F=17.74, p.<0.001) relative to the control group. Moreover depression (F=24.09, p.<0.001) was decreased in program participants, whereas self-esteem (F=40.24, p.<0.001) was increased. Conclusion: The program could be a useful intervention because the results show that the group cognitive improvement program has a significant effect on cognitive function, depression and self-esteem in elderly with mild cognitive impairment.

경도인지장애 노인의 우울 예측 요인 (Prediction of Depression among Elderly with Mild Cognitive Impairment Living in the Community)

  • Purpose: This study aimed to identify depression and its predictors among Korean community-dwelling elderly with mild cognitive impairment. Methods: Secondary data analyses of the data collected by the "Study on tailored integration program for reinforcing cognitive and physical function of the frail elderly. The study used data from one-to-one interviews using structured questionnaires. The subjects were 346 community-dwelling elderly who visited a healthcare center in Seoul, Korea. A descriptive correlational study design was utilized to explore depression and its predictors including physical factors and psychosocial factors among the elderly with mild cognitive impairment. Data were analyzed using t-test, Pearson's correlation, and hierarchical regression. Results: Mean score of depression was within normal limit $11.61{\pm}6.69$. Somatic symptoms, negative life events (severity), social support were shown as significant predictors of depression. The most influential predictor for depression was somatic symptoms (${\beta}$=.340, $p$ <.001). Conclusion: To prevent and relieve depression in the elderly with mild cognitive impairment, nursing intervention strategies which consider well-balanced physical and psychosocial aspects are needed. In particular, a specific nursing intervention strategy is required to improve physical health of the elderly with mild cognitive impairment.
