중도시내(中都市內) 도서(島嶼)와 육지거주부인(陸地居住婦人)의 모성보건실태비교(母性保健實態比較) (Comparative Study on Maternal Health Status With Island and Land Women in A Middle City Area)

  • To compare the maternal health status between women in island and land, the study was conducted to adjacent land (kukdong) and island (kyonghodong) areas in Yosoo city during April 1976. The results obtained from 174 interviewee in island and 192 in land areas were the same as follow, 1. General characteristics of both area. Age: Land, group 20-29 year of age was the largest. Island, 30-39 year of age was the largest. Education; Low educated group, less than primary grade (land 88.6%, island 93.1%), was predominant in both area. Occupation of herself; agriculture and labor was the main occupation (land 50%, island 82.2%) in both area. Occupation of husband: labor and fishery was general (81.2%) in land, and agriculture and fishery was general (81.6%) in island. Duration of residence; Land, 25.0% being resided here more than 10 years. Island, 64.3% being resided here more than 10 years. 2. Marriage, Pregnancy, and Present children. Average age at marriage; Land, 20.7 years. Island, 20.9 years. Average frequency of pregnancy; Land, 4.4 times. Island, 4.3 times. Wastage of pregnancies: Land, 236 per 1000 pregnancies. Island, 151 per 1000 pregnancies. Wastage occupied by induced abortion; land, 73.5%. island, 60.5%. Number and sex of present children; Land, 3.3 per family, sex ratio 52.4 to 47.6. Island, 3.6 per family, sex ratio 53.3 to 46.7. 3. Prenatal and postnatal care. Prenatal consutation: Land, 16.1% received by doctor or midwife. Island 9.2% received by doctor or midwife. Complications during last pregnancy; Land, 46.6% complained. Island, 51.1% complained. Return to work within 1 week after delivery; Land, 40.6%. Island, 50.6%. 4. Delivery environment Home delivery; Land, 97.4% Island, 97.3%. Delivery attdended by non medical professions at home; Land, 80.2% Island, 93.7%. Solo home delivery; Land, 13.0%, Island, 12.9%. Delivery attended by layman without taking any disinfective preparations; Land, 48.1%, Island, 49.1%. Material mainly used to cut umbilical cord at home; Land, scissors (97.4%). Island, scissora (98.8%). Cord cutting material used without giving any disinfective preparations; Land, 79.9%, Island, 84.0%. Delivery sheets used at home; Cement bag paper (land 50.0%, island, 31.3%). Vinyl sheets (land,17.5%, island, 27.6%). News paper (land, 3.2%. island, 11.7%). No sheets (land 19.5%. island, 12.9%). etc. Maternal Meal; Seaweed soup with rice was the most general in both area. (land 95.3%, island, 91.4%).

수도권 소재 산부인과의원의 제왕절개율과 관련 요인분석 (Factors Determining Cesarean Section Frequency Rates of the OBGY Clinics in Metropolitan Area)

  • This study aims to find factors that affect variations in cesarean section frequency rates among OBGY clinics in Metropolitan areas. The factors include patient, medical supplier characteristics and economic factors. This study is a cross-sectional analysis using health insurance delivery claims from July to December 2000 and files of the NHIC(national health insurance corporation). Multiple regression was used to analyze the dependent variable of cesarean section frequency rate at each clinic. The results are as follows : Cesarean section frequency rate is increasing in proportion to the number of the following patients : repeated caesarean section, disproportion, obstructed labour, fetal distress, emergency caesarean section and self-employed patients. There are geographic variations as well. Cesarean section frequency rates are higher in Inchon and Gyonggi province than in Seoul. The higher number of total delivery the clinic has, the lower rate of cesarean section it has. Clinics with high frequency rates in 1999 showed higher rates the next year. Further research is required to develop evidence based delivery modes and change strategies for increasing normal delivery and activating midwife clinics.

간호대학생의 조산원 분만 참여 경험 (Experiences of Nursing College Students attended to Delivery Practice at Midwifery Clinic)

  • Purpose: The purpose of this study was to explore the experiences of nursing college students who attended delivery practices at midwifery clinic. Methods: This was a qualitative study using focus groups. Data were collected by group interviews with a total of 12 students who observed natural childbirth at midwifery clinic. Data were analyzed through Colaizzi's method in which meaningful statements were extracted. Results: The meanings of experiences were identified five theme clusters from thirteen themes and thirty-one sub-themes. The five theme clusters were 'ultimate process of natural childbirth', 'allowed birth', 'role recognition of the midwife', 'positively changed perception', and 'barriers'. Conclusion: This study showed that observation of delivery at midwifery clinic was helpful for nursing students in terms of positive impression about natural childbirth with maternal and baby being centered. Nursing students expressed value and concern about delivery at midwifery clinic. Further in-depth study of natural childbirth in terms of husband and family perspective is required. For quality improvement of maternity nursing care, natural childbirth process needs to be included in nursing care at hospitals as well as students' clinical practicum.

Comparison of professionalism and job satisfaction between Korean midwives in birthing centers and midwives in hospitals

  • Purpose: Midwives working in hospitals (MWH) have limited roles in managing and assisting births independently. To find ways to successfully integrate midwifery into care systems, exploring midwives' work-related perceptions might be the first step. The purpose of this study was to compare professionalism and job satisfaction between Korean midwives working in birthing centers (MWBC) and MWH. Methods: A descriptive comparative design was used, querying 19 MWBC and 53 MWH in Korea. Data were accrued from October to November 2017 using the Professionalism Inventory Scale and the Job Satisfaction Scale. Results: Age, marital status, monthly income, length of career as a midwife, and length of career in the current workplace were significantly different between MWBC and MWH. The level of professionalism in MWBC showed significant differences by position at the birthing center (t=16.19, p=.001). Professionalism and job satisfaction among MWH showed significant differences depending on perceived professional performance (F=9.95, p<.001 and F=11.04, p<.001, respectively). Levels of professionalism and job satisfaction were higher for MWBC than for MWH. Conclusion: Educational programs designed to enhance professionalism and expand the role of MWH are suggested. Also, policy changes that clearly define job roles and improvement of the legal system is required to enable MWH in Korea to effectively perform their midwifery work and be properly reimbursed.

저출산 대응 공공서비스 관점에서 본 「토론토 출산지원센터」 현지조사연구 (A Field Study of 「Toronto Birth Centre」 in the Perspective of a Public Service Strategy Coping with Low Birth-rate)

  • Korea has shown the lowest birth rate among OECD countries for over 10 years. This caused various social tensions and problems including negative impacts on national economy. To deal with these problems and to cope with future risk, diverse strategies must be actively explored. The purpose of this research is to delineate the characteristics of Toronto Birth Centre in Canada which was established currently to support families. Field visit study was the major methodology which includes interview with a staff and walk-through guided observation accompanied by visual recording. As a result, the specific functions and the logic of utilizing the center along with its spatial characteristics were delineated. The result has significant implication in developing coping strategies to release the social tension. The birth center can be an efficient strategy to include as a public strategy to Korean Society who has suffered from long lasting lowest birth rate.

조산원 분만을 선택한 여성의 특성, 선택동기 및 출산경험 조사 (Characteristics, Motivation of Choice and Childbirth Experience of Women Who Selected Delivery at Midwifery Clinic)

  • Purpose: This study is descriptive research to identify characteristics and childbirth experience of women who selected delivery with midwives. Methods: The research methods included structured questionnaires, open questions and charts and data were collected from March 2009 to May 2010 at one midwifery clinic. The 108 data of primipara and multipara were analyzed using descriptive statistics and grouping in same meaning. Results: The general characteristics of participants were age of 31~35, housewife, college or university graduate, religion 'yes', and economic state 'middle'. The obstetric characteristics of participants were abortion rate of 23.1%, none complication to pregnancy and delivery of 97.2%, and none postpartum complication. This study was analyzed using positive experiences of 3 categories, that is 'comfortable and natural childbirth', 'satisfaction and trust to personal care of midwives', and 'experience of baby-and-family-centered childbirth' and negative experiences of 3 categories, that is 'improvement of healthcare environment', 'insufficient facilities and nursing care', and'burden of cost'. Conclusion: This study is significant, since it investigates in the absence of domestic research on the characteristics of women who delivered at midwifery clinic. Thus, this study provided basic data on the characteristics of women who delivered at midwifery clinic.

임산부의 교육요구도에 관한 연구 (A Study on Educational Needs of Pregnant Women)

  • This study was conducted to assess to the knowledge and educational needs of pregnant women relating to antepartum, delivery, postpartum periods, and to identify the variables which influenced their knowledge and educational needs. The study subjects were 282 postpartum women who had delivered fullterm normal babies without any complications. Data were collected from women those who were not more than two days after delivery at general hospitals, 3 maternal child health care centers and 4 midwife's offices in Pusan using questionnaire method from Feb. 1. 1991 to Mar. 20. 1991. The results of this study are as follows 1. Relating to the antepartum period, the knowledge about the time when the sex of fetus was fixed was low, and relating to delivery, the knowledge about the method reducing labor pain was low. 2. Relating to the antepartum period, the educational needs about child rearing was high, and relating to delivery, the educational needs about newborn baby were high. Relating to the postpartum period, the educational needs about emergency care of new born baby were high. 3. There were statistically significant differences in total educational needs by educational level(P<.01), duration of marriage (P<.01), and number of pregnancy(P<.05). 4. There were positive correlation between the total knowledge and educational level and negative correlations between the total knowledge and duration of marriage and age.

우리나라 가족계획사업(家族計劃事業)의 현황소고(現況小考) ('Recent Progress of Family Planning in Korea')

  • Korean family planning program has been adopted as a part of the Economic Development Plan with strong national government backup. After initiation of family planning program, the increase rate of total population declined from 2.6 percent during 1955-1960, to 2.1 percent in 1960-1975, and 1.6 percent in 1979. Of course, we do not ascribe this population increase rate decline to the national family planning program alone. Other contributing factors have been changes such as growing numbers of induced absortions, a rising marriage age and economic development. Currently, 2,600 family planning workers are assigned in all myun of the country. 21 percent of the works are registered nurse, 9 percent are midwife and aid nurses occupy 70 percent (Table 1). Authorized clinics are 2,329 which composed 1,765 IUD clinics, 1,070 vasectomy clinics and 1,150 Fimale sterilization clinics (Table 2). Cumulative contraceptive services provided by government program, 1962-1974 is illustrated in Table 3. After government program in family planning has been initiated (1962-1978), estimated number of births averted by each methods was measured (Table 4). From 1962 to 1978, tendency of contraceptive acceptors is illustrated in Table 5 showed that IUD, oral pill and condom program is decreasing and in other hand, sterilization program is increasing very much. Attitude change toward family size (1965-1978), contraceptive practice and son preference are showed in Table 7, 8, respectively. Auther concluded that future program in family planning should target to the point, that smaller family size norm for maternal health should be emphasized rather than smaller size family for national development.

Is there an association between labor induction and attention-deficit/hyperactivity disorder among children?

  • Background: There is evidence of destructive effects of oxytocin use during labor on neurodevelopment. Purpose: This meta-analysis pooled all observational studies to determine the association between labor induction and the risk of attention-deficit/hyperactivity disorder (ADHD) among children. Methods: All observational studies (case-control and cohort) were reviewed until September 2020 after searches of the PubMed, Scopus, and Web of Science databases, the gray literature, and conference proceedings. The pooled odds ratios (ORs), relative ratios (RRs), and 95% confidence intervals (CI (swere calculated as random effect estimates of association among studies. Results: All observational studies were reviewed, including 4 cohort studies including 2,885,743 participants and 3 case-control studies including 51,135 participants (10,961 with ADHD and 40,174 in control groups) with a sample size of 2,936,878 participants. The pooled estimates of OR and RR did not indicate a significant association between labor induction and ADHD among children (OR, 1.13; 95% CI, 0.90-1.35) and (RR, 1.10; 95% CI, 0.96-1.24). Conclusions: The findings showed that labor induction is not associated with an increased risk of ADHD among children. However, more studies are needed to investigate the relationship between labor induction and ADHD.

Distribution Performance of Practice Midwives Through Entrepreneurial Leadership, Motivation, Organizational Learning and Commitment

  • Purpose: to explore more deeply the variables of knowledge construction in influencing performance, through entrepreneurial leadership, motivation, organizational learning, and commitment to the performance of midwives in providing maximum service to patients and the community. Research design, data and methodology: using quantitative methods with hypothesis testing, data was obtained through direct visits and surveys to midwife practice locations through coordination with the Indonesian Midwives Association (IBI) regarding surveys to be carried out and needed. Results: there are 3 direct paths that have significant value. The path between the motivation variable to commitment was found to be significant, then the effect of organizational learning on commitment was found to be significant and finally the effect of the path variable from commitment to distribution performance was found to be significant. The indirect effect was found to be insignificant for the influence of entrepreneurial leadership through commitment to distribution performance, but different results found a significant indirect effect for the relationship between motivation through commitment to performance and organizational learning through commitment to distribution performance. Conclusion: there is a high commitment to the work of midwives, commitment as a good mediation in influencing distribution performance between organizational learning and work motivation.
