골격성 III급 부정교합자에서 상악골 전진술을 동반한 양악 수술 시 중안면 연조직 형태의 변화 (Soft tissue change of the midface in skeletal class III orthognathic surgery patients)

  • 본 연구는 중안면 함몰이 있는 골격성 III급 부정교합자의 중안면의 형태를 정상 표본과 비교하고, 악교정수술에 의하여 연조직이 어떤 변화를 보이는 지를 알아보기 위하여 시행하였다. 골격성 III급 부정교합으로 진단 받고 상악 전진술과 하악 후퇴술을 시행 받은 환자 34명(남자:15명, 여자:19명)을 대상으로 수술 전후 측모 두부방사선 계측사진을 분석하였다. 골격성 III급 부정교합자의 중안면의 경조직 계측치 S'-Or, Or${\bot}$NA, S'Or/SN, ${\angle}$SNOr에서 남녀모두에서 정상 교합자와 큰 차이가 있었다. 상악골이 평균 5.03 mm 전방이동 될 때 연조직 Orbitale는 2.26 mm전방으로 이동하였으며 상관계수는 0.599였다 (p<0.05). 상악 평면각의 변화(시계방향으로 회전)와 상악골의 수직이동은 연조직 Orbitale의 전방이동과 유의성있는 상관관계가 없었다. 상악골 전방이동에 대한 연조직 Orbitale와 Subnasale의 전방이동 비율은 각각 43.57%, 81.54%였다. 이상의 결과에서 중안면 함몰이 있는 환자는 상악골 전방이동시 상순과 비부 외에도 연조직 Orbitale를 포함한 중안면 연조직이 전방으로 이동하지만, 상악골의 회전이동 및 수직이동에 대해서는 연조직의 변화량이 적었다는 것을 알 수 있었다. 이는 상악골의 수술에 따른 중안면 함몰의 해소를 예측하는데 도움이 될 것이다.

정상 한국인 성인 남녀에서 협골을 중심으로 한 중안모의 측모 두부방사선 규격사진 분석법 (A LATERAL CEPHALOMETRIC ANALYSIS OF MIDFACE FOCUSING ON ZYGOMATIC BONE IN KOREAN ADULTS)

  • Purpose : A new method of lateral cephalometric analysis for midface, focusing on zygomatic bone, was created in this study, and measured in Korean adults. The purposes of this study are understanding about new approach for midfacial depression, especially zygomatic bone, and using to make adequate diagnosis and treatment plan. Materials and methods : In this study, esthetic Korean adults, 25 males and 25 females who were between $0^{\circ}$ and $4^{\circ}$ in ${\angle}ANB$, and between 62% and 70% in P/A facial height ratio, and had normal overbite and overjet, were used. Orbitale(Or) and Soft tissue orbitale(Or') were used for indicators of anteroposterior position of zygomatic area. And, mean value and standard deviation of linear and angular measurements, and ratio about each linear measurements, were obtained. then, standard deviation diagram - wiggle diagram - was made for visualization of results. Results : Mean values, standard deviations and ranges of 19 measurements were obtained, and among them, 5 linear measurements that had large standard deviation were excluded and the others were used for making standard deviation diagram. In standard deviation diagram, the following results were obtained. 1. If the measurements are located on more left side of mean-value-vertical -line, the potential of midfacial hypoplasia are stronger, especially zygomatic area. 2. If the measurements are located on more right side of mean-value-vertical-line, the potential of midfacial hypoplasia are decreased. Conclusion : This study presented a new method of lateral cephalometric analysis focusing on zygomatic bone in Korean adults. We expect that the results of this study can be used as parameter when clinicians make decisions about diagnosis and treatment plan for rehabilitation of esthetics and function. But, it is necessary to prove its usefulness, and to further evaluate the results.

A safe, stable, and convenient three-dimensional device for high Le Fort I osteotomy

  • Background: Le Fort I osteotomy is a highly effective treatment for skeletal jaw deformities and is commonly performed. High Le Fort I osteotomy is a modified surgical procedure performed for improving the depression of the cheeks by setting the osteotomy higher than the conventional Le Fort I osteotomy. Developments in three-dimensional (3D) technology have popularized the use of 3D printers in various institutions, especially in orthognathic surgeries. In this study, we report a safe and inexpensive method of performing a high Le Fort I osteotomy using a novel 3D device and piezosurgery, which prevent tooth root injury without disturbing the operation field for patients with a short midface and long tooth roots. Results: A 17-year-old woman presented with facial asymmetry, mandibular protrusion, a short midface, and long tooth roots. We planned high Le Fort I osteotomy and bilateral sagittal split ramus osteotomy. Prevention of damage to the roots of the teeth and the infraorbital nerve and accurate determination of the posterior osteotomy line were crucial for clinical success. Le Fort I osteotomy using 3D devices has been reported previously but were particularly large in size for this case. Additionally, setting the fixing screw of the device was difficult, because of the risk of damage to the roots of the teeth. Therefore, a different surgical technique, other than the conventional Le Fort I osteotomy and 3D device, was required. The left and right parts of the 3D device were fabricated separately, to prevent any interference in the surgical field. Further, the 3D device was designed to accurately cover the bone surface from the piriform aperture to the infra-zygomatic crest with two fixation points (the anterior nasal spine and the piriform aperture), which ensured stabilization of the 3D device. The device is thin and does not interfere with the surgical field. Safe and accurate surgical performance is possible using this device and piezosurgery. The roots of the teeth and the infraorbital nerve were unharmed during the surgery. Conclusions: This device is considerably smaller than conventional devices and is a simple, low-cost, and efficient method for performing accurate high Le Fort I osteotomy.

구순 및 구개열을 가진 상악 후퇴증 환자의 교정-외과적 치험 1례 (A Case of Orthognathic Surgery in Congenital Alveolar-Palatal cleft patient)

  • 유아기에 구순성형술 및 구개성형술 등에 의한 중 안면부의 성장장애를 동반한 성인환자에서 만기 이차성 골이식술(Late secondary bone graft)을 동반한 Le Fort I osteotomy를 시행한 결과 기능적, 심미적으로 양호한 개선 효과를 얻었다. 1. 증례에서는 골지지가 거의 없는 우측 상악 중절치 및 측절치 부위의 치아를 발거하고, 술전 교정치료로 변위된 치아의 배열과 소실된 공간을 회복한 후 장골이식을 동반한 상악골 전진술을 시행하였다. 2. 파열 변연부위에 골점막 절개를 시행한 후 순측 구개측 봉합 및 비점막을 거상하여 구비강 누공을 폐쇄한 후 장골능에서 얻은 골수-망상골을 이식하였다. 3. 수술후 구비강 폐쇄로 비음이 개선되었고, 술후 8개월된 방사선 사진상 파열부위의 골 재생을 확인한 후 보철치료를 시행하였다. 4. 표준 두부방사선 계측상 상악골의 전진에 의한 측모의 현저한 개선을 관찰할 수 있었다.

유지 인대의 외부 고정을 통한 제1형 신경섬유종증 환자의 안면부 변형 교정 (External Fixation of Retaining Ligament in Correction of Facial Disfigurement in Type-1 Neurofibromatosis Patients)

  • Purpose: In neurofibromatosis patients, complete surgical excision of the mass is almost impossible and surgical treatment usually consists of multiple serial excisions that only result in a debulking effect. Remnant tumor mass has a gravitational effect on facial soft tissues that leads to sagging of skin and soft tissue, and eventually, facial disfigurement and asymmetry. The purpose of our surgical method is to perform soft tissue lifting with longer lasting effect with less surgical risk of damaging facial nerve and vessels. With external fixation using K-wire or surgical screw, the procedure only called for a short incision length and had additional adhesive properties that enabled anchoring of soft tissue in a lifted position for a longer postoperative period. Methods: A total of 5 neurofibromatosis patients (NF-1) visited our clinic for mass reduction and face lifting. The age of patients ranged from 13 to 42 (mean 28.8 years), and most patients had a long history of multiple excisions in the past. Face lifting was performed in 2 different areas, the periorbital area in 3 patients, and the midface in 2 patients. The materials used in fixation of retaining ligament were K-wire (n=3) and titanium screw (n=2). Results: Follow up period was from 5 month to 3 years and 1 month (mean=2 years and 1 month). All patients conveyed satisfaction with the results and no major complications were reported. The lifting effect lasted for as long as 3 years, and there were no complaints of relapse of soft tissue depression or sagging within the operated area. 1 patient (M/13) needed secondary k-wire insertion and additional mass excision in 1 year and 10 months postoperatively due to tumor growth. In two patients with K-wire fixation, mild dimpling and tenderness were observed in the follow up period, but in about 2 months postoperatively, dimpling was relieved and there was no need for removal of fixating material. Conclusion: Surgical lifting in neurofibromatosis patients can be challenging, for mass excision cannot be done completely and gravitational effect by residual mass can be persistent. External fixation of the retaining ligament in patients with neurofibromatosis can give satisfactory results-for incision length is relatively shorter, and the lifting effect can last longer compared to other various face lifting techniques.
