Prevalence of methicillin-resistant Staphylococcus aureus in Iranian children: a systematic review and meta-analysis

  • Background: Antibiotic resistance is associated with longer hospitalizations, higher treatment costs, and increased morbidity and mortality rates. Purpose: This study aimed to determine the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) in Iranian children. Methods: International databases, including Web of Science, PubMed, Embase, and Scopus, and Iranian databases, including Scientific Information Database (www.sid.ir), Magiran, and Iranian Database for Medical Literature (idml.research.ac.ir), were systematically searched for articles published between January 2000 and August 2019. Sources of heterogeneity were determined using subgroup analysis and meta-regression. Results: Overall, 343 studies were identified; of them, 20 were included in the meta-analysis to estimate the pooled prevalence. The pooled prevalence of MRSA was 42% (95% confidence interval [CI], 29-55) among culture-positive cases of S. aureus, 51% (95% CI, 39-62) in hospitalized children, and 14% (95% CI, 0.05-27) in healthy children. Conclusion: The overall pooled prevalence of MRSA in children was 42%. Appropriate infection control measures and effective antibiotic therapy are needed.

  • Methicillin-resistant Staphylococcus (S.) aureus (MRSA) is one of the most important nosocomial pathogens worldwide and the emergence of this strain has become a major clinical problem. In this study, we investigated the prevalence of MRSA and their genetic characteristics in 69 S. aureus isolated from humans and animals. In human isolates, higher antimicrobial resistance rates were observed against penicillin (80.6%), followed by erythromycin (11.9%) and tetracycline (9.0%). All of them were susceptible to clindamycin, enrofloxacin, novobiocin, pirlimycin, trimethoprim/sulfamethoxazole and vancomycin. The resistance patterns in animal isolates were similar to those of human isolates. Two (2.9%) MRSA strains were isolated from human (n = 1) and animal (n = 1), and these isolates were confirmed as carrying the mecA gene. One isolate originating from human was resistant to 7 drugs and the other isolate derived from animal was resistant to 11 drugs. Staphylococcal cassette chromosome mec (SCCmec) variant IIIB was identified in animal isolate but SCCmec type of an isolate from human was not exactly determined. Two MRSA isolates showed unrelated PFGE pattern between them. Our results indicated although the frequency of MRSA isolates from humans and animals was low, a continuous surveillance and monitoring should be called for to prevent the contamination and spread of MRSA in the community. To our knowledge, this is the first time that SCCmec type variant IIIB was detected from animals in Korea.

Pattern of Methicillin-resistant Staphylococcus aureus in Dental and Medical Environments

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most prevalent pathogens in hospitals. To investigate cross contamination by this bacterium in both dental and medical settings, the pathogens that cause acute pyogenic infection and one of the major microbes responsible for nosocomial infection were isolated from health care providers, nurses and patients. We used VITEK II to measure drug sensitivity, and we further performed biochemical testing, coagulase serotype testing and pulse-field gel electrophoresis (PFGE) for isolated MRSA colonies. The isolation rate of Staphylococcus aureus from nasal swabs was 75.0% from dental health care providers and 18.8% from the medical health care providers. A total of 10 MRSA strains were isolated from 40 health care providers and 2 patients and the prevalent coagulase serotype from patients and health care providers was VII. The antimicrobial drug resistance and partial PFGE types of the isolated MRSA strains showed a similar pattern. These results suggest that MRSA may be one of the principal causes of nosocomial infection in dental and medical hospitals.

Isovitexin Protects Mice from Methicillin-Resistant Staphylococcus aureus-Induced Pneumonia by Targeting Sortase A

  • The rise of methicillin-resistant Staphylococcus aureus (MRSA) has resulted in significant morbidity and mortality, and clinical treatment of MRSA infections has become extremely difficult. Sortase A (SrtA), a virulence determinant that anchors numerous virulence-related proteins to the cell wall, is a prime druggable target against S. aureus infection due to its crucial role in the pathogenicity of S. aureus. Here, we demonstrate that isovitexin, an active ingredient derived from a variety of traditional Chinese medicines, can reversibly inhibit SrtA activity in vitrowith a low dose (IC50=24.72 ㎍/ml). Fluorescence quenching and molecular simulations proved the interaction between isovitexin and SrtA. Subsequent point mutation experiments further confirmed that the critical amino acid positions for SrtA binding to isovitexin were Ala-92, Ile-182, and Trp-197. In addition, isovitexin treatment dramatically reduced S. aureus invasion of A549 cells. This study shows that treatment with isovitexin could alleviate pathological injury and prolong the life span of mice in an S. aureus pneumonia model. According to our research, isovitexin represents a promising lead molecule for the creation of anti-S. aureus medicines or adjuncts.

Antibacterial Activity of Curcuma longa against Methicillin-resistant Staphylococcus aureus

  • Methicillin-resistant Staphylococcus aureus (MRSA) has been emerging worldwide as one of the most important hospital and community pathogens. Therefore, new agents are needed to treat the MRSA. In the present study, we investigated antimicrobial activity of ethyl acetate, methanol, and water extracts of Curcuma longa L. (C. longa) aganist clinical isolates of MRSA. The ethyl acetate extract of C. long a demonstrated a higher antibacterial activity than the methanol extract or water extract. Since the ethyl acetate extract was more active than other extracts, we examined whether ethyl acetate extract may restore the antibacterial activity of β-lactams and alter the adhesion and invasion of MRSA to human mucosal fibroblasts (HMFs). In the checkerboard test, ethyl acetate extract of C. longa markedly lowered the MICs of ampicillin and oxacillin against MRSA. In the bacterial adhesion and invasion assay, MRSA intracellular invasion were notably decreased in the presence of 0.125 - 2 mg/ml of C. longa extract compared to the control group. These results suggest that ethyl acetate extract of C. longa may have antibacterial activity and the potential to restore the effectiveness of β-lactams against MRSA, and inhibit the MRSA adhesion and invasion to HMFs.

  • We evaluated the performance of a novel screening test, PBP2a MRSA rapid kit (Dinona Inc., Iksan, Korea), for methicillin-resistant Staphylococcus aureus (MRSA) based on a immunochromatographic assay. The test is able to detect penicillin-binding protein 2a (PBP2a) using the nasal specimens from health care workers. The nasal specimens were obtained from 69 healthcare workers and were incubated in enrichment broth followed eight hours incubatin in BHI with cefoxitin $4{\mu}g/mL$. These broth were tested by PBP2a Rapid Kit. The enrichment broths were also directly tested for tube coagulase using the conventional identification method. 19 of 22 MRSA showed positive results by PBP2a rapid test and direct coagulase test (the sensitivity for detection of MRSA, 86.36%). While, 8 of 47 non-MRSA showed false positive results for the two tests. All of the 8 non-MRSA which showed false positive were co-colonizing isolates with MRCNS and MSSA. In addition, 46 of 49 methicillin-resistant staphylococci (MRS) showed positive results for PBP2a MRSA rapid kit (the sensitivity for detection of MRS, 93.8%), and all of 20 non-MRS showed negative results (specificity, 100%). The combination of PBP2a MRSA rapid kit and direct coagulase test showed the good sensitivity for detection of MRSA from anterior nares but frequently showed false positive results from the co-colonizing carrier with MRCNS and MSSA.

Patterns of Antimicrobial Resistance and Detection of mecA Gene from Methicillin Resistant Staphylococcus aureus Isolated from Healthcare Facilities and U.S. Military Hospital in Korea

  • A total of 108 strains of MRSA (Methicillin-resistant Staphylococcus aureus) clinical isolates was collected from $121^{st}$ general hospital (U.S. military hospital), Korean healthcare facility from January to March in 2005 and Wonju Christian hospital in 1999. Antimicrobial susceptibility test by Vitek System and MIC test using oxacillin and cephalothin stripes by E-test were executed. PCR based detection of mecA gene was performed on the all of MRSA clinical isolates, too. MRSA clinical isolates were characterized with antimicrobial resistance patterns, PCR based detection of mecA gene and validation of the multiplex PCR strategy of SCCmec among clinical isolates.

황금작약탕(黃芩芍藥湯)의 MRSA에 대한 항균활성에 관한 연구 (Antibacterial Effect of Huanggeumjakyak-tang against Methicillin-resistant Staphylococcus aureus)

  • Objectives The aim of this study was to investigate antimicrobial activity of Huanggeumjakyak- tang water extract against MRSA. Methods The antibacterial activities of Huanggeumjakyak-tang were evaluated against 3 strains of Methicillin-resistant Staphylococcus aureus (MRSA) and 1 standard Methicillinsusceptible S. aureus (MSSA) strain by using the disc diffusion method, minimal inhibitory concentrations (MICs) assay, colorimetric assay using MTT test, checkerboard dilution test and time-kill assay was performed under dark. Results The MIC (minimum inhibitory concentration) of Huanggeumjakyak-tang water extract against S. aureus strains ranged from 1,000 to $2,000{\mu}g/ml$. So we confirmed that it has a strong antibacterial effect. Also the combinations of Huanggeumjakyak-tang water extract and conventional antibiotics exhibited improved inhibition of MRSA with synergy effect. Conclusions The results obtained in this study suggest that Huanggeumjakyak-tang water extract showed antibacterial effect against MRSA, and it also showed reducing effect on the side-effect problems that are the major weak points of traditional antibiotics.

신생아황달 환아에서의 메티실린내성 황색포도알균 보균율에 관한 연구 (Carriage Rates of Methicillin-resistant Staphylococcus aureus in Neonates with Neonatal Jaundice)

  • 목 적 : 황색포도알균의 보균율은 신생아에서 가장 높다고 알려져 있고, 황색포도알균의 보균은 곧 그 균주에 의한 감염증으로 이어질 위험이 높아진다. 본 연구에서는 신생아황달로 입원한 환아에서의 황색포도알균과 MRSA의 보균율을 조사하였고, 신생아황달의 대부분이 모유황달이나 생리적황달임을 감안하여 이를 통해 간접적으로 건강한 신생아의 황색포도알균 및 MRSA 보균율을 추정하고자 하였다. 방 법: 2006년 1월부터 2010년 12월까지 성애병원과 광명성애병원 신생아집중치료실에 황달로 입원한 환아 545명을 대상으로 하였다. 입원 첫날 멸균된 면봉으로 비강과 서혜부를 도찰하여 얻은 검체를 세균배양을 통해 황색포도알균을 동정하고 다시 항생제감수성 검사를 통해 메티실린내성 여부를 판정한 결과를 후향적으로 조사하였다. 결 과:총 545명의 환아가 본 연구에 포함되었고, 이중 318명의 비강과 서혜부에서 황색포도알균이 분리되어 보균율은 58.3%였고 214명의 환아에서는 MRSA가 분리되어 MRSA 보균율은39.3%였다. 또한 분리된 MRSA를 항생제 감수성 결과를 토대로 분석하였을 때, CA-MRSA로 추정되는 균주는 65.7% (142/216), HA-MRSA로 추정되는 균주는 34.3% (74/216)였다. 결 론: 39.3%의 MRSA 보균율로 미루어볼 때, 외부에서 전원되는 신생아를 대상으로 한 MRSA 감시배양검사는 필요하다고 생각된다. 또한, MRSA 균주 중 CA-MRSA 가능성 균주가 65.7%로 높게 나와 이미 지역내산부인과 및 분만실 등에 CA-MRSA 균주가 정착해 있을 가능성이 높다고 생각되며 이에 대한 지속적이고도 정기적인 MRSA 감시배양검사도 필요할 것으로 생각된다.

통현이팔단 에탄올 추출물의 Methicillin Resistant Staphylococcus aureus에 대한 항균활성 (Antibacterial activity of Tonghyeonipal-dan against Methicillin-resistant Staphylococcus aureus)

  • Objectives : Methicillin-resistantStaphylococcus aureus(MRSA) is a human pathogen. New antibacterial agents are needed to treat MRSA-related infections. This study investigated the antibacterial activity of EtOH 70% extracts ofTonghyeonipal-dan(THD) which prescription is composed of oriental medicine against MRSA.Methods : The antibacterial activity of THD was evaluated against MRSA strains by using the Disc diffusion method, broth microdilution method, Checkerboard dilution test, and Time-kill test; its mechanism of action was investigated by bacteriolysis, detergent or ATPase inhibitors were used.Results : The minimum inhibitory concentration (MIC) of THD is 1,000~2,000 μg/mL against MRSA. In the checkerboard dilution test, fractional inhibitory concentration index (FICI) of THD in combination with antibiotics indicated synergy or partial synergism againstS. aureus. Furthermore, a time-kill assay showed that the growth of the tasted bacteria was considerably inhibited after 24 h of treatment with the combination of THD with selected antibiotics. For measurement of cell membrane permeability, THD 500 μg/mL along with concentration of Triton X-100 (TX) and Tris-(hydroxymethyl) aminomethane (TRIS) were used. In the other hand, N,N-dicyclohexylcarbodimide (DCCD) and Sodium azide (NaN3) were used as an inhibitor of ATPase. TX, TRIS, DCCD and NaN3 cooperation againstS. aureusshowed synergistic action.Conclusions : Accordingly, antimicrobial activity of THD was affected by cell membrane and inhibitor of ATPase were assessed. These results suggest that THD has antibacterial activity, and that THD extract offers great potential as a natural antibiotic against MRSA.
