50대 폐경 여성의 대사증후군 위험인자 분석 (Analysis of Metabolic Syndrome Risk Factors among the Menopausal Women in Her Fifties)

  • Purpose: The purpose of this study was to assess the difference of metabolic syndrome (MS) risk factors according to the MS diagnosis in the age of fifties women. Methods: Forty-two subjects (non metabolic syndrome 30, metabolic syndrome 12) were recruited from the health promotion center of a tertiary care hospital in an urban city. MS was defined by third report of the national cholesterol education program (NCEP) expert panel on detection, evaluation, and treatment of high blood cholesterol in adults (Adult Treatment Panel III) (ATP III), and abdominal obesity was determined by Asia-Pacific criteria in waist circumference. Results: The prevalence of MS by definition of NCEP-ATP III and Asia-Pacific criteria in waist circumference was 28.6% in the age of fifties women. The educational level and the median HDL-cholesterol were significantly lower in the MS patient group than in the Non-MS group. The median of weight, triglyceride, and fasting glucose were significantly higher in the MS patient group than in the Non-MS group. Conclusion: These results indicate that the nursing care should be focused on weight, triglyceride, HDL-cholesterol, and fasting glucose of the MS patients in the fifties women.

베이지안 네트워크를 이용한 대사증후군 모델링 (Modeling of Metabolic Syndrome Using Bayesian Network)

  • 대사증후군은 뇌졸중이나 심혈관 질환 등 다양한 합병증으로 발전될 수 있어 그 심각성이 커지고 있으며, 우리나라의 유병률이 증가하는 추세를 보이고 있어 연구의 필요성이 강조되고 있다. 본 연구는 베이지안 네트워크를 활용하여 대사증후군과 그 진단기준이 되는 5가지 이상 징후인 복부비만, 고중성지방혈증, 고혈압, 저 HDL 콜레스테롤혈증, 고혈당의 관계에 대한 모델을 구축 하고자 하였다. 추가적으로 대사증후군의 가장 위험한 진단조합을 선별하였고, 개개인의 특성에 따라 대사증후군의 특징도 다르게 나타난다는 것을 확인하였다. 사용된 데이터는 제5기 국민건강영양조사 중 2010년 자료로써 건강설문조사의 모든 문항에 응답한 성인 4,489명의 데이터이다.

  • Purpose: This study was aimed at identifying levels of compliance of patients with metabolic syndrome and the factors influencing their compliance. Methods: Data were collected from patients with metabolic syndrome at K medical center in 2009 using questionnaires. The data were analyzed using ANOVA, t-test, Scheffe test, Pearson correlation, and stepwise multiple regression. Results: The mean score of health behavior compliance was 2.82 (range: 1.43~3.87). Of the factors significantly influencing compliance with health behavior, health perception, exercise efficacy, age and perceived severity explained the 42.8% variance of compliance with health behavior. The factor explaining the highest level of variance was health perception. Conclusion: It is essential for health professionals to consider the aforementioned four factors when developing interventions to increase compliance with health behavior of the patient with metabolic syndrome.

대사증후군 노인의 운동지속 영향요인 (Factors affecting on maintenance of exercise among elderly with metabolic syndrome)

  • 본 연구의 목적은 대사증후군 노인의 운동유지 여부에 따라 운동유지에 대한 영향요인을 확인하기 위해 시행된 횡단적 조사연구이다. 본 연구의 대상자는 총 136명이며, 자료의 수집은 대사증후군 노인의 운동유지 영향요인을 알아보기 위해 운동유지, 운동기대감, 운동 관련 환경, 운동 사회적지지, 운동자기효능감에 대한 도구로 수집되었다. 수집된 자료는 SPSS Win 20.0 program을 사용하여 requency, X2, t-test, Logistic regression으로 분석하였다. 연구 결과 연령, 운동기대감, 운동자기효능감이 이들의 운동유지 영향요인임이 밝혀졌다. 이러한 결과는 대사증후군 노인의 운동유지를 위해서는 내적 동기에 해당하는 자기효능감을 함양시키고 운동기대감을 증진시키는 것이 중요하다고 할 수 있다. 또한 대사증후군 노인을 위한 건강증진프로그램을 계획할 때 운동에 대한 결과를 알려주고 이에 대한 이점을 부각시켜 스스로 운동을 잘 해낼 수 있도록 계획해야 하는 데에 본 연구결과가 기초자료로 활용할 수 있음이 기대된다.

중년 건강검진 수진자에서 대사증후군과 한방비만변증의 관련성 (Relation between Metabolic Syndrome and Obesity Pattern Identification Questionnaire in Middle-aged Health Check-up Examinees)

  • Objectives: Metabolic syndrome is considered a coronary heart disease risk factor and its prevalence rate is increasing in Korea. Because obesity is relevant to metabolic syndrome, we investigated the relationship between metabolic syndrome and the Obesity Pattern Identification Questionnaire in middle-aged health check-up examinees. Methods: This was a cross-sectional study with 125 patients who visited a health promotion center of university hospital from October 2012 to January 2013. We analyzed the association of Obesity Pattern Identification Questionnaire and the diagnostic criteria of metabolic syndrome. Results: Pi deficiency (脾虛), phlegm (痰飮), liver stasis (肝鬱) and food accumulation (食積) pattern showed significantly highs score in the group with hypertriglyceridemia. Also, females demonstrated significantly high scores of liver stasis (肝鬱) and food accumulation (食積) in the group with hypertriglyceridemia. The questions of Pattern Identification that showed especially significant high score in the group of hypertriglyceridemia are as follows: 'Easily get annoyed', 'Usually worried', 'Frequently overeating or bingeing', and 'Having more after getting full'. There are positive correlations between triglyceride and the score of Pi deficiency (脾虛), phlegm (痰飮) and food accumulation (食積) pattern. Conclusions: Obesity Pattern Identification Questionnaire can be used for the management of hypertriglyceridemia in an effort to prevent metabolic syndrome.

Associations among the Degree of Nonalcoholic Fatty Liver Disease, Metabolic Syndrome, Degree of Obesity in Children, and Parental Obesity

  • Purpose: To analyze the associations among the degrees of nonalcoholic fatty liver disease (NAFLD) by ultrasonography and metabolic syndrome, degrees of obesity in children, and degrees of parental obesity. Methods: A total of 198 children with obesity who visited a pediatric obesity clinic were prospectively enrolled in this study. The severity of NAFLD based on ultrasonography was classified into no, mild, moderate, or severe NAFLD group. The degree of obesity based on the percentage over standard weight for height per sex was classified into mild, moderate, or severe. Results: Of 132 patients evaluated for the degree of NAFLD and metabolic syndrome, the p-value of correlation between the two factors was 0.009. Therefore, metabolic syndrome might significantly affect the degree of NAFLD. Of 158 patients evaluated for the degree of NAFLD and the degree of obesity, the p-value of correlation between the two factors was 0.122. Of 154 patients evaluated for the degree of obesity and father's obesity, the p-value was 0.076. Of 159 patients evaluated for the degree of obesity and mother's obesity, the p-value was 0.000, indicating that mother's obesity could significantly affect the degree of obesity in children. Of 142 patients evaluated for the degree of obesity and metabolic syndrome, the p-value was 0.288. Conclusion: Metabolic syndrome might significantly affect the degree of nonalcoholic fatty liver in children. In addition, mother's obesity might be a significant factor that affects the degree of obesity in children.

Positive Effects of Adiponectin, BDNF, and GLP-1 on Cortical Neurons Counteracting Palmitic Acid Induced Neurotoxicity

  • The prevalence of metabolic syndrome caused by diets containing excessive fatty acids is increasing worldwide. Patients with metabolic syndrome exhibit abnormal lipid profiles, chronic inflammation, increased levels of saturated fatty acids, impaired insulin sensitivity, excessive fat accumulation, and neuropathological issues such as memory deficits. In particular, palmitic acid (PA) in saturated fatty acids aggravates inflammation, insulin resistance, impaired glucose tolerance, and synaptic failure. Recently, adiponectin, brain-derived neurotrophic factor (BDNF), and glucose-like peptide-1 (GLP-1) have been investigated to find therapeutic solutions for metabolic syndrome, with findings suggesting that they are involved in insulin sensitivity, enhanced lipid profiles, increased neuronal survival, and improved synaptic plasticity. We investigated the effects of adiponectin, BDNF, and GLP-1 on neurite outgrowth, length, and complexity in PA-treated primary cortical neurons using Sholl analysis. Our findings demonstrate the therapeutic potential of adiponectin, BDNF, and GLP-1 in enhancing synaptic plasticity within brains affected by metabolic imbalance. We underscore the need for additional research into the mechanisms by which adiponectin, BDNF, and GLP-1 influence neural complexity in brains with metabolic imbalances.

한국인 대학생군 대상의 청.장년층 대사이상증후군 위험성 평가 (Evaluation of the Risk of Metabolic Syndrome for the Young Adults in Korean Students of a University)

  • Metabolic syndrome, defined as the clustering of several metabolic disorders including obesity (waist circumference ${\geq}90$ if male or ${\geq}80$ if female, cm), dyslipidemia ($TG{\geq}150$ or HDL-C<40 if male or <50 if female, mg/dl), hypertension ($BP{\geq}130/85mmHg$) and hyperglycemia (fasting plasma $glucose{\geq}110mg/dl$), increases the cardiovascular risk of the general population. Recently, risk of this syndrome arises in young adults world widely. Therefore, we randomly selected and evaluated the risk of metabolic syndrome of total 43 people (group I-22, group II-21) for 2 years. Group I was 22 peoples (15 males, 7 females) with age of 22 thru 35 year old (average 28 year old) and group II was 21 people (19 male, 2 female) with age of 22 thur 32 years old (average 24 year old) in Cheongju area from March 1st thru 30th of 2008 in Cheongju area from September 1st thru 30th of 2007 in order to find out how serious this phenomenon is in young adult of Korea. 13.95% (n=7) of total people has a metabolic syndrome by NCEP/ATPIII definition among this group (group I-6, group II-1). Those of 6 have 3 or over risk factor for metabolic syndrome such as obesity, hypertension, fasting blood glucose and hypetriglyceridemia at the same time (group I-5, group II-1). Group I have more risk factor because of more higher age than group II. Therefore we need aggressively to monitor and provide them for early diagnosis, educational programs and assistance for lifestyle changes in order to prevent metabolic syndrome among young adults.

폐경 후 여성의 대사증후군 지표와 영양소 섭취에 대한 비교 융합연구 : 2010~2012 국민건강영양조사자료 이용 (Comparison of metabolic syndrome indicators and nutrient intakes in postmenopausal women : from the Korean National Health and Nutrition Examination Survey, 2010~2012)

  • 본 연구는 제 5기 국민건강영양조사(2010~2012) 결과를 바탕으로 폐경여성의 대사증후군 지표에 따른 영양소 섭취를 비교 조사하였다. 조기폐경 여성 214명과 일반폐경 여성 2,545명을 분류하였으며, 폐경여성의 영양섭취에서 비만인 경우 레티놀 섭취와의 연관을 나타내고 있음을 확인하였고, 당뇨인 경우 리보플라빈 섭취량에 있어서 유의함을 확인하였다. 본 연구 결과 폐경여성에서 레티놀과 리보플라빈, 조섬유, 칼슘 및 나트륨 등의 미량영양소가 대사증후군 위험과 관련성이 있음을 보였는데, 추후 대규모 전향적 연구가 실시되어 각 영양소 섭취량과 폐경 후 대사증후군의 인과관계를 더욱 명확히 규명할 필요가 있을 것이라 사료된다.

농촌지역 주민의 대사증후군 발생률과 위험요인 (The Incidence and Risk Factors of Metabolic Syndrome in Rural Area)

  • 본 연구는 대상증후군 발생률과 대사증후군 발생과 관련된 위험요인을 알아보고자 지역사회 주민을 대상으로 2006년부터 2010년까지 5년 간격으로 동일인을 추적조사하였다. 1차 기초조사와 2차 추적조사에 참여한 대상자는 620명이었으며, 이중 1차 기초조사에서 대사증후군에 진단되지 않았던 460명이 본 연구의 최종 연구대상자가 되었다. 2차 추적조사에서 새로이 대사증후군으로 판정된 대상자는 108명(23.5%)이었으며, 대사증후군의 발생률은 1,000인년당 25.2이었다. 대사증후군의 발생률은 남자가 여자보다 유의하게 높았고(p<0.01), 정상체중에 비해 과체중일때와 비만일때 유의하게 높았으며 (p<0.01), 비흡연보다 과거흡연이나 현재흡연 상태일 때 대사증후군의 발생률이 유의하게 높았다(p<0.01). 연령, 교육수준, 배우자유무, 경제상태, 직업유무, 총콜레스테롤수치, 음주, 운동에서는 유의한 차이가 없었다. 대사증후군의 위험요인을 파악하고자 다중로지스틱회귀분석을 실시한 결과 비만이 다른 변수들은 보정한 후에도 유의한 변수로 선정되었으며, 정상체중에 비해 과체중일수록(OR=2.691, 95% CI=1.170-6.192), 정상체중에 비해 비만일수록(OR=4.727, 95% CI=2.137-10.457) 대사증후군의 발생률이 높았다. 따라서 대사증후군의 관리를 위하여 비만에 대한 관리가 매우 중요하며, 이를 충분히 고려한 건강관리지원이 필요하리라 여겨진다.
