Effects of Sarcopenic Obesity on Metabolic Syndrome in Korean Elders: Using Data from the Korea National Health and Nutrition Examination Survey (2008-2011)

  • Purpose: This study was conducted to examine effects of sarcopenic obesity on metabolic syndrome in Korean elders. Methods: This study is based on the analysis of the Korea National Health and Nutrition Examination Survey (KNHANES) with 1,155 subjects (524 men, and 631 women) aged 60 or older, from 2008 to 2011. Sarcopenia was defined as an appendicular skeletal muscle (ASM), divided by weight (%) of <1 SD (standard deviation) below the sex-specific mean for young adults. Obesity was defined as a total body fat percent (men${\geq}$25%, women${\geq}$35%). Results: The prevalence of SO (sarcopenic obesity) was 13.3% among men and 22.5% among women. Both sexes showed a higher total body fat percent, and the SMI (skeletal muscle index) was the lowest in the SO group. Metabolic syndrome was highly prevalent in the SO group (52.5% men, 60.4% women). The SO group showed a higher risk for metabolic syndrome (odds ratio men 6.57 [95% CI 5.19~7.27], women 3.89 [95% CI 2.41~6.29]) than the obese group (men 3.14 [95% CI 1.76~4.14], women 2.54 [95% CI 1.38~4.65]). Conclusion: SO is a major risk factor for metabolic syndrome in Korean elders. Therefore, a nursing program should be given to the Korean elderly SO group to prevent metabolic syndrome.

The Association between Food Group Consumption Patterns and Early Metabolic Syndrome Risk in Non-Diabetic Healthy People

  • We investigated the association between dietary habits/food group consumption patterns and early risk of metabolic syndrome (MetS), a main cause for metabolic disease. Study participants were recruited from the health promotion center in Dong-A University Hospital and public advertisement. Study subjects (n = 243, 21-80 years) were categorized into three groups: Super-healthy (MetS risk factor [MetS RF] = 0, n = 111), MetS-risk carriers (MetS RF = 1-2, n = 96), and MetS (MetS $RF{\geq}3$, n = 27). Higher regularity in dietary habits (breakfast-everyday, regular eating time, non-frequent overeating, and non-frequent eating-out) was observed in the Super-healthy group than in the MetS-risk carriers, and particularly in the MetS subjects. The relationship between food group consumption patterns and MetS-risk related parameters were investigated with adjustment for confounding factors. Fruit consumption was positively associated with HDL-cholesterol, and tended to be negatively associated with waist circumference, triglyceride, LDL-cholesterol, and insulin resistance (IR). The consumption of low-fat meats and fish, and vegetables was negatively associated with hs-CRP. Specifically, the consumption of seafoods belonging to the low-fat fish was negatively associated with fasting glucose, hs-CRP, and interleukin (IL)-6. Anchovy/dried white baits consumption was negatively associated with fasting insulin and IR. Green-yellow vegetables consumption was negatively associated with fasting insulin, IR, and hs-CRP. On the other hand, sugars and fast-foods were positively associated with LDL-cholesterol. Additionally, fast-foods consumption was positively associated with hs-CRP and IL-6 levels. In conclusion, dietary habits/food group consumption patterns are closely associated with MetS-risk related parameters in Koreans. It may suggest useful information to educate people to properly select healthy foods for early prevention of MetS.

소아청소년에서의 대사증후군 역학 (Epidemiology of the metabolic syndrome among Korean children and adolescents)

  • The prevalence of obesity in Korean children is estimated to be around 10%, and has increased significantly over the past 20 years. Metabolic syndrome, which includes central obesity, glucose intolerance, insulin resistance, dyslipidemia, and hypertension, is a well documented risk factor for cardiovascular diseases, but there is no general consensus concerning its definition for children. In this study, ablut 30-40% of overweight or obese children had metabolic syndrome compared to less than 10% in the general population. Dyslipidemia was the most common component and hyperglycemia was the least common component of metabolic syndrome. If the prevalence of childhood obesity continues to increase, it is highly likely that the prevalence of the metabolic syndrome will also increase. These findings emphasize the importance of preventing obesity in addition to the diagnosis and management of metabolic syndrome in Korean adolescents.

Targeting the Gut Microbiome to Ameliorate Cardiovascular Diseases

  • The bacterial cells located within the gastrointestinal tract (GIT) outnumber the host's cells by a factor of ten. These human digestive-tract microbes are referred to as the gut microbiota. During the last ten years, our understanding of gut microbiota composition and its relation with intra- and extra-intestinal diseases including risk factors of cardiovascular diseases (CVD) such as atherosclerosis and metabolic syndrome, have greatly increased. A question which frequently arises in the research community is whether one can modulate the gut microbial environment to 'control' risk factors in CVD. In this review, we summarized promising intervention methods, based on our current knowledge of intestinal microbiota in modulating CVD. Furthermore, we explore how gut microbiota can be therapeutically exploited by targeting their metabolic program to control pathologic factors of CVD.

  • 1. Objectives The purpose of this study was to investigate the high-sensitivity C-reactive protein (hsCRP) related characteristics found in each Sasang Constitutional type. 2. Methods This cross-sectional epidemiological study was conducted using data from 2,842 men and women. After typing each participant into one of the four Sasang constitutional types, they were analyzed on various cardiovascular disease-related variables. 3. Results 1) The predictors of cardiovascular disease (metabolic syndrome, high risk FRS group, high risk hsCRP group) was found to show the highest prevalence in the Taeeum type. 2) In the group with metabolic syndrome, the mean hsCRP concentration in the Taeeum and Soyang types were higher than in the Soeum type. In the group without metabolic syndrome, the mean hsCRP concentration in the Taeeum type were higher than in the Soyang and Soeum types. 3) In the FRS low risk group, the mean hsCRP concentration were higher in order of Taeeum type > Soyang type > Soeum type. In the FRS high risk group, the mean hsCRP concentrations were not significantly different among the Sasang constitutional types. 4. Conclusions The results of this study suggest that the Taeeum constitutional type is a risk factor for high hsCRP and cardiovascular disease. These findings suggest that the prevention of cardiovascular disease is more important in the Taeeum type compared to other constitutional types. Moreover, preventive measures are warranted even in the Taeeum-type persons with low clinical cardiovascular risk.

한국여성의 폐경에 따른 대사증후군과 영양섭취에 대한 단면 연구 (A Cross-sectional Study of Nutrient Intake for Korean Women with Menopause and Metabolic Syndrome)

  • Menopausal status and hormonal changes are important factors related to the prevalence of metabolic syndrome in women. Nutrient intake is also a risk factor for metabolic syndrome. Although, postmenopausal status and hormonal changes result in a 60% increased risk for metabolic syndrome, there has been no method to elucidate the effects of nutrient intake on metabolic syndrome following menopause. This study was conducted to evaluate the effects of nutrient intake by menopausal women on the prevalence of metabolic syndrome. All data were obtained from the Korean National Health and Nutrition Examination Survey 2007-2009. Menopausal women showed a 1.8-fold increase in the prevalence of metabolic syndrome. Metabolic syndrome group showed significantly lower values in calcium, iron, vitamin A, carotin, retinol and riboflavin intake than those of normal group in premenopausal women. In postmenopausal women, there are significant differences in crude fiber, calcium, iron, potassium, riboflavin and niacin. Indeed, different patterns of nutrient intake were observed by menopausal status and metabolic syndrome. As menopause cannot be controlled, a diet with adequate nutrient intake may be useful to control the rapid increase in the prevalence of metabolic syndrome due to menopause.

근로자의 서서하는 작업시간과 대사증후군의 관련성 (Association Between Occupational Standing Time and Metabolic Syndrome in Korean Male Workers)

  • Purpose: In this study, we sought to understand the risk factor for chronic disease of workers by studying the association between occupational standing time and metabolic syndrome(MetS) in full-time 300 male workers. Materials: Data on age, life habit, work related information of the subjects were surveyed using self-reported questionnaire and interview. MetS was identified based on the report of Alberti et al.(2009). As for the data analysis, SPSS 19.0 was used to conduct the descriptive statistic, ANOVA, Pearson correlation coefficient and multiple logistic regression analysis. Results: Daily working hour affects on the lifting heavy objects, running and strenuous exercise(r=-0.137, p<0.01), and total physical function decreased with the increase in age(r=-0.145, p<0.01). Also, obese was significantly associated with genuflection and bend over(r=-0.110, p<0.05). On multiple logistic regression analysis for the diagnostic indices of MetS, occupational standing time were significantly associated with waist circumference(odds ratio=0.885, ${\beta}$ value=-0.122, 95% CI=0.797-0.983, p<0.05) and triglyceride (odds ratio=0.873, ${\beta}$ value=-0.136, 95% CI=0.800-0.953, p<0.01). Conclusions: These results suggest that the working posture may be important risk factor in pathogenesis and growing of MetS and cardiovascular disease.

  • Metabolic syndrome (MS) is increasing as obesity increases in Korea. In this study, we investigated the relationship between MS and gallbladder polyp (GBP). We examined 9,861 subjects who visited a single health-screening center in Jeju between January 2009 and December 2019. MS was defined according to the revised National Cholesterol Education Program criteria. Multiple logistic regression analysis was performed after adjusting for age to evaluate the association between the components of MS and GBP. The estimated rates of MS and GBP among subjects were 28.5% and 4.8%, respectively. Multivariate analysis suggested that MS (odds ratio[OR] 1.405, P=0.006) was the only independent factor affecting GBP. In our study sample, MS was correlated with GBP. The prevalence of gallbladder polyp increased with an increase in the risk factors indicating MS.

사상체질에 따른 대사증후군과 Adiponectin의 상관성 (Association Between Metabolic Syndrome and Adiponectin according to Sasang Constitution)

  • 1. Objectives: The objective of this study is to investigate the relationship between adiponectin and metabolic syndrome according to Sasang Constitution. 2. Methods: Six hundred sixty six participants were included in this cohort study. Sex, age, BMI(Body Mass Index), smoking, drinking, adiponectin level and Metabolic syndrome components were considerd. Sasang constitutional diagnosis was carried out by a sasang constitutional specialist using photos, questionnaires and PSSC(Phonetic System for Sasang Classification). 3. Results: In binary logistic analysis after adjustment of age, sex, BMI, smoking, drinking, adiponectin level and sasang constitution were related with Metabolic syndrome. 4. Conclusions: We suggest that adiponectin and sasang constitution are the important biomarker in Metabolic syndorme.

사상체질병증 임상진료지침: 예방 및 위험인자 (Clinical Practice Guideline for Sasang Constitutional Medicine: Prevention and Risk Fators of Sasangin Disease)

  • Objectives This research was proposed to present Clinical Practice Guideline (CPG) for Prevention of Sasangin disease pattern of Sasang Constitutional Medicine (SCM) and diseases closely related with Sasang constitution. Each CPG was developed by the national-wide experts committee consisting of SCM professors. Methods At first, we searched the literatures related to SCM such as "Dongeuisusebowon", Textbook of SCM and Clinical Guidebook of SCM. Also we searched the articles related to the studies about risk factors for Sasangin disease pattern of both at home and abroad. Finally, we selected leading risk factors of Sasangin disease pattern and developed CPG for prevention of Sasangin disease pattern of SCM. And then, we searched the literatures related SCM such as "Dongeuisusebowon" and the articles on the correlation between disease and Sasang constitution using case-control studies, observational studies or cross sectional studies of both at home and abroad. Next, we selected diseases closely related with Sasang constitution on the basis of articles including prevalence rate and odds ratio between disease and Sasang constitution and finally developed CPG for these diseases. Results and Conclusions We categorized risk factors of Sasang disease pattern into 2 types: non-modifiable and potentially modifiable. 3 items (age, sex and genetic factors) were classified as non-modifiable risk factors of Sasang disease pattern. 6 items (original symptom, stress, diet and nutrition, physical activity, alcohol and drug misuse) were classified as less well-documented or potentially modifiable risk factors of Sasangin disease pattern. We found out Sasang constitution is more likely to develop some diseases. It was proven that Sasang constitution increase the risk of hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. And there is high probability of Sasang constitution being potential risk factor for obesity, hyperlipidemia, allergy and cancer. Also, we found out Taeeumin is independent risk factor for hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. Therefore we recommend that Taeeumin need to prevent these disease by regular checkups and aggressive management.
