BACKGROUND/OBJECTIVES: Nobiletin (NOB), a citrus flavonoid, is reported to have beneficial effects on cardiovascular and metabolic health. However, there is limited research investigating the effect of long-term supplementation with low-dose NOB on high-cholesterol diet (HCD)-induced hypercholesterolemia and non-obese nonalcoholic fatty liver disease (NAFLD). Therefore, we investigated the influence of NOB on hypercholesterolemia and NAFLD in HCD-fed mice. SUBJECTS/METHODS: C57BL/6J mice were fed a normal diet (ND) or HCD (35 kcal% fat, 1.25% cholesterol, 0.5% cholic acid) with or without NOB (0.02%) for 20 weeks. RESULTS: HCD feeding markedly reduced the final body weight compared to ND feeding, with no apparent energy intake differences. NOB supplementation suppressed HCD-induced weight loss without altering energy intake. Moreover, NOB significantly decreased the total cholesterol (TC) levels and the low-density lipoprotein (LDL)/very-LDL-cholesterol to TC ratio, and increased the high-density lipoprotein-cholesterol/TC ratio in plasma, compared to those for HCD feeding alone. The plasma levels of inflammatory and atherosclerosis markers (C-reactive protein, oxidized LDL, interleukin [IL]-1β, IL-6, and plasminogen activator inhibitor-1) were significantly lower, whereas those of anti-atherogenic adiponectin and paraoxonase were higher in the NOB-supplemented group than in the HCD control group. Furthermore, NOB significantly decreased liver weight, hepatic cholesterol and triglyceride contents, and lipid droplet accumulation by inhibiting messenger RNA expression of hepatic genes and activity levels of cholesterol synthesis-, esterification-, and fatty acid synthesis-associated enzymes, concomitantly enhancing fatty acid oxidation-related gene expression and enzyme activities. Dietary NOB supplementation may protect against hypercholesterolemia and NAFLD via regulation of hepatic lipid metabolism in HCD-fed mice; these effects are associated with the amelioration of inflammation and reductions in the levels of atherosclerosis-associated cardiovascular markers. CONCLUSIONS: The present study suggests that NOB may serve as a potential therapeutic agent for the treatment of HCD-induced hypercholesterolemia and NAFLD.

Background: Prevalence of adenoma in males aged 40-49 years in Korea was higher than expected. The aim of this study was to investigate the prevalence and risk factors of colorectal adenoma in males aged 40-49 years. Methods: Total 1,902 asymptomatic subjects with a mean age of $47.9{\pm}6.7years$, who underwent a screening colonoscopy in a health promotion center of Myongji Hospital from 2010 to 2013 were enrolled in this study. We conducted a case-control study to determine the risk factors for adenoma. The subjects were classified into two groups (adenoma vs. controls). To validate the diagnostic value of carcinoembryonic antigen (CEA) for adenoma, area under the receiver operating characteristic curve (AUROC) was calculated. Results: At least one colorectal adenoma was identified in 385 subjects (20.2%). Among these 385 subjects, 372 subjects were found to have a non-advanced adenoma, 13 subjects had an invasive adenoma. One subject had cancer. Male sex, age, smoking, metabolic syndrome, and elevated CEA level were significantly associated with a colorectal adenoma in univariate analysis. However, metabolic syndrome was not significant in multivariate analysis. In the male group, the AUROC of CEA for colorectal adenoma was 0.600 (0.543 to 0.656) in non-smokers under 50 years of age, and 0.615 (0.540 to 0.690) in smokers under 50 years of age. Conclusion: Male sex, smoking, and high levels of CEA seem to be associated with colorectal adenoma. High levels of CEA and smoking may be diagnostic markers for any colorectal adenoma in Korean males aged 40-49 years.
본 연구는 칼슘고갈식이와 칼슘회복식이를 급여한 SD 종 흰쥐에서 골격대사 지표와 골밀도를 측정함으로써 어린소재 Ca (Tilapia mossambica 비늘을 이용한 칼슘소재)의 유용성을 $CaCO_3$와 비교 평가하였다. 1) 칼슘고갈식이인 LoCa군에 비해 칼슘회복식이를 먹인 AdCa군과 AdFa군에서 체중증가, 사료섭취량이 유의하게 높았고, 이는 어린소재 Ca의 식이효율이 적정수준에 있다고 할 수 있다. 2) 혈청 PTH와 칼시토닌은 실험군 사이에 차이가 없었다. 3) 혈청 ALP와 오스테오칼신은 LoCa군에서 유의하게 증가하였고, AdCa와 AdFa군에서 저하되었다. 뇨의 DPD는 LoCa군에서 현저히 높은 수치를 보인 반면, AdCa와 Ad-Fa군에서 낮은 수치를 보임으로써 뼈의 형성지표와 흡수지표는 칼슘회복식이에 의해 개선되었다. 4) 대퇴골의 습윤무게, 건조무게, 체중 100g 단위당 중량이 모두 AdFa > AdCa > LoCa 순으로 높았고, 대퇴골의 회분함량과 칼슘함량도 모두 같은 순서를 보임으로써 뼈의 형성과 칼슘 축적에 AdCa군과 마찬가지로 AdFa군에서도 효과적이었음을 시사한다. 5) 대퇴골의 골밀도는 AdCa군과 AdFa군에서 LoCa군에 비해 25~26%의 유의하게 높은 수치를 보였다. 이상에서와 같이 칼슘회복식이 (AdCA와 AdFa)는 ALP, 오스테오칼신, DPD의 지표를 개선하였고, 대퇴골의 중량과 회분함량의 증가와 함께 골밀도를 향상시키는 것으로 나타났다. 따라서 어린소재 칼슘은 대조 칼슘급원으로 많이 사용되는 $CaCO_3$에 비교하여 뼈 건강에 대등한 효과를 보이므로, 칼슘 소재로서 개발될 가치가 있을 것으로 평가된다.
Intestinal microorganisms interact with various immune cells and are involved in gut homeostasis and immune regulation. Although many studies have discussed the roles of the microorganisms themselves, interest in the effector function of their metabolites is increasing. The metabolic processes of these molecules provide important clues to the existence and function of gut microbes. The interrelationship between metabolites and T lymphocytes in particular plays a significant role in adaptive immune functions. Our current review focuses on 3 groups of metabolites: short-chain fatty acids, bile acids metabolites, and polyamines. We collated the findings of several studies on the transformation and production of these metabolites by gut microbes and explained their immunological roles. Specifically, we summarized the reports on changes in mucosal immune homeostasis represented by the Tregs and Th17 cells balance. The relationship between specific metabolites and diseases was also analyzed through latest studies. Thus, this review highlights microbial metabolites as the hidden treasure having potential diagnostic markers and therapeutic targets through a comprehensive understanding of the gut-immune interaction.
본 연구의 목적은 대사적으로 비만한 건강인과 비건강강인의 신체활동과 좌식생활의 행태를 비교분석하는 것이었다. 본 연구의 대상자는 2014년 국민건강영양조사 중 1585명을 대상으로 하였으며, 대사적으로 건강한 비만인(metabolically healthy obesity; MHO)와 대사적으로 비건강한 비만인(metabolically unhealthy obesity; MUO)의 분류는 NCEP-ATP III(National Cholesterol Education Program-Adult Treatment Panel III)의 기준을 근거로 분류하였다. 연구결과, 남자대상자의 경우 MHO와 MUO 간의 신체활동 수준 및 규칙적인 운동 참여 수준은 차이가 없었다. 여자대상자의 경우 MHO와 MUO 간에 총 신체활동량과 근력운동 참여에서 MHO 집단이 더 많은 신체활동량과 근력운동 참여를 하는 것으로 나타났다. 결론적으로 여성의 경우, 신체활동과 운동참여 수준은 동일한 수준의 비만인에서 건강상태에 영향을 주는 하나의 중요한 요인이라 사료된다.
혈소판제제는 출혈질환이나 혈소판의 기능장애 치료를 위해 사용되고 있다. 혈소판성분채집술은 혈액성분채집기를 사용하여 혈소판 성분을 채집하는 방법으로, 성분채집혈소판의 질 평가에는 다양한 지표들이 이용되고 있다. 채집된 혈소판의 생산성은 채집효율(collection efficiency)과 채집률(collection rates)로 평가되고 보관된 혈소판의 질은 in vitro 상에서 시각적 외양, 대사활성도, 양, 혈소판 수, 백혈구 수, 미세입자(microparticles), 그리고 다양한 혈소판 활성 표지자들로 평가된다. 혈소판 활성 표지자들은 여러 연구분야에서도 이용되고 있다. 수혈된 후의 혈소판의 질은 교정증가치(corrected count increment)와 혈소판회복퍼센트(percentage platelet recovery)로 평가된다. 본 논문은 혈소판성분채집술의 채집과정, 사용되는 장비, 성분채집혈소판의 질, 전반적인 효율성, 그리고 질평가 지표들에 대한 리뷰(review)와 함께 다른 혈액성분채집기 간의 기능의 차이점을 비교하였고, 또한 혈소판성분채집술은 구연산염 항응고제에 의한 부작용을 일으킬 수는 있지만 전혈 헌혈시에 실신의 전구증상이나 실신을 일으킬 위험이 있는 헌혈자들에게는 더 안전한 방법임을 보여주고 있다.
People with higher genetic predisposition to obesity are more susceptible to cardiovascular diseases (CVDs) and healthy plant-based foods may be associated with reduced risks of obesity and other metabolic markers. We investigated whether healthy plant-foods-rich dietary patterns might have inverse associations with cardiometabolic risk factors in participants at genetically elevated risk of obesity. For this cross-sectional study, 377 obese and overweight women were chosen from health centers in Tehran, Iran. We calculated a healthy plant-based diet index (h-PDI) in which healthy plant foods received positive scores, and unhealthy plant and animal foods received reversed scores. A genetic risk score (GRS) was developed based on 3 polymorphisms. The interaction between GRS and h-PDI on cardiometabolic traits was analyzed using a generalized linear model (GLM). We found significant interactions between GRS and h-PDI on body mass index (BMI) (p = 0.02), body fat mass (p = 0.04), and waist circumference (p = 0.056). There were significant gene-diet interactions for healthful plant-derived diets and BMI-GRS on high-sensitivity C-reactive protein (p = 0.03), aspartate aminotransferase (p = 0.04), alanine transaminase (p = 0.05), insulin (p = 0.04), and plasminogen activator inhibitor 1 (p = 0.002). Adherence to h-PDI was more strongly related to decreased levels of the aforementioned markers among participants in the second or top tertile of GRS than those with low GRS. These results highlight that following a plant-based dietary pattern considering genetics appears to be a protective factor against the risks of cardiometabolic abnormalities.
목적: 경동맥 내막-중막 두께와 경동맥 경화반의 존재유무는 죽상동맥경화증의 임상 전단계를 나타내는 지표로 널리 사용되고 있다. 경동맥 내막-중막 두께를 측정할 때의 측정자 의존성 때문에, 다기관 연구에서는 경동맥 내막-중막 두께와 경화반 측정의 기관간 신뢰도를 확인하는 것이 중요하다. 이 연구의 목적은 심뇌혈관 및 대사질환원인 연구센터에 속해 있는 세 임상기관 사이의 경동맥 내막-중막 두께와 경화반 측정의 측정자간 신뢰도를 평가하는 것이다. 방법: 심뇌혈관 질환 과거력이 없는 사람 20명이 2014-2015년 사이에 이 연구에 참여하였고(연령 37-64세), 미리 정해진 프로토콜에 따라 연구참여자의 좌, 우 경동맥을 세 임상기관에서 반복적으로 측정하였다. 총 경동맥의 원위부에서 측정한 경동맥 내막-중막 두께의 최대값과 평균값을 기록하였다. 경동맥에서의 경화반 존재유무는 측정자에 의해 확인되었다. 경동맥 내막-중막 두께와 경화반 존재유무의 신뢰도를 급내상관계수와 카파 통계량을 통해 각각 평가하였다. 결과: 계산된 급내상관계수는 최대 경동맥 내막-중막 두께를 평가하였을 때 0.647이었고 (95% CI: 0.487-0.779), 평균 경동맥 내막-중막 두께를 평가하였을 때 0.758 (95% CI: 0.632-0.854) 이었다. Bland Altman plot에서, 관측치의 대부분은 평균의 차이에서 ${\pm}1.96$ 표준편차 사이에 분포하였다. 각 기관 사이의 경화반 존재유무에 대한 카파 통계량은 0.304 (기관 1과 2), 0.507 (기관 1과 3), 0.606 (기관 2와 3)이었다. 전반적인 일치를 평가하는 Fleiss카파값은 0.445였다. 결론: 세 임상기관 사이의 경동맥 내막-중막 두께의 측정자간 신뢰도는 훌륭하였으며, 경화반 존재유무에 대한 신뢰도는 적정하였다.

The purpose of the present study was to investigate cardiac damage biomarkers after a triathlon race in elite and non-elite athlete groups. Fifteen healthy men participated in the study. Based on performance, they were divided into elite athlete group (EG: n=7) and non-elite athlete group (NEG: n=8). Participants' blood samples were obtained during four periods: before, immediately, 2 hours and 7 days after finishing the race. creatine kinase (CK), creatine kinase-myoglobin (CK-MB), myoglobin, and lactate dehydrogenase (LDH) were significantly increased in both groups immediately after, and 2 hours after finishing the race (p<.05). CK, CK-MB, and myoglobin were completely recovered after 7 days (p<.05). Hematocrit (Hct) was significantly decreased in both groups (p<.05) 7 days after the race. LDH was significantly decreased in the EG (p<.05) only 7 days after the race. Homoglobin (Hb) was significantly decreased in the NEG (p<.05) only 2 hours after the race. Although cardiac troponin T (cTnT) was significantly increased in the EG but not in the NEG 2hours after the race (p<.05), there was no group-by-time interaction. cTnT was completely recovered in both groups 7 days after the race. In conclusion, cardiac damage occurs during a triathlon race and, is greater in elite than in non-elite. However, all cardiac damage markers return to normal range within 1 week.
In patients with type 2 diabetes, oxidative stress could be increased by their metabolic changes. Elevated plasma homocysteine is considered as one of markers of enhanced oxidative stress. Due to oxidative stress, some complications like cardiovascular or renal diseases may develop in type 2 diabetes patients. Plasma homocysteine concentration may be increased if folate status were inadequate. Protective effects against oxidative stress may be diminished if the status of anti-oxidative nutrient as vitamin C was poor. It is, therefore, important to maintain adequate status of folate and vitamin C in type 2 diabetes patients. Thus, this study was performed to determine the effects of supplementation of folate and/or ascorbate on blood glycated hemoglobin ($HbA_{1c}$) level, serum concentrations of homocysteine and cholesterol, plasma oxidized low density-lipoprotein (LDL), concentration and plasma glutathione peroxidase (GSH-Px) activity in the patients with type 2 diabetes. A total of 92 type 2 diabetes patients participated voluntarily with written consents. They were divided into one of the four experimental groups; Control (C), Folate-supplemented (F), Ascorbate-supplemented (A), and Folate plus ascorbate-supplemented (FA). The subjects in C were taken placebo, those in F were supplemented 1 mg of folate, those in A received 1,000 mg of ascorbate, and those in FA were given 1 mg of folate plus 1,000 mg of ascorbate daily for 4 weeks. Supplementation of folate or ascorbate resulted to increase serum folate level or plasma ascorbate concentration apparently, respectively. Folate supplementation not ascorbate seemed to decrease plasma concentrations of homocysteine and oxidized LDL and reduce plasma GSH-Px activity. There might not be synergic effect of the supplementation of folate plus ascorbate. The results indicate that oxidative stress in the patients with type 2 diabetes may lower mainly by folate supplementation.
