Ginseng for Reducing the Blood Pressure in Patients with Hypertension: A Systematic Review and Meta-Analysis

  • Ginseng is one of the most-widely used herbal remedies. This systematic review evaluates the current evidence for its use in the reducing blood pressure (BP) in patients with hypertension. Systematic searches of 12 electronic databases were conducted without language restrictions. All randomized clinical trials (RCTs) of ginseng as a treatment for hypertension were candidates for inclusion. Methodological quality was assessed using the Cochrane risk of bias. Five RCTs met the inclusion criteria. The risk of bias was low in most of the trials. Four of the included RCTs compared the effectiveness of ginseng to placebo. The meta-analysis of these data failed to show a statistically significant acute effect on systolic BP (SBP) or diastolic BP (DBP). However, subgroup analyses showed beneficial effects of Korean red ginseng (KRG) on both SBP (n=54, mean difference [MD], -6.52; 95% confidence interval [CI], -9.99 to -3.04; p=0.0002) and DBP (n=54, MD, -5.21; 95% CI, -7.90 to -2.51; p=0.0001). Two RCTs tested the long-term effects of ginseng for BP for 24hours. One of these trials failed to show any benefits of KRG compared to no treatment, and the other failed to show superior effects of North American ginseng compared to placebo. Adverse events with ginseng were none in one trial or not assessed. Collectively, these RCTs provide limited evidence for the acute effectiveness of KRG in the treatment of high BP. The total number of RCTs included in the analysis and the total sample size were insufficient to draw definitive conclusions. More rigorous studies are warranted.

A Meta Analysis of Group Music Therapy with Adolescents (청소년 대상 그룹 음악치료 효과에 대한 메타분석)

  • The purpose of this study was to analyze data regarding the effects of group music therapy with adolescents. 27 master's, doctorial theses and published articles between 2000 and 2012 were selected for meta analysis. Subjects, independent variables, dependent variables, and the number of session were used to compare the effectiveness of group music therapy program. 49 effect sizes using means and standard deviation from 27 studies were calculated. The results were as follows. The overall mean effect size was -0.14, which is small size effect, according to Cohen's ES interpretation criteria. The mean ES was influenced by the subjects(adolescents/maladjustment students/addictive students/students in facility), the ES of adolescents was the biggest. However there were no significant difference among subjects. During the independent variables(active music therapy/receptive music therapy/combined music therapy), combined music therapy showed the biggest efficacy. When results were sorted by the dependent variables (social/emotional/behavior/cognitive category), the ES of behavioral category was the biggest. However, there was no significant difference among them. The ES of 11~15 session was the most effective.

  • Many researches have achieved to reuse existent experience at each step of software life cycle. It is achieved various study about Design Pattern serving that abstract level solves design problem at high level. But, design step is not supplying design information that is abstracted so that new designers may reuse standardizing defined good design information. Therefore, in this paper, to do so that can reuse existent design information because composes Design Pattern can solve problem at design step, and extend design information that also is standardized XMI to base meta model of Design Pattern. And I did so that can compose Design Pattern utilizing MetaModel of Design Pattern standardized by XMI. Therefore, software designer can reuse composing existent design information that is could standardize problem at design because uses MetaData of standardized Design Pattern, and designs by Design Pattern base.

  • A meta-analysis is a statistical integration method that delivers an opportunity to overview the entire result of integrating and analyzing many quantitative research results. This study will find meaningful independent variables for criterion variables that affect influencing on purchase intention in social network service, on the basis of the results of a meta-analysis. We reviewed a total of 29 studies related on purchase intention in social network service published in Korean journals between 2000 and 2015, where a cause and effect relationship is established between variables that are specified in the conceptual model of this study. Thus, we present the theoretical and practical implications of these results and discuss the differences between these results through a comparative analysis with previous studies.

The Anti-hepatotoxic Effect of Ginseng in Rats: Meta-analysis

  • The aim of this meta-analysis was to systematically investigate the anti-hepatotoxic effect of ginseng in rats induced toxicity which damage to liver. Primary researches were gained on the ScienceDirect database, the DBpia, and the KISS, and the data about the effect factors in plasma and in enzyme were listed as many as possible. The effect factors were alanine transaminase (ALT), aspartate transaminase (AST), liver aminopyrine N-demethylase (AD), liver aniline hydroxylase (AH), liver 3,4-Methylenedioxyamphetamine (liver MDA), cytochrome P450 (P450), serum alkaline phosphatase (ALP), serum lactate dehydrogenase (LDH), cytochrome b5 (Cyto b5), glutathione reductase (GR), Liver glutathione S-transferase (GST), liver glutamyltransferase (GT), Liver (${\gamma}-GCS$), serum liver 3,4-Methylenedioxyamphetamine (serum MDA), serum sorbitol dehydrogenase (SDH), serum total protein (TP), serum ${\gamma}-glutamyltransferase$ (${\gamma}-GT$). To investigate the effect of ginseng, the mean difference (MD) between the group of rats induced by toxicity (RH) and the group of rats induced by toxicity with ginseng (RHG) were combined, and the significance of MDs were tested. The combined MDs were checked the biases caused by heterogeneity among studies and the publication biases, and adjusted by using random effect model and trim and fill method, respectively. The effect about ALT, AST, ALP, LDH, SDH, TP and ${\gamma}-GT$ in plasma factors were significant, and about AD, liver MDA, P450, Cyto b5, GR, GST, GT and ${\gamma}-GCS$ in enzyme factors were significant. The treatment with ginseng supplementation was significantly effected on plasma and enzyme factors of damaged-rats.

Effectiveness of Non-Pharmacologic Interventions in Chemotherapy Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis (항암화학요법 관련 말초신경병증에 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study was conducted to evaluate the effectiveness of non-pharmacologic interventions in chemotherapy-induced peripheral neuropathy (CIPN). Methods: PubMed, Cochrane Library CENTRAL, EMBASE, CINAHL, and several Korean databases (Until August 2017) were searched. The main search strategy combined terms for peripheral neuropathy and presence of neoplasms. The risk of bias was assessed using the Cochrane's Risk of Bias tool for randomized studies and the Risk of Bias Assessment tool for non-randomized studies. To estimate the effect size, a meta-analysis of the studies was performed using the Rev Man 5.3 program of the Cochrane Library random-effects models were used in the analyses. Results: Twenty-two studies with a total of 954 participants met the inclusion criteria. Of the 22 studies, 12 were used to estimate the effect size of the non-pharmacologic interventions. The non-pharmacologic interventions used in patients with CIPN were exercise, acupuncture, massage, and foot bath. The acupuncture significantly reduced CIPN symptoms and signs (d=-0.71) and CIPN pain (d=-0.73) (p<.001). Massage and foot bath were also effective in reducing CIPN symptoms (d=-0.68; 95% CI=-1.05, -0.30; p<.001; $I^2=19%$).Exercises were effective in improving muscle strength and endurance(d=-0.55) and quality of life (d=-2.96), but they were not significantly effective in improving CIPN. Conclusion: Although these results provide little evidence of the effectiveness of acupuncture, massage, and foot bath in the treatment of CIPN, they suggest that these interventions can reduce CIPN symptoms in patients with cancer. However, the findings of this study should be interpreted with caution as there is a relative lack of data in this field, and additional well-designed studies are needed. A Research Trends for Domestic Public Design by Meta Analysis (국내 공공디자인 연구동향에 대한 메타분석)

  • There are various form of public elements in modern urban space. These public elements mingle with sociocultural elements which consist of each city, determining urban environment's image. Especially, since the social value of design becomes important, the social role of public design has been emphasized as an effective value for public interest and environment. It is just about 10 years since design mind in the public sector has been made domestically. Even though domestic public design has been studied not so long, it has developed with radical change of social culture and science technology. This study targets public environment in modern dense urban space. Also, it grasps the trend of public design studies through analysing and documentary survey of the existing studies, and based on that result, aims at presenting the direction that domestic public design studies should progress. This study selected 253 theses(1989.6~2015.11) by collecting primary, secondary data about public design studies, and implemented overall meta-analysing about the trend of domestic public design studies. As a result, public design has been studied intensively in the field of design about environment, architecture, etc. The trend analyzing of study purpose and keyword has a tendency which is weighted towards exploratory study focused on suggestion and development about object and realm of public design. The trend analyzing of study area tends to study overall part of public space or public facility rather than specific area and object. The most parts of studies analyze domestic area, especially Seoul metropolitan area. Also, the study method weighted in favor of some qualitative analysis has been utilized.

Effectiveness of Soyo-san (Xiaoyao-san) and its Modifications on Chronic Fatigue Syndrome: a Systematic Review and Meta-analysis (만성피로증후군에 대한 소요산(逍遙散)과 그 변방(變方)의 효과 : 메타분석을 통한 체계적인 문헌고찰)

  • The purpose of this review is to confirm whether Soyo-san (Xiaoyao-san) and its modifications is effective on alleviating clinical symptoms in chronic fatigue syndrome (CFS) patients. We collected clinical trials (randomized controlled trial, quasi-randomized controlled trial, controlled clinical trial) to investigate the effects of Soyo-san and its modifications on general symptoms, fatigue, depression and anxiety in CFS patients. The databases used for data retrieval were Pubmed, Central Cochrane, Embase, CNKI, CQVIP, Wanfang, CiNii, OASIS, RISS, and Koreamed. We performed selection/exclusion process from the found studies to conform with prespecified criteria, and assessed the final included trials according to the Cochrane risk of bias tool. The included studies were classified based on the interventions in experimental and control group. Eight randomized controlled trials and one controlled clinical trial (total 921 participants) were eligible and their results were synthesized in the meta analysis. The synthesis showed a considerable effect of Soyo-san and its modifications on improvement of general symptoms (relative risk 0.27 [95% CI 0.19 to 0.39], Z=7.03, P<0.00001; I2=0%) and fatigue severity (SMD -1.20 [95% CI -1.46 to -0.93], Z=8.78, P<0.00001; I2=52%) in CFS patients, while Effect on depression and anxiety were inconclusive. We found that Soyo-san and its modifications were effective for improvement of general symptoms and fatigue severity in CFS post-treatment.

Constructing a Metadata Database to Enhance Internet Retrieval of Educational Materials

  • This paper reports the GEM (Gateway to Educational Materials) project whose goal is to develop an operational framework to provide the K-12 teachers in the world with 'one-stop/any-stop' access to thousands of lesson plans, curriculum units and other Internet-based educational resources. To the IS-element Dublin Core base package, the GEM project added an 8-element, domain-specific GEM package. The GEM project employed the conceptual data modeling approach to designing the GEM database, used the Sybase relational database management system (RDBMS) to construct the backend database for storing the metadata of educational resources, and also employed the active server page (ASP) technology to provide Web interfaces to that database. The consortium members catalog lesson plans and other Internet-based educational resources using a cataloging module program that produces HTML meta tags. A harvest program collects these meta tags across the Internet and outputs an ASCII file that conforms to the standard agreed by the consortium members. A parser program processes this file to enter meta tags automatically into appropriate relational tables in the Sybase database. The conceptual/logical schemas of Dublin Core and GEM profile are presented. The advantages of conceptual modeling approach to manage metadata are discussed. A prototype system that provides access to the GEM metadata is available at http://lis.skku.ac.kr/gem/.

A Systematic Review and Meta-analysis on the Effect of Delirium Prevention Intervention in Korean Intensive Care Units (국내 중환자실 섬망 예방 중재에 관한 체계적 고찰 및 메타분석)

  • Purpose : This study aimed to systematically review the preventive interventions for delirium in Korean intensive care unit (ICU) patients and evaluate their efficacy. Methods : For this systematic review and meta-analysis, we searched the literature and selected studies from data sources that included the RISS, KISS, National Central Library, National Assembly Library, DBpia, Science on, MEDLINE, and Cochrane Library. We used Cochrane's revised tool for risk of bias in randomized trials and non-randomized studies of intervention tools to assess the quality of the selected studies. The effect size of the intervention was calculated as odds ratio (OR) and standardized mean difference (SMD). Results : Preventive interventions reported in 23 studies with a total of 4,799 ICU patients were effective in reducing the occurrence of delirium (OR=0.64, 95% CI : 0.49~0.91, p=.011), but not the duration (SMD=-0.22, 95% CI : -0.51~0.08, p=.148). As a result of a subgroup analysis, non-pharmacological interventions were effective in reducing the occurrence of delirium (OR=0.66, 95% CI : 0.47~0.94, p=.020), while pharmacological interventions had no effect (OR=0.68, 95% CI : 0.33~1.40, p=.295). Among the non-pharmacological interventions, multi-component intervention had the largest effect size (OR=0.38, 95% CI : 0.26~0.55, p<.001). Conclusion : Non-pharmacological interventions were effective in reducing the occurrence of delirium. We recommend the development and application of multi-component interventions to prevent delirium in the Korean ICU patients.
