Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting. Purpose: The results of recent published studies focusing on IL-8 polymorphism in colorectal cancer susceptibility have often been inconsistent. We therefore carried out a meta-analysis based on independent studies to assess the association. Methods: Nine case-control studies with 7,003 individuals (3,019 cases and 3,984 controls) were included in this meta-analysis through searching the databases of PubMed, Excerpta Medica Database (EMBASE), and Chinese Biomedical Literature Database (CBM; Chinese) (up to Aug 1st, 2012). The odds ratio (OR) and 95% confidence interval (95%CI) were used to assess the strength of the association. Meta-analysis was conducted in a fixed/random effect model. Results: No obvious associations were found for all genetic models when all studies were pooled into the meta-analysis (for A vs. T: OR = 1.084, 95% CI = 0.971-1.209, P = 0.019; for TA vs. TT: OR = 1.18, 95% CI = 0.943-1.475, P = 0.001; for AA vs. TT: OR = 1.155, 95% CI = 0.916-1.456, P = 0.014; for AA+TA vs. TT: OR = 1.170, 95% CI =0.953-1.437, P = 0.001; for AA vs. TT+TA: OR = 1.044, 95% CI = 0.886-1.230, P = 0.097). In the subgroup analyses by ethnicity (Caucasian) and source of controls (population based), also no significant associations were found for all genetic models. Conclusions: Result suggests that the IL-8-251T>A polymorphism is not associated with colorectal cancer risk. Because of the limitations of this meta-analysis, this finding demands further investigation.
Recently many organizations have attempted to build hypermedia systems to expand their working areas to internet-based virtual work places. It is thus important to manage corporate hypermedia documents effectively. Metadata play a critical role for managing these documents. This paper identifies metadata roles and components to build its schema. Furthermore a meta-information system HyDoMiS(Hyperdocument Meta-information System) is proposed by the use of this metadata schema. HyDoMiS performs three functions; metadata management search and reporting The metadata management function is concerned with workflow document and database. The system is more likely to help implement and maintsain hypermedia information system effectively.
Since XML (eXtensible Markup Language) was highlighted as an information interchange format, there is an increasing demand for incorporating XML with databases. Most of the approaches are focused on RDB (Relational Databases) because of legacy systems. But these approaches depend on the database system. Countless researches are being focused on DTD (Document Type Definition). However XML Schema is more comprehensive and efficient in many perspectives. We propose a meta-model for XML Schema that is independent of the database. There are three processes to build our meta-model: DOM (Document Object Model) tree analysis, object modeling and storing object into a fixed DB schema using model mapping approach. We propose four mapping rules for object modeling, which conform to the ODMG (Object Data Management Group) 3.0 standard. We expect that the model will be especially useful in building XML-based e-business applications.
Background: Antibiotic resistance is associated with longer hospitalizations, higher treatment costs, and increased morbidity and mortality rates. Purpose: This study aimed to determine the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) in Iranian children. Methods: International databases, including Web of Science, PubMed, Embase, and Scopus, and Iranian databases, including Scientific Information Database (www.sid.ir), Magiran, and Iranian Database for Medical Literature (idml.research.ac.ir), were systematically searched for articles published between January 2000 and August 2019. Sources of heterogeneity were determined using subgroup analysis and meta-regression. Results: Overall, 343 studies were identified; of them, 20 were included in the meta-analysis to estimate the pooled prevalence. The pooled prevalence of MRSA was 42% (95% confidence interval [CI], 29-55) among culture-positive cases of S. aureus, 51% (95% CI, 39-62) in hospitalized children, and 14% (95% CI, 0.05-27) in healthy children. Conclusion: The overall pooled prevalence of MRSA in children was 42%. Appropriate infection control measures and effective antibiotic therapy are needed.
하이퍼미디어 응용에 나타나는 다양한 형태의 멀티미디어 데이타와 이들 사이의 동기화 등을 지원하기 위해 하이퍼미디어 전자 문서의 기술 표준인 HyTime(Hypermedia/Time-based Structuring Language)이 사용되고 있다. 그런데 이러한 HyTime 문서를 효과적으로 관리하기 위해서는 HyTime문서의 논리적 구조 정보를 체계적으로 데이타베이스에 저장하여 여러 사용자가 공유할 수 있도록 관리할 필요가 있다. 본 논문에서는 하이퍼미디어 문서의 논리적인 구조를 정의하는 HyTime DTD(Document Type Definition)를 객체 지향 데이타베이스에서 체계적으로 관리하기 위해 메타 데이타베이스 스키마를 설계하고 이를 관리하는 방법을 기술한다.
Background: An excessive pronated foot is defined as a flattening or complete loss of the medial longitudinal arch. Excessive foot pronation is considered to have high risk factors of overuse injuries in the lower limb. Various treatments have been investigated in attempts to control excessive pronation. Objects: This meta-analysis identifies the effects of an anti-pronation taping technique using different materials. Methods: The electronic databases used include MEDLINE, the Physiotherapy Evidence Database (PEDro), Science Direct, the Korean Studies Information Service System (KISS), the Research Information Sharing Service (RISS), the Korea National Library, and the Korean Medical Database (studies published up to July 31, 2019). The database search used the following keywords: "foot drop" OR "foot arch" OR "foot pronation" OR "flat foot (pes planus)" AND "taping" OR "support." Eight eligible studies were analyzed to determine the effectiveness of anti-pronation taping in study and control groups. Results: The overall random effect size (Hedges'g) of the anti-pronation taping technique was 0.147 (95% confidence interval [CI]: -.214 to .509). When the effect (Hedges' g) was compared by the type of tape material, rigid tape (RT; Lowdye taping) was .213 (95% CI: -.278 to .704) and kinesiotape (KT; arch support taping) was -.014 (95% CI: -.270 to .242). Based on this meta-analysis, it was not possible to identify the extent to which anti-pronation taping was effective in preventing navicular drop, improving balance, or changing foot pressure. Only three of the eight eligible studies applied KT on excessive pronated feet, and the outcome measure areas were different to those of the RT studies. The KT studies used EMG data, overall foot posture index (FPI) scores, and rear foot FPI scores. In contrast, the RT studies measured navicular heights, various foot angles, and foot pressure. Conclusion: This review could not find any conclusive evidence about the effectiveness of any taping method for patients with pronated feet. Future studies are needed to develop the anti-pronation taping technique based on the clinical scientific evidence.
Objective : The purpose of this study is to work on the problems and on the direction of the study regarding the evidence-based study of Acupuncture. Methods : We reviewed the PubMed electronic database. Eight articles of systematic review without meta-analysis and six articles without meta-analysis were selected. Results : In the study of eight articles of systematic review without meta-analysis, there were two articles on effectiveness of acupuncture and one on non-effectiveness of acupuncture. Four articles were about the changes according to the quality of randomized controlled trials. Also, one article was about the side-effect of acupuncture. In the study of six articles of systematic review with meta-analysis, there were two articles on Odds Ratio, two on Relative Risk, one on number-needed-to-treat, and two on 18 predefined methodological criteria. Conclusion : As the quality of RCTs was high, positive response decreased and as the quality of RCTs was low, positive response increased. For the study of evidence-based acupuncture, it is necessary that we conduct research on well-planned placebo acupuncture, large sample size, statistical method, method of searching and assessing literature and method of meta-analysis.
본 논문에서는 SGML 문서의 효율적인 검색을 위한 방법론을 제안한다. 이를 위하여 메타 데이터, 색인 범위, 엘리먼트 이롬의 재정의, 그리고 데이터베이스 생성에 관한 정보를 기술할 수 있는 언어로 IDDL을 정의한다. 또한 메타 데이터와 구조에 대한 복합 질의를 효과적으로 표현할 수 있는 질의 언어인 IDQL을 제안한다. 본 논문에서는 제안된 방법론의 유효성을 입증하기 위하여 IDDL과 IDQL에 기반한 검색시스템을 개발하였으며 이를 상이한 구조의 대용량의 문서 집합에 실험하였다. 그 결과, 제안된 방법론은 데이터베이스를 동적으로 구성하며 사용자에게 편리한 검색 환경을 제공하였다.
그리드 컴퓨팅 환경에서 대용량 데이터의 처리와 가용성 향상, 데이터 통합을 위해 그리드 데이터베이스 관리 시스템이 사용된다. 그리드 데이터베이스 관리 시스템은 효율저인 질의처리를 위해 여러 노드로 질의를 분산하여 처리한다. 하지만 질의 처리가 임의의 노드에 집중되어 처리 성능이 감소되고, 작업 부하의 불균형이 발생한다. 본 논문에서는 그리드 데이터베이스에서 질의 처리를 위한 캐쉬 관리 기반의 부하분산 기법을 제안한다. 제안 기법에서는 여러 노드에 있는 캐쉬들을 관리하기 위해 캐쉬 관리자를 사용하며, 캐쉬 관리자는 노드를 지역별 그룹으로 연결하고 자신의 그룹 안에 있는 노드의 캐싱된 메타 정보를 관리한다. 노드는 캐쉬 관리자를 통해 질의를 전달 할 최적의 메타 정보를 캐싱한다. 노드에서 캐싱된 메타 정보를 통해 질의를 전달하여 노드의 부하를 분산한다. 제안 기법은 캐쉬 기반으로 부하가 적은 노드에서 질의를 처리하여 노드들의 부하를 분산하여 질의 처리시에 향상된 성능을 보인다.
