Purpose: This study investigated the factors influencing mental health among university students based on ecological perspectives. Method: The subjects for this study were 416 students in two colleges in K province. The instruments used for measurement were: Symptom Checklist (Derogatis, 1977), Ego Resilience Scale (Block & Block, 1980), Perceived Stress Inventory (Cohen, Kamarck, & Mermelstein, 1983), and Social Support Scale (Park, 2002). The data was analyzed using the IBM SPSS program. Result: Ego resilience and perceived stress as intrapersonal factors and friends' support as interpersonal factor were the significant predictors of mental health. It was showed that these factors explained 47% of mental health. Conclusion: This study suggests that the factors influencing mental health are both intrapersonal and interpersonal factors. Thus, the findings of this study provide the basic data to develop a program for mental health of university students based on ecological perspectives.
이 연구는 농촌지역 정신보건의료서비스의 필요도와 공급수준을 파악하고 일차정신보건시설로서 보건소가 운영하는 정신보건사업을 평가하고자 하였다. 필요도는 유병률과 정신보건서비스 이용률로 한정하여 2001년도에 보건복지부가 주관하여 실시한 정신질환 실태 역학조사 결과를 이용하여 도농간 차이를 비교하였다. 공급수준은 입원 및 요양 병상수와 일차정신보건의료시설의 기초자치 단체별 설치율로 한정하여 전자는 2002년도에 보건복지부 산하 지역사회 기술지원단이 작성한 보고서 결과내용 중 일부를 발췌하였고 후자는 보건복지부의 2003년도 정신보건사업 안내서와 정신보건의료기관 총람의 내용을 토대로 재구성하였다. 농촌지역의 보건소 정신보건사업을 평가하기 위하여 2002년도에 각 기초자치단체가 중앙에 제출한 제3기 지역보건 의료계획서들 중 농촌형지역과 도농복합형지역으로 구분하여 각각 중앙 및 광역자치단체 정부의 지원 여부에 따라 지원한 경우 1-2개, 지원하지 않는 경우 광역자치단체별로 각각 2개소를 무작위 추출하여 정신보건사업내용부문을 사업대상 및 등록, 구조, 과정으로 나누어 정신보건사업안내서에 제시한 기준을 가지고 평가하였고 담당자들이 작성한 정신보건사업의 문제점들을 분석하였다. 이 연구는 2003년 4월 1일부터 5월 10일까지 이루어졌고 그 결과는 다음과 같다. 첫째, 니코틴 장애를 제외한 평생유병률과 일년유병률은 도시지역이 24.9%, 13.2%, 농촌지역이 28.2%, 17.7%이었으며 정신분열증, 주요우울증, 불안장애, 알코올 사용장애 등 주요 정신질환 역시 도시보다 농촌지역이 더 높은 것으로 나타났다. 또한 지난 일년간 정신보건의료서비스 이용률은 전체적으로 8.9%에 불과하였다. 둘째, 입원 및 요양 병상수는 2001년 현재 인구 천명당 0.97병상이며 요양시설을 포함할 경우 1.27병상으로 세계 보건기구가 권장하는 인구 천명당 1.0병상, 그리고 병상 감소정책을 추진하고 있는 유럽이 0.93병상임을 고려할 때 양적으로 부족하지는 않다. 그러나 광역자치단체간 편차가 큰 것으로 나타났는데 서울, 인천, 울산 등 대도시지역이 충북, 충남, 경남, 전남지역의 1/6내외수준이었다. 셋째, 일차정신보건의료시설의 기초자치단체 충원율을 살펴보면, 정신과 의원은 광역시 및 특별시형은 100%인 반면 농촌형지역 89곳 중 15곳(16.9%)만이 설치되어 있었으며 사회복귀시설은 전반적으로 낮았으며 농촌지역의 충원율이 7.9%로 가장 낮은 것으로 나타났다. 보건소 정신보건사업에 대한 중앙 및 광역자치단체의 지원율이 광역시 및 특별시형이 42%인 반면 농촌형지역은 22.5%인 것으로 나타났다. 다섯째, 농촌형지역과 도농복합형지역 모두에서 지원을 받지 않은곳이 자체예산을 추가로 확충하지 않아 사업인력 및 예산이 매우 취약하고 중앙정부가 제시하는 기본사업에 대한 충실도가 매우 낮은 것으로 나타났다. 이상의 결과를 종합해볼 때 농촌지역이 도시지역에 못지 않게 정신보건의료서비스에 대한 필요도가 높으나 서비스 이용률은 매우 낮았으며 도시지역에 비해 농촌지역의 일차정신보건의료시설들이 매우 부족한 것으로 나타났다. 또한 일차정신보건의료시설로서 보건소가 도시지역 보다 농촌지역에서 더 중요한 역할을 수행해야 하나 중앙의 지원율은 오히려 농촌지역 보다 도시지역이 더 높았고 지원을 받지 않는 보건소의 정신보건사업프로그램은 매우 형식적으로 진행되고 있었다. 앞으로 중앙정부는 일반의료서비스에 있어서 농어촌의료서비스 개선정책을 추진하듯이 정신보건정책에 있어서도 이러한 고려가 필요하다.
When parents as primary care takers to the mentally disabled adult are no longer taking care of their care-needed offsprings because of their own death or illness, instead themselves. who take care of their offsprings with the mental disability? Therefore, 'permanency planning' is very important for reduction of parents' care burden and social integration of mentally disabled adults. Accordingly, this study aims to find out factors which are related to permanency planning for adults with the mentally disability For the purpose of the study, 192 parents of the adult with the mental illness and mental retardation were conducted a survey regarding type of permanency planning, and its related factors including social functioning level of the mentally disabled, care burden, parents' self-perception of being aged, help from offspring without mental disability, social support, and financial ability. Furthermore, this study examined correlation between these factors and residential planning. Results obtained by the study were as follows: 1) 51% of the parents are having a plan for institution and most parents want other family member to take care for financial planning for their mentally disabled offsprings. 2) As a result of multiple regression for finding out factors which affect parents' permanency planning, social functioning level of the mentally disabled, parents' self-perception of being aged, help from offspring without mental disability, social support, and financial ability were statistically significant influenced factors, which has 23.3% of explanatory power. 3) As a result of step-wise multiple regression, financial ability, parents' self-perception of being aged, and help from offspring without mental disability were the most powerful influenced factors for permanency planning. 4) In case of having a plan for residential types-which are institution and community living-, parents who have a plan for the mentally disabled offsprings' future residence as community living than institution have the offsprings with more social functioning and also have more help from offspring without mental disability. Therefore, this study concluded that welfare policy for mental health and the handicapped which secure various types of community living facilities and income security is strongly needed. At the same time, mental health profession is needed to have more active interest and intervention for permanency planning for their adult clients and parents.

Purpose: The purpose of this study was to examine the knowledge, caregiving performance, stress levels, and mental health of family caregivers of terminal cancer patients with delirium, insofar as these characteristics are relevant for delirium. Methods: Between May 1, 2019, and June 1, 2020, 96 family caregivers of terminal cancer patients with delirium completed a structured survey, the results of which were analyzed. Results: The average correct answer rate for delirium-related knowledge was 53.2% across all subcategories, which included knowledge of causes (41.5%), symptoms (65.4%), and caregiving (51.7%). The average score for family caregivers' performance of caregiving for delirium was 2.60±0.5, with subcategories including caregiving for patients without delirium (2.16±0.95), caregiving for patients with delirium (2.84±1.01), and stress related to caregiving for delirium (39.88±16.55), as well as categories such as patient-related caregiving (44.32±28.98), duty-related caregiving (44.21±30.15), and interpersonal relationship-related caregiving (22.35±25.03). For mental health, the average score among family caregivers was 1.96±0.70, with the highest score being for the category of additional items (2.28±0.84). Family caregivers of patients with hyperactive delirium as the delirium subtype had higher scores for caregiving performance than caregivers of patients with mixed delirium. Conclusion: Scores for the delirium-related knowledge and caregiving performance of family caregivers were low, while their caregiving stress levels were high due to their lack of knowledge and experience. This indicates the importance of delirium-related education for family members of patients with delirium and the necessity of developing nursing intervention programs to help manage stress and promote mental health among family caregivers.
Objectives: The purpose of this study was to compare the physical and mental health status of disaster victims according to disaster types, such as a typhoon disaster and an oil spill disaster, and to suggest adequate health care services for them. Methods: A total of 484 people who suffered disasters were selected for this study, and data were collected from July to August, 2008. The data-set for this study included 286 victims of typhoon disasters in Jeju and Jeollanamdo district in South Korea, and 198 victims of the oil spill disaster in Taean. Physical health status was measured using revised Patient Health Questionnaire and mental health status was measured using the Korean version of 'Post-traumatic Diagnostic Scale'. Results: According to the comparative analyses of typhoon disaster victims and oil spill disaster victims, poorer physical health outcomes were shown among the oil spill disaster victims when compared to the typhoon disaster victims. Also, the oil spill disaster victims showed symptoms of depression, anxiety, and post-traumatic stress disorder, at rates higher than those found among the typhoon disaster victims. Conclusions: These findings suggest that there is a need to provide adequate physical and mental health-related care services for oil spill disaster victims. The seriousness of oil spill disaster should be realized and reconsidered in developing recovery strategies and disaster preparedness for physical and mental health services.
이 연구는 어린이집 보육교사의 감정부조화, 직무소진과 정신건강 간의 연관관계를 규명하는 것을 목적으로 467명의 보육교사를 대상으로 조사하였으며, 5점 척도로 구성한 측정도구를 활용하였다. 수집된 자료는 통계패키지 IBM SPSS 23과 AMOS 21을 사용하여 유의수준 5% 수준에서 검정하였다. 주요 결과는 다음과 같다. 첫째, 어린이집에 종사하는 보육교사의 감정부조화가 정신건강 하위요인 중 직무스트레스(학부모관계, 업무과부하)에 유의한 영향을 미치는 것으로 나타났다. 둘째, 감정부조화는 직무소진에 모두 유의한 영향이 있는 것으로 나타났다. 셋째, 직무소진은 정신건강 하위요인인 직무스트레스, 심리적 안녕감에 부분적으로 유의한 영향을 미치고 있었다. 넷째, 직무소진은 감정부조화와 정신건강에 대해 부분적 매개효과가 있는 것으로 나타났다. 이러한 연구결과는 어린이집에서 종사하는 보육교사의 정신건강 증진의 중요성을 제시하고 있으며, 정신건강 증진을 위한 제도적 지원을 모색할 필요성을 시사한다.
Purpose: The purpose of this study is to analyze the concept of recovery in relation to those living with mental illness and provide a better understanding to the definition, perspectives, and paradigm of recovery in phenomenon, as a conceptual knowledge. Methods: A literature review was conducted to define the concept of recovery from a mental illness by using key words, "recovery", "mental health and illness", "concept analysis" and "recovery-oriented nursing", and searching the Cumulative Index to Nursing and Allied Health Literature, PubMed, Cochrane library and RISS4U database. Concept analysis of recovery was done, by using the Walker and Avant's framework of concept analysis. Results: Attributes of recovery for those living with mental illness included regain, life reconstruction, hope, adjustment, and health. Antecedents of recovery from mental illness included instilling hope, recovery vision, belief, peer support, recovery-oriented services, empowerment, personal accountability, education, human rights and culture. The consequences, as meanings of recovery included self-esteem, hopeful life, positive adjustment, and healthy life. Conclusion: Concept of recovery is important for a nurse to understand when caring for a person living with mental illness. This concept of recovery from mental illness may apply to future studies to develop a recovery-oriented nursing intervention.
Objectives: The purpose of this study was to analyze the medical cost for patients with vertigo and to examine associations between chronic vertigo and mental disorder using 2014 Health Insurance Review & Assessment Service-National Patients Sample (HIRA-NPS). Methods: We analyzed sociodemographic characteristics, medical cost and medical care use pattern for vertigo patients. We used hierarchical multiple logistic regression analysis to examine odds ratio between chronic vertigo and mental disorder. Results: A total of 46,502 people and 118,504 claims data were identified for vertigo cases. Characteristics of vertigo patients have significant differences on proportion of female patients (68.36%), patients' average age (54.98) and proportion of medical assistance (5.76%) compared with non-vertigo patients. Results revealed that Korean medicine are one of frequent methods among total treatments for vertigo patients. Total days of medical care and total costs are 2.78 days and 111,362 won, respectively, and days for outpatients in Korean medical care (mean: 2.26 days) are more than those (mean: 5.05 days) in Western medical care. There is significant difference relative to sex between acute vertigo and chronic vertigo. The odds ratio between chronic vertigo and mental disorder is estimated as 1.34, that means risk of becoming chronic is 34% higher for vertigo patients with mental disorder. Conclusions: This study assessed socio-demographic characteristics, medical care use and expenses related to vertigo, and estimated associations between chronic vertigo and mental disorder. Findings provide a basis for economic evaluation studies on vertigo patients and development of clinical practice guidelines for vertigo patients with mental disorder.
The purpose of this study is to evaluate the causal relationship among health care criteria in Korean National Mental Hospitals, using Malcolm Baldrige National Quality Award(MBNQA). The survey instrument consists of 92 Questions from the seven the MBNQA health care criteria. Structural Equation Modeling (SEM) is used to analyze the empirical data and estimates the path coefficients among the seven categories. The result of this study indicates that Leadership drives Foundation and Direction, which influence on Systems that creates Results. Conclusively, among 18 hypotheses, 15 are statistically significant.
본 연구는 대학생을 대상으로 정신건강 자가관리를 위한 모바일 앱을 개발하기 위한 연구로, 대상자의 요구를 반영하여 개발하기 위해 사용자 중심 설계를 이용하였다. 사용자 중심설계란 대상자에게 적합하게 적용하기 위해 대상자 요구도 사정-분석-설계-개발-평가-수정 및 보완-적용하는 연구설계이다. 대학생들의 정신건강 자가관리를 위해 이들의 주요 정신건강 문제인 음주·수면·우울·스트레스 등 4가지 영역으로 구성하였다. 각 영역별 현재 상태에 대한 자가테스트 콘텐츠, 점검 결과 분석 및 알림, 현재 상태에 대한 관리 방안 제시 등이 가능하도록 설계하였다. 이를 기반으로 안드로이드 기반 정신건강 자가관리 앱을 개발하였다. 대상자가 자신의 정신건강 상태자료를 입력함으로써 각각의 결과에 대한 정상 혹은 위험 정도를 설명하고 이에 따라 대상자는 자신이 수행할 수 있는 적절한 중재법을 선택할 수 있다. 또한 4가지 영역에 대한 현재 상태를 일자별로 나타낼 수 있는 정신건강자가관리 캘린더와 현재상태를 애니메이션과 상태바를 통해 통합적으로 표현할 수 있도록 개발하였다. 본 연구는 대상자 중심으로 다가갈 수 있는 정신건강 중재로, 지속적이고 더 향상된 프로그램으로 개선할 수 있는 정신건강 자가관리 앱 개발의 기초적인 연구로서 시도하였다.
