  • Manifestations in tuberous sclerosis such as seizure, mental retardation, end-stage renal disease (ESRD), and heart problems present a number of challenges to the dentist and anesthesiologist. Lack of cooperation in addition to the severe medical condition makes dental treatment more difficult. General anesthesia is often required for mentally and physically handicapped patients undergoing extensive dental treatment. We experienced a case of dental treatment under general anesthesia in a 36-year-old male patient with tuberous sclerosis associated with ESRD and mental retardation. He was treated on an outpatient basis followed by hemodialysis without any complications.

A 95% accurate EEG-connectome Processor for a Mental Health Monitoring System

  • An electroencephalogram (EEG)-connectome processor to monitor and diagnose mental health is proposed. From 19-channel EEG signals, the proposed processor determines whether the mental state is healthy or unhealthy by extracting significant features from EEG signals and classifying them. Connectome approach is adopted for the best diagnosis accuracy, and synchronization likelihood (SL) is chosen as the connectome feature. Before computing SL, reconstruction optimizer (ReOpt) block compensates some parameters, resulting in improved accuracy. During SL calculation, a sparse matrix inscription (SMI) scheme is proposed to reduce the memory size to 1/24. From the calculated SL information, a small world feature extractor (SWFE) reduces the memory size to 1/29. Finally, using SLs or small word features, radial basis function (RBF) kernel-based support vector machine (SVM) diagnoses user's mental health condition. For RBF kernels, look-up-tables (LUTs) are used to replace the floating-point operations, decreasing the required operation by 54%. Consequently, The EEG-connectome processor improves the diagnosis accuracy from 89% to 95% in Alzheimer's disease case. The proposed processor occupies $3.8mm^2$ and consumes 1.71 mW with $0.18{\mu}m$ CMOS technology.

Deterioration of Mental Health in Children and Adolescents During the COVID-19 Pandemic

  • This paper reviews the global effect of the coronavirus disease 2019 (COVID-19) pandemic on the mental health of children and adolescents in South Korea, the U.S., Japan, and China. We reviewed research on deteriorated mental health, including increased suicide, suicidal thoughts, and self-harm. Various studies have shown that students' mental health issues, such as depression and anxiety, have worsened during the COVID-19 pandemic. Furthermore, the number of students who committed suicide has significantly increased in the U.S. and Japan. Factors such as prior mental health status, change in daily routine, reduced physical activity, excessive screen time, overuse of electronic devices, and reduced social support have been reported to have a significant effect. The chain of deteriorating mental health among the youth began at the onset of COVID-19, social distancing, and school closure. As youths began to stay at home instead of going to school, they lost opportunities to connect with their friends or teachers, who could provide support outside of their homes. Young people spent less time on physical activity and more time online, which damaged their sleeping schedule and daily routine. In preparing for the post-pandemic phase, we should thoroughly analyze the long-term effects of the pandemic on youth mental health, while simultaneously tackling current imminent issues.

  • The development of inexpensive high throughput methods to identify individual DNA sequences is important to the future growth of medical genetics. This has become increasingly apparent as psychiatric geneticists focus more attention on the molecular basis of complex multifactorial diseases at which most of psychiatric disease is estimated. Furthermore, candidate gene approaches used in identifying disease associated genes necessitate screening large sequence blocks for changes tracking with the disease state. Even after such genes are isolated, large scale mutational analysis will often be needed for risk assessment studies to define the likely medical consequences of carrying a mutated gene. This review provide basic knowledge of up-to-date technology, cDNA microarray which enables above mentioned various research themes.

정신장애 환자의 치과치료 (DENTAL TREATMENT OF PSYCHIATRIC PATIENTS)

  • Patients with severe mental problems have difficulty maintaining good oral hygiene, so that the incidence of dental disease is greater, the disease is more severe, and the progression is much faster in these patients. Generally, they require physical restraint, drug-induced sedation, and general anesthesia since they are not cooperative. In many cases, these patients have systemic disease, so that the prognosis after dental treatment depends highly on the management of existing diseases. The problems related to treating these patients are examined and methods of using general anesthesia and sedatives are further examined to propose better ways of treating these patients.

  • Restless Legs Syndrome/Willis-Ekbom Disease (RLS/WED) is a sleep disorder characterized by sensorimotor symptoms such as unpleasant sensations before sleep, akathisia, and periodic limb movements during sleep. It is also closely related to hyperarousal and is often accompanied by insomnia. Although the mechanism is not clear, the understanding of etiology and pathophysiology has greatly expanded through recent advances in genetic and neurobiological research. The most important pathophysiology of RLS/WED is brain iron deficiency. Such iron deficiency in the brain is caused by complex interactions between several genetic factors and various environmental factors, including comorbidities. Iron deficiency in the brain results in dysfunction of several neurotransmitters. A decrease in adenosine activity appears first, followed by an increase in the activity of glutamate and dopamine. A decrease in adenosine activity and an increase in glutamate activity stimulate the brain arousal system, resulting in hyperarousal. In addition, overproduction of dopamine and glutamate leads to dysfunction of the cortical-striatal-thalamic circuit, resulting in symptoms such as akathisia and periodic limb movements during sleep.

정신과의 분자생물학 적용 (Molecular Application in Psychiatry)

  • The development of molecular biology has brought many changes in psychiatry. Molecular biology makes us possible to know the cause of mental disorders that provide the way to prevent the disorders, and to develop various accurate diagnostic and treatment methods for mental disorders. The author discusses the concept, cause, and treatment of mental disorders in the aspect of molecular biology. Importing the methods of molecular biology into psychiatry, we can anticipate to get a number of the goals of psychiatric genetics, including identification of specific susceptibility genes, clarification of the pathophysiological processes whereby these genes lead to symptoms, establishment of epigenetic factors that interact with these genes to produce disease, validation of nosological boundaries that more closely reflect the actions of these genes, and development of effective preventive and therapeutic interventions based on genetic counseling, gene therapy, and modification of permissive or protective environmental influences. In addition to their capacity to accelerate the discovery of new molecules participating in the nervous system's response to disease or to self-administered drugs, molecular biological strategies can also be used to determine how critical a particular gene product may be in mediating a cellular event with behavioral importance. Molecular biology probably enables us discover the environmental factors of mental disorders and allow rational drug design and gene therapies for mental disorders, by isolation of gene products that facilitate a basic understanding of the pathogenesis of these disorders. A specific genetic linkage may suggest a novel class of drugs that has not yet been tried. With respect to gene therapy, the hypothetical method would use a gene delivery system, most likely a modified virus, to insert a functional copy of a mutant gene into those brain cells that require the gene for normal function.

정신지체 및 발달지연으로 수용된 인구의 임상, 내분비 및 대사 질환 평가 (Clinical and Biochemical Evaluation of Institutionalized Population with Mental Retardation or Developmental Delay)

  • Purpose: Developmental delay and mental retardation are frequently occurring disorders that present major socio-economic burden on the affected individual's family and society. Both can be congenital or acquired. However, a large number of people are institutionalized without exact diagnosis and, as a result, have not received proper care. Methods: 508 subjects with mental retardation or developmental delay from six institutions in Chung Buk Province were clinically evaluated and screened for metabolic and endocrinologic problems between 2000 and 2012. Results: Clinical genetic disorders were observed in 52 (10.2%) subjects. Cerebral palsy attributed to 21% of the institutionalized. 18 (3.5%) were diagnosed with metabolic disorders and 13 (2.6%) exhibited secondary endocrinologic dysfunction. Over 16% showed metabolic evidence of malnutrition. Conclusion: 21% and 3.5% of the population institutionalized due to mental retardation or developmental delay were afflicted by preventable cerebral palsy and metabolic disorders, respectively. Through early identification of the causes and early treatment, it may be possible to prevent, reduce, or alleviate the disability of many institutionalized individuals. Further research is imperative for establishing guidelines for diagnostic investigation for mental retardation.

Impact of the Coronavirus Disease Pandemic on Mental Health Among School Students in Korea During the COVID-19 Pandemic

  • Objectives: The coronavirus disease (COVID-19) pandemic has had various effects on mankind, especially children and adolescents. Because children and adolescents spend a lot of time at school, COVID-19 has had a great impact on school mental health. In this study, we investigated the effect of prolonged COVID-19 on school mental health. Methods: We prepared self-report questionnaires for depression (Children's Depression Inventory, CDI), anxiety (Korean version of the Penn State Worry Questionnaire for Children; Generalized Anxiety Disorder-7, GAD-7), and post-traumatic stress (Primary Care Post-traumatic Stress Disorder, PC-PTSD) for administering to students aged between 7 and 18 years, recruited by a COVID-19 psychological prevention support group in the Gwangmyeong Mental Health Welfare Center for 2 years, in 2020 and 2021. Results: For children aged 7-12 years, there was no significant difference between the years 2020 and 2021 in the assessment of depression, anxiety, and post-traumatic stress. Conversely, for adolescents aged 13-18 years, there was a significant increase in the scale scores (CDI, PC-PTSD, and GAD-7). Conclusion: Prolonged COVID-19 might have had a significant impact on the mental health of adolescents who spent a lot of time at school. When comparing the years 2020 and 2021, middle and high school students were more affected by COVID-19 than elementary school students.

알레르기 비염 유병자의 정신건강에 영향을 미치는 요인 (Factors Affecting the Mental Health of Allergic Rhinitis Patients)

  • 본 연구의 목적은 알레르기 비염 환자의 정신건강에 영향을 미치는 요인을 파악하여 연구대상자의 정신건강 증진을 위한 정책 마련의 기초자료를 제시하고자 한다. 분석 자료는 질병관리청의 국민건강영양조사 자료를 사용하였고, 성인이면서 알레르기 비염 환자 881명을 최종 분석에 사용하였다. SPSS 26을 이용하여 빈도분석, 교차분석, 로지스틱 회귀분석을 실시하였다. 연구결과, 스트레스 인지율에 영향을 미치는 요인으로 연령이 낮을수록, 교육수준이 높을수록, 통증/불편을 느낄수록, 불안/우울을 느낄수록, 주관적 건강인지 수준이 낮을수록, 1주일간 걷기를 하지 않았을 때 영향을 미쳤다. 우울증에 영향을 미치는 요인으로는 의료급여일수록, 일상 활동에 지장이 있을 경우, 불안/우울이 있을 경우 영향을 미쳤다. 이는 알레르기 비염 환자의 정신건강 증진을 위해 스트레스와 우울증에 영향을 미치는 요인을 관리하고 해결하는 방안을 모색한다면 효율적으로 대상자의 정신건강 상태 증진을 기대할 수 있을 것이다.
