Purpose: This study was performed to obtain a quantitative evaluation of the cortical and cancellous bone graft harvestable from the mental and canine regions, and to evaluate the cortical vestibular thickness. Materials and Methods: This study collected cone-beam computed tomographic (CBCT) images of 100 Italian patients. The limits of the mental region were established: 5 mm in front of the medial margin of each mental foramen, 5 mm under the apex of each tooth present, and above the inferior mandibular cortex. Cortical and cancellous bone volumes were evaluated using SimPlant software (SimPlant 3-D Pro, Materialize, Leuven, Belgium) tools. In addition, the cortical vestibular thickness (minimal and maximal values) was evaluated in 3 cross-sections corresponding to the right canine tooth (3R), the median section (M), and the left canine tooth (3L). Results: The cortical volume was $0.71{\pm}0.23mL$ (0.27-1.96 mL) and the cancellous volume was $2.16{\pm}0.76mL$ (0.86-6.28 mL). The minimal cortical vestibular thickness was $1.54{\pm}0.41mm$ (0.61-3.25 mm), and the maximal cortical vestibular thickness was $3.14{\pm}0.75mm$ (1.01-5.83 mm). Conclusion: The use of the imaging software allowed a patient-specific assessment of mental and canine region bone availability. The proposed evaluation method might help the surgeon in the selection of the donor site by the comparison between bone availability in the donor site and the reconstructive exigency of the recipient site.
Purpose : This study evaluated the potential use of the computed tomography indices (CTI) on cone beam CT (CBCT) images for an assessment of the bone mineral density (BMD) in postmenopausal osteoporotic women. Materials and Methods : Twenty-one postmenopausal osteoporotic women and 21 postmenopausal healthy women were enrolled as the subjects. The BMD of the lumbar vertebrae and femur were calculated by dual energy X-ray absorptiometry (DXA) using a DXA scanner. The CBCT images were obtained from the unilateral mental foramen region using a PSR-$9000N^{TM}$ Dental CT system. The axial, sagittal, and coronal images were reconstructed from the block images using $OnDemend3D^{TM}$. The new term "CTI" on CBCT images was proposed. The relationship between the CT measurements and BMDs were assessed and the intra-observer agreement was determined. Results : There were significant differences between the normal and osteoporotic groups in the computed tomography mandibular index superior (CTI(S)), computed tomography mandibular index inferior (CTI(I)), and computed tomography cortical index (CTCI). On the other hand, there was no difference between the groups in the computed tomography mental index (CTMI: inferior cortical width). Conclusion : CTI(S), CTI(I), and CTCI on the CBCT images can be used to assess the osteoporotic women.

Intraoral lipomas are a rare clinical entity, comprising only 0.1% to 5% of all benign tumors in the intraoral cavity. A 56-year-old woman suffering from diabetes presented with this relatively rare intraoral lipoma and was treated by surgical excision under general anesthesia. Because the mass was located adjacent to the mental foramen, a precise dissection was necessary to ensure minimal nerve damage. No abnormalities or recurrence was noted at 1-year follow-up and the patient did not complain of numbness. We studied the occurrence of oral lipoma in this diabetic patient and reviewed the relationship between oral lipoma and diabetes in the literature.

Objectives: The aim of this study was to evaluate different anatomical variants of the anterior loop of the inferior alveolar nerve (IAN) via cone-beam computed tomography (CBCT). Materials and Methods: CBCT images of 71 patients (36 males and 35 females) were evaluated. We used the classification described by Solar for IAN evaluation. In this classification, three different types of IAN loops were introduced prior to emerging from the mental foramen. We classified patients according to this system and introduced a new, fourth type. Results: Type I was seen in 15 sites (10.6%), type II in 39 sites (27.5%), and type III in 50 sites (35.2%). We found a new type in 38 sites (26.8%) that constituted a fourth type. Conclusion: We found that type III was the most common variant. In the fourth type, the IAN was not detectable because the main nerve was adjacent to the cortical plate and the incisive branch was thinner than the main branch and alongside it. In this type, more care is needed for surgeries including inferior alveolar and mental nerve transposition.
신경섬유종증은 상염색체 우성 유전성 질환으로, 17번 염색체의 장완에 위치한 종양 억제 유전자의 변성에 의해 발생한다. 이는 두가지 형이 있으며 신경섬유종의 발생은 제1형 신경섬유종증의 임상적 진단 기준 중 하나이다. 신경섬유종증의 임상적 증상으로는 피부병소, 골변형, 중추신경계의 종양 등이 있으며 환자의 25%가 구강내 신경섬유종을 보인다. 악골내 신경섬유종은 드물며 방사선학적으로 하악공, 하악관, 이공을 포함하고 단방성으로 잘 경계된 방사선 투과성으로 나타난다. 신경섬유종은 하나 또는 그 이상의 병소에서 신경육종으로 전이된다는 점에서 임상적으로 중요하며 현재 특이한 치료법은 없으나, 외과적 절제술이 좋은 치료법으로 여겨지고 있다. 본 증례는 제1형 신경섬유종증 진단을 받은 환아로, 구강내 신경섬유종의 외과적 절제 후 양호한 결과를 보여 이를 보고하는 바이다.
Noonan증후군에서는 정상 핵형이면서도 Turner증후군과 같은 표현형과 다양한 형태의 지능저하, 양안격리증, 골격이상 및 선천성 심혈관계 이상과 같은 선천적 기형의 특징을 보인다. Noonan증후군의 2/3에서 심기형을 가지며, 그중 폐동맥협착이 반을 차지한다. Noonan증후군에서 폐동맥협착과 그외 다른 심기형을 가졌던 두 명의 환자 증례로 첫번째 환자는 31세 남자로 Noonan증후군의 특징적인 표현형을 보이면서 중증의 폐동맥 판막하협착과 난원공 개존증을 가지고 있었다. 두번째 28세 남자환자는 전형적인 Noonan증후군의 얼굴모양 과 저신장이었으며 폐동맥판 및 폐동맥판막하 협착을 동반하고 있었다. 상기 2례에서 모두 폐동맥판 교련절제와 우심실의 비후된 근육층을 제거하였고 첫번째 환자에서의 난원공 개존증은 일차봉합을 시행하였다. 술후 좋은 결과를 보였고 추적관찰중이다.
Purpose: The aim of this study was to analyze radiologically the location and course of the mandibular canal and to observe the alveolar and basal bone changes during the remodeling procedures of atrophic mandible. Materials and Methods: CT scanning was performed on dry 30 edentulous or partially dentulous mandibles. In 48 edentulous lower halves, measuring areas were determined by three points in the length of the mandibular canal. The distance from the mandibular canal towards cranial and caudal edges, buccal and lingual external borders of the body of the mandible were measured. A statistical comparison between the mean values of different classes of mandibular body was carried out in the selected areas. Results: The distance between the mandibular canal and caudal borders of the body of the mandible and lingual borders dose not change in the atrophic process of mandible. The mandibular canal within the mandible courses downwards from mandibular foramen towards mesial and subsequently it gets to the mental foramen. The distance between the mandibular canal and buccal external border of basal bone changes similar to the change of cranial borders of alveolar bone in the atrophic process of mandible. Conclusion: CT scanning was very effective and practicable to analyze the location and course of the mandibular canal and to observe the alveolar and basal bone changes of atrophic mandible. Also more detailed investigation of basal bone changes observed during the remodeling procedures of atrophic mandibles seems reasonable to rely on the massive anthropologic collections of atrophic mandibles combined with CT scanning.
The purpose of this study was to investigate the effect of various electroacupuncture duration induced by acupuncture point-Zusanli ($S_{36}$) electrical stimulation on inhibition of amplitude of digastric electromyogram (dEMG) evoked by noxious electrical stimuli around the mental foramen. intraperitoneal sodium pentobarbital in an initial dose of 50mg/kg and maintenance doses of 4.5mg/kg/h were given through a cannula in the femoral vein using a constant infusion pump. A pair of stimulating electrodes were inserted for noxious stimuli around the mental foramen. An irritant electronic stimuli pulse (0.2 Hz, 0.1 ms duration) was produced with an intensity of about $1.5{\times}2$ times threshold for evoking the dEMG. The anterior belly of the digastric muscle was exposed and a pair of 0.1mm wire electrodes were inserted for dEMG recording. Acupuncture point stimulation on Zusanli (2 Hz, 250 ${\mu}s$, biphasic pulse, 2 V) was delivered by Dental Electronic Anesthesia (3M, U.S.A). For periods of electronic stimulation of 10, 20, and 30min, the amplitudes of dEMG were measured on the oscilloscope and on the monitor connected to the amplifier. The following results were obtained: The dEMG was decreased to 73.4% of that in the control set after 10 min electroacupunture stimulation (Group I); The dEMG was decreased to 77.1% (10min), 54.0.% (20min) of that in the control set after 20minutes of electroacupunture stimulation (Group II). The dEMG was decreased to 73.3% (10min), 61.9% (20min), 76.2% (30min) of that in the control set after 30 min of electroacupunture stimulation (Group III). From these results, it may be that in the electroacupuncture stimulation on the Zusnali resulted in a reduction of amplitude of dEMG and that the most effective electroacupuncture stimulation period was 20min.
Dysfunction of the inferior alveolar nerve may result from trauma, diseases or iatrogenic injury. The development and refinement of an objective method to evaluate this clinical problem is highly desirable and needed, especially concerning for an increasing medico-legal issue. Evoked potential techniques have attracted considerable attention as a means of assessing the function and integrity of nerve pathways. The purpose of this study was to characterize the Sensory Evoked Potentials(SEPs) and Somatosensory Evoked Potentials(SSEPs) elicited by electrical stimulation of mental nerve. SEPs and SSEPs were measured and analyzed statistically before and after needle injury on the inferior alveolar nerve of Sprague-Dawalye rats. Measuring SEPs was more sensitive in evaluation of the recovery of sensory function from inferior alveolar nerve injury then measuring SSEPs but we measured SSEPs in the hope of providing a safe, simple and objective test to check oral and facial sensibility, which is acceptable to the patient. We stimulated mental nerve after needle injury on the inferior alveolar nerve and SEPS on the level of mandibular foramen and SSEPs on the level of cerebral cortex were recorded. Threshold, amplitude, and latency of both of SEPs and SSEPs were analyzed. The results were as follows ; 1. Threshold of SEPs and SSEPs were $184{\pm}14{\mu}A$ and $164{\pm}14{\mu}A$ respectively. 2 SEPs were composed of 2 waves, i.e., N1 N2 in which N1 was conducted by II fibers and N2 was conducted by III fibers. 3. SSEPS were composed of 5 waves, of which N1 and N2 shower statistically significant changes(p<0.01, unpaired t-test). 4. SEPs and SSEPs were observed to be abolished immediately after local anesthesia and recovered 30 minutes later. 5. SEPs were abolished immediately after injury. N1 of SSEPs was abolished immediately and amplitued of N2 was decreased($20.7{\pm}12.2%$) immediately after 23G needle injury, but N3, N4 and N5 did not change significantly. Recovery of waveform delayed 30 minutes in SEPs and 45 minutes in SSEPs. 6. The degree of decrease in amplitude of SEPs and SSEPs, after 30G needle injury was smaller than those with 23G. SEPs recorded on the level of mandibular foramen were though to be reliable and useful in the assessment of the function of the inferior alveolar nerve after injury. Amplitude of SSEPs reflected the function and integrity of nerve and measuring them provided a safe, simple and abjective test to check oral and facial sensibility. These results suggest that measuring SEPs and SSEPs are meaningful methods for objective assessment in the diagnosis of nerve injury. N1 and N2 of SSEPs can be useful parameters for the evaluation of the nerve function following a needle injury.
이부는 안면 외모에서 뚜렷한 부위를 차지하고 있어 사람의 특징을 나타내며 인상을 결정짓는다 할 수 있겠다. 따라서 옛부터 이 부위는 안면 미용에서 관심의 대상이 되었고 이에 대한 성형 수술도 시행되었다. 그러나 요즘 시행되는 Genioplasty는 짧은 역사를 가졌을 뿐만 아니라 방법도 부위 및 구조적 특성 등으로 다양성이 제한되었다. 기존의 Horizontal reduction genioplasty는 턱이 정상적인 형태 및 전방폭을 가졌을 때에 이부의 높이를 줄이거나 후방위치시킬 때에는 좋은 효과를 볼 수 있으나 하악골의 전방 폭경 및 높이의 결손등이 있을 때에는 mental foramina의 위치, 전방 하악골의 높이와 치근의 위치 등에 의해 사용이 제한된다할 수 있다. 따라서 이러한 환자에 있어서는 다른 방법이 요구된다 하겠다. 본 교실에서는 Hinds와 Kent에 의해 서술된 Horizontal step osteotomy for advancement genioplasty에 대해 변형된 방법으로 narrow, pointd chin과 짧은 전방하악골 높이를 가진 chin prominence환자에 대해 시술한 바 비교적 양호한 결과를 얻었기에 이에 보고하는 바이다.
