개에서 누낭비강조영술시 조영방법 및 조영제의 비교 (Comparison of Radiograpic Contrast Techniques and Contrast Media on Dacryocygtorhinography in Dogs)

  • This study was undertaken to evaluate image quality and level of discomfort during examination with various contrast media(meglumine ioxitalamate: Telebrix $30^{\circledR}$ , diatrizoate sodium: Hypaque $50^{\circledR}$, sodium ioxaglate + meglumine ioxaglate: Hexabrix , iopamidol: Iopamiro $300^{\circledR}$ , iodized poppyseed oil: $Lipiodol^{\circledR}$ and barium sulfate: Baritop$^{\circledR}$and various radiographic contrast techniques(eye dropping: group 1, single injection to the superior punctum: group 2 and continuous injection to the superior punctum: group 3) on dacryocystorhinography in dogs. The results were as follows; 1. The Image quality of radiographic contrast technique of continuous Injection of contrast media(group 3) was better than the other techniques. 2. Average scores for dacryocystorhinography quality of sodium ioxaglate + meglumine ioxaglate, iopamidol, barium sulfate, iodized Poppyseed oil, meglumine ioxitalamate and diatrizoats sodium were 3.67${\pm}$0.47, 3.67${\pm}$0.47, 3.50${\pm}$0.50, 3.33${\pm}$0.75, 2.50${\pm}$0.50 and 2.50${\pm}$0.76, respectively in group 3. 3. Diatrizoate sodium produced more discomfort than the other media. It was considered that the best radiographic contrast technique of dacryocystorhinography was continuous injection of contrast media to the superior punctum and sodium ioxaglate + meglumine ioxaglate, iopamidol, iodized Poppyseed oil and barium sulfate were the excellent contrast media for dacryocystorhinography in dogs.

  • 수정란이식의 수태율을 높이기 위해서 가급적 자궁각 심부에 이식하려고 한다. 그러나 자궁경관경유법에 의한 수정란이식에서 자궁각 심부에 무리하게 이식하면 출혈 또는 이식지연으로 수태율이 저하된다. 수정란이식시 자궁내 이식기의 조작은 자궁내 PG 분비를 촉진하며, 자궁내에 분비된 PGF₂a는 수정란의 발육에 악영향을 준다고 보고하고 있다. 따라서 수태율 향상을 위해 수정란이식 직전에 PGF₂a 분비억제 제제를 투여하면 수태율을 높일 수 있다고 Schrick 등(Theriogenology, 2001;55:370)은 보고하였다. Comparison Study of Image Performance with Contrast Agent Contents for Brain Magnetic Resonance Imaging

  • The purpose of study was to evaluate SNR and CNR with different contrast agent contents (1.0 mmol/mL gadobutrol and 0.5 mmol/mL gadoterate meglumine) for spin echo (SE) and 3-dimension contrast-enhanced fast field echo (3D CE-FFE) pulse sequences. In this study, we compared the SNR and the CNR between 0.5 mmol/mL gadoterate meglumine and 1.0 mmol/mL gadobutrol according to the concentration of contrast agent in brain MRI. When we compared between SE and 3D CE-FFE pulse sequences, the higher SNR and CNR using 3D CE-FFE pulse sequence can be acquire regardless of contrast agent contents. Also, a statistically significant difference was found for SNR and CNR between all protocols. In conclusion, our results demonstrated that the SNR and CNR have not risen proportionately with contrast agent contents. We hope that these results presented in this paper will contribute to decide contrast agent contents for brain MRI.

2-HP-β-시클로덱스트린과 메글루민을 이용한 란소프라졸의 포접화합물 제조 및 평가 (Preparation and Evaluation of Inclusion Complex of Lansoprazole with 2-HP-β-Cyclodextrin and Meglumine)

  • To enhance the solubility and stability of lansoprazole (LAN), new proton pump inhibitor, we were prepared various molar ratio of inclusion complex with $2-hydroxypropyl-{\beta}-cyclodextrin$ (HPCD) and organic alkali agent, meglumine (MEG). Inclusion complex formation of LAN with HPCD was investigated by Differential Scanning Calorimetry and X-ray diffractometry. The aqueous solubilities of inclusion complexes, and the stabilities of 1:4 and 1:5 inclusion complexes in aqueous solutions containing different concentrations of MEG were examined. The stability of 1:5 LAN-HPCD inclusion complex containing MEG, which was equaled to amount of LAN, was performed in 0.9% NaCl and 5% dextrose solution. The formation of inclusion complex of LAN with HPCD was $A_L$ type and the molar ratio of complex was 1:1. The stability constant was $41.557\;M^{-1}$. As molar ratio of LAN to HPCD was increased, solubility of inclusion complex was increased. 1:5 LAN-HPCD inclusion complex was more stable than 1:4 LAN-HPCD inclusion complex. And as contained MEG amount in LAN solution was increased, stability of 1:4 and 1:5 LAN-HPCD inclusion complexes was improved. Also stability of 1:5 LAN-HPCD-MEG inclusion complex in 0.9% NaCl solution and 5% dextrose solution was similar to it in water at room temperature, but it was unstable at $40^{\circ}C$.

악관절조영술이 악관절 조직에 미치는 영향에 대한 병리조직학적 연구 (A HISTOPATHOLOGIC STUDY OF THE EFFECTS OF ARTHROGRAPHY ON THE TISSUES OF TEMPOROMANDIBULAR JOINT)

  • This study was performed to observe the effects of arthrography on the tissues of temporomandibular joint histopathologically. Among 17 rabbits (34 joints), 2 (4 joints) were used as normal control group, others (30 joints) were experimentally arthrographed using 0.03㎖ diatrizoate meglumine (Hypaque meglumine 60, Wintrop, U.S.A). The rabbits were sacrificed at 2-hour, 1-, 2-, 4- and 7-day after experiment and the tissues of temporomandibular joint were prepared according to the usual method for light microscopic examination. The results were as follows: 1. There were no changes of the fibrous connective tissues of the condylar fossa, the fibrous connective tissues and hyaline cartilages of the condyle, and the articular dislc 2. In 2-hour experimental group, the proliferation of the surface synovial membrane cells, the congestion and hemorrage of the vessels were observed. 3. In 1 and 2-day experimental group, minimal lymphocytes infiltration was observed with the proliferation of the surface synovial membrane cells, the congestion and hemorrage of the vessels. 4. In 4-day experimental group, the diminution of the proliferation of the surface synovial membrane cells was seen, but there was no vascular changes. 5. In 7-day experimental group, the tissues showed similar appearance to the normal control group.

오메프라졸과 메글루민의 복합체 형성과 안정성 (Complexation of Omeprazole with Meglumine and its Stability)

  • To investigate the interaction of omeprazole (OMP) and meglumine (MEG), a complex was prepared by freeze-drying method in ammoniacal aqueous medium at room temperature and subjected to IR, DSC, and 1H NMR analysis. In addition, the stability of the complex was tested by accelerated stability analysis, and the dissolution rate of both powder and enteric coated was determined pellet by paddle method. The results are as follows; i) IR, DSC, and $^{1}H$ NMR studies indicate the formation of inclusion complex between OMP and MEG probably by electrostatic forces as $[OMP]\;[MEGH]^+$ form in a stoichiometric ratio (1:1) of OMP : MEG. ii) The dissolution rate of enteric coated OMP-MEG complex pellet in simulated enteric fluid was 90.6% in 10 minutes, which may satisfy the requirement for the regulation of dissolution. iii) OMP-MEG complex were decomposed according to pseudo 1st order kinetics: while the decomposition of OMP showed a rate constant $(k_{25^{\circ}C})$ of $5.13{\times}10^{-4}{\cdot}\;day^{-1}$, a half-life$(t_{1/2})$ of 1,350 days, a shelf-life$(T_{90%})$ 205 days and an activation energy of 23.53 kcal/mole. OMP-MEG complex inhibited a rate $(k_{25})$ of $2.92{\times}10^{-4}{\cdot}\;day^{-1}$, a half-life$(t_{1/2})$ of 2,373 days, a shelf-life $(T_{90%})$ of 306 days and an activation energy of 20.18 kcal/mole. iv) OMP was stabilized markedly by the formation of OMP-MEG complex between OMP and MEG, and the humidity increased the stability of OMP-MEG complex by decreasing the decomposition rate$(k_{50^{\circ}C})$ from $1.27{\times}10^{-2}{\cdot}\;day^{-1}$ at 31% R.H. to $2.54{\times}10^{-2}{\cdot}\;day^{-1}$ at 90% R.H.

Childhood cutaneous leishmaniasis: report of 117 cases from Iran

  • Cutaneous leishmaniasis (CL), due to Leishmania major, is endemic in different parts of Iran and has long been recognized in most provinces of Iran. This study was conducted to determine the prevalence of childhood leishmaniasis in 3 areas at the southeast of Kashan, A descriptive study was carried out on all children referred to central laboratories during a 3-year period. Initial information including age, sex, sites of ulcer on the body, number of lesions, address, and the place of the disease was obtained. The study gathered 117 children, and the results showed a prevalence of 7.2% in patients with lesions among the population and 4.2% of people displayed lesion and scar. The ages of subjects were from 6 to 15 years (average 9.75 years). The boy: girl ratio was 1.2. All of our patients lived in an endemic area. The face was affected in 47.0% of cases. The encountered forms of leishmaniasis are as follows: papulonodular 27.4%, ulcer 60.7%, sporotrichoid 6%, impetiginous 2.5%, and erysipeloid 3.4%. Treatment with intramuscular meglumine antimoniate 20-30 mg/kg/day was done for 93 patients. Meglumine antimoniate treatment was tolerated with no side effects. All leishmaniasis lesions healed within an average period of 2-14 months. Hyperpigmented scars were formed in 25.6% of the patients, atrophic scars in 4.3%, and hypopigmented scars were in 3.4%, respectively The findings of this study indicate increased prevalence of CL in the villages at the area of Kashan and Aran-Bidgol. The clinical finding patterns belonged to different endemic strains of L. major in Isfahan, which indicates the possible transmission of infection from Isfahan to this area.

말의 그물막구멍포착 교정을 위한 소장 절제 및 문합 1례 (Small Intestine Resection and Anastomosis for Epiploic Foramen Entrapment of a Horse)

  • A 10-year-old gelding Warmblood weighing 560 kg was referred to J&C Equine Hospital with the history of hyperpnea, depression, pawing, and rolling for 7 hours. According to the results of clinical and ultrasound examination, it was considered that intestines were distended with thickened wall. The horse had been treated with lactated Ringers' solution (14 L, IV), flunixin meglumine (1.1 mg/kg, IV), and mineral oil (1 L, PO), but he did not show any responses to those treatments. Exploratory laparotomy was performed and identified incarcerated small intestine through the epiploic foramen. The horse received resection and anastomosis of the entrapped small intestine. After surgery, the horse was treated with intensive postoperative care of fluid therapy (5 L with 20 mEq/L KCl, every 2 hours), flunixin meglumine (1.1 mg/kg, IV, sid), antibiotics (penicillin 22,000 IU/kg, IV, qid and gentamicin 6.6 mg/kg, IV, sid), lidocaine constant rate infusion (bolus 1.3 mg/kg over 15 minutes then 0.05 mg/kg/minute), common nutritional supplements, nasogastric intubation every 2 hours and trunk bandage. Postoperative feeding program had started with small amount of hay every 4 hours and gradually increased to normal amount till 5 days. At 77 days after surgery, he showed sudden outbreak of colic and was euthanized. The causes of colic were small intestinal strangulation by passing through the mesenteric rents and postoperative adhesion between small intestines. According to the results, it is recommended to perform perioperative intensive care of horse with colic and to use several methods to prevent adhesions during abdominal surgery of horses.

Visceral Leishmaniasis without Fever in an 11-Month-Old Infant: a Rare Clinical Feature of Kala-azar

  • Visceral leishmaniasis or kala-azar is an endemic parasitic disease in some parts of the world which is characterized by fever, splenomegaly, and pancytopenia in most of the cases. Herein we report an 11 month-old male infant with diagnosis of kala-azar who presented with pallor, hepatosplenomegaly, failure to gain weight, and no history of fever. Surprisingly, fever started after beginning of meglumine antimoniate treatment in this patient. As far as we are aware of, this is a rare presentation of visceral leishmaniasis. Therefore, clinicians especially in endemic areas are highly recommended to include kala-azar among differential diagnosis of unexplained anemia without fever to prevent misdiagnosis of this potentially fatal, but treatable condition.

Cutaneous Leishmaniasis of the Eyelids: A Case Series with Molecular Identification and Literature Review

  • Cutaneous leishmaniasis (CL) is a protozoan disease which is endemic in Iran. It is transmitted by the Phlebotomus sand fly. The eyelid is rarely involved possibly because the movement of the lids impedes the sand fly from biting the skin in this region. Here, we report 6 rare cases of eyelid CL. The patients were diagnosed by skin scraping, culture, and PCR from the lesions. Skin scraping examination showed Leishmania spp. amastigotes in the cytoplasm of macrophages. Culture examination was positive for Leishmania spp. PCR was positive for Leishmania major and Leishmania tropica. The lesions were disguised as basal cell carcinoma, chalazion, hordeolum, and impetigo. The patients were treated with intramuscular meglumine antimoniate (20 mg/kg/day) for at least 3 weeks. They showed a dramatic response, and the lesions almost completely disappeared. We emphasized the importance of clinical and diagnostic features of lesions, characterized the phylogenetic relationship of isolated parasites, and reviewed the literature on ocular leishmaniasis.
