Development of Portable Astral Light using the High Power 3-Color LEDs (고출력 3색 LED를 이용한 휴대용 무영등의 개발)

  • We was designed the portable LED light which can shadowless shooting and developed using a high-power LED that medical attention as a new lighting components for structural confirmation of oral dental area, medical and surgical lesions in the local area. LED which applied to the development, was used the 3-color LEDs for possible of implement a variety of colors and adjusted the light intensity. It is being magnified of delicacy expressiveness and three-dimensional for tone of the subject-specific. It has been developed a highly efficient light module that LED is used to consider the electrical characteristics and optical properties.   • The space planning and interior layout of recovery unit should be determined carefully for patient's nursing and observation which is considered to prevent infection and to cope quickly with emergency situation after surgical operation. Recently, the recovery unit in medical facilities is planned and managed without consideration of operating system, sanitary facilities and prior condition of space. Therefore, there is required to the logical criterion of architectural planning for patient's recovery unit which is concerned about opinion of medical team, anesthetist, sanitarian, architect and patient as a user. As the result, this study would apply to design guideline for the architectural planning of patient's recovery unit.

The Effect of a Telephone follow-up on the Self-care and the Satisfaction on Nursing Care in G-I Surgical Patients (전화추후관리가 위,장관계 수술환자의 자기간호수행 및 환자만족도에 미치는 효과)

  • This study was a quasi-experimental study to investigate the effect of a telephone follow-up on the self-care and the satisfaction on nursing care in G-I surgical patients. The subjects of the study were 38 patients, 19 in experimental group and 19 in control group, who discharged after gastrointestinal surgery at a university hospital in Taegu between August 11. 1998 and October 16, 1998. The telephone follow-up program for the experiment was developed by the researcher, which contained consultation, support and encouragement for self-care after discharge. Number of the instruments used for measurement of the outcome variables were 5, self-care after discharge, satisfaction(1)${\sim}$(2) on Nursing care. intention to visit the hospital again. intention to recommend the hospital to others. The results of this study were as follows ; 1. The first hypothesis, "The subjects of experimental group will show a higher level of self-care than the subjects of control group" was rejected (F=.48. p=.4937). 2. The Second hypothesis. "The subjects of experimental group will show a higher level of satisfaction than the subjects of control group" was accepted partly as follows. 1) The hypothesis 2-1, "The subjects of experimental group will show a higher level of satisfaction(1) than the subjects of control group" was accepted(F=4.04. p=.0496). 2) The hypothesis 2-2. "The subjects of experimental group will show a higher level of satisfaction(2) than the subjects of control group" was accepted(F=8.48. p=.0064). 3) The hypothesis 2-3, "The subjects of experimental group will show a higher level of intention to visit hospital again than the subjects of control group" was rejected(F=1.95. p=.1723). 4) The hypothesis 2-4. "The subjects of experimental group will show a higher level of intention to recommend the hospital to others than the subjects of control group" was rejected(F=1.43. p=.2411) . From the results of this study, the telephone follow-up can be regarded as an effective nursing intervention for the discharged patients.

The Actual Experiences of the Living World among Cancer Patients (암환자의 생활세계 경험)

  • Purpose: The purpose of this study was to understand the meanings and nature of living in the world among cancer patients. The present study adopted a hermeneutic phenomenological method which was developed by van Manen. Method: The participants for this study were 5 men and 6 women, who were over the age of 20 with admission or a follow up visit in the medical or surgical department. Data were collected by using in-depth interviews and observations from February to September, 2007. The contents of the interviews were tape-recorded with the consent of the subject. Result: The essential themes that fit into the context of the 4 existential grounds of body, time, space and other people were: a body that cannot be restored, a body that endures and lives, waiting in uncertainty, a valued calculation for the living day, being in a world of invisible power, reestablishing relationships, and reflection on his or her life. Conclusion: These findings revealed that living in the world is affected to varying degrees by the cancer. It is important for nurses to identify and take care of disabilities and to support the reorientation in the disintegrated life situation. The result of this study can give nurses some insight into these experiences and help promote empathetic care.

Factors Influencing Intra-Operative Body Temperature in Laparoscopic Colectomy Surgery under General Anesthesia: An Observational Cohort

  • Purpose: This study aimed to identify factors influencing intra-operative core body temperature (CBT), and to develop a predictive model for intra-operative CBT in laparoscopic abdominal surgery. Methods: The prospective observational study involved 161 subjects, whose age, weight, and height were collected. The basal pre-operative CBT, pre-operative blood pressure, and heartbeat were measured. CBT was measured 1 hour and 2 hours after pneumoperitoneum. Results: Explanatory factors of intra-operative hypothermia (< $36^{\circ}C$) were weight (${\beta}=.361$, p< .001) and pre-operative CBT (${\beta}=.280$, p= .001) 1 hour after pneumoperitoneum (Adjusted $R^2=.198$, F= 7.56, p< .001). Weight was (${\beta}=.423$, p< .001) and pre-operative CBT was (${\beta}=.206$, p= .011) 2 hours after pneumoperitoneum (Adjusted $R^2=.177$, F= 5.93, p< .001). The researchers developed a predictive model for intra-operative CBT ($^{\circ}C$) by observing intra-operative CBT, body weight, and pre-operative CBT. The predictive model revealed that intra-operative CBT was positively correlated with body weight and pre-operative CBT. Conclusion: Influence of weight on intra-operative hypothermia increased over time from 1 hour to 2 hours after pneumoperitoneum, whereas influence of pre-operative CBT on intraoperative hypothermia decreased over time from 1 hour to 2 hours after pneumoperitoneum. The research recommends pre-warming for laparoscopic surgical patients to guard against intra-operative hypothermia.

A Study of Role-Conflict, Stress and Job Satisfaction of Nurses According to Their Current Work Posts (근무병동별 간호사의 역할갈등, 직무스트레스 및 업무만족에 관한 연구)

  • This research has been conducted in order to compare and analyze the role-conflict, job stress and job satisfaction of nurses according to their current work posts. The subjects of this research were carefully selected 426 nurses from 2 university hospitals. The research tool were a role-conflict measurement index, job stress a tool, and job satisfaction Minnesota Satisfaction Questionnaire. The data have been collected using a structured questionnaire from March to May 2003. The collected data have been analyzed using the SPSS PC 10.0V program. The nurses at Intensive Care Unit (ICU) showed highest level of job stress while the nurses at surgical ward showed the lowest level of job stress (F=8.37, p=.000). There was no significant difference in role-conflict and job satisfaction according to the current work posts. According to the results of this research, there was a difference in stress according to the current work posts, and it turned out that the nurses at ICU had the highest level of stress compared to the nurses at other departments, and it causes a fall in job satisfaction.

Risk Factors for Deliberate Self-extubation (기관 내 삽관환자의 의도적 자가발관 위험요인)

  • Purpose: This study was conducted to analyze intubation survival rates according to characteristics and to identify the risk factors affecting deliberate self-extubation. Methods: Data were collected from patients' electronic medical reports from one hospital in B city. Participants were 450 patients with endotracheal intubation being treated in intensive care units. The collected data were analyzed using Kaplan-Meier estimation, Log rank test, and Cox's proportional hazards model. Results: Over 15 months thirty-two (7.1%) of the 450 intubation patients intentionally extubated themselves. The patients who had experienced high level of consciousness, agitation. use of sedative, application of restraints, and day and night shift had significantly lower intubation survival rates. Risk factors for deliberate self-extubation were age (60 years and over), unit (neurological intensive care), level of consciousness (higher), agitation, application of restraints, shift (night), and nurse-to-patient ratio (one nurse caring for two or more patients). Conclusion: Appropriate use of sedative drugs, effective treatment to reduce agitation, sufficient nurse-to-patient ratio, and no restraints for patients should be the focus to diminish the number of deliberate self-extubations.

Effects of Foot Mats on Levels of Fatigue, Discomfort, Lower Extremity Edema and Skin Temperature in Scrub Nurses (발매트 적용이 수술실 소독간호사의 피로도, 불편감, 하지 부종 및 피부온도에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effects of foot mats on reducing levels of fatigue, discomfort, lower extremity edema and skin temperature in scrub nurses during surgery. Methods: This study employed one group repeated measures design in which 16 scrub nurses (ages 22-38) stood on steel footstools (SF) during scrub as the conventional test and stood on SF with foot mats as the experimental test. Levels of fatigue, discomfort, lower extremity edema and skin temperature were measured before the first surgical hand scrub and right after the first surgery of the day for 3 consecutive days in both tests. Results: The changes in levels of fatigue and discomfort were significantly different between tests (p<.001~.033). Edema, determined by the differences in circumferences of the ankles and calves before and after the surgery was also significantly different between groups (p=.001~.011). However, the change in skin temperature on the foot tops of both feet was not different between tests (p=.351~.467). Conclusion: The results of this study show that using foot mats on top of the steel footstool is effective in reducing the levels of fatigue, discomfort and lower extremity edema in scrub nurses during surgery.

Effects of Education for Safe Handling of Cytotoxic Drugs on Knowledge and Practice in Oncology Nurses (항암제 안전관리 교육이 간호사의 안전관리 수칙 지식 및 실천에 미치는 효과)

  • Purpose: The present study explored the effects of education for safe handling of cytotoxic drugs on knowledge and practice in nurses. Method: Data were collected using a single group with pre and posttest design from 59 nurses at medical and surgical wards in C university hospital from April to June 2005. Questionnaires on the knowledge and practice of the guidelines for safe handling of cytotoxic drugs were asked to complete before and after the education. Result: The education improved nurses' knowledge and practice of the guidelines for safe handling of cytotoxic drugs. The knowledge and practice had a significant positive correlation. Conclusion: These findings show that the education for safe handling of cytotoxic drugs improved the knowledge and practice of nurses. As the knowledge of the guidelines for safe handling of cytotoxic drugs makes a positive association with the practice, it is thought to be required to enhance the education program for nurses.

  • Purpose: The study was done to identify variations in Nurses' CPR knowledge, attitude and performance before and after BLS Education. Method: This study was designed to evaluate the differences in Nurses' CPR knowledge, attitude and performance. For the study we selected 167 nurses working in Daegu K University hospital. The study group was given a test before (pre test) and after (post test) BLS education. The BLS education was given by two BLS instructors certified by the American Heart Association, using a DVD and manikin. Result: The results showed a statistically significant difference the nurse's knowledge, belief, emotion and performance but there was no significant difference in their behavior after BLS education. CPR knowledge and performance was higher among emergency room nurses compared to surgical ward nurses. Conclusion: CPR education has shown to have a positive effect on nurses' attitudes towards CPR. It also improved their related knowledge and performance. Therefore, cardiopulmonary education should be given continuously, and different steps of CPR education programs need to be developed in order to fill the knowledge deficiency gap.
