Objectives : The purpose of this study is to investigate the anti-oxidative and the anti-inflammatory effects of Danpitang(DPT) extract in RAW 264.7 macrophages. Methods : The macrophage cell line RAW 264.7 cells were used and MTT assay was performed to measure the cell viabilities at the various concentrations of DPT($50-400{\mu}g/m{\ell}$). Nitric oxide(NO) was measured in LPS-induced RAW 264.7 cells. Expressions of iNOS, COX-2, $TNF-{\alpha}$, $IL-1{\alpha}$, $IL-1{\beta}$ and IL-6 were also performed by real-time PCR. Protein expression of iNOS and COX-2 was confirmed by western blot. The anti-oxidant activities of DPT was measured by DPPH radical scavenging activity. Results : 1. There was no cytotoxicity in RAW 264.7 cells treated with DPT compared to the control. 2. DPT treated group significantly inhibited NO production compared to the LPS treated group. 3. DPT treated group significantly decreased mRNA expressions of iNOS, COX-2, $TNF-{\alpha}$, $IL-1{\alpha}$, $IL-1{\beta}$ and IL-6 compared to the LPS treated group. 4. To evaluate the safety of the products for the human body, Adverse events, SCORAD Index Assessment were conducted; There were no severe adverse events during this study. And SCORAD Index showed a statistically significant decrease in treatment group in baseline, 2 weeks and 4 weeks. Therefore, it is suggested that products, if used for certain period, should be safe for the human body. 5. DPT was found to have high DPPH free radical scavenging ability. Conclusions : According to the above results, DPT can be used as a therapy in various anti-inflammatory skin diseases.
Objectives: To assess the effect and safety of transcranial direct-current stimulation (tDCS) in primary chronic insomnia. Methods: A one-month, double-blind, randomized, sham-controlled trial was performed. A total of 7 patients with primary chronic insomnia received tDCS using anodal (n=3), cathodal (n=2), or sham stimulation (n=2). They were followed up at 1 week and 1 month after treatment. The primary outcome measures included improvement in total sleep time (TST), sleep latency (SL), and sleep efficiency (SE) at 1 month follow-up. Results: TST and SE were improved with tDCS at 1 month follow-up in all patients (100%) of the anodal group, one (50%) of the cathodal group, and one (50%) of the sham group. tDCS improved SL at 1 month follow-up in two patients (67%) of the anodal group, one (50%) of the cathodal group, and none (0%) of the sham group. With respect to adverse events, transient itching sensation occurred in one patient of the anodal group. None of the other groups reported adverse events. Conclusions: Our results suggest that tDCS may be effective and safe for treatment of primary chronic insomnia. A larger controlled study needs to be further investigated.
Background: The role of clinical pharmacists in medication therapy to improve clinical and economic outcomes has been reported in the literature. This study was conducted to analyze the changes in details of medication interventions before and after the introduction of clinical pharmacists into the care of neurocritical care unit (NCU) patients, and to evaluate the economic effects of clinical pharmacists by calculating the avoidance cost. Methods: A retrospective study was conducted reviewing the electronic medical records from June 2013 to May 2014 (before), and from June 2016 to May 2017 (after). We calculated the number and rates of intervention, the acceptance rates of it, and also reviewed the list of interventions. We calculated avoidance cost if there was no intervention. Results: The monthly mean number of interventions increased from 8.0 (${\pm}5.7$) to 31.7 (${\pm}12.8$) (P<0.001) and the frequency of intervention also increased from 0.8% to 1.6% (P=0.003). The most frequently provided pharmacist intervention was nutritional support before introduction of clinical pharmacists and discussions on the medication plan after. The number of classified interventions was 14 before introduction of clinical pharmacist services and 33 after. The calculated cost avoidance associated with a clinical pharmacists' integration was 77,990,615 won per year. Conclusion: Introduction of clinicals pharmacist into the NCU was associated with increased intervention rates and expanded types of clinical interventions. The cost avoidance achieved by the pharmacists' interventions can be further explored to evaluate if similar expansions of pharmacists' services achieve similar results in other settings.
Objective: This study was conducted to report the glucose-lowering effect and safety of herbal extracts in patients with type 2 diabetes mellitus. Methods: We investigated 21 patients with type 2 diabetes mellitus who were administered Daeshiho-tang, Bojungikgi-tang, Jowiseunggi-tang, and Hoechunyanggyeok-san at Kyung-Hee University Korean Medical Hospital from 2014 to 2019. The hypoglycemic effect of the herbal extracts was assessed by comparing blood glucose levels, including fasting blood sugar (FBS) and 2-hour postprandial glucose (PP2) levels. For safety assessment, the effects of herbal extracts on liver and kidney function were analyzed by liver function tests, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), and γ-glutamyltransferase (GGT), and kidney function tests, including blood urea nitrogen (BUN) and creatinine (Cr). Patients were stratified according to their glycated hemoglobin (<6.5 or >6.5) levels and the kind of herbal extract used for treatment. Results: After administration of herbal extracts, FBS and PP2 significantly decreased to 20.24 mg/dL and 35.0 mg/dL respectively. Subgroup analysis revealed that, regardless of the glycated hemoglobin level, FBS and PP2 were significantly reduced in both groups. The safety profile showed no significant difference before and after taking herbal extracts. Conclusions: Daeshigo-tang, Bojungikgi-tang, Jowiseunggi-tang, and Hoechunyanggyeok-san may show the further glucose-lowering effects on patients with type 2 diabetes mellitus who have already treated with anti-hyperglycemic agents.
본 연구의 목적은 코로나-19 확산 속에서 손 씻기 교육 프로그램 적용이 지적장애 아동들의 손 씻기 빈도와 방법에 미치는 영향을 분석하는데 있다. 연구대상은 지적장애 아동 14명이며, 시각적 영상 자료를 활용한 손 씻기 교육에 참여하였다. 손 씻기 교육 프로그램은 소개, 이론교육, 실습으로 구성되었으며, 매주 2회로 16회기, 총 8주간 실시하였다. 손 씻기 이론교육은 손 씻기 중요성과 적응증, 올바른 손 씻기 단계와 방법, 손 씻기 도구 사용방법, 그리고 손 소독제 특성을 포함한 손 소독제 사용방법, 손 씻기로 인한 피부 손상 예방으로 구성되었다. 손 씻기 평가는 손 씻기 빈도와 방법 관찰 기록지를 활용하여 실시되었다. 손 씻기 빈도는 일상생활의 일과 중에서 손을 씻는 빈도를 평가하였으며, 일별, 월별 빈도를 분석하였다. 손 씻는 방법은 손 씻는 도구사용, 시간, 부위, 손 씻은 후 건조방법으로 구분하여 평가하였다. 그 결과 손 씻기 교육 프로그램 적용 후 대상자들의 손 씻는 빈도가 증가하였고, 손 씻는 방법도 향상되었다. 코로나-19 확산 속에서 적극적인 감염예방 방법으로 손 씻기는 매우 중요하다. 따라서, 본 연구결과를 토대로, 지적장애 아동들의 손 씻기 빈도를 증가시키고, 손 씻기 방법을 향상시키기 위해서 손 씩기 교육의 적용을 고려해야 할 것이다.

Background: The remarkable efficacy of osimertinib in non-small cell lung cancer (NSCLC) with acquired T790M mutation has been widely documented in clinical trials and real-world practice. However, some patients show primary resistance to this drug. Even patients who initially show a favorable response have inconsistent clinical outcomes later. Therefore, the aim of this study was to identify additional clinical predictive factors for osimertinib efficacy. Methods: A prospective cohort of patients with acquired T790M positive stage IV lung adenocarcinoma treated with osimertinib salvage therapy in Hallym University Medical Center were analyzed. Results: Sixty-one eligible patients were analyzed, including 38 (62%) women and 39 (64%) who never smoked. Their mean age was 63.3 years. The median follow-up after treatment with epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) was 36.0 months (interquartile range, 24.7-50.2 months). The majority (n=45, 74%) of patients were deceased. Based on univariate analysis, low baseline neutrophil-to-lymphocyte ratios (NLR), age ≥50 years, never-smoking history, stage IVA at osimertinib initiation, and prolonged response to previous TKIs (≥10 months) were associated with a significantly longer progression-free survival (PFS). Multivariate analysis showed that never-smoking status (hazard ratio [HR], 0.54; 95% confidence interval [CI], 0.30-0.98; p=0.041) and a baseline NLR less than or equal to 3.5 (HR, 0.23; 95% CI, 0.12-0.45; p<0.001) were independently associated with a prolonged PFS with osimertinib. Conclusion: Smoking history and high NLR were independent negative predictors of osimertinib PFS in patients with advanced NSCLC developing EGFR T790M resistance after the initial EGFR-TKI treatment.
본 연구는 키네시오 테이핑 자가 적용 교육 프로그램의 참여를 통해 만성요통환자들의 통증관리 인식과 건강관련 삶의 질의 변화를 알아보는 데 목적이 있다. 본 연구의 대상은 만성요통환자 30명이며, 실험군(n=15)과 대조군(n=15)으로 구분하였다. 실험군은 키네시오 테이핑 자가적용 교육을 적용하였으며, 대조군은 치료사에 의한 테이핑 처리를 적용하였다. 이에 따라, 통증관리 인식의 변화를 분석하기 위해 캐나다작업수행측정 도구를 활용하여 대상자들의 수행도와 만족도의 변화를 측정하였다. 건강관련 삶의 질의 변화를 분석하기 위해서 간이 건강상태 조사 설문지 Short term-8 health survey(SF-8)을 활용하여, 전반적 건강상태, 신체적 기능, 신체적 역할, 신체 통증, 활력, 사회적 기능, 정신건강, 정서적 역할의 변화를 측정하였다. 통증관리 인식의 분석결과 실험군의 수행도 만족도가 통계적으로 유의미하게 향상되었으며, 사후 검사에서 집단간 차이가 나타났다. 건강관련 삶의 질의 분석결과 실험군의 전반적 건강상태, 신체적 기능, 신체적 역할, 신체 통증, 활력, 사회적 기능, 정신건강, 정서적 역할이 통계적으로 유의미하게 향상되었으며, 사후 검사에서 집단간 차이가 나타났다. 키네시오 테이핑 자가적용 교육 프로그램은 일상생활에서 통증을 관리하여 건가관련 삶의 질을 향상시키는데 의미 있는 방법이다. 따라서, 카네시오 테이핑 자가적용 교육 프로그램을 만성요통환자들에게 고려해야 할 것이다.
본 연구의 목적은 입원환자가 낙상사고 전후에 따라 건강상태 관련 특성의 차이를 확인하기 위함이며, 2016년 1월 1일부터 2020년 12월 31일까지 입원 후 낙상사고가 발생하여 환자안전보고시스템에 보고된 환자 328명의 데이터를 활용한 후향적 연구이다. 연구대상자의 연령은 평균 68.57±14.13세로서 70대가 30.49%로 가장 많았고, 입원 이후 평균 13.86±25.03일이 경과 시에 낙상사건이 발생하였고, 발생 시간대는 야간(22:30- 06:59)이 42.99%로 가장 많았다. 입원 후 낙상 전후에 배변문제(x2=314.0, p<.001), 배뇨문제(x2=284.0, p<.001), 정맥수액요법(x2=85.16, p<.001), 걸음걸이의 허약감(x2=69.77. p<.001), 와상(x2=51.60, p<.001), 의식이 불명하거나 자신의 기능을 과대평가한 경우(x2=17.52, p<.001) 비협조적 태도(x2=220.17, p<.001)에서 통계적으로 유의한 차이가 있었다. 낙상 전후 환자의 건강상태 관련 특성의 차이를 파악하여 환자의 특성에 맞는 적절한 낙상예방 및 개별적인 교육중재활동을 모색할 필요가 있다.
목적 : 본 연구의 목적은 국내에서 개발된 아동감각처리척도(Sensory Processing Scale for Children)를 사용하여 일반발달 아동과 감각처리문제 아동의 감각처리 능력을 비교하여 판별타당도를 알아보고자 하였다. 연구방법 : 연구대상자는 3-5세의 일반발달 아동 68명과 감각처리문제 아동 83명의 보호자로 총 151명을 대상으로 하였다. 연구 참여에 동의한 아동의 보호자에게 연구의 목적과 방법을 설명한 후 연구목적, 동의서, 아동감각처리척도(Sensory Processing Scale for Children), 단축감각프로파일(Short Sensory Profile), 일반적 특성 문항을 포함한 설문지를 발송 또는 전달하여 자료를 수집하였다. 분석방법은 아동감각처리척도(Sensory Processing Scale for Children)의 판별타당도를 알아보기 위해 성별을 공변량으로 하여 공분산분석을 실시하였다. 결과 : 연구결과 일반발달 아동과 감각처리문제 아동의 감각처리능력의 차이는 공변량인 성별의 영향(F=6.663, p=.807, partial =η2.000)을 받지 않은 것으로 나타났으며, 두 군에서 감각처리능력에 차이가 있는 것으로 나타났다(F=37.629, p=.000). 아동감각처리척도(Sensory Processing Scale for Children)의 감각영역에서 총점과 모든 감각 영역에서 감각처리문제 아동이 일반발달 아동보다 평균점수가 낮았으며, 고유감각(p=.097) 영역을 제외하고 통계적으로 유의한 차이가 있었다. 아동감각처리척도(Sensory Processing Scale for Children)의 감각 요소에서 감각과민반응, 감각저반응, 그리고 감각구별 요소에서 감각처리문제 아동과 일반발달 아동은 통계적으로 유의미한 차이(p=.000)가 있었으나 감각찾기 요소는 통계적으로 유의한 차이가 없었다(p=.112). 결론 : 아동감각처리척도(Sensory Processing Scale for Children)는 감각처리문제 아동을 선별할 수 있는 판별타당도가 있는 도구로 제시할 수 있으며, 앞으로 임상 환경에서 감각처리의 문제를 선별하거나 분석, 해석, 그리고 중재계획 시 사용할 수 있으리라 생각한다.
유전자가위 CRISPR-Cas9은 유전자편집을 위한 생명과학기술 가운데 하나로서 무한한 잠재력을 지니고 있다. 이는 인류에게 많은 혜택을 줄 수도 있으며, 또는 예측하기 어려운 도전과제를 남겨줄 수도 있다. 유전자편집의 표준 또는 규제를 위한 거버넌스는 이러한 기술로부터 발생할 수 있는 연구자의 과학 발전을 위한 목적과 윤리적 책무 사이의 충돌을 합리적으로 해결해기 위한 절차적 방식이다. 관련 연구자들을 비롯한 그의 연구로부터 영향을 받을 수 있는 당사자들이 거버넌스 절차를 통해서 연구자들의 유전자가위 CRISPR-Cas9을 남용한 연구를 경계하기 위한 그들의 책임의식 향상에 기여하여야 한다. 이러한 거버넌스 절차는 연구자들의 책임의식을 어떻게 효과적이고 실효적으로 고양시켜줄 것인지 확인시켜줄 수 있다. 즉 세계적으로 논의되고 있는 유전자가위를 활용한 인간 배아에 대한 연구가 누구를 위해서 진행되는지 명확히 하여야 한다. 연구를 통한 과학적 호기심의 해결이 유전자가위를 활용한 인간 배아 연구의 목적이 될 수도 있다. 하지만 이러한 연구의 목적인 인간 배아는 단순한 연구의 도구로 취급되어서는 안된다. 그러므로 거버넌스는 유전자치료에 관한 연구를 장려하는 것과 더불어 연구자들의 책임의식을 고양하여야 한다. 그렇지 않으면, 과학 발전만을 위한 연구만 남을 것이고, 이러한 연구는 인류에게 기술을 통한 혜택보다 오히려 불행을 초래할 수 있다. 그러므로 이 글은 연구자의 책임의식을 고양하기 위한 거버넌스를 중심으로 연구하는 것을 내용으로 한다.

제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
