Effect of Gamiondam-tang (GMODT), a Polyherbal Formula on the Pharmacokinetics Profiles of Tamoxifen in Male SD Rats

  • Objectives: The effects of Gamiondam-tang (GMODT) co-administration within 5min on the pharmacokinetics (PK) of tamoxifen were observed as a process of the comprehensive and integrative medicine, combination therapy of tamoxifen with GMODT to achieve synergic pharmacodynamics and reduce toxicity on the breast cancer. Methods: After 50mg/kg of tamoxifen treatment, GMODT 100mg/kg was administered within 5min. The plasma were collected at 30 min before administration, 30 min, 1, 2, 3, 4, 6, 8 and 24 hrs after end of GMODT treatment, and plasma concentrations of tamoxifen were analyzed using LC-MS/MS methods. PK parameters of tamoxifen (Tmax, Cmax, AUC, $t_{1/2}$ and $MRT_{inf}$) were analysis as compared with tamoxifen single administered rats using noncompartmental pharmacokinetics data analyzer programs. Results: Co-administration with GMODT induced increased trends of plasma tamoxifen concentrations to 1hr after end of administration, and then showed decreased trends of plasma tamoxifen concentrations, and especially significant (p<0.05) increases of plasma tamoxifen concentrations were demonstrated at 0.5hr after end of co-administration with GMODT and also related significant (p<0.05) decreases of $AUC_{0-inf}$ and $MRT_{inf}$ as compared with tamoxifen single formula treated rats, at dosage levels of tamoxifen 10 mg/kg and GMODT 100 mg/kg within 5 min, in this experiment. Conclusion: Based on the results of the present study, it is considered that single co-administration GMODT within 5min significantly inhibited the oral bioavailability of tamoxifen through variable influences on the absorption and excretion of tamoxifen, can be influenced on the toxicity or pharmacodynamic of tamoxifen.

Effects of Oriental Nursing Intervention Program for Health Promotion of the Elderly in the Community (지역사회 재가노인의 건강증진을 위한 한방간호중재 프로그램의 적용효과)

  • Purpose: This study was carried out to examine the applied effects of an oriental nursing intervention program on the physical health status, activities of daily living (ADL), depression, life satisfaction, and Yangseng of the elderly living in the community. Methods: This was a quasi-experimental study using a non-equivalent control group pretest-posttest. Study participants were total 79 elderly (experimental: n=39, control: n=40) who aged 65 or over with normal cognitive status and available communication in D community, Seoul, South Korea. They were recruited by convenient sampling, and assigned randomly. The oriental nursing intervention program composed Pal-Dan-Gum as Qi-gong therapy and Kyunglak exercise. This program consisted of 20 sessions total for 10 weeks. Measures were CMI(Cornell Medical Index), Instrumental ADL (Activities of Daily Living), GDS(Geriatric Depression Scale), geriatric life satisfaction scale, and Yangseng scale. Data were analyzed by SPSS WIN21.0 using descriptive statistics, x2-test, and independent t-test. Results: There were statistically significant differences in the mean scores of physical health status (t=10.295, p<.001), ADL (t=7.571, p<.001), depression (t=-15.434, p<.001), life satisfaction (t=21.257, p<.001), and Yangseng (t=9.527, p<.001) between intervention and control groups. Conclusions: Oriental nursing intervention program was effective for improving the physical health status, ADL, depression, life satisfaction, and Yangseang of the elderly living in the community. This intervention program can be implied for health promotion of elderly living in the community.

The Administration Patterns of Analgesics and Sedatives for Patients in SICU (외과중환자를 위한 진통제와 진정제의 투여 양상)

  • Major purposes of this study were to investigate the administration patterns of analgesics and sedatives in SICU and to identify the factors influencing the use of prn analgesics and sedatives by ICU nurses. The sample of this descriptive study was 50 adult patients in SICU and 53 ICU nurses. Patient's medical records were reviewed to investigate names, doses, the routes of administration, the interval of administration, and the type of prescription of sedatives and analgesics administered. Study medications were narcotics, hypnotics, and antipsychotics. To identify the factors influencing the use of prn analgesics and sedatives, 53 ICU nurses checked 9 items, and rank them from first to fifth. The selection of items was based on the previous studies and the experience of the investigator. The results of the study are as follows: 1. The mean age of the subjects was 53 years, 24 patients out of 50 subjects had received mechanical ventilation therapy. Most of the patients received neurosurgeries and abdominal surgeries. 2. For 4 days, 13 total study medications and combination of these were administered to the patients. Commonly prescribed drugs were Ketoprofen and Midazolam. Twenty six to fourty two percent of the patients did not receive any drugs for at least one day during the four days. 3. On the average, the study drugs were administerd 1.4 to 2.6 times per day during 4 days. 4. More than 50 percent of the prescription was as-needed (prn) except those of the POD 3. Fourteen percent of the patients did not have any prescription for sedation and pain control after surgery. 5. Examination of the frequency of sedatives and analgesics bolus administration revealed that a greater number of doses were given during daytime (from 7 am. to 7 pm.) than nighttime (from 7 pm. to 7 am.). The difference was significant at Alpha, .05. 6. First factor that most influenced nurses to administer sedatives and analgesics for intubated patients was the evaluation of patient's vital signs (51%). For non- intubated patients, the factors that nurses considered important were the patients' complaints of pain (64%) and evaluations of patients' vital signs (23%). In conclusion, the results of this study indicate that patients in SICU might not receive enough analgesics and sedatives to feel completely free from pain during the post operational period. Future study should be focused on the evaluation of the adequacy of current practice for pain and anxiety control in terms of the SICU patient's response.

An Analysis of Nursing Decision Tasks, Characteristics, and Problems with Decision Making (환자 간호에 대한 간호사의 의사결정 내용과 특성 및 의사결정 장애요인에 관한 분석)

  • The purpose of this study was to describe nursing decision tasks, their characteristics, and problems associated with decision making. The subjects were 32 nurses who had at least one-year nursing experience and worked on medical-surgical units or intensive care units(ICU). They were asked to describe their decision making experiences in patient care situations and to identify the characteristics of each decisions. They were also asked to describe perceived problems associated with decision making in nursing. The responses on nursing decision tasks and problems were analyzed with content analysis and the decision characteristics were identified by statistical analysis of variance. It was found that there were 16 nursing decisions which are as follows : decisions related to interpreting and selecting appropriate strategies for pain management(6.6%) ; decisions related to providing emotional support (0.7%) ; decisions related to explaining the patient's condition and rationale for procedures(1.1%) ; decisions related to assisting patients to integrate the implications of illness and recovering into their lifestyles(2.9%) ; decisions related to detecting significant changes In patients and selecting appropriate intervention strategies (17.2%) ; decisions related to anticipating problems and selecting preventive measures(4.2%) ; decisions related to identifying emergency situations(0.4%) ; decisions related to effective management of patient crisis until physician assistance becomes available(2.8%) ; decisions related to starting and maintaining intravenous therapy(2.6%) ; decisions related to administering medications(8.1%) ; decisions related to combating the hazards of immobility(7.3%) : decisions related to treating wound management strategies(5.5%) ; decisions related to relieving patient discomfort(13.9) ; decisions related to selecting appropriate strategy according to the changing situation of the patient(18.2%) ; decisions related to selecting the best strategy for patient management(5.3%) ; and decisions related to coordinating, ordering, and meeting the various needs of the patient (3.1%). The nurses reported the fellowing problems in decision making : difficulties due to lack of knowledge and experience (18.6%) ; uncertainty and complexity of decision tasks(15.2%) ; lack of time to make decisions(2.9%) ; personal values which conflict with other staff(15.7%) ; lack of selection autonomy(30.0%) ; and organizational barriers(7.6%). Continuing education programs and decision support systems for frequent nursing decision tasks can be established on the basis of these results. Then decision ability in nurses will increase through the education programs and decision support systems, and then quality of nursing service will be better.

  • Trigeminal neuralgia is defined as "a sudden, usually unilateral, brief stabbing recurrent pain in the distribution of one or more branches of the fifth cranial nerve" by the International Association for the Study of Pain(IASP). Trigeminal neuralgia is classified as an idiopathic trigeminal neuralgia with no apparent cause and a symptomatic trigeminal neuralgia which is caused by a structural lesion such as brain tumor. Over 80% of the tumors are meningioma, acoustic neuroma, and epidermoid tumors. Symptomatic trigeminal neuralgia can not be excluded even if old-aged patient does not have abnormal neurologic sign and symptom, and good response to pharmacotherapy. Therefore, initial examinations such as MRI or CT are essential to exclude symptomatic trigeminal neuralgia. When compared with CT, MRI, especially gadolinium enhanced MRI, has an increased sensitivity in the detection of intracranial lesions. The most effective medical treatment of trigeminal neuralgia is carbamazepine. The most common side effects of carbamazepine include drowsiness, dizziness, unsteadiness, nausea, anorexia. Hepatotoxicity, bone marrow depression are the most feared side effect of carbamazepine therapy but occurs rarely. It require periodic complete blood cell counts as well as hepatic and renal function tests. It has been recommended that complete blood cell counts is done every 2 weeks for the first 2months and then quaterly thereafter. Oxcarbazepine can be used if neutropenia occurs.

The effect that the composition handling of aromatherapy and abdominal massage treatment reaches the middle-aged woman's body ingredient transformation (향요법과 복부마사지의 복합처치가 중년 여성의 체 성분 변화에 미치는 영향)

  • The purpose of this study is to determine the effect of aromatherapy, showing how the composition handling of aromatherapy and abdominal massage treatment reaches body ingredient transformation. The subjects for this research were total 18 middle-aged females in Seoul; aroma massage group of 7 females, aroma inhalation and water bathe group of 5 females, abdominal massage group of 6 females by jojoba oil without any medical effect. This clinic trial was held from July 1, 2008 to Aug.14, 2008. I held this clinic trial under the same condition after and before this clinic. A standard tape and OLYMPIA 3.5 of S hospital were used at the body measuring for subjects after and before clinic trials. I got Average and standard deviation by data analysis by SPSS Win. Ver.14.0. I did paired t-test for the comparison of before and after, and repeated measure ANOVA for between two groups or among three groups'. The verification was held Duncan Test. The results of this study were as follows: 1. Body mass quotient (F=2.86, p= .063) and Body region (F=1.34, p= .279) among three groups showed no meaningful difference, but weight meaningful difference and aroma massage group showed the greatest difference of body measure change quantity. 2. In change quantity of abdomen girth, Waist circumference and WHR, abdomen girth (F=4.56, p= .012) and Waist circumference (F=4.37, p= .031) showed a meaningful statistical difference. The result of subsequent inspection showed that there was a meaningful difference among three groups and aroma massage group was best. 3. In Cell quantity, Body region quantity and Muscle volume, Body region quantity (F=2.76, p= .182) and Muscle volume (F=3.12, p= .054) showed no difference, but Cell quantity (F=3.79, p= .040) showed a meaningful difference. In the comparison of three groups there was no difference, but aromatherapy group showed more change quantity than any other group. According to the result of this study, the composition handling of aromatherapy and body massage was effective in the decrease of Abdominal fatness and Waist circumference, Weight and the increase of Cell quantity. so I suggest that woman use this therapy in the program of obesity management for her health improvement.

The effects of music listening, autogenic training, and music-assisted autogenic training on the quality of life, relaxation responses, and daily living of migraine patients

  • The purpose of this study was to investigate the effect of music listening, autogenic training, and music-assisted autogenic training on the quality of life, physiological and psychological relaxation responses, and daily living in a population of migraine patients. Forty migraine patients, ranging 20 to 60 years, were referred to the researcher by their physicians and participated in the study. A convenience control-group pretest-posttest design was employed. The participants were randomly assigned to one of four groups: music listening (ML), autogenic training (AT), music-assisted autogenic training (MAT), or a control group participants, with n = 10 participants per group. The participants in experimental groups received four 30-minute sessions with an assigned treatment, once a week during a 4-week experiment period. The participants in the control group continued their regular medical treatment as prescribed by the doctor without receiving any other relaxation treatment. However, they were still aware of the research and their responsibilities for the study. The Migraine Assessment (MIDAS) and Migraine-Specific Quality of Life (MSQOL) questionnaire were used to investigate a migraine patients' quality of life collected before and after the 4-week experiment for all subjects. The physiological and psychological relaxation responses of migraine patients were measured by relaxation levels and forehead temperature recorded before and after each treatment session for the participants in three treatment conditions. The effect of the relaxation treatments on daily living of migraine patients was examined through the frequency and intensity of migraine headaches, and the amount of medication taken for migraine headaches during the 4-week experiment as recorded in participants' diary for all participants. The results found significant differences from pre- to posttest on the MIDAS, MSQOL, and relaxation levels while no significant was found among the groups. The analysis of forehead temperature showed no significant difference from pre- to posttest and among the groups. A one-way ANOVA was performed on the frequency, intensity, and amount of medication taken for migraine headaches during the 4-week experiment period. While results yielded no significant difference among the groups, the data indicate that the participants in the three treatment groups reported fewer migraine headaches, lower degrees of headache intensity, and less medication taken for migraine headaches than participants in the control group. A conclusion drawn from this study is that music listening itself as a relaxation treatment, or as an adjunct to other relaxation techniques can be effective in the treatment of migraine headaches.

Comparison of Film Measurements, Convolution$^{}$erposition Model and Monte Carlo Simulations for Small fields in Heterogeneous Phantoms (비균질 팬텀에서 소조사면에 대한 필름측정, 회선/중첩 모델과 몬테 카를로 모사의 비교 연구)

  • Intensity-modulated radiation therapy (IMRT) often uses small beam segments. The heterogeneity effect is well known for relatively large field sizes used in the conventional radiation treatments. However, this effect is not known in small fields such as the beamlets used in IMRT. There are many factors that can cause errors in the small field i.e. electronic disequilibrium and multiple electron scattering. This study prepared geometrically regular heterogeneous phantoms, and compared the measurements with the calculations using the Convolution/Superposition algorithm and Monte Carlo method for small beams. This study used the BEAM00/EGS4 code to simulate the head of a Varian 2300C/D. The commissioning of a 6MV photon beam were performed from two points of view, the beam profiles and depth doses. The calculated voxel size was 1${\times}$1${\times}$2$\textrm{cm}^2$ with field sizes of 1${\times}$1$\textrm{cm}^2$, 2${\times}$2$\textrm{cm}^2$, and 5${\times}$5$\textrm{cm}^2$. The XiOTM TPS (Treatment Planning System) was used for the calculation using the Convolution/Superposition algorithm. The 6MV photon beam was irradiated to homogeneous (water equivalent) and heterogeneous phantoms (water equivalent + air cavity, water equivalent + bone equivalent). The beam profiles were well matched within :t1 mm and the depth doses were within ${\pm}$2%. In conclusion, the dose calculations of the Convolution/Superposition and Monte Carlo simulations showed good agreement with the film measurements in the small field.

Clinical Approaches to Vascular Anomalies of the Lip

  • Background The incidence of vascular anomalies in the head and neck is higher than in the extremities. It is especially common for vascular anomalies to involve the lip. The lips are a functionally and aesthetically important component of the head and neck area. A retrospective analysis of data from our vascular anomaly center was performed in order to understand the characteristics and treatment requirements of vascular anomalies of the lip and to establish which treatments are likely to lead to the best outcomes. Methods A retrospective review was performed of the medical records of patients diagnosed with vascular anomalies of the upper or lower lip from January 2001 to September 2013. Using clinical photographs, radiologic findings, and patient records, the diagnosis of each case and the location of the vascular anomaly were recorded, along with information about treatment and follow-up. Results A total of 1,606 patients were diagnosed with vascular anomalies over this time period, of whom 127 (7.9%) were found to have vascular anomalies in the lip only. Surgical treatment with or without embolization, sclerotherapy, laser therapy, medication only, and observation were the treatment strategies adopted in these cases. Conclusions Vascular anomalies of the lip should be diagnosed accurately. Radiologic diagnosis played a crucial role in treatment planning, and several techniques were used to treat vascular anomalies of the lip. When surgical excision is indicated for the correction of vascular anomalies of the lip, the aesthetic and functional characteristics of the lip should be considered.

Prognosis of Malignant Melanoma on Lower Extremity (하지에 발생한 악성 흑색종의 예후)

  • Purpose: Malignant melanoma is recognized as the most serious skin cancer. We examined anatomical distribution and 5 - year survival rate of each stage of malignant melanoma on lower leg. Methods: We retrospectively analyzed the medical records of 91 patients(46 males and 45 females) with malignant melanoma on lower leg from 1985 to 2008. Age, sex, anatomical distribution and 5 - year survival rates of each stage of malignant melanoma on lower leg were investigated. Also, 5 - year survival rates of each stage and invasion depth of malignant melanoma on heel pad were investigated. Results: On lower leg, most frequently 32 cases(35.1%) occurred on heel pad, 27 cases(29.7%) occurred on dorsum of foot, 18 cases(19.8%) in toe and 14 cases(15.4%) on others in lower leg. We used the excision margin as 3 ~ 5 cm. After wide excision, in stage III, IV, the patients underwent the immunologic / chemo - therapy. The incidences of each stage were 22 cases(24.2%) in stage I, 47(51.6%) in II, 17(18.7%) in III and 5(5.5%) in IV. The 5 - year survival rates of each stage were 85%, 53.2%, 47.1% and 40%. On heel pad, the incidences of each stage were 5 cases(15.6%) in stage I, 19 cases(59.4%) in II, 7 cases(21.9%) in III and 1 case(3.1%) in IV. The 5 - year survival rates of each stage were 80%, 63.2%, 42.9% and 100%. On heel pad, incidence of local recurrence was 2 and 5 - year survival rate of this case was 100%. And systemic recurrence was 9 and 5 - year survival rate of this case was 55.6%. Conclusion: The 5 - year survival rate of malignant melanoma on heel pad was higher than previous study. To maintain the weight - bearing function of foot, we recommend the active reconstructive surgery for heel pad reconstruction after wide excision of heel pad malignant melanoma.
