부산권 치과위생사의 정서적 조직몰입 영향 요인 분석 -근로생활의 질을 중심으로- (Analysis of Impact Factors of Affective Commitment in Organization of Dental Hygienist in Susan Area -Focused on Quality of Working Life(QWL)-)

  • 이 연구는 치과위생사의 정서적 조직몰입에 영향을 미치는 근로생활의 질 요인을 분석함으로써 이들의 근로생활의 질 향상과 더불어 조직에의 애착과 몰입을 추구하고 근로생활의 보람과 궁극적인 삶의 만족도를 높이는 방안을 찾고 치과경영에서의 인적관리의 방향성을 제시하고자 하였다. 조사대상은 부산 경남 지역에 위치한 치과 의료기관에 근무하는 치과위생사를 대상으로 설문조사 하였고 197부의 설문지를 연구에 이용하였다. 연구결과 근로생활의 질과 정서적 몰입의 구성요소 간 요인별로 높은 상관관계를 보였고, 정서적 조직몰입에 영향을 미치는 근로생활의 질 요인 분석 결과 근무환경, 인간관계, 사회적 유익성이 정서적 조직몰입에 유의한 영향을 미쳤다. 이에 치과 의료기관은 인적 경영관리 차원에서 근무환경 및 복리후생 개선과, 직원 간 원활한 인간관계를 형성할 수 있도록 하며, 정기적 사회봉사나 지역주민을 위한 서비스 제공으로 자긍심과 애사심을 고취시키고, 치과위 생사들은 보다 긍정 적 인 생각으로 근무기관과 자기발전을 위해 끊임 없는 노력으로 업무능력 향상과 직업적 가치를 높이는 것이 중요하다고 생각된다.

간호사의 직장 내 괴롭힘 관련 온라인 뉴스기사 댓글에 대한 토픽 모델링 분석 (A Topic Modeling Analysis for Online News Article Comments on Nurses' Workplace Bullying)

  • Purpose: This study aimed to explore public opinion on workplace bullying in the nursing field, by analyzing the keywords and topics of online news comments. Methods: This was a text-mining study that collected, processed, and analyzed text data. A total of 89,951 comments on 650 online news articles, reported between January 1, 2013 and July 31, 2018, were collected via web crawling. The collected unstructured text data were preprocessed and keyword analysis and topic modeling were performed using R programming. Results: The 10 most important keywords were "work" (37121.7), "hospital" (25286.0), "patients" (24600.8), "woman" (24015.6), "physician" (20840.6), "trouble" (18539.4), "time" (17896.3), "money" (16379.9), "new nurses" (14056.8), and "salary" (13084.1). The 22,572 preprocessed key words were categorized into four topics: "poor working environment", "culture among women", "unfair oppression", and "society-level solutions". Conclusion: Public interest in workplace bullying among nurses has continued to increase. The public agreed that negative work environment and nursing shortage could cause workplace bullying. They also considered nurse bullying as a problem that should be resolved at a societal level. It is necessary to conduct further research through gender discrimination perspectives on nurse workplace bullying and the social value of nursing work.

Dental Hygienists in Japan

  • The purpose of this study was to determine the status of dental hygienists in Japan. The study explicated the history, core curriculum, National Examination for Dental Hygienists, work roles of dental hygienists, and workplaces of dental hygienists. This study was based on the government policy report on dental hygienists and the information published by each public institution. The latest statistics presented by the institutions were collected through official websites. The employment information of graduates from Hiroshima University was analyzed based on actual field study. The results of the study revealed that social demand for dental hygienists has steadily increased and policies and education have been revised accordingly. The work roles of dental hygienists have expanded to meet the needs of the treatment and those of public health fields. In line with major policy changes, the educational period has been extended from 1 year to 3 or 4 years, while the mandatory credits for graduation have been established. Licensing examinations were being performed by the local governments since 1948 due to the different situations of dental hygienists in different areas. In 1992, they were converted into a single national examination. The work roles have expanded from assisting dental treatments to health guidance, home care, and perioperative care. Consequently, the number of dental hygienists has increased, especially in healthcare facilities for the elderly. Dental hygienists perform various roles. However, the most essential role is to provide the best oral care services to the patient. The expected role of dental hygienists has expanded in alignment with public healthcare needs and support for the elderly. The government and universities are expected to bring about improvements such as diversifying the channels of education and establishing policies to respond to growing patient needs by cultivating excellent dental hygiene professionals.

개원 전문의 소득에 영향을 미치는 요인분석 (Analysis of influencing factors on self-employed physician's income)

  • This study describes the relation of physician's income and price of medical service and social welfare through microeconomic view, reviews the literature of influencing factor on physician's income, and it describes general distribution of physician's income, and analyzes influencing factor of physician's income. A total of 844 persons responded to the mail survey, through stratified sampling by 23 branches of medical society in Korean RBRVS study. The design of the study is cross sectional study, and the unit of analysis is a physician. To examine the change of average income per month, multiple regression was used to test the change according to physician's characteristics, demographic characteristics, scale of clinic, average intensity of ordinary work, and specialty. The major findings of this study are as follows; 1. As for self-employed physicians, the difference of average income per month among specialties was mcreased \4,850,000, but the difference was \6,020,000 under the control of control variables. 2. The number of average out-patients per month and number of nurses and nursing aides significantly positively associated, and average income per month was significantly higher for physicians who had sick-beds than physicians who had no sick-beds. In conclusion, the number of out-patient and number of nurses and nursing aides is the major influencing factor, and the difference of average income per month among specialties existed in self-employed physicians. So this study suggests basic hypothesis that the price of medical service and supply of physician by specialties are not pertinent. Being a cross-sectional study, this study can not suggest causal explanations. In the future, further study is needed for causal explanations.

  • This research is purposed to find Dong-mu's thought on 'philosophy and medical interrelationship' in his writings. It was researched with Dong-mu's chief medical writings such as ${\ulcorner}$Gyukchigo(格致藁)${\lrcorner}$ , ${\ulcorner}$Dongyi Soose Bowon(東醫壽世保元) ${\lrcorner}$ , ${\ulcorner}$Dongyi Soose Bowon Sasang Chobongyun(東醫壽世保元四象草本卷)${\lrcorner}$ , ${\ulcorner}$Dong-mu's posthumos work(東武遺稿)${\lrcorner}$ Dong-mu describe his philosophy conception with the system of The Book of change(周易). But he changes the meaning of Taegeuk(太極), LangYui(兩儀), Sasang(四象) on the respect of human being. Gyuchigo(格致藁) can be understood as social behavior pattern of Sasang constitute. Affairs- Mind - Body-Objects(事心身物) was distributed at Heaven-Human -Nature-Order(天人性命) on the respect of Human Moral. In the respect of Form-Use(體用), It is assigned to Form(體) and metaphysical philosophy. After The theory of Sa-Dan(四端論) in ${\ulcorner}$Dongyi Soose Bowon(東醫壽世保元) ${\lrcorner}$, It is focused on Nature-Emotion(性情). Affairs - Mind - Body - Objects(事心身物)is distributed at Heaven-Human -Nature-Order (天人性命) on the respect of human internal mind. In the respect of Form-Use(體用), It is assigned to Use(用) and Physical science. Sasang Constitution can be expanded to Eight COnstitution in the respect of dominant position among Nature(性) and Emotion(情).

지역사회기반 호스피스완화케어에서의 의료지원서비스 요구분석 (A Need Analysis for Medical Supporting Service as a Part of Community-based Hospice Palliative Care)

  • Objectives : This study was conducted to investigate the need of medical supporting service (MSS) as a part of community-based hospice palliative care from the view point of beneficiaries and providers. Methods : This study adopted a methodological triangulation design. A questionnaire regarding intention to use MSS was completed by 175 patients under home-based cancer patient management program. And three focus groups consisted of hospice nurses, public health physicians, and public officials were interviewed to obtain the perceived needs, obstacles, and solutions of MSS. Results : Mean age of home-based cancer patient was 70.18 year old, 48.0% of them were living alone. Only 53.7% of them were treated pain and 93.7% intend to take pain medication prescribed by public health physician. All participants of focus group interviews agreed necessity and importance of MSS. Physicians' lack of confidence and unwillingness to prescribe opioid to terminal patients was the biggest obstacle to provide MSS in the public health center. Conclusions : The necessity and demand of MSS for community-dwelling cancer patients were verified. MSS is urgent issue to meet their needs.

Predicting Ability of Dynamic Balance in Construction Workers Based on Demographic Information and Anthropometric Dimensions

  • Background: Difficulties in walking and balance are risk factors for falling. This study aimed to predict dynamic balance based on demographic information and anthropometric dimensions in construction workers. Methods: This descriptive-analytical study was conducted on 114 construction workers in 2020. First, the construction workers were asked to complete the demographic questionnaire determined in order to be included in the study. Then anthropometric dimensions were measured. The dynamic balance of participants was also assessed using the Y Balance test kit. Dynamic balance prediction was performed based on demographic information and anthropometric dimensions using multiple linear regression with SPSS software version 25. Results: The highest average normalized reach distances of YBT were in the anterior direction and were 92.23 ± 12.43% and 92.28 ± 9.26% for right and left foot, respectively. Both maximal and average normalized composite reach in the YBT in each leg were negatively correlated with leg length and navicular drop and positively correlated with the ratio of sitting height to leg length. In addition, multiple linear regressions showed that age, navicular drop, leg length, and foot surface could predict 23% of the variance in YBT average normalized composite reach of the right leg, and age, navicular drop, and leg length could predict 21% of that in the left leg among construction workers. Conclusion: Approximately one-fifth of the variability in the normalized composite reach of dynamic balance reach among construction workers using method YBT can be predicted by variables age, navicular drop, leg length, and foot surface.

간호사의 내부지향적 정책, 사회적지지 및 직무배태성이 이직의도에 미치는 영향 (Effects of Internal Oriented Policy, Social Support, and Job Embeddedness by Turnover Intention of Nurses)

  • 본 연구는 간호사의 내부지향적 정책, 사회적지지 및 직무배태성이 이직의도에 미치는 영향에 대해 알아보기 위한 서술적 조사연구이다. 연구대상자는 연구 참여에 동의한 D광역시에 위치한 2개 대학병원 간호사들이며, 자료 수집은2018년 10월 2일부터 2019년 3월 20일 까지 실시되었다. 자료 분석은 SPSS 22.0 프로그램을 이용하였으며, 빈도와 백분율, 평균과 표준편차, 상관분석, t-test, ANOVA, Pearson's correlation, Multiple regression을 이용하였다. 연구결과 내부지향적 정책은 3.04점, 사회적지지는 상사 3.08점, 동료 3.81점, 가족 3.67점이었고, 직무배태성은 2.98점, 이직의도는 3.55점이었으며, 내부지향적 정책, 사회적지지 및 직무배태성이 높을수록 이직의도는 감소하는 것으로 나타났다. 일반적 특성에서 이직의도에 미치는 요인을 파악한 결과 임상경력과 근무부서가 유의미한 결과를 보였다. 임상경력은 높을수록 이직의도가 높았고, 근무부서는 외과병동, 내과병동, ICU, OR순으로 이직의도가 높았다. 이상의 결과를 바탕으로 간호사의 이직의도를 낮추기 위한 중재방안으로 병원 내부지향적 정책, 사회적지지 및 간호사의 직무배태성을 높이기 위한 프로그램 개발 및 교육이 적극적 이루어져야 할 것이다.

미국과 캐나다 노인밀집도시의 노인주거관련 사회적지원에 관한 연구 : 농촌지역 소도시를 중심으로 (A Study on Social Supports for the Elderly Housing in Senior Concentrated Cities in the United States and Canada : Focused on Small Cities along Rural Counties)

  • The purpose of this study is to explore social supports for elderly housing and their residential lives in small cities along rural counties of the United States and Canada, and suggest future implications for age-concentrated rural villages in Korea. In this study, five small and medium cities in non-metropolitan counties of California and Ontario province were visited and elderly residents and service experts were interviewed about their perceptions of community integrated social support networks for senior residences. The senior housing complexes were built due to influx of both metropolitan and rural residents seeking warm localities, traffic connections, business purposes in active production areas. and leisure attractions. There are five main social support networks for senior housing issues in these areas. First, the areas are claimed for senior zones and accordingly health industries are encouraged by local authorities. Second, the community is homogeneously constructed as a senior friendly environment and include features such as an RV park and mobile cottages. Third, senior-helping seniors are offered active work through golf-cluster active retirement communities. Fourth, traditional theme production camps are mobilized by the elderly workers. Lastly, an information system is maintained for screening volunteers and for senior abuse prevention. On the other hand, residential lives are occasionally negatively influenced by unbalanced concentrations of elderly facilities such as nursing stations and funeral homes. For the future of Korean rural elderly policies, suggestions are made as follows: first, an integrated urban and rural township that contains attractive places for early retiring people who seek a warm atmosphere in later life needs to be constructed. Second, an integrated model retirement village of urban and rural retirement life needs to be initiated as a measure of evaluating the adaptation process of movers in senior concentrated zones. Third, a cooperation system among governmental ministries needs to be formed with the long- term goal of establishing a traditional rural town of independent housing districts and medical facilities in rural areas. Fourth, productive and active lifestyles need to be maintained as the local community and government develop successful retirement rural villages, by limiting the expansion of nursing related facilities. Finally, generation integrated visiting welfare programs and services need to be further developed for the housing areas especially in the winter, when social integration and activity are relatively low.

작업환경실태조사 및 작업환경측정자료(2018~2022) 결과를 활용한 우리나라 전자산업에서의 고위험물질 노출 특성 -디클로로메탄, 트리클로로메탄, 수산화테트라메틸암모늄 중심으로- (Characteristics of Exposure to High-Risk Substances in the Electronics Industry Using the Work Environment Survey and Work Environment Measurement Database (2018~2022) in South Korea -Dichloromethane, Trichloromethane, and Tetramethylammonium Hydroxide-)

  • Background: Social interest is increasing due to frequent accidents caused by chemicals in the electronics industry. Objectives: The purpose of this study is to present a management plan by evaluating the exposure characteristics of dichloromethane (DCM), trichloromethane (TCM), and tetramethyl ammonium hydroxide (TMAH), which are high-risk substances to which people may be exposed in the electronics industry in South Korea. Methods: To investigate the handling companies and status of the hazardous chemicals DCM, TCM, and TMAH, the handling status of the three substances was classified based on electronics industry-related codes from the 2019 Work Environment Survey (Chemical Handling and Manufacturing) data with work environment measurement results for five years. Results: DCM, TCM, and TMAH are commonly used as cleaning agents in the electronics industry. For DCM, it was found that all work environment measurement results from 2018 to 2021 but not 2022 exceeded the exposure standard. Conclusions: Identifying the distribution channels of hazardous chemicals is an intervention point that can reduce exposure to hazardous chemicals. It requires management through tracking systems such as unique verification numbers at the import and manufacturing stages, and proper cultivation of and related support for handling chemicals by business managers.
