Analysis of Image Distortion on Magnetic Resonance Diffusion Weighted Imaging

  • The purpose of this study is to improve diagnostic efficiency of clinical study by setting up guidelines for more precise examination with a comparative analysis of signal intensity and image distortion depending on the location of X axial of object when performing magnetic resonance diffusion weighted imaging (MR DWI) examination. We arranged the self-produced phantom with a 45 mm of interval from the core of 44 regent bottles that have a 16 mm of external diameter and 55 mm of height, and were placed in 4 rows and 11 columns in an acrylic box. We also filled up water and margarine to portrait the fat. We used 3T Skyra and 18 Channel Body array coil. We also obtained the coronal image with the direction of RL (right to left) by using scan slice thinkness 3 mm, slice gap: 0mm, field of view (FOV): $450{\times}450mm^2$, repetition time (TR): 5000 ms, echo time (TE): 73/118 ms, Matrix: $126{\times}126$, slice number: 15, scan time: 9 min 45sec, number of excitations (NEX): 3, phase encoding as a diffusion-weighted imaging parameter. In order to scan, we set b-value to $0s/mm^2$, $400s/mm^2$, and $1,400s/mm^2$, and obtained T2 fat saturation image. Then we did a comparative analysis on the differences between image distortion and signal intensity depending on the location of X axial based on iso-center of patient's table. We used "Image J" as a comparative analysis programme, and used SPSS v18.0 as a statistic programme. There was not much difference between image distortion and signal intensity on fat and water from T2 fat saturation image. But, the average value depends on the location of X axial was statistically significant (p < 0.05). From DWI image, when b-value was 0 and 400, there was no significant difference up to $2^{nd}$ columns right to left from the core of patient's table, however, there was a decline in signal intensity and image distortion from the $3^{rd}$ columns and they started to decrease rapidly at the $4^{th}$ columns. When b-value was 1,400, there was not much difference between the $1^{st}$ row right to left from the core of patient's table, however, image distortion started to appear from the $2^{nd}$ columns with no change in signal intensity, the signal was getting decreased from the $3^{rd}$ columns, and both signal intensity and image distortion started to get decreased rapidly. At this moment, the reagent bottles from outside out of 11 reagent bottles were not verified from the image, and only 9 reagent bottles were verified. However, it was not possible to verify anything from the $5^{th}$ columns. But, the average value depends on the location of X axial was statistically significant. On T2 FS image, there was a significant decline in image distortion and signal intensity over 180mm from the core of patient's table. On diffusion-weighted image, there was a significant decline in image distortion and signal intensity over 90 mm, and they became unverifiable over 180 mm. Therefore, we should make an image that has a diagnostic value from examinations that are hard to locate patient's position.

치과 방사선 검사에서 유효선량 평가 (Evaluation of Effective Dose in Dental Radiography)

  • 과학 기술의 발달로 인해 의료 방사선 장비 또한 첨단화 되고 있으며 정확한 진단과 치료를 할 수 있게되어 의료의 질적 향상은 물론 양적인 면에서도 그 사용이 날로 증가하고 있는 추세이다. 특히 환자 피폭선량에 대부분을 차지하는 CT(Computed Tomography) 촬영의 건수는 크게 증가하고 있다. 치과 방사선 검사의 경우 정확한 병변 진단을 위한 CT 및 CBCT(Cone Beam Computed Tomography) 의 도입으로 과거에 비해 환자는 많은 선량을 받게 되었다. 본 연구에서는 인체 팬텀과 TLD-100H를 이용하여 치과 방사선 검사별 조직의 흡수선량을 측정하고 ICRP 60, 103에 따라 유효선량을 계산하였다. ICRP 60, 103에 따른 유효선량 값은 파노라마 검사의 경우 $5.1{\mu}Sv$, $29.5{\mu}Sv$, 세팔로 검사의 경우 $11.2{\mu}Sv$, $14.4{\mu}Sv$, 이며 CBCT는 상악골의 경우 $53.7{\mu}Sv$, $209.6{\mu}Sv$, 하악골은 $129{\mu}Sv$, $391.5{\mu}Sv$ 그리고 CT에서는 상악골의 경우 $93.3{\mu}Sv$, $139.5{\mu}Sv$이며 하악골은 $282.7{\mu}Sv$, $489.7{\mu}Sv$로 평가 되었다. CBCT 검사의 경우 유효선량은 CT 검사에 비해서는 적지만 파노라마 검사 및 세팔로 검사에 비해서는 높게 평가되었다.

선형가속기를 기반으로 한 뇌정위 방사선 수술 시 전용 콘과 정방형 소조사면의 선량 특성에 관한 고찰 (Evaluation of Dosimetric Characteristics of Small Field in Cone Versus Square Fields Based on Linear Accelerators(LINAC) for Stereotactic Radiosugery(SRS))

  • 선형가속기를 기반으로 하는 뇌정위적 방사선 수술에 있어 전용 콘과 동일한 정방형 조사면에서의 소조사면 선량특성에 관해 알아보고자 6 MV 광자선과 소조사면 전용 검출기를 이용해 측정하였다. 선원 표면간 거리는 100 cm로 하고, 전용 cone ${\Phi}1\;cm$, ${\Phi}2\;cm$, ${\Phi}3\;cm$과 정방형 조사면 $1{\times}1\;cm^2$, $2{\times}2\;cm^2$, $3{\times}3\;cm^2$ 조사면 으로 1.5 cm, 5 cm, 10 cm, 20 cm, 30 cm 지점에서 심부선량을 측정하였고, 최대 선량점에서 각 조사면에서 출력선량을 측정하여 비교하였다. 반음영의 측정을 위한 선량측면도 측정은 ${\Phi}1\;cm$, ${\Phi}3\;cm$ 및 동일한 정방형 조사면에서 선원 표면간 거리 95 cm, 깊이 5 cm에서 SFD를 이용하여 측정하였다. 물리학적 조사면(90%) 내에서는 1 mm 간격으로, 반음영 영역(20~80%)에서는 0.5 mm 간격으로 측정하였다. 전용 콘 과 정방향 조사면의 PDD 변화는 기준조사면을 기준으로 5 cm 깊이에서 4.3~7.9% 낮게 나타났으며, 콘의 크기와 조사면 크기가 커질수록 PDD 변화는 적게 나타났다. 선질 비교를 위해 $PDD_{20,10}$를 비교한 결과 ${\Phi}1\;cm$ 콘과 $1{\times}1\;cm^2$을 제외한 다른 조사면에서는 1% 이내의 변화를 보였다. 출력선량은 콘을 이용한 방법에서 정방형 조사면에서 보다 3.1~4.6% 출력선량이 증가하였다. 반음영 영역은 ${\Phi}1\;cm$, ${\Phi}3\;cm$ 동일한 정방형 조사면에서 콘을 이용한 방법이 반음영 영역이 20% 감소하였다. 뇌정위적 방사선 수술은 소조사면을 이용해 1회 대선량을 조사하는 방법으로 조사면의 선량특성을 고려한 처방이 중요하게 된다. 본 실험을 통해 뇌정위적 방사선 수술에 있어 정방형 소조사면 보다 전용 콘을 이용한 방법이 유효하다고 사료되며, 향후 다양한 측정기를 이용한 소조사면의 선량특성에 관한 연구가 필요하다고 생각된다.

공동(air cavity)의 존재 시 실험적 선량분포와 치료계획상의 선량분포 비교 (Comparison of Experimental and Radiation Therapy Planning (RTP) Dose Distributions on Air Cavity)

  • 고 에너지 광자선 치료 시 공동의 존재로 인한 실험적 선량분포와 치료계획상의 선량분포의 변화를 비교, 평가 하고자 하였으며, 선형가속기의 6 MV 광자선을 이용해서 폴리스틸렌 팬텀, 자체 제작한 아크릴 팬텀으로 공동을 만들고 표면에서 공동까지의 거리는 3 cm로 하고 선원-측정기간 거리는 100 cm로 고정하였고 공동의 크기는 가로 $\times$ 세로 $\times$ 높이로 정하였다. 공동의 넓이, 높이, 존재 유무, 그리고 조사면과 공동의 크기비율에 따른 깊이에 대한 선량변화를 평판형전리함과 미소전류계를 이용하여 측정하였다. 치료계획상의 선량분포는 불균질 보정을 하고 치료계획을 하여 비교하였다. 그 결과 공동의 넓이가 커짐에 따라 선량은 점차 감소하였다. 공동의 존재 시에, 공동후면 이후 깊이선량은 공동의 비존재시보다 크게 나타났다. 공동의 크기를 $5{\times}5{\times}3\;cm^3$로 고정했을 때 조사면이 $4{\times}4\;cm^2$, $5{\times}5\;cm^2$, $6{\times}6\;cm^2$일 경우에 rebuild-up이 일어났다. 그러나 조사면이 $10{\times}10\;cm^2$에서는 선량감소만이 나타났다. 또한 조사면을 $5{\times}5\;cm^2$로 고정했을 때, 공동의 넓이가 $4{\times}4\;cm^2$, $5{\times}5\;cm^2$일 경우에는 rebuild-up현상이 일어났지만, $2{\times}2\;cm^2$, $3{\times}3\;cm^2$일 경우에는 일어나지 않았다. 모든 경우에서 치료계획상의 선량분포에서 rebuild-up 현상이 나타나지 않았다. 따라서 공동이 위치한 곳에 종양이 존재할 때는 치료계획상의 선량분포에 차이가 있으므로 주의를 할 필요가 있다.

유리선량계를 이용한 요추검사의 장기선량 및 영상의 평가 (Evaluation on Organ Dose and Image Quality of Lumbar Spine Radiography Using Glass Dosimeter)

  • 본 연구는 디지털 방사선촬영 시스템에서 요추검사 시 조사야 크기에 따른 주변 장기선량과 요추 영상의 화질 변화를 정량화하여 의료피폭 저감화를 위한 자료를 제공하고자 하였다. 요추검사 시 조사야 크기는 $8^{\prime\prime}{\times}17^{\prime\prime}$에서 $14^{\prime\prime}{\times}17^{\prime\prime}$ 크기까지 변화하였으며, RANDO 팬텀을 대상으로 요추 전-후방향, 측방향 검사를 실행하였다. 장기선량은 간, 위, 췌장, 신장, 생식선을 선택하여 유리선량계로 측정하였으며, Image J로 영상 분해능을 비교 분석하였다. 전-후방향 촬영에서 요추의 주변 장기선량은 조사야 크기가 작아짐에 따라 감소하였으나, 생식선에서는 조사야의 가로축 길이가 1" 작아질 때마다 3.83%로 감소율의 변화가 없었다. 측방향 촬영에서 간, 신장과 같이 표면과 가까운 장기에서 장기선량은 높은 선량이 나타났으나, 조사야의 가로축 길이가 1" 작아질 때마다 감소율이 5% 이내로 나타났다. 그러나 생식선에서는 조사야의 가로축 길이가 1" 작아질 때마다 감소율이 24.34%로 나타났다. 조사야 내, 외부에 대한 선량 차이는 간에서 $549.8{\mu}Gy$, 위에서 $264.6{\mu}Gy$로 감소하였으나, 생식선에서는 평균 $1,135.1{\mu}Gy$로 큰 변화가 없었다. 조사야 크기에 따른 영상은 전-후방향 촬영에서 $9^{\prime\prime}{\times}17^{\prime\prime}$, 측방향 촬영에서 $10^{\prime\prime}{\times}17^{\prime\prime}$ 이하의 크기에서 30 dB 이상으로 영상의 질적 차이를 구분할 수 없었다. 따라서 요추검사는 다른 검사 부위보다 주변장기들이 많이 포함하고 있기 때문에 불필요한 환자의 피폭선량을 줄이고 양질의 영상을 얻기 위하여 조사야 크기 설정에 관한 권고기준안이 제시되어야 할 것으로 사료된다.

인터벤션 시술 시 면적선량계를 이용한 환자 방사선 선량 평가 (Evaluation of Patient Radiation Doses Using DAP Meter in Interventional Radiology Procedures)

  • 해외 각국에서 발표된 중재적 방사선 영역에서의 환자선량을 조사하였으며, 전국 23개 주요 병원에서 국내에서 많이 시행되는 13개의 주요 중재적 방사선 시술에 대한 환자선량을 DAP meter를 이용하여 측정하였으며, 시술별 피폭선량에 관한 8,415 건의 국내 자료를 확보하고, 각 주요 시술별로 참고 선량치를 제시하였다. 연구결과는 경동맥화학색전술 $237.7Gy{\cdot}cm^2$, 투석용 동정맥루 인터벤션시술 $17.3Gy{\cdot}cm^2$, 하지 혈관질환의 인터벤션시술 $114.1Gy{\cdot}cm^2$, 뇌혈관조영술 $188.5Gy{\cdot}cm^2$, 뇌동맥류 코일색전술 $383.5Gy{\cdot}cm^2$, 경피경간 담즙 배액술 $64.6Gy{\cdot}cm^2$, 담도 스텐트설치술 $64.6Gy{\cdot}cm^2$, 요로 폐색에 대한 신루설치술 $22.4Gy{\cdot}cm^2$, 다목적 중심정맥 카테터 삽입시술 $4.3Gy{\cdot}cm^2$, 항암제 주입 목적의 매몰형 중심정맥 카테터 삽입시술 $2.8Gy{\cdot}cm^2$, 혈액 투석 목적의 중심정맥 카테터 삽입시술 $4.4Gy{\cdot}cm^2$, 카테터를 이용한 배액시술 $17.1Gy{\cdot}cm^2$ 그리고 장기혈관색전술 $357.9Gy{\cdot}cm^2$이다. 본 연구를 통해 얻은 선량참고치는 방사선 인터벤션 시술에서 환자선량을 줄일 수 있는 최소한의 가이드라인으로 활용될 수 있을 것으로 생각되며, 참고선량치에 대한 연구는 장비의 발달과 시술방법, 재료의 발전으로 변화될 수 있음을 감안하고 선진국에서는 5년마다 시행되고 있음을 참고할 때, 향후 면적선량을 유효선량으로 변환할 수 있는 국내시술에 적합한 한국형 변환계수가 연구되어야 할 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
