Characteristics and Limitations of Bedside Teaching Instructional Models (침상 옆 교육의 교수설계모형의 특징과 한계)

  • As a preliminary approach to developing a bedside teaching program, this study analyzed the instructional models that have been suggested for bedside teaching. The objects of analysis were four models: the 'Cox model,' which is composed of an experience cycle and an explanation cycle; the 'best teaching practice model' by Janicik and Fletcher; the 'twelve tips to improve bedside teaching' by Ramani; and the SNNAPS model for outpatient education by Wolpaw, Wolpaw, and Papp. This study was conducted in three steps. First, we identified the major components of each model and analyzed their characteristics and limitations. Second, we compared each model in terms of four aspects: the learner, learning interaction, learning context, and organization management. Third, on the basis of prior analysis, the possibilities and potential problems of the models were explored. Based on this review of the existing instructional design models, we proposed an additional four key elements for designing a bedside teaching program: multi-layered learners, various learning environments and contexts, time management by using media, and self-directed design.

Outcome criteria in nocturnal enuresis treatment (야뇨의 치료 평가 기준에 관한 연구)

  • Objectives : Nocturnal enuresis is one of common disorders in children. However, there are not a lot of researches going on about this disease, and also standardized criteria for analyzing were insufficient in Korea. Therefore, clinical researches were not in the confidence level. Methods : Korean journals which were published in 1990 to 2006, and online journals about nocturnal enuresis were used for analyzing based on outcome criteria. Results : The International Children's Continence Society and the World Health Organization have published outcome criteria about nocturnal enuresis, but different, often idiosyncratic, outcome criteria and/or definitions have been adopted in published research on treatment for nocturnal enuresis in recent years. But a new set of criteria suggested by Butler, Robinson, and et. al. referred to as a "dryness scale", which focuses on the percentage of dry nights accomplished at a point in time, will be an alternative proposal. These criteria will be of help to make standardized and proper outcome criteria in oriental medical studies. Conclusions : Agreed standardized outcome criteria in nocturnal enuresis treatment is needed.

Primary Glioblastoma of the Cerebellopontine Angle : Case Report and Review of the Literature

  • Glioblastoma multiforme (GBM) is located most frequently in the cerebral hemispheres. Glioblastoma presenting as an extraaxial mass of cerebellopontine angle (CPA) is very rare in adults. We report a rare case of GBM arising in the CPA. The patient was a 71-year-old female, who complained of progressive gait disturbance and poor memory. Initial magnetic resonance imaging (MRI) revealed a $1.4{\times}1.3cm$ mass in the left CPA, with broad base to the petrous bone, showing homogenous enhancement. Follow-up MRI showed a rapid increase in size of mass ($2.7{\times}2.2cm$) with a necrotic portion. A stereotactic biopsy was done under the guidance of navigation system, and the histopathologic diagnosis was GBM, World Heath Organization grade IV. Further surgical resection was not performed considering her general condition, and the patient underwent concurrent chemotherapy with radiation therapy. Although rare, the possibility of glioblastoma should be included in the differential diagnosis of atypical CPA tumor.

금연 성공률을 높이기 위한 전략

  • Although it has been well known that smokimg is one of the major cause of various disease and conditions, the smoking rate is still very high in Korea. A variety of smoking cessation program are provided by public organization and also by healthcare institutions. In this social enviroment, the smoker's intension and trial rates for smokimg cessation increasing, but it is also true that the succes rate is low about 30%. Therefore this study was conducted to suggest the strategies for providing the effective smoking cessation programs by exploring the factors related to recognition and behavioral intention or programs. To explain the health behavior for smoking and smoking cessation programs, the behavioral model was constructed. The model is composed of five-stages such as recognition of the program, past exprience, present smoking status, intention for smoking, and behavioral intention for cessation programs. It is results that there were very low recognition and and purchase rates for most of smoking cessation programs. Evidenced-based and effective smoking cessation progrms need to be encouraged to smokers by medical doctors, and the strategies of eucationm public realtions, and advertisement are recommended. In addition, cotinuing legal and systematic supports for smoking cessation would lower the smoking rate and ultimately ontribute to the nation;s health promotion, Recognitionm Behavioral Intention.

A Study of Relationships Hardiness, Coping Behaviors and Organizational Commitment of General Hospital Employees (종합병원 종사자의 강인성, 대응행위 및 조직몰입과의 관계)

  • Purpose: This descriptive correlation study was done to identify the hardiness, coping behavior, and organizational commitment of general hospital employees and show how these variables affect increases in role performance and problem solving ability to have a positive influence on organizational harmony. Methods: A quantitative, descriptive research design was used with a sample of 368 employees working in general hospitals in M and C cities. A survey was used to collect the data. Results: The score for perception of hardiness of general hospital employees was 2.85, and coping behavior was 2.40, both out of a possible 4 points, and organizational commitment, 3.03 out of a possible 5 points. There were statistically significant positive correlations between hardiness and coping behavior (r=.33, p<.001), also between hardiness and organizational commitment (r=.51, p<.001), and also between coping behavior and organizational commitment (r=.22, p<.001). Conclusion: The results of the study indicate that hardiness, coping behavior, and organizational commitment in general hospital employees have positive correlations, and thus hospital administrators should explore ways of increase individual employee hardiness and coping behavior, and make efforts to harmonize their organizations by enhancing organizational commitment.

  • Purpose - The aim of this work is to study the reorientation that the export industrial sectors in Costa Rica have experienced during the last 20 years. Research design, data, methodology - The study employs the Cluster Analysis with the export data (20 years of cut-off period) from Costa Rica to the U.S-market. To make the predictions, the technique of the time series was used, with official data (from 2001 to 2010) from the U.S. Department of Commerce and the U.S. International Trade Commission. Results - The Cluster Analysis, show how the economic sectors of traditional products exports of Costa Rica have progressively become in exporters of non-traditional products, meanwhile,the time series confirms that this trend will continue, at least during the next five years. Conclusions - The industry of traditional products exports of Costa Rica (dressmaking, vegetables, coffee, mate, species, etc.) will progressively become in exporters of non- traditional products with a high-tech component (i.e., mechanical equipment and devices, electronic devices and medical equipment),as a consequence of the Chinese (Costa Rica's main competitor) economy's presence in the Organization for Economic Co-operation and Development (OCDE). This fact has enabled the potential improvement of Costa Rica's international competitiveness in the U.S. market.

A Survey on Professionalism and Job Satisfaction of Korean Veterinarians (한국 임상수의사의 전문직업성 및 직업만족도에 대한 연구)

  • This survey on professionalism and job satisfaction was aimed to analyze the veterinarians' attitude toward their profession and educational needs in Korea. Two hundred and ninety nine completed questionnaires were analyzed for this study. The participants considered nontechnical competences, such as communication skills and ethical decision making skills, important as medical knowledge and skills for veterinarians. There were significant differences in veterinarians' job satisfaction among the subgroups of gender, region, specialty, employment status, and work hours. The factor analysis of adjusted Hall's professionalism scale showed 4 dimensions of Veterinarians' professionalism; belief in self-regulation, belief in public service, using the professional organization as a major referent, and autonomy. Veterinarians' professionalism was significantly related to the job satisfaction. The survey results suggest that the educational program of professionalism should be needed for the professional development of veterinary students and veterinarians.

No Estrogenic Activity of Phthalate Esters in Ovariectomized Rat Uterotrophic Assay (랫드 자궁비대반응시험(Uterotrophic assay)을 이용한 phthalate esters의 에스트로겐성 작용 연구)

  • The rodent uterotrophic assay is currently recommended as one of the primary in vivo assays far endocrine disrupting chemicals by the Organization for Economic Cooperation and Development (OECD) and Endocrine Disruptor Screening and Testing Advisory Committee (US EPA EDSTAC). Generally, this assay relies on the rapid increase in uterus and vagina weights when exposed to estrogenic compounds. Phthalate esters have been used extensively as a plasticizer in the manufacture of plastic products such as PVC films and medical devices. Recently, phthalate esters have been shown to induce endocrine system mediated responses. However, a flew studies have been conducted for the screening of their estrogenic activity. In this study the estrogenic activity of seven phthalate esters, butyl benzyl phthalate (BBP), di(2-ethylhexyl) phthalate (DEHP), di-n-butylphthalate (DBP), diethylphthalate (DEP), di-n-pentylphthalate (DPF), di-n-propylphthalate (DPrP) and dicyclohexylphthalate (DCHP), was examined in uterotrophic assay. Phthalate esters dissolved in corn oil were administered to ovariectomized (OVX) female Sprague-Dawley rats by sub-cutaneous injection for three consecutive days. fiats were sacrificed 24h after final treatment, and then uterus and vagina weights were deter mined. All phthalate esters tested in this assay did not change talc uterus and vagina weights at dosage levels up to 200 mg/kg/day treatment. These results demonstrated that phthalate esters did not exhibit estrogenic activity in vivo uterotrophic assay.

Educating Healthcare Professionals in Pharmacovigilance: Global Trends and Korea's Status (보건의료인을 위한 약물감시교육의 해외 동향 및 국내 현황)

  • This narrative review introduces global trends in pharmacovigilance (PV) education for healthcare professionals and the status of PV education in Korea. Proactive participation of healthcare professionals, including physicians, pharmacists, and nurses in reporting suspected adverse events is the main driving force for effective operation of the spontaneous adverse event reporting system database, which in turn facilitates early safety signal detection of otherwise unknown suspected adverse events. The World Health Organization recognizes PV education curriculum as a key aspect in promoting awareness of PV and adverse event reporting among healthcare professionals, and multiple studies have demonstrated that PV educational interventions for healthcare professionals have increased overall adverse event reporting. Considering the global trends in PV education, the curriculum in Korean universities still has room for improvement in promoting PV obligation among future healthcare professionals. Further research is needed to develop PV education curriculum. We suggest a three-step project for innovating PV education in Korea to meet the global PV educational standards: a survey to gauge current PV competencies among healthcare professionals, reform of current PV academic curriculum, and evaluation and fine-tuning of the reformed curriculum.

The Effect of Drug Vintage on Mortality : Economic Effect of New Drug (약의 허가시점분포가 사망률에 매치는 영향 : 신약의 거시경제적 효과)

  • Technological innovation has been regarded as the core competence for the economic growth of individual, as well as organization and country. Pharmaceutical innovation, what we call new medicines, influence people's longevity and productivity by increasing output per hour worked. Therefore, using claims data on virtually all the drugs and diseases of over 550,000 people enrolled in National Health Insurance Program in Korea, we examined the impact of the vintage (original FDA and KFDA approval year) of drugs used to treat a patients from July 1st to December 31st in 2002 on the patient's mortality at the end of 2004, controlling for demographic characteristics(age and sex), utilization of medical services, and the nature and complexity of illness. We found that people using newer drugs are less likely to die at the end of 2004, conditional on covariates. The estimated mortality rates were declining with respect to drug vintage for 1970s, 1980s and 1990s and highly significant. In addition to estimating the model for the entire sample, we estimated the model separately for several disease categories classified by Korean Classification of Disease. Estimates of three drug vintage variables for subgroups of people with (1)neoplasms, (2)endocrine, nutritional and metabolic diseases, and (3)the diseases of circulatory system displayed similar patterns.
