Analysis of 'Sleep Disease' Medical Service Delivery system Through In-Depth Interview

  • As the world gradually advances to an aging society, the quality of human life is valued. Among them, 'quality of sleep' is very closely related to quality of life. Recently, Korea expanded health insurance coverage for "sleep disorders". Particularly, as the number of sleep multiple tests and prescriptions for sleep aids has increased rapidly, much attention has been focused on the related medical service environment. Therefore, this study looked at an in-depth interview of 11 hospitals to see what treatment delivery system is being established when the government applies health insurance for 'sleep disorders'. In conclusion, the organizations with the most average number of sleep polyp tests per day were found to have more sleep polyp labs (hardware) and more full-time specialists. Also, the polysomnography lab (hardware) and the specialist's full-time status (software) did not necessarily result in a "positive pressure regulator prescription" that can solve "sleep apnea" caused by "sleep ailments". Rather, it was found that the number of days of sleep multiple laboratories (hardware), the number of full-time specialists (software) or the specialty majors (software) had a greater impact. In particular, the higher the specialist's full-time personnel (software) index (=6.000), the higher the sleep-inducing agent prescription rate(=1.000), and the lower the specialist's full-time personnel (software) index (=1.000), the higher the sleep-inducer's prescription rate(= 0.010) Was low. In addition, even if the professional full-time personnel(software) index was the same (=1.000), the hospital type was lower as it was closer to the public hospital(=0.067) and higher at the specialized hospital (= 0.933). In the case of university hospitals, when the full-time specialists (software) are in the same condition (= 1.000), the frequency of use of the sleep laboratory (=1.000) and the sleep test rate (= 1.000) were all the same.

Qualitative Study for Barriers for Medication and Health Care Service Use among the Visually Impaired and Hearing Impaired in Korea (시각장애인 또는 청각장애인의 의료기관 이용 및 의약품 안전사용 저해요인 관련 심층면접조사)

  • Objective: The disabled are in a blind spot for obtaining information on drugs, and the pharmacies' counseling on drug use is centered on non-disabled people. Few studies have investigated the current statuses of drug use by type of disability. The purpose of this study is to understand the drug use by type of disability and by life cycle of visually impaired and hearing impaired in Korea. Methods: The study participants consisted of 16 people with visually impairments, 12 people with hearing impairments. One in-depth interview was conducted per participant, and each interview was recorded and documented. Results: Common barriers against safe medication and medical service uses across disability types are 'lack of consideration and service for the disabled, limited access to medical facilities due to disability, limited access to information regarding medication use, psychological anxiety about drug use and side effects, and inconvenience regarding COVID-19 epidemic. The specific factors were 'difficulties in identifying proper medicines and following prescribed dosages' in the case of visually impaired, and 'problems with sign language interpretation system' for the hearing impaired. Conclusion: Disabled people are hindered from using medicines properly due to various factors. Based on the content derived from this study, it is necessary to eliminate the inhibition factors and devise specific measures for the safety of each type of disorder such as developing a method for medication counseling considering disabilities and establishing communication support systems.

  • Objectives : This study aimed to evaluate present status and future of traditional medical service market, focusing on the Korean medical center. Methods: We chose the subject with simple random sampling and investigated through interview and internet with questionnaire. Total 319 people helped us. Results: 1. It was investigated that, the number of the Korean medical centers which was operating traditional medical service was 9,910. And the number of people in the business of traditional medical service was estimated by total 46,577 in 2005. 2. Average sales of Korean medical center in 2003 was 24.8 million won. increased by 25.6million won in 2004, 28.3million won in 2005. 3. At the end of 2005, the scale of traditional medical service market was estimated by 2 trillion 7,676 hundred million won. Conclusions : Korean traditional medical service industry has been developed and will be developing by 2.8 times in 2015.

The Predictors of Employees' Personnel Rating at a University Hospital in Korea (일개 대학병원 직원의 인사고과성적 예측요인)

  • This study purports to investigate the determinants of individual personnel rating of the employees at a university hospital in Seoul, Korea. The sample used in this study consisted of 63 nurses, 41 para-medical staff (Clinical Pathologist, and Radiologist), and 67 administrative staff. Independent variables of the study included the achievement level of the selection test (English, major subject, and interview), post-entrance development factors (education and training, career development, supervisory support, co-worker support, and organizational support), and demographic characteristics. Data for the achievement level of the entrance exam and years for the first promotion were collected from the administrative records of the study hospital, while data for the post-entrance development factors were collected from the survey with self-administered questionnaires using 5-point Likert Scale during June 10-25, 2003. Collected data were analyzed using hierarchical multiple regression. The results of the study showed that achievement level of the interview and English exam at the selection test, education and training, organizational support, and supervisory support while working at the hospital, and length of duration (below 8 years) and educational background (4-year college graduates) among demographic variables had significant positive effects on the personnel rating. The results of the study imply that hospital administrators should make an effort to improve the validity of the selection test, and to motivate the employees to receive more education and training.

Effectiveness of Music-listening intervention on Hwa-byung Patients (화병환자를 대상으로 한 음악청취 중재의 효과에 대한 연구)

  • Objective: The objectives of this preliminary study was to develop cost-effective and accessible self-intervention program and to assess the effects of music listening self intervention program for Hwa-byung patients, who had undergone conflicts with family and economical problems, in quantity and quality. Subjects: Seven middle age women who bad been diagnosed with Hwa-byung by Hwa-Byung Diagnotic interview Schedule participated in this program. Method: Participants learned music listening techniques and visited hospital three times a week during 2 weeks. Music-listening self intervention for Hwa-byung patients was consisted of three sad musics(in minor) and three sedative musics(in major) and perlO1med at home for 2 weeks. Patient's somatic symptoms and self awareness (of Hwa-byung scale), anxiety(STAD, anger(STAXD, depressive mood(BDD were evaluated before and after intervention and 6 weeks later after study completed. And various and subjective responses of Music-listening self intervention for Hwa-byung patients were collected by semi- structured interview made for this study. Results: Their Somatic symptoms(especially insomnia), self-awareness of Hwa-byung and depressive mood were improved except anxiety, anger. This improvement had maintained over a 6-week follow-up period. Patients had used constantly the music-listening program for Hwa-byung patients after the study completed. Conclusion: The music-listening program will be an effective for Hwa-byung patient's somatic symptoms and depressive mood and had been used constantly by them. But, much methodological supplementation will be needed to assess clinical effects on this program.

The Case-Control Study of Risk Factors of Silent Cerebral Infarction (무증상 뇌경색의 위험요인에 대한 환자;대조군 연구)

  • Background : Cerebrovascular disease is a major cause of death and disability in adults. Silent cerebral infarction (SCI) portends more severe cerebral infarction or may lead to insidious progressive brain damage resulting in vascular dementia. Known cardiovascular risk factors, such as arterial hypertension, diabetes mellitus, smoking, hyperlipidemia and ischemic heart disease may increase the risk of SCI. This study was designed to evaluate the risk factors of SCI in an apparently normal adult population. Methods : We divided 340 neurologically normal adults (mean age=59.90$\pm$8.30, men:women = 146:194) who underwent brain computed tomography (CT) or magnetic resonance imaging (MRI) at the Stroke Medical Center in Daejeon University Oriental Medicine Hospital in two groups, Silent inf. and Controls,and analyzed risk factors of SCI by interview, physical examination and blood test. Risk factors of SCI were assessed by interview, physical examination and blood test. We performed Pearson's chi-square test and two-sample t-test for univariate analysis and multiple logistic regressions for multivariate analysis to evaluate risk factors of SCI. Results : Old age, diabetes mellitus, and high lactate dehydrogenase (LDH) levels were associated with SCI on univariate analysis. Diabetes mellitus was demonstrated to be an independent risk factor for SCI on multivariate analysis. Conclusions : Advanced age, diabetes mellitus, and LDH levels are associated with SCI.

  • The opinion mining is that to use the existing data mining technology also uploaded blog to web, to use product comment, the opinion mining can extract the author's opinion therefore it not judge text's subject, only judge subject's emotion. In this paper, published opinion mining algorithms and the text using speech recognition API for non-voice data to judge the emotions suggested. The system is open and the Subject associated with Google Voice Recognition API sunwihwa algorithm, the algorithm determines the polarity through improved design, based on this interview, speech recognition, which implements the model.

The nature of the homeless tuberculosis screening and directed to the distance homeless specific location are likely to evaluate the actual incidence low and aggressive or management needs, the rationale is allowed insufficient reality is. Through this research, future, for tuberculosis high risk tuberculosis patient, such as homeless to expand the tuberculosis screening of infectious tuberculosis patients in private medical institutions, and one-stop service that chest X-ray examination and sputum examination is carried out at the same time introduced immediately to prevent the inspection and examination, cure, and should establish a foundation that can be up to post administration.

Exploring the Contributory Factors of Confined Space Accidents Using Accident Investigation Reports and Semistructured Interviews

  • Background: The oil and gas industry is one of the riskiest industries for confined space injuries. This study aimed to understand an overall picture of the causal factors of confined space accidents through analyzing accident reports and the use of a qualitative approach. Methods: Twenty-one fatal occupational accidents were analyzed according to the Human Factors Analysis and Classification System approach. Furthermore, thirty-three semistructured interviews were conducted with employees in different roles to capture their experiences regarding the contributory factors. The content analyses of the interview transcripts were conducted using MAXQDA software. Results: Based on accident reports, the largest proportions of causal factors (77%) were attributed to the organizational and supervisory levels, with the predominant influence of the organizational process. We identified 25 contributory factors in confined space accidents that were causal factors outside of the original Human Factors Analysis and Classification System framework. Therefore, modifications were made to deal with factors outside the organization and newly explored causal factors at the organizational level. External Influences as the fifth level considered contributory factors beyond the organization including Laws, Regulations and Standards, Government Policies, Political Influences, and Economic Status categories. Moreover, Contracting/Contract Management and Emergency Management were two extra categories identified at the organizational level. Conclusions: Preventing confined space accidents requires addressing issues from the organizational to operator level and external influences beyond the organization. The recommended modifications provide a basis for accident investigation and risk analysis, which may be applicable across a broad range of industries and accident types.

A Case of Panic Disorder Patient Improved by Gyeji-tang(Guizhi-tang) Based on Shanghanlun Provision (상한론(傷寒論) 변병진단체계(辨病診斷體系)에 근거한 계지탕(桂枝湯)의 투여로 호전을 보인 공황장애 환자 1례)

  • Objectives: The objective of this case report is to find a therapeutic effect of Gyeji-tang in panic disorder patients, based on Shanghanlun provision. Methods: At patient's first visit, we had conducted a medical interview in order to diagnose the physical and mental symptoms of the patient. A questionnaire called BAI had been also used to measure the degree of anxiety in panic disorder. After all the diagnoses had been done, the patient was allowed to take 120cc of Gyeji-tang, three times a day for 25 weeks. 25 weeks later, we conducted a second medical interview to compare with the initial treatment and to evaluate whether his physical and mental symptoms had been decreased or not. Likewise, the patient was asked to fill out the BAI questionnaire to measure the decline of anxiety degree. Results: Based on Shanghanlun provision, the patient with panic disorder due to overwork was diagnosed as Tai-yang-bing. Considering that the patient was having dyspnea, 15th provision of Gyeji-tang was selected and administered for 25 weeks. After 25 weeks, we could have observed that the main physical symptoms such as palpitation, dyspnea, stifling, headache, and fatigue were moderately improved. Moreover, the BAI score, which was evaluated to measaure the degree of anxiety, was also decreased from 49 points to the level of normal condition, 16 points. Conclusions: A 44 year old male patient who suffered from panic disorder due to overwork was allowed to take 15th provision Gyeji-tang for 25 weeks, based on Shanghanlun Provision. In results, the main physical symtpoms including dyspnea, palpitation, stifling, headache, and fatigue, and mental symptom such as extreme anxiety were all effectively improved.
