Study UBC Inspection of the Over Result (UBC 검사의 이상결과에 대한 연구)

  • Purpose: As UBC IRMA is being tested, patients out of the reference value are reacting within the value again a few days later the urine collection tested, which causes the reliability of the test to decrease as a result. In this study, we can assume that the physiological changes in the factors would affect the results. The purpose of the study is to find out whether hematuria and proteinuria in the sample as well as the interval time (3 hours or more recommended) have affected the results. As a result, we could discover the changes in factors and increase the reliability of the test. Materials and Methods: 468 people (female: 249, male: 219) who came for the check-up were presented herein for medical examination from 2013.3.15 to 2013.1.2. Some people out of 468 who have reacted onto the reference value were divided into group low titer zone, ow-middle titer zone, and middle-high titer zone and tested for hematuria and proteinuria. During that period, 48 outpatients were asked to fill in a questionnaire regarding the urination interval time. The reagents used were (IDL Biotech AB, Sweden) and UBC IRMA. Results: Of the patients that are formed in the reference value of ($0.1-34.0{\mu}g/L$) turn out to be 52.7 years average age in their low concentration, ($mean{\pm}SD$) of the value of $0.10{\pm}0.02{\mu}g/L$. Among 80 people (50.8%, female: 49.2%), 16 patients (20%) have shown reaction to microscopic hematuria and 10 patients (12.5%) responded to proteinuria. In the average low concentration under 52.5 years of average age, 43 people (53%) have shown reaction to microscopic hematuria and 21 people (26.3%) are proteinuric patients out of 80 patients (male: 50.8%, female: 51.3%). In the middle high concentration of $11.8{\pm}4.82{\mu}g/L$ under the average age 51.7 years, 35 patients (53%) have responded to the microscopic hematuria and proteinuric patients are 26 people (39.3%) out of 66 people (men: 44%, women: 56%). In addition, in the concentration of $51.7{\pm}43.5{\mu}g/L$, some patients who get out of the reference value are observed as the average age of 52.0. 11 patients (78.6%) out of 14 (male: 35.7%, female: 64.3%) react to the microscopic hematuria. There show 6 people (42.8%) who turn out to be as proteinuric patients. As for the interval time, $1.67{\pm}3.71{\mu}g/L$ was the average value among 48 patients (female: 45.8%, male: 54.2%). Conclusion: We cannot see if proteinuria and hematuria directly affect abnormal results of inspection of 8,18 cytokeratin; however, we can find out that they statistically have an influence on highly generating UBC among several mechanisms. Also, although urination interval time was various every 15 minutes, we it does not affect these results.

  • CATCH22 syndrome or DiGeorge syndrome is a medical acronym of cardiac defects, abnormal facial appearances, thymic hypoplasia, cleft palate, and hypocalcemia. Patients with CATCH22 syndrome are susceptible to infection due to an absent or hypoplastic thymus and often have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with CATCH22 syndrome under general anesthesia. A 4-year-old, 14.8 kg boy with CATCH22 syndrome visited Seoul National University Dental Hospital for dental check up. The patient had multiple caries requiring dental treatment. He experienced the corrective heart surgery due to Tetralogy of Fallot a few years ago. General anesthesia was planned because his heart rate and vital sign had shown unstable during the previous conscious sedation procedure. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with CATCH22 syndrome could be performed with the help of general anesthesia and careful monitoring.

  • Since the late 1990s, 'smart clothing' has been developed in a various way to meet the need of users and to help people more friendly interact with computers through its various designs. Recently, various applications of smart clothing concept have been presented by researchers. Among the various applications, smart clothing with a health care system is most likely to gain the highest demand rate in the market. Among them, smart clothing for check-up of health status with its sensors is expected to sell better than other types of smart clothing on the market. Under this circumstance, research and development for this field have been accelerated furthermore. This research institution has invented biometric sensors suitable for the smart clothing, and has developed a design to diagnose various diseases such as cardiac disorder and respiratory diseases. The newly developed smart clothing in this study looks similar to the previous inventions, but people can feel more comfortable in it with its fabric interaction built in it. When people wear it, the health status of the wearers is diagnosed and its signals are transmitted to the connected computer so the result can be easily monitored in real time. This smart clothing is a new kind of clothing as a supporting system for preventing various cardiac disorder and respiratory diseases using its biometric sensor built-in, and is also an archetype to show how smart clothing can work on the market.

  • Identify the effects of ISO quality management system on the durability of the enterprise by applying it to early startups and small - and medium-sized enterprises. In particular, the quality management system of a technology start-up small and medium sized companies is important in applying the management system to the financial, management and quality control services rather than to research and development. The quality management system is applied to the medical equipment manufacturers, mobile phone brokers, foreign Chinese companies, and components manufacturers for diesel engines.Try it. Check the case where the quality management system has been applied to the company concerned and the nonconformities have been improved. In addition, the success of the project was determined by applying the quality control system to maintain the stability and perpetuation of the company through the case of the failed selection. The company explains that recently, it must appoint internal auditors to carry out continuous improvement activities and that those who want the development and continuity of the company must reflect the quality management system.

Relationship between Change in Body Mass Index and Blood Pressure among Industrial Workers (산업장 근로자들의 BMI 변화와 혈압 변화의 관련성)

  • This study was performed to elucidate the association of blood pressure and incidence of hypertension in response to changes in body mass index. A total of 28,249 industrial workers (male: 25,548, female: 2,701) who aged 30~69 years old, received regular medical check-up at least once per year from 2002 to 2012 (over 11 years) were included. In the analysis, the averages of blood pressure were stratified by BMI which was obtained from the initial examination results of each individual. In addition, averages of changes in blood pressure were analyzed by stratifying with changes in BMI over 10 years(from 2002); annual occurrence rates of high blood pressure, stratified per BMI, were further assessed in which patients were in normal ranges of blood pressure. As a result, the averages blood pressure over 10 years were more significantly elevating in the group of obesity compared to those of the low weight group as well as the normal weight group. Similarly, when it comes to the occurrence rates of high blood pressure, the group of higher BMI represented elevated risks compared to the groups of low BMI. In conclusion, the increase of BMI over 10 years(2002-20012) were significantly influenced on the increase of blood pressure.

Relationship Between Fatty Liver and Coronary Risk Factors among Health Examined Adult Women in an University Hospital (한 대학병원 건강검진센터에 내원한 성인 여성의 지방간과 관상동맥질환위험인자와의 관련성)

  • The present study was to elucidate such a relationship by comparing the coronary risk factors with and without fatty liver adjusted for age and/or BMI. Study subjects were 665 women of 30 years and over, who underwent health package check-up at the health promotion center of an university hospital from July, 2009 to June, 2010. As a results, the prevalence rates of fatty liver of study subjects were 11.6%, and the rates were significantly higher in older age group, in the group of higher level of BMI. The group of subjects with fatty liver had significantly lower mean HDL-cholesterol and higher levels of body fat rate, TG, TC, LDL-cholesterol, FBS and ALT, then those parameters in subjects without fatty liver, even after adjustment for age and/or BMI. In age and BMI adjusted logistic regressions, The odds ratio of fatty liver was increased significantly as there is an increase in the abnormal group with TG, TC, LDL-C and ALT.

  • Diagnostic performance of polymerase chain reaction (PCR) for detecting Dirofilaria immitis in dogs was evaluated when no gold standard test was employed. An enzyme-linked immunosorbent assay test kit (SnapTM, IDEXX, USA) with unknown parameters was also employed. The sensitivity and specificity of the PCR from two-population model were estimated by using both maximum likelihood using expectation-maximization (EM) algorithm and Bayesian method, assuming conditional independence between the two tests. A total of 266 samples, 133 samples in each trial, were randomly retrieved from the heartworm database records during the year 2002-2004 in a university animal hospital. These data originated from the test results of military dogs which were brought for routine medical check-up or testing for heartworm infection. When combined 2 trials, sensitivity and specificity of the PCR was 96.4-96.7% and 97.6-98.8% in EM and 94.4-94.8% and 97.1-98% in Bayesian. There were no statistical differences between estimates. This finding indicates that the PCR assay could be useful screening tool for detecting heartworm antigen in dogs. This study was provided further evidences that Bayesian approach is an alternative approach to draw better inference about the performance of a new diagnostic test in case when either gold test is not available.

Fasting Blood Sugars and Their Association with Serum Lipids, and Obesity Indices in Manufacturing Workers (제조업 근로자의 공복 시 혈당과 혈청지질 및 비만지표와의 관련성)

  • The purpose of this study was to investigate the relationship between the fasting blood sugar and serum lipid levels (TC, TG, HDL-C, LDL-C) and obesity indices (BMI, body fat rates, waist circumference, waist to hip ratio). The study sample consists of 1,473 manufacturing workers aged from 30 to 59 years, who underwent a health check-up at a university hospital during the period from Jan. to Dec. 2015. A data analysis was conducted to classify the subjects into the normal and abnormal groups according to their fasting blood sugar levels depending on the average values of the serum lipids and obesity indices. Multiple regression analyzes adjusted for sex and age were conducted for the factors affecting the fasting blood sugar level. As a result, the Serum TC, TG, LDL-C, BMI and waist circumference were found to be significantly higher in the abnormal fasting blood sugar level group than in the normal one, but the HDL-C was significantly lower in the abnormal group than in the normal one The fasting blood sugar level had a significant positive correlation with the TC, TG, LDL-C, BMI and waist circumference. The TC, TG, BMI and body fat were the significant factors affecting the fasting blood sugar. The above results suggest that the fasting blood sugar and serum lipid levels (TC, TG, HDL-C, LDL-C), obesity indices (BMI, body fat rates, waist circumference, waist to hip ratio) of manufacturing workers are significantly associated with each other.

Meta-analysis of Change in Weight and Heart Rate for Phentermine in Obesity (비만환자의 펜터민 복용에 따른 체중과 심박수 변화에 대한 메타분석)

  • Objectives: This study aimed to evaluate the change in weight and heart rate associated with the use of phentermine through meta-analysis based on the published literatures. Methods: Eight electronic databases, PubMed, EMBASE, Cochrane library, and five domestic databases were used to search the literature. Randomized controlled trials that evaluated the change in weight and heart rate with the use of phentermine compared with placebo were included in this study. The fixed-effect model weighted by the Mantel-Haenszel method was used in the meta-analysis, and the random-effects model was used when heterogeneity was present. Results: We included 12 studies comprising 677 patients. The change in weight observed with the use of phentermine (SMD = -1.37, 95% CI: -1.55, -1.19) was statistically significant compared with that observed with placebo. As per the subgroup analysis results, the change in weight by publication year, country, phentermine dosage, follow-up check was not heterogeneous. The change in heart rate observed with the use of phentermine (SMD = 0.64, 95% CI: 0.35, 0.92) was significant compared with that observed with placebo. Conclusions: Weight loss and increased heart rate were confirmed in phentermine compared with placebo.

Health Status and Health-promoting Lifestyle for Living Donors after Kidney Donation Through Survey (설문조사를 통한 생체 신장 공여자의 기증 후 건강상태와 건강증진 생활방식)

  • Background: Normal renal function and health have been recognized as important factors in living donors after kidney donation. The purpose of this study was to evaluate the health status and health-promoting lifestyle in living donors after kidney donation. Methods: A total of 678 living-kidney donors were counted in our center from January 1990 to December 2011. Only 84 donors agreed to participate in the survey by telephone. We received consent for participation in our survey from 48 donors (57.1%). Data were collected from May to August 2013 using donor characteristics, health status, and Health Promoting Lifestyle Profile I (HPLP-I). Results: The donors were predominantly female (62.5%) and the average age was 48.9±11.8 years, and the average period after nephrectomy was 9.7±5.7 years. The characteristics of donors included ideal body weight (37.5%), overweight (37.5%) in body mass index, and good health status (81.3%). Most donors underwent an annual medical check-up (56.2%), no health problem (81.3%), and no disease (64.6%). However, one patient was treated with dialysis for renal failure due to diabetes. The total average score for HPLP-I was 128.3±13.9. Higher than average scores (116.3±19.1) were observed for the general middle-aged woman. There were statistically significant differences in self-realization and nutrition in subsection of HPLP-I. Self-realization showed a higher score for Christian (F=2.743, P=0.041) and good health (F=3.389, P=0.017). Nutrition showed a higher score for overweight, obesity (F=6.783, P=0.000), and older than 60 (F=3.854, P=0.009). Conclusions: Most living kidney donors were healthy after their donation and had relatively high scores for health-promoting lifestyle. However, one patient had a serious health problem. In addition, younger, longer period after donation, and the rare health examination of donors showed a lower health-promoting lifestyle. Designed and continuous health-care management after transplantation is needed for kidney donors.
