Retrospective study of conical connection dental implant (Ankylos dental Implant). (Conical connection 임프란트(Ankylos dental implant)에 대한 후향적 임상연구)

  • Objectives. The standardization of connection between fixture and abutment has not been defined. The success of dental implants was not always depends on connection. However, the connection mechanism is one of the most important things for dental implant treatment success. Most implant systems are very comparable in their design and engineering. They share many common characteristics and have similar strengths and weaknesses. Their significant weaknesses are connection, microgap and the resulting micromovement allowing bacterial contamination and bone loss. In the present study, we investigated the clinical performance of Ankylos implant (conical connection implant) Patients and Methods. The clinical performance of conical connection implant was studied under well-controlled clinical conditions. A total of 133 conical connection implants were placed in 50 patients from April 2005 to March 2006. The mean follow-up loading period of implants which was considered successful was 220$\pm$29 days. We recorded the age, sex, installation site, reason of edentulous region, bone density of installation site, diameter and length of dental implants and periods from installation to uncovering surgery using patients medical chart. Results Four Ankylos implants were lost during pre-loading period. 129 implants provided excellent clinical performance during 220$\pm$29 days on an average. The short-term success rate of this conical connection implant system was 96.99%.

Anti-hypertensive effect by Single adminstration of Chunghyul-dan : A CASE SERIES (청혈단 단회 투여에 따른 혈압 강하 효과 : 증례 집적 연구)

  • Objectives: This study was designed to investigate antihypertensive effect by single adminstration of Chunghyul-dan in stroke patients. Methods: This is a 31-patient case series from retrospective chart review of inpatients who diagnosed with stroke at a Kyung Hee University Korean Medicine Hospital (Seoul) between October 2016 to October 2017. We reviewed patient's charts with transient and sudden increases in blood pressure who were treated with Chunghyul-dan and analyzed blood pressure over time. In addition we classified patients by using the standardized predictive models for Korean medical diagnostic pattern-identification to find out if there are some differences in responder ratios. Results: Thirty one patients who took Chunghyul-dan were identified. 60 minutes, 120 minutes after 1200mg administration, systolic and diastolic blood pressure decreased significantly compared to baseline. We could not identify the difference between the groups of each pattern-identification because of small number of some groups. Conclusions: Although limited by its retrospective nature, this study suggests that Chunghyul-dan may be effective as short term antihypertensive method for stroke patient.

Epidemiology of Hyperbilirubinemia in a Quaternary Pediatric Emergency Department over a Three-Year Period

  • Purpose: There is a lack of scholarly reports on pediatric emergency department (PED) exposure to hyperbilirubinemia. We aimed to describe the epidemiology of hyperbilirubinemia in patients presenting to a PED over a three-year period. Methods: This was a retrospective cohort study, completed at an urban quaternary academic PED. Patients were included if they presented to the PED from 2010 to 2012, were 0 to 18 years in age, and had an elevated serum bilirubin for age. A chart review was completed to determine the incidence of hyperbilirubinemia, etiology, diagnostic work up and prognosis. The data set was stratified into four age ranges. Results: We identified 1,534 visits where a patient was found to have hyperbilirubinemia (0.8% of all visits). In 47.7% of patients hyperbilirubinemia was determined to have arisen from an identifiable pathologic etiology (0.38% of all visits). First-time diagnosis of pathologic hyperbilirubinemia occurred in 14% of hyperbilirubinemia visits (0.11% of all visits). There were varying etiologies of hyperbilirubinemia across age groups but a male predominance in all (55.0%). 15 patients went on to have a liver transplant and 20 patients died. First-time pathologic hyperbilirubinemia patients had a mortality rate of 0.95% for their initial hospitalization. Conclusion: Hyperbilirubinemia was not a common presentation to the PED and a minority of cases were pathologic in etiology. The etiologies of hyperbilirubinemia varied across each of our study age groups. A new discovery of pathologic hyperbilirubinemia and progression to liver transplant or death during the initial presentation was extremely rare.

  • Objectives: The purpose of this study was to investigate liver function test results in admitted patients before and after herbal treatment. Methods: 54 subjects admitted to hospital had liver function tests (aspartate aminotransferase, alanine amino transferase, alkaline phosphatase, total bilirubin) before and after herbal treatment from 1 March 2017 to 30 June 2019. Results: Aspartate aminotransferase, alanine aminotransferase, and alkaline phosphatase were not significantly changed (p>0.05). On admission, 11 patients had abnormal liver function, while seven patients had abnormal results in alkaline phosphatase upon discharge. Three of the seven were normal when they were hospitalized. Conclusions: This study suggests that herbal treatment may have no effect on liver injury.

Discovery of Urinary Biomarkers in Patients with Breast Cancer Based on Metabolomics

  • A metabolomics study was conducted to identify urinary biomarkers for breast cancer, using gas chromatography-mass spectrometry (GC-MS) and liquid chromatography-mass spectrometry (LC-MS), analyzed by principal components analysis (PCA) as well as a partial least squares-discriminant analysis (PLS-DA) for a metabolic pattern analysis. To find potential biomarkers, urine samples were collected from before- and after-mastectomy of breast cancer patients and healthy controls. Androgens, corticoids, estrogens, nucleosides, and polyols were quantitatively measured and urinary metabolic profiles were constructed through PCA and PLS-DA. The possible biomarkers were discriminated from quantified targeted metabolites with a metabolic pattern analysis and subsequent screening. We identified two biomarkers for breast cancer in urine, ${\beta}$-cortol and 5-methyl-2-deoxycytidine, which were categorized at significant levels in a student t-test (p-value < 0.05). The concentrations of these metabolites in breast cancer patients significantly increased relative to those of controls and patients after mastectomy. Biomarkers identified in this study were highly related to metabolites causing oxidative DNA damage in the endogenous metabolism. These biomarkers are not only useful for diagnostics and patient stratification but can be mapped on a biochemical chart to identify the corresponding enzyme for target identification via metabolomics.

  • Purpose: The purpose of this study is to review the material handling system of the Airborne Infection Isolation Hospitals which is the one of the key elements of infection control and to improve the basic data for the planning and design of those facilities. Methods: Research was conducted by literature reviews and case studies for the material handling system of domestic and foreign Isolation hospitals. Results: The result of this study can be summarized into three points. First, a general isolation unit and a high level isolated unit need to be distinguished in terms of efficiency and safety. In particular, it is desirable that a high level isolated unit have to completely separate clean and soiled circulations, and soiled corridor should be installed by those means. By doing this, the medical staff can observe patient rooms and supply clean materials directly in the clean zone without wearing PPE, so that safety and work efficiency can be improved at the same time. Second, for the safe disposal of wastes, it is desirable to install a dedicated sterilizer per ward and sterilize it at least in the ward. In addition, It is desirable to install a central waste treatment room and a dedicated soiled corridor in consideration of the inadequate handling capacity and emergency situation. Third, the characteristics of material flow chart in the negative pressured isolation hospitals and the corresponding material handling system have been presented. Implications: Infection control is very important in safety, but it is necessary to respond to the symptoms of the patient.

The Situational, Clinical and Psychosocial Factors Related to Treatment-Seeking Behavior Among Those with Acute Myocardial Infarction (심근경색증 환자의 상황적, 임상적 요인 및 사회심리적 요인과 치료추구행위에 관한 연구)

  • The purpose of this study was to determine the impact of situational, clinical and psychsoical factors on treatment-seeking behavior among those with acute myocardial infarction(AMI). This study used a retrospective, descriptive design. The sample consisted of 72 patients aged over 30 and who were diagnosed with an acute myocardial infarction at two large university-affiliated medical centers from July 1, 1998 to March 30, 2000. But of 72, patients 5 who were an outlier in treatment-seeking time were deleted. Data were collected by using questionnaires, which included demographic data, situational, clinical and psychosocial data. Also patient interviews and chart review were used to obtain information related to treatment-seeking time. The results of this study were summarized as follows ; 1. Mean time from the onset of AMI symptoms to arrival at the hospital was $12.09{\pm}11.44$ hours; 2. Treatment-seeking time was not significantly different by age, gender, or education; 3. Most(44 or 65.78%) patients were at home when they began having AMI symptoms. The remaining patients were either in a public area, workplace or in a car. Patients at home delayed longer than those who had their first symptoms elsewhere, but not significantly different. Also, most patients were with another person when they began to experience AMI symptoms: a spouse(25 or 37.3%), other family member(31 or 46.3%); the remaining 11 were alone. There were no significant differences in treatment-seeking time based on whether alone or with others. Most patients(46 or 68.7%) used an ambulance rather than taking private transportation, and patients who used an ambulance were delayed longer than those who used private transportation, but there were no significant differences; 4. Time to treatment-seeking was not significantly different by blood pressure, heart rate on admission and the peak CK-MB, CPK and Cholesterol level, Killips class; 5. There were no significant statistical differences in treament-seeking times by anxiety level, mood status or control ability.

  • Background: The incidence of rectal cancer is increasing in younger age groups. Limited data is available regarding survival outcome in younger patients with conflicting results from western world. The goal of this study was to determine survival in patients with rectal cancer <30 years of age and compare it with their older counterparts in the Pakistani population. Materials and Methods: A retrospective chart review of patients operated for rectal adenocarcinoma between January 2005 and December 2010 was performed. Patients were divided into two groups, Group 1 aged ${\leq}30years$ and Group 2 aged >30years. Patient characteristics, surgical procedure, histopathological details and number of loco-regional and distant failures were compared. Expected 5 year survival was calculated using Kaplan Meier curves and significance was determined using the Log rank test. Results: There were 38 patients in group 1 and 144 in group 2. A significantly high number of younger patients presented with poorly differentiated histology (44.7% vs 9.7%) (p=0.0001) and advanced pathological stage (63.1% vs 38.1%) (p=0.04). Predicted overall 5 year survival was 38% versus 57% in groups I and II, respectively (p=0.05). Disease free survival was 37% versus 52% and was significantly different (p=0.007). Conclusions: Early onset rectal cancer is associated with poor pathological features and a worse outcome in Pakistani population.

HF-IFF: Applying TF-IDF to Measure Symptom-Medicinal Herb Relevancy and Visualize Medicinal Herb Characteristics - Studying Formulations in Cheongkangeuigam - (HF-IFF: TF-IDF를 응용한 병증-본초 연관성(relevancy) 측정과 본초 특성의 시각화 -청강의감 방제를 대상으로-)

  • Objectives : We applied the term weighting method used in the field of data search to quantify relevancy between symptoms and medicinal herbs, and, based on this, we aim to introduce a method of visualizing the characteristics of medicinal herbs. Methods : We proposed HF-IFF, an adaptation of TF-IDF, which is a term weighting measurement method adapted in the field of data search. Using this method, we deduced relevancy between symptoms and medicinal herbs In Cheongkangeuigam that was published in 1984 by organizing the medical theory of Cheongkang, Kim Younghoon, and visualized this as a graph in order to compare the characteristics of medicinal herbs used for different symptoms. Results : HF-IFF is the product of HF and IFF, where HF is the frequency of the relevant medicinal herb for a set of symptoms, and IFF is the inverse of the number of formulations (FF) containing that herb. A total of 251 types of medicinal herb are used in Cheongkangeuigam, and 1538 formulations are classified according to 67 types of symptom. The overall mean for HF-IFF was 0.491, with a maximum of 4.566 and a minimum of 0.013. Conclusions : In spite of several limitations, we were able to use HF-IFF to measure relevancy between symptoms and medicinal herbs, with formulations as an intermediate. We were able to use the quantified results to visually express the characteristics of the herbs used for symptoms by bubble chart and word-cloud from HF-IFF.
