위탁급식업체 급식안전사고 실태 및 조리종사원의 조리작업안전에 대한 인식 (Assessment of Accidents Occurrence and Cuisine Employees' Awareness of Workplace Safety in Contract Foodservice Management Company)

  • The study was conducted in two phase. In phase I, workplace safety accidents were investigated that happened from 2004 to 2008 in 52 medium-sized contract managed foodservice companies located in Seoul. In phase II, a survey was conducted to examine the cuisine employees' awareness of workplace safety. The survey was administered to the same foodservice management companies from April 12 to April 20, 2009. The final response rate was 84% (N=336), and the data were analyzed using SPSS Windows (ver. 12.0). The analysis showed that more accidents occurred in the age groups of 51~60 and 41~50 and among women. Forty-four percents of the accidents were reported by the employees who had been engaged in foodservice for 1~4 years. The majority of the safety accidents occurred in May~June of the year and the injuries were related to the 'hands and arms'. Approximately 38.5% of the injuries happened during 'movement and transportation'. More than half of the accidents were not managed as 'industrial accidents'. By type of injuries, 28.6% of the accidents were 'fractures'. In terms of the companies' actions, 44% of the accidents were dealt by companies' paying medical bills. The results of this study could be useful to develop evaluation indicators for safety education programs, decrease cuisine employees' safety accidents, protect assets, and prevent various worker and industrial accidents to create a pleasant work environment.

Types of injuries caused by isolated electric scooter accidents

  • Purpose: The recent increase in electric scooter (e-scooter) use has been accompanied by an increase in injuries from e-scooter-related accidents. Studies have reported that most such injuries are minor, and physicians may therefore underestimate the severity of such injuries. This study investigated the types and severity of injuries caused by isolated e-scooter accidents (i.e., those that did not involve colliding with other cars or falling from heights). Methods: This prospective observational study was conducted from May to December 2021 at Dankook University Hospital tertiary medical center. The demographic data of patients injured in isolated e-scooter-related accidents were collected. All injuries were categorized by body part. Results: Fifty eligible patients visited our emergency department during the study period. Of these, 76% were categorized as nonemergency, and 62% were discharged after initial evaluation and treatment at the emergency department. Another 10% were admitted to the intensive care unit (ICU) and 18% to the ward, with nine patients receiving at least one operation. The average hospital stays were 2.4 days in the ICU and 9 days in the ward. One death occurred due to traumatic brain injury (overall mortality rate, 2%). Multiple fractures of the left third through ninth ribs combined with lung laceration and fractures of T12 and L4 were noted. Conclusions: Various types and severities of injuries can occur in isolated e-scooter accidents. While most such injuries are minor, some will require ICU admission or surgery, and deaths can occur. Physicians should not underestimate the severity of such injuries.

단일병원에서 관찰한 최근 10년간의 소아 우발사고에 관한 연구 : 이물흡인을 중심으로 (Clinical Study of Childhood Accidents from a Hospital Over Ten Years with Regard to Foreign Body Aspiration)

  • 목 적: 소아에서 우발사고는 경제 및 문화수준의 향상으로 인한 생활의 기계화와 자동화에 의해 점차 증가하는 추세이며 소아기 사망의 중요한 원인이 되고 있다. 이에 저자들은 응급실을 방문한 우발사고 환아들을 대상으로 상부 위장관이나 기도내 이물흡인과 중독을 중심으로 후향적으로 의무기록을 조사하여 소아 우발사고의 예방대책을 수립하는데 도움이 되고자 본 조사를 시행하였다. 방 법 : 1990년 1월 1일부터 1999년 12월 31일까지만 10년간 서울아산병원 응급실을 방문한 만 15세 미만의 외상, 추락, 화상, 이물흡인 및 중독 환아 6410명을 대상으로 하여 사고의 유형별 분석 및 이물질의 종류, 성과 연령, 위치, 치료 방법에 대하여 병원의 입원기록지를 바탕으로 후향적으로 조사하였다. 결 과 : 1) 사고의 유형별 분포를 보면 외상환아가 5,038례로 가장 많았고 다음이 추락 803례, 화상 353례, 이물 흡인 188례, 중독 28례의 순으로 발생하였고, 남녀비는 1.8 : 1이었다. 2) 이물흡인의 경우 총 188례 중 남녀비는 2 : 1이었으며, 연령별 발생 분포는 6세 이하가 169례(89%)로 가장 많았고 특히 1-2세가 57례(30%)로 많은 분포를 나타내었다. 중독의 경우 남녀비는 1.3 : 1이었고, 연령별 분포는 1-2세가 12례(42%)로 가장 많았고 1세 이하에서 6례(21%), 6세 이상에서 6례(21%)가 관찰되었다. 3) 이물흡인의 위치는 상부 위장관이 112례(59%)로 가장 많은 분포를 보였고, 호흡기관 57례(30%), 외이도가 15례(7%) 순이었다. 상부 위장관 이물의 경우 식도가 69례(61%)로 가장 많았고 위가 43례(38%)를 보였다. 기도내 이물의 경우 우측 및 좌측 주 기관지가 각각 27례(47%)로 동일한 분포를 보였다. 4) 이물의 종류를 보면 상부 위장관의 경우 동전이 65례(58%)로 가장 많았고, 핀이 12례(10.7%), 장난감 및 열쇠가 각각 6례(5.3%), 반지 및 바둑알이 각각 5례(4.5%)였으며, 기도내 이물의 경우 땅콩이 36례(63%)로 가장 많았고, 장난감 부속품이 11례(19%)였다. 5) 이물흡인의 치료 방법으로 상부 위장관 이물의 경우 경성 식도내시경으로 제거된 경우가 60례(54%), 굴곡성 내시경으로 제거된 경우가 29례(26%), 수술을 받은 경우가 7례(6%)였으며, 16례(14%)는 자연 배출되었다. 식도내 이물과 위내 이물 중 핀과 같은 날카로운 경우 진단 즉시 내시경으로 제거하였고, 위내 이물 43례 중 20례는 진단 후 24시간 이내에 제거하였다. 그러나 추적 관찰시 시행한 단순 복부 방사선사진에서 위내 이물이 십이지장을 지난 23례 중 16례(70%)는 2주 이내에 모두 합병증이 없이 자연 배출되었고, 7례(30%)는 장폐쇄 증상 등이 동반되어 수술적으로 제거되었으며 수술 후 합병증은 동반되지 않았다. 기도내 이물의 경우 51례에서 경성 기관지내시경으로 제거되었으나 6례에서는 이미 심한 질식과 뇌손상으로 사망하였다. 6) 중독의 원인별 분포를 보면 약물이 원인인 경우가 14례(50%)였고, 그 중에서 경련성 질환을 가진 환아에서 Carbamazepine이 5례(18%), Valproic acid가 4례(14%)였다. 결 론: 지난 10년간 우발사고의 원인으로 외상이 가장 많았고, 추락, 화상, 이물흡인 그리고 중독의 순이었다. 최근에 이물흡인과 중독이 영유아에서 많이 증가하고 있으며, 특히 기도내 이물흡인의 경우 사망률이 가장 높기 때문에 기도 유지를 위한 적절한 처치가 필요함을 알 수 있다.

일부지역에서 구급차운전자의 구급차 안전운전 운행행태에 관한 연구 (A Study on Behavioral Factors for the Safely of Ambulance Driving)

  • This is the first Korea study to evaluate the effects od the safety of ambulance driving and the occurrence of ambulance traffic accidents and to provide basic informaion for the description of various factors to reduce the ambulance traffic accidents. The major insturment of this study were Krean Self-Analysis Driver Opinionnaire. Questionnaire contains 8 items which measure driver's opinions or attitudes : driving courtesy, emotion, traffic law, speed, vehicle conditions, the use of drugs, high-risk behaviors, and human factors. To take the analysis of data, the total of 350 divers were investigated ambulance divers and others in Taejon City and others (6 City) from 1996. 1. July to 1996. 31. July. The data were analyzed by the descriptive statistics and the logistic regression - path analysis - with SPSS and SAS package program. The result are as follows : 1. There was desirable attitude group(16.2%) and undesirable attitude group(17.6%) on safety ambulance driving. 2. It have suggested that risk factors of ambulance traffic accidents much affected with emotion and speed control on safety ambulance driving < Y(Accdient) = -2.64 + 0.57 $X_1$ (Emotion Control) + 0.30 $X_2$(Seed control) + E > and motor traffic acident much affected with emotion control and high-risk behavior on safety driving < Y(Accident) = -1.11 + 0.33 $X_1$(Emotion Control) + 0.29 $X_2$(High-risk Behvior) + E > 4. The primary emphassis of ambulance drivers was make us realized that improthatnt factors on safety ambulance driving were 1)making way for emergent ambulance, 2)driver's career, 3)The ability of emergency medical technics, and the knowledge or under standing of ambulance way difficut(or easy) of accdess. 5. Almost 96.6% of respondents have agreed to necessity of emergency medical technics for ambulance drivers. 6. Almost 94.6% of respondents have consented to necessity of emergtency medical technicians for ambulance driving. 7. It have suggested that the proportion of traffic accident proportion by desitable attitude group(16.7%) was much less than that of undesirable attitude group(30.8%) on safety ambulance driving(P < 0.05)/Ps) Accidents are unplanned, unforesen incidents which can lead to harmful or unfortunate outcomes, Collisons are not accidents, since the basic cause of the majority of collisons invovles high-risk human behavior. Although there are many factors which contribute to accident causation, four basic factors seem to predominate in most traffic related situations. These four factors include: the human factor, the vehicle factor, the environmental factors and destination factor(Peto G. et al. 1995).

일부지역의 구급차 안전사고에 영향을 주는 요인 분석 (A Study on Behavioral Factors for the Safety of Ambulance Driving by Coefficiecial Structural Analysis - focus on Gwangju Metropolitan City-)

  • This is a study to evaluate the effects of the safety of ambulance driving and the occurrence of ambulance traffic accidents and to provide basic information for the description of various factors to reduce the ambulance traffic accidents. The major instruments of this study were Korean Self-Analysis Driver Opinionnaire. To take the analysis of data, the total of 187 drivers were investigated ambulance drivers in Gwangju Metropolitan City from 2002. 1. September to 2002. 20. September. The data were analyzed by the path analysis SPSS program. The result are as follows : 1. There was desirable attitude group(58.4%) and undesirable attitude group(41.7%) on safety ambulance driving. 2. It have suggested that rist factors of ambulance traffic accident much affected with emotion and speed control on safety ambulance driving(Y(Accident) = -2.00 + 0.6 X1(Emotion Control) + 0.4 $X_2$(Speed control) + E). 3. Almost 92.1% of respondents have agreed to necessity of emergency medical technics for ambulance drivers.

119 구급업무의 정책논쟁분석 (Argumentation Analysis for 119 First aid Service and Policy)

  • Since a few years ago, great disasters have often took place in domestic and abroad, such as Sampoong Department store collapse, Sea-land disaster accident, Incheon Hof fire accidents. Whenever a lot of accidents occurred such as fire, gas explosion, traffic accident, mountain accident, various disease, people required 119 first aid service. As the result, even for the last year, average 2,608 person per day were temporary treated and carried to near hospital by 119 system. But first-aid team or first grade EMT(emergency medical technician) that should provide high quality service and save important lives, might have negatively treated or made their duty only for simple transfer, when they were imminent of emergency case in every kinds of accident spot. Because they worried about medical dispute for "task restriction for EMT" decided by medical laws or rules of first aid. So it was proposed that "task restriction for EMT" should be mitigated or released in the possible range on this occasion.

정면충돌 시 고령운전자 상해 특성에 관한 실사고 연구 (Real-world Accident Study on Injury Characteristics of Elderly Driver in Car-to-Car Frontal Crashes)

  • Real-world accident cases were investigated to understand injury characteristics of the elderly driver. A total 10 cases of car-to-car frontal crash accidents from passenger car including SUV claimed to domestic car insurance company were reviewed. The injury characteristics of the elderly were analyzed from personal information (gender, age), medical treatment record (medical certificate, curative days), vehicle information (model, air-bag, seatbelt) and damage information. This study showed that elderly driver has higher possibility of thorax injury than non-elderly's. Moreover, Injury type and severity were more severe than non-elderly driver at similar type accident conditions. Also, elderly driver's medical treatment period needs 3 times more than non-elderly driver's.

산업재해로 인한 수부손상환자의 물리치료에 관한 연구 (A Study on the Physical Therapy and Rehabilitation Service to the Hand Injury Patients of Industrial Accidents)

  • Even though the automation of industrial machinery has dramatically increased, most industries still require human hands for the production of goods. Our hands are essential to our existence. For this reason, hand injuries caused by industrial accidents have become an important issue lately. Most hand injuries are external wounds and constitute 38.6% of disabling industrial accidents. This significant statistic cannot be ignored by rehabilitation programs. The rate of success in operations of finger and hand injuries has increased, but relatively less consideration has been given to physical therapy, rehabilitation, research, or systematic installment in terms of welfare, which would help the injured carry on the nominal life they lived before their injury. Therefore, it is necessary to study systematically all related aspects to provide patients with physical therapy, rehabilitation, and social welfare, in order to restore their social, professional, and economical capabilities. Physical considerations, functional usability, and cosmetic restoration, as well as the patients' mental state must be addressed. The results of the study shows several problems. Medical appliances, manpower, and the environment of the treatment rooms are very poor. The patients cannot receive treatment early enough nor enough treatment because of too many patients for too few physical therapists. Close cooperation between physical therapists and doctors, nurses, or related departments is lacking. Furthermore, it is irrational that industrial accident premiums for the exert; e treatment of hand injury patients are not itemized. Lack of recognition of the importance of specialty in hand injury therapists leads to the lack of professionalization of systematic hand injury treatment In order to solve these problems, the professonalism of physical therapy should be acknowledged and particular treatment courses should be available and/or required. Based on the understanding of the hand injury patients' needs, new theories of physical therapy should be developed and modem medical appliances and facilities should be available, in order to provide the patients with qualitative medical treatment, which then will facilitate the patients' recovery.

Occupational Exposure to Potentially Infectious Biological Material Among Physicians, Dentists, and Nurses at a University

  • Objective: The objective of this study was to evaluate the prevalence and incidence of accidents with biological material, the level of knowledge, and compliance to standard precautions (SPs) among dentists, physicians, nurses, and dental and medical students. Methods: A closed cohort study with a prospective and retrospective component was conducted between August 2014 and September 2015. The participants were contacted in two moments during the follow-up period, during which a structured questionnaire divided into six sections was used; the interviews were conducted during the follow-up period (Month 6) and at the end of the observation period (Month 12). Results: The global prevalence of accidents in the previous 12 months was 10.2%, with a difference between professionals and students (13.0% vs. 5.1%, respectively; p < 0.003). The incidence rate was 6.49 per 100 person/year, with difference between the groups (6.09 per 100 person/year in professionals and 7.26 per 100 person/year in students), type of specialization (hazard ratio, 3.27), and hours worked per week (hazard ratio, 2.27). The mean of compliance to SP was 31.99 (±3.85) points, with a median of 33 (30, 35) points against the expected 27.75 points. Adherence to SP was associated with the accident report (p < 0.020). Conclusion: We conclude that the proportion/incidence rate of accidents with biological material was high in relation to that in the literature, being higher in professionals and especially among physicians. The levels of knowledge and adherence to SP were good, with the best found in dentists and dental students.

119 구급대원의 화학사고 대응역량 및 교육요구도 (Chemical accident response competencies and educational needs of 119 EMTs)

  • Purpose: This study aimed to evaluate and assess the response capabilities and educational needs of 119 emergency medical technicians (EMTs) in chemical accidents. Methods: A self-reported questionnaire was completed by 167 119 EMTs between December 1st and December 31, 2023. The questionnaire comprised 8 questions on general characteristics, 2 on chemical accidents experienced by the participants, 29 on response capabilities, and 15 on educational needs. Data analysis was performed using t-tests, analysis of variance, Duncan's test for post-hoc analysis, and Pearson's correlation coefficient, using SPSS 27.0. Results: The participants scored 2.69 points on response capacity to chemical accidents. The EMT-Paramedics scored high in 'patient triage,' 'patient treatment,' 'patient transport,' and 'collaborative support' (F=3.924, p=.010; F=5.843, p=.001; F=3.698, p=.013; F=5.272, p=.002), followed by educational experience (t=-4.962, p<.001; t=-2.685, p=.008; t=-3.455, p=.001; t=-3.593, p<.001; t=-3.034, p=.003). The participants scored 4.19 points on educational needs, with high scores for 'patients treatment and transport' (4.280.93). The scores for 'patient triage competency', and 'patient triage' (r=.169, p=.024) correlated positively. Furthermore, the scores for 'patient treatment competency' and all sub-factors of educational needs (r=.185, p=.013; r=.215, p=.004; r=.199, p=.008; r=.190, p=.011; r=.197, p=.008) correlated positively. Conclusion: To strengthen the response capabilities of 119 EMTs, it is imperative to develop an educational program that focuses on first-aid responses.
