의료기관 인증 후 환자안전 및 질 관리 변화와 경영활동 변화 간의 관계: 정신병원과 요양병원 대상 (Relationship between Changes of Patient Safety & Medical Service Quality and Changes of Management Activity after Medical Institution Accreditation: Mental Hospitals and Geriatric Hospitals)

  • 본 연구는 의료기관 인증제의 의무 대상인 정신병원과 요양병원을 대상으로 환자안전 및 질 변화와 경영활동의 변화 간의 관계에 대해 의료기관 인증을 받은 5개 정신병원과 5개 요양병원에 근무하는 종사자들을 대상으로 조사 연구한 결과, 정신병원과 요양병원 종사자들의 인증업무 참여유무는 환자안전 및 질 관리 변화 측정내용 중 '안전보장활동 수행정도' '환자를 위한 양질의 의료서비스 제공정도' '환자의 권리와 책임에 대한 존중정도' '감염발생 위험예방 위한 감염관리활동 수행정도'의 향상 변화에 유의하게 긍정적인 상관관계를 보였다. 그리고 환자안전 및 질 관리 내용 중 안전보장활동 지속적인 질 향상 활동 환자의 권리와 책임에 대한 존중의 지속적인 발전적 변화를 추구하기 위해서는 교육훈련을 통해서 조직구성원들의 역량을 강화시키는 경영활동이 필요하며, 진료전달 체계와 평가의 관리 환자를 위한 양질의 의료서비스 제공의 지속적인 발전적 변화를 위해서는 고객지향성 업무프로세스를 사회 환경의 변화에 맞추어 추구하는 경영활동이 필요함을 알 수 있었다.

  • 해외 의료서비스 시장의 메가 트렌드는 글로벌 융합, 인접 분야와의 융합, ICT기술의 적극적 도입, 공공과 민간의 파트너십(Public Private Partnership)을 통한 혁신 4가지로 요약될 수 있다. 의료서비스는 더 이상 local industry가 아니며, Global Convergence 되어가고 있다. 선진국의 경우 소득 수준의 증가, 새로운 의학기술의 발전, 전문화된 의료서비스의 증가, 인구 고령화 등 의료 수요가 증가하면서 외국 의료진의 이주 증가하고 있으며, 지리적 근접성이나 비용에 상관없이 최고 의료기술에 대한 선택 증가하고 있다. 고품질이면서 상대적으로 가격이 낮은 외국 의료서비스에 대한 수요 증가하며 특히 미국 JCI등 국제적 병원인증 기준의 확산되고 있다. 병원 수출은 관련 기술을 더욱 더 효율적으로 융합해서 수출 할 수 있는 'ICT 융합병원' 수출 산업화의 길을 열었다고 평가되고 있다. 현재 국내의 병원은 이미 포화상태에 이르렀고, 국내병원들의 세계화가 필요한 시점임. 따라서 국가별로 전략을 달리하고, 기술 이전뿐만 아니라 건물마련, 의료장비 구매, 현지 의료인력(의사 및 간호사) 선발 및 교육훈련, 홍보마케팅 등 토탈 수출도 가능하다. 이에 공공성 유지와 해외 진출을 위해 현행 의료법이 전향적으로 개정될 필요가 있으며, 해외 의료서비스의 국내법 적용에 보다 신축성 있는 법적용과 더 나아가 적극적 정책지원이 필요하다.

의과대학 학생의 소진에 대한 지도사례 분석 (A Case Study on Approaches to Supporting Medical Students with Burnout Experience)

  • Medical students often need high levels of specialized institutional and personal support to recover from a burnout experience and to prevent student failure. This study aimed to present approaches to supporting medical students who had experienced burnout using case studies. The study subjects were 45 burnout cases who had been supported by the student support system and committee in one medical school. The factors associated with burnout include poor social network: the lack of belongingness, immature sense of identity, poor mental health, a lack of intrinsic motivation and aptitude, or a major personal crisis. We summarized some tips according to the factors associated with burnout. These tips should provide guidance to organizations and individual support providers to encourage the best quality support for medical students with burnout experiences.

간호사의 가정간호를 위한 교육요구 분석 (A study on educational need of nurses for home care)

  • This study was conducted from July to December 1990, in order to diagnose nurses' educational need for home care. The study subjects consisted of 145 nursing educators, and the 3 groups of nurses, namely 250 senior nursing students of diploma and collegiate program, 235 health center nurses, 521 university' hospital nurses in Seoul. Four types of questionaires were formulated by Delphi method. Two questionaires for the nursing educators were designed to measure their expectations of nurses' knowledge and of their skill for home care, and another two questionaires for the nurses to measure their actual home care knowledge and skill. The results of the study were as follows : 1) The mean scores of educators' expectation for home care knowledge were 17.68 for the care of dependence on medical equipment, 17.44 for the care of mobility impairment patient, 16.56 for the care of cardiopulmonary impairment patient, 16.40 for the care of nutrition and elimination impairment patient, '1.20 for the care of psychiatric disorder patient and 9.03 for the care of cancer and terminally ill patient,. 2) The mean scores of nurses' home care knowledge tested by 20 items were 14.36 for the care of mobility impairment patient, 13.28 for the c8;re of dependence on medical equipment, 13.78 for the care of cardiopulmonary impairment patient, 12.92 for the care of nutrition and elimination impairment patient, and those of tested by 10 items were 7.08 for the care of psychologic disorder patient, 7.80 for the care of cancer and terminally ill patient. The sum of means marked 69.23. As for the nurses' home care knowledge categorized by tasks in terms of the group, significant difference were shown in the care of mobility impairment(P=0.00), cancer and terminally ill(P=0.03), nutrition and elimination impairment(P=0.00) and psychologic disorder patient(P=0.00). No significant difference were shown in the care of dependence on medical equipment and cardiopulmonary impairment patient. 3) Regard to educational need of nurses' home care knowledge categorized by task according to the group it was found that all sampled nurses had educational need in the care of mobility impairment, dependence on medical equipment, cardiopulmonary impairment, cancer and terminally ill patient. It was found that health center nurses had educational need in the care of psychologic disorder. No educational need were found in the health center nurses whose career less than 2 years, in the care of mobility impairment, cardiopulmonary impairment and psychologic disorder patient, and in those of career with 2-5 year in the care of psychologic disorder patient. No educational need were found in the hospital nurses whose career more than 15 years, in the care of cardiopulmonary impairment patient and in those of career with 11-15 year, in the care of cancer and terminally ill patient. 4) The mean scores of educators' expectation for home care skill measured by Likert 5 points scale were 4. 21 for assessing, 4.49 for planning, 4.29 for basic care, 4.42 for curative care, 4.40 for rehabilitative care, 4.36 for emergency care, 4.53 for medication, 4.31 for nutritional care, 4.32 for other means for care, and 4.38 for evaluation. 5) Regard to nurses' home care skill measured by Likert 5 points scale of self evaluation, there was a significant difference between the nurses' home care skill and group(P=0.00l). The higher scores reported by students were vital sign checking and basic care while the scores of below medium were curative care and emergency care. The higher scores reported by health center nurses were vital sign checking, other means for care and care of specimen while the scores below medium were curative, emergency and nutritional care. The higher scores reported by hospital nurses were vital sign checking, care of specimen and basic care, while the score below medium was emergency care. 6) Regard to educational need of nurses' home care skill by nursing process activity according to the group it was found that health center nurses had educational need in all nursing skills including vital sign checking, care of specimen, health assessment, socioeconomic assessment, nursing diagnosis, care plan, basic care, curative care, rehabiitative care, psychological care, emergency care, medication, nutritional care, other means for care and evaluation. And students had educational need in all nursing skills except vital sign checking, and hospital nurses had educational need in all nursing skills except vital sign checking, care of specimen and basic care. 7) In short, the result of this study suggests that the curriculum should be organized in accordence with nurses' educational background and their career for the education of nurses for home care. It should be considered to develop the short term educational program focused on curative and rehabilitative care for health center nurse or community health nurse practitioner and which was focused on family care for hospital nurse. Concerning about this field practice for home care nurse, they are required not only community practice but also . clinical practice including emergency, curative and rehabilitative care.

《東医宝鑒》 方劑引文与代表性中医原著比較硏究 (A Comparative Study on the Difference Between Herbal Formulations in Dongeuibogam and their Original Chinese Medical Texts)

  • This is a study on the difference between herbal formulations cited in Dongeuibogam and their original Chinese texts. We focused on the changes of herbal formulations from their original Chinese texts in the course of compilation of Dongeuibogam. We searched for the reason of this change on various aspects. Our findings confirmed the need for further studies in this field Through this study, we concluded that the changes between the original Chinese texts and Dongeuibogam are a consequent phenomena of historical progression. We hope lor this study to act as a foundation for further advanced studies.

지역의료보험의 실시에 따른 의료이용변화 분석 : 소득계층별 의료필요충족도를 중심으로 (Effects of Regional Health Insurance on Access to Ambulatory Care)

  • The effects of regional health insurance on access to ambulatory care are examined in this paper. Access is measured as use-disability ratios. The data are collected in a household interview survey at Hwachon county before and after the introduction of regional health insurance. Before the introduction of regional health insurance, low-income class has less contacts with physicians than high-income class. This disparity in accessibility among economic classes is reduced with the health insurance coverage, but not removed, even after adjusting for health need.

의과대학과 의학전문대학원생들의 진로적응성에 따른 직업관, 진로선택, 전공미결정요인 비교 (Work Value, Career Choice, and Specialty Indecision Based on Career Adaptability of Medical College and Graduate School Students)

  • The purpose of this study was to investigate the differences between medical college students and graduate school students with regard to the factors that shape career choice and specialty indecision. One hundred and thirty-three students from a medical school participated in this study. The students completed a survey, which collected information on career choice, specialty indecision, and career adaptability. Significant differences were found between high and low career adaptability groups in factors that affected specialty choice and indecision. Students with high career adaptability were significantly more affected by 'intellectual content of specialty,' 'ages and characteristics of patients,''patient contact experience during the early years of medical school,' and 'greater opportunity for research.' Among the specialty indecision factors, students with high career adaptability were affected by 'several specialties equally appealing to me' and 'many interests,' while students with low career adaptability were affected by being 'unaware of my abilities,''unaware of my interests' and having 'learned my choice was not possible for me.' The factor having the greatest influence on specialty indecision was 'need more information and support,' and there were no significant differences in these factors between the two groups. The results suggest that the development of career counseling and education programs need to be designed for medical college students and graduate students in terms of career adaptability and specialty indecision.

신규간호사와 프리셉터가 인지하는 프리셉터의 교수효율성 비교 (A Comparison of Preceptors' and New Graduate Nurses' Perception of Teaching Effectiveness)

  • Objective: The purpose of this study was to compare preceptors and new graduate nurses on their perception of preceptor teaching effectiveness. Methods: The participants were 90 new nurses and 90 preceptors who worked in A medical center. The data were collected from July 1 to September 30, 2008. A cross-sectional descriptive survey was done using a structured questionnaire. Results: New nurses' perception ($4.07{\pm}.44$) of the preceptor teaching effectiveness was significantly higher than perception of the preceptors ($3.57{\pm}.37$). Fifty five percent of new graduate nurses reported a lack of coherence in the preceptor's practice guidelines. For 5 causal factors classified on the teaching effectiveness, the factor of 'Professional knowledge and ability' showed the highest score, but 'Interpersonal and communication skill' got lowest score for both group. Preceptors responded that they did not have enough time to teach well because of their heavy workloads. Conclusions: These results suggest that the preceptors need appropriate compensation and education opportunities, and new graduate nurses need consistent education by the teaching professionals. Therefore, it is important to give preceptors full charge of the preceptorship. Also, it will be necessary to develop education programs to enhance interpersonal and communication skill for preceptors and new nurses.

Medical Educators' Response to Changes in Medical Education due to COVID-19

  • To analyze medical professors' evaluation of their online education experiences in an attempt to support faculty members and indicate the future direction of medical education. Faculty members who taught online in the first semester of 2020 at Yonsei University College of Medicine in South Korea were surveyed. The results of the survey were analyzed in June 2020. There were 88 respondents (35.1% of 251): 59 professors (67.0%), 16 associate professors (18.2%), and 13 assistant professors (14.8%). Their specialties lay in basic medicine (25.0%), clinical medicine (65.9%), and research and special purposes (9.1%). Sixteen participants (18.2%) had previous experience in online lectures; 23 (26.1%) reported that preparation time for online lectures was the same as before; 65 (73.9%) reported that the preparation time had increased; 38 (43.2%) faced difficulties in preparation and lecturing online, and among them 16 (42.1%) indicated inadequate interaction with students; 11 (28.9%) needed extra preparation time; and 11 (28.9%) attributed their difficulties to technical issues with the online platform. Participants' satisfaction with online lectures was low (p<0.001). In the free response question on overall experience with online education, 38.3% mentioned the need for an instructional design that allows students to actively participate and interact with professors, 29.5% mentioned the need for the establishment of an information & communications technology system, and 17.0% mentioned the necessity of faculty development. To prepare for the current pandemic and more in the long term, an appropriate educational support system must be constructed, and a learner-centered instructional design that enables wider interactions and active learning is needed.

공적개발원조(ODA)의 효과적인 의료기기 운영관리를 위한 교육훈련방안: 베트남을 중심으로 (Effective Education and Training method for Operations Management of Medical Equipment of Official Development Assistance(ODA): focusing on Vietnam)

  • 코로나 19 팬데믹으로 인하여 보건의료분야 공적개발원조(ODA)의 중요성이 강조되고 있다. ODA 사업으로 지원된 의료기기의 운영관리, 유지보수에 대한 사후관리, 이를 실현하기 위한 직종별 차별화된 교육훈련 제공이 필요하다. 본 연구에서는 효과적인 의료기기 사후관리, 운영관리를 위해 직종(사용자와 유지보수자)간 교육훈련 성취도의 차이분석을 t-test 검증을 통해 분석하였다. 분석결과, 의료기기 사용자와 유지보수자 간 중점관리 대상 의료기기 6종류의 사용법과 관리방법(Q5), 각 의료기기의 사용 매뉴얼 이해(Q6)에서는 집단 간 차이가 있는 것으로 분석되었지만, 교육의 필요성, 목적, 강사에 대한 만족도 및 직종 간 커뮤니케이션의 필요성에서는 차이가 없는 것으로 분석되었다.
