간암 호흡동조 방사선치료 환자의 호흡신호분석 (Respiratory signal analysis of liver cancer patients with respiratory-gated radiation therapy)

  • 목 적 : 외부표지자 호흡움직임 측정 장치(RPM; Real-time Position Management, Varian Medical System, USA)를 이용한 간암 호흡동조 방사선치료 시 호흡신호와 방사선 조사 시간 및 실제 조사된 호흡위상을 분석하여 호흡움직임 측정 장치를 이용한 호흡동조 방사선 치료의 정확도를 평가하였다. 대상 및 방법 : 2014년 5월부터 9월까지 Novalis Tx.(Varian Medical System, USA)와 RPM을 이용하여 간암 호흡동조 방사선치료(Duty Cycle 20%, Gating window 40% ~ 60%)를 시행한 환자 총 16명의 치료 시 기록된 호흡움직임을 분석하였다. RPM에 기록된 외부표지자의 호흡움직임을 후행적 분석을 통해 호흡위상으로 재구성하였으며, 재구성된 호흡위상을 이용하여 기록된 Beam-on Time과 Duty Cycle에 대해 RPM을 사용한 호흡동조 방사선치료의 예측 정확도를 분석하고, 호흡움직임의 재현성에 따른 Duty Cycle과 예측 정확도의 상관관계를 분석하였다. 결 과 : 대상 환자 16명의 치료계획 시와 실제 치료 시 호흡주기 차이는 평균 -0.03초(범위 -0.50초 ~ 0.09초)로 분석되었으며 두 호흡간의 통계적 차이는 확인할 수 없었다(p=0.472). 치료 시 평균 호흡주기는 4.02 sec (${\pm}0.71sec$), 치료 중 호흡주기 표준편차의 평균값은 7.43%(범위 2.57% ~ 19.20%)로 분석되었다. 실제 Duty Cycle은 평균 16.05%(범위 13.78% ~ 17.41%)로 나타났고 이 중 후행적 분석을 통해 평균 56.05%(범위 39.23% ~ 75.10%)가 계획된 호흡위상(40% ~ 60%)에서 조사되었음을 확인하였다. 호흡주기의 표준편차와 Duty Cycle과 계획된 호흡위상에서 조사된 비율의 상관관계는 각각 -0.156 (p=0.282)와 -0.385 (p=0.070)으로 분석되었다. 결 론 : 본 연구는 실제 치료 중 기록된 외부표지자의 호흡움직임을 후행적으로 분석하여 치료 중 호흡움직임의 재현성 및 Duty Cycle, 계획된 호흡동조창에서의 실제 치료 비율 등을 확인하였다. 4DCT를 이용한 치료계획과의 오차를 최소화하고 효율적인 치료를 위해 호흡훈련 및 호흡신호 모니터링의 강화가 필요 할 것으로 판단된다.

공공설치용 자동제세동기의 사용성 연구 (Usability Study on Public Access Defibrillator)

  • Usability study of the CU Medical "i-PAD" AED was conducted to validate the usability of the device, primarily focused on the trained user as well as the utility of the training material. Comparing these results of the trained participants to those obtained by Callejas et al. (2004), who studied video-trained users of two commercially available Philips AEDs (FR2 and HS1) using a male manikin only, the participants in this study were more successful (100% vs. 85.7%-FR2 vs. 88.9%-HS1) and faster to shock (58.5s-i-PAD vs. 91s-FR2 vs. 90s-HS1). While the focus of this study was on validating ease of use for trained bystanders, the results of the untrained condition showed that, from a usability perspective, these devices perform as well as any other AEDs already approved and available for purchase for untrained bystander use, as evidenced by the comparison to various published research results.

경기도 지역 전략산업 선정에 관한 시론적 연구 (A Study on the Selection of Strategic Industry in Gyeonggi Province)

  • The aim of this study is to find the strategic industries that fulfills the on-site demands of the Gyeonggi province region related to the advancement and development of the local industries. As the results of the analysis, the 4 major strategic industries in the Gyeonggi province region were determined to be (1) electronic component, computer, visual, acoustic and telecommunication equipment manufacturing, (2) pulp, paper and paper product manufacturing, (3) medical substances and pharmaceutical product manufacturing, and (4) rubber and plastic product manufacturing. The industry of concordance between the 4 major strategic industry of the Gyeonggi province determined in this Study and the 13 future growth engine industry of Korea was the area of intelligent semiconductor. Accordingly, it was analyzed that there is a need to strategically cultivate the electrical and electronic telecommunication device industry, which is ranked the $1^{st}$among the strategic industries of Gyeonggi province by generating synergic effects with the policy of the government for nurturing of intelligent semiconductor industry.

지능형 메디컬 기기 개발을 위한 KANO-QFD 모델 제안: AI 기반 탈모관리 기기 중심으로 (A Study on the Development Methodology of Intelligent Medical Devices Utilizing KANO-QFD Model)

  • AI 기술이 결합된 지능형 제품은 기술적 차별화를 실현하며 시장 경쟁력을 높일 수 있는 잠재성을 지닌다. 하지만 시장 수용도를 극대화 할 수 있는 AI 기반의 신제품 개발 방법론은 부재하다. 본 연구는 AI 기반의 지능형 제품 개발에 대한 방법론으로서 KANO-QFD 통합 모델을 제안한다. 실증적인 분석을 위한 구체적 사례로 탈모 예측 및 치료 기기에 대한 소비자 요구조건(Customer Requirements)의 유형을 분류하고, 이를 구현하기 위한 기술적 요구사항(Engineering Characteristics)의 상대적 중요도 및 우선순위를 도출하여 지능형 메디컬 신제품 개발의 방향을 제시하였다. 소비자 130명을 대상으로 실시한 설문조사 분석 결과, KANO 카테고리 중 매력적 품질(Attractive Quality) 요소로 미래 탈모 진행 상황에 대한 정확한 예측, 미래 탈모 모습 및 치료 후 개선된 미래 모습을 실물화하여 스마트폰으로 보고, 세련된 디자인, 레이저와 LED 빛 복합 에너지를 이용한 치료 등이 도출되었다. QFD의 품질의 집(House of Quality)을 기반으로 분석한 결과, 탈모 진단 및 예측을 위한 학습 데이터, 두피 스캔용 Micro 카메라 해상도, 탈모 유형 분류 모델, 맞춤화를 위한 개인별 계정 관리, 탈모 진행상황 진단 모델 순으로 상대적 중요도 및 우선순위가 도출되었다. 본 연구는 기존에 선행되지 않았던 AI 기반의 지능형 메디컬 제품 개발에 대한 방향을 제시하였다는 면에서 의의를 지닌다.

Association of Congestive Heart Failure and Death with Ankylosing Spondylitis : A Nationwide Longitudinal Cohort Study in Korea

  • Objective : We attempted to discover that Ankylosing spondylitis (AS) has a comprehensive relationship with congestive heart failure and death. Methods : We used a nationwide database managed by the Korean National Health Insurance Service from 2010 to 2014. Twelve thousand nine hundred eighty-eight patients with a diagnosis of AS and 64940 age- and sex- stratified matching subjects without AS were enrolled in the AS and control groups. Incidence probabilities of 6 years congestive heart failure and death in each group were calculated. The Cox proportional hazard regression analysis was used to estimate the hazard ratio. We divided the AS and control groups into subgroups according to sex, age, income, and comorbidities. Results : During the follow-up period, 102 patients (0.79%) in the AS group and 201 patients (0.32%) in the control group developed congestive heart failure (p<0.0001). In addition, 211 (1.62%) subjects in the AS group died during the follow-up period compared to 639 (0.98%) subjects in the control group (p<0.0001). The adjusted hazard ratio of congestive heart failure and death in the AS group was 2.28 (95% confidence interval [CI], 1.80-2.89) and 1.66 (95% CI, 1.42-1.95), respectively. The hazard ratios of congestive heart failure and death were significantly increased in all of the subgroups. Conclusion : The incidence rates of congestive heart failure and death were increased in AS patients.

우리나라 의약외품 허가심사제도 고찰 및 개선 방안 (Regulatory System of Quasi-drugs in Korea)

  • The study was aimed to recommend the ways for improving regulatory system of quasi-drugs in governmental authority by comparing with other countries. According to the regulations, the scope of quasi-drugs includes 3 categories of 1) the health aids made of textile, rubber and paper, 2) the health aids which have very minimal effects or no any effects on humans, and 3) disinfectants and pesticides. In US, these quasi-drugs in Korea are classified into 5 categories of medical device, cosmetics, OTC drugs, dietary supplements and pesticides. To improve quasi-drugs administration in Korea, it is concluded that several measures should be implemented : 1) establish clear criteria for classifying into quasi-drugs and more detailed guidelines on designation of quasi-drugs, 2) reform current regulations to meet 3-categories characteristics, supplement detailed guidelines on quasi-drugs administration for effective application process, and update relevant regulations for efficacy, safety and quality, 3) update quasi-drug monographs, 4) re-evaluate current classification of individual quasi-drugs, 5) develop comprehensive list by ingredients, 6) reform post-marketing management system for safety and quality, 7) strengthen the review agency function by increasing the number of experts, 8) develop the database for quasi-drugs for effective information management.

  • There is a new evolution in technological advancement taking place called the Internet of Things (IoT), The IoT enables physical world objects in our surrounding to be connected to the Internet. ISM (Industrial Scientific Medical) band that is 2.4GHz band authorized free of charge is being widely used for smart devices. Accordingly studies have been continuously conducted on the possibility of coexistence among nodes using ISM band. In particular, the interference of IEEE 802.11b based Wi-Fi devices using overlapping channel during communication among IEEE 802.15.4 based wireless sensor nodes suitable for low-power, low-speed communication using ISM band. Because serious network performance deterioration of wireless sensor networks. In this paper, we will propose an algorithm that identifies the possibility of using more accurate channels by mixing utilization of interference signal and RSSI (Received Signal Strength Indicator) Min/Max/Activity of Interference signal by wireless sensor nodes. In addition, it will verify our algorithm by using OPNET Network verification simulator.

  • 부산광역시는 지역사회기반 마을건강센터를 운영하고 있는데 이는 경제적 취약계층을 중심으로 제공되는 보건의료서비스와는 달리, 개인이 아닌 마을 공동체를 중심으로 건강증진 서비스 제공하는 목적을 갖고 있다. 2021년 기준 부산 지역 내 71개소 센터가 설치되어 운영 중이고 본 논문에서는 마을건강센터를 디지털 전환하는 방법을 모색하고자 한다. 마을건강센터에서 활용가능한 디지털 기술은 첫째, 스마트 기기를 활용한 건강지표 구축으로 각종 질환의 관리체계를 고도화하는 것, 둘째, 사물인터넷으로 모바일 기술과 함께 로그 라이프 등 각종 건강지표관리 관리하는 것, 셋째, 지능형 로봇으로 독거노인의 말벗과 위급상황 알리미로 활용하는 것이다.

  • The computer system validation consulting industry is growing as domestic GMP companies' overseas exports or advancements increase, and computer system validation has been mandatory in Korea since the 2010s, but domestic CSV consulting companies are small in size and have a short history, revealing many shortcomings in terms of service quality and customer satisfaction when conducting consulting. Research related to consulting has been mainly focused on management consulting and IT consulting, and research related to computer system validation is very insufficient. This study confirmed the effect of consultant competency and service quality on consulting performance, customer satisfaction, and intention to renew contract when performing computer system validation through empirical research on food, pharmaceutical, cosmetics, and medical device companies, which are representative companies in the GMP industry. As a result, it was confirmed that consultant competency and service quality had a significant effect on consulting performance, customer satisfaction, and intention to renew contract. In addition, it was confirmed that the reputation and expertise of consulting companies had a moderating effect on the relationship between consultant competency and consulting performance.

투시 검사 시 장비에 따른 환자와 시술자의 입사표면선량 연구 (Fluoroscopic the equipment study in accordance with the entrance surface dose study of patients and practitioners)

  • Purpose : Fluoroscopy equipment, depending on the type of changes that occur in the patient's position ESD and study the patient's scatter ray of ESD Practitioners considered a comparative analysis was to evaluate the correct dose. Materials and Methods : HITACHI four overtube type TU-8000 Flat Detector and Under tube C-Arm Philips' Multi Diagnost Eleva with Flat Detector type were measured by. Each devices is a measure of the patient's esd randophantom position in tabel unfors Xi multi funtion then fixed to the abdomen fluoroscopy and 10 seconds, spot was measured three times, practitioners of the incident surface dose by considering the patient's scatter ray of the table for each device in the average human stomach 21cm thickness acrylic phantom ($25cm{\times}25cm$) Place the practitioner position after position randophantom unfors Xi multi funtion in the thyroid and stomach 1 minute by a fixed one-time fluoroscopy and measured. Results : 10 seconds and the patient perspective of the c-arm ESD 1.2 times smaller on the AP and oblique measurements were measured in the 6-13 times smaller. spot positions to changes in the measured three times on the AP of the abdomen, ESD is 18 times smaller c-arm measurements and the oblique measurement was 19-30 times smaller. And 1 minute at practitioners fluoroscopy esd in the thyroid 2.12 times the c-arm, chest 1.75 times less the dose was measured. On the AP, depending on the device, but the lack of dose difference oblique positions of the two devices depending on changes in the area due to changes in both the AP than on the dose increased, the difference in dose between the two devices, the maximum difference was approximately 27 times. Conclusion : Fluoroscopic equipment at the time of inspection in accordance with changes in dose according to the patient and the patient's positions changes, because the area of the scatter ray considering the change of dose measurements be made, and study of the equipment according to the characteristics of the efficiency and the exposure of the patient and practitioner is considered smooth study equipment manufacturers that can be done is to build the system and think that is also important. Various fluoroscopy when you check future changes in many factors of change in dose for the equipment in the laboratory system by considering the scatter ray radiation shielding for the management to take advantage of reckless undertube have been utilized as more exposure Reduction activities can help is considered as the direction.
