A Study of Sasang Constitution in Kimchi (김치에 대한 사상체질의학적(四象體質醫學的) 고찰(考察)(I))

  • Purposes This study is for the constitutional health in daily life by adding the Sasang Constitutional points to the kimchi's nutritional ones. It lays the foundation of the development of both new medical kimchis and production of various kinds of kimchis according to different constitution. Methods We have examined the Keemy(氣味) and effect(效能) of kimchi represented in the Korean medicine [Dongyibogam(東醫寶鑑), Hyangyaggibseongbang(鄕藥集成方), Choongyakdaesajeon(中葯大辭典)] and Leejema's literature [DongyiSoosebowon(東醫壽世保元), Dongmooyugo(東武遺稿), Chobon kwon(草本卷)]. Results 1) There are 18 kinds of materials in kimchi, 9 main materials and 9 additional ones. 2) There are mustard, red pepper, garlic, leek, ginger, salt, green onion, and dropwort as good materials of kimchi for Soeumin, which are mostly Shinon(辛溫) or Yeoll(熱). 3) There are eggplant, oyster, chinese cabbage, shrimp, cucumber, and sesame as good materials of kimchi for Soyangin, which are mostly Gam(甘) or Ham(寒) or Ryang(凉), Han(寒). 4) There are bracken, perilla, radish, and taro as good materials of kimchi for Taeumin. which are mostly ShinGam(辛甘) or OnPyeong(溫平), Ryang(凉) Han(寒).

Significant Efficacy of Additional Concurrent Chemotherapy with Radiotherapy for Postoperative Cervical Cancer with Risk Factors: a Systematic Review and Meta-analysis

  • Background: Whether concurrent chemotherapy treatment is superior to radiotherapy alone as an adjuvant regimen for postoperative cervical carcinoma with risk factors remains controversial. Materials and Methods: A literature search strategy examined Pubmed, Embase, the Cochrane Library, the China National Knowledge Internet Web, the Chinese Biomedical Database and the Wanfang Database. Article reference lists and scientific meeting abstracts were also screened. Controlled trials comparing concurrent chemoradiotherapy versus radiotherapy alone in postoperative cervical cancer were included. The methodological quality of non-randomized controlled trials was evaluated using the Newcastle-Ottawa Scale. Randomized controlled studies were evaluated with the Cochrane handbook. A meta-analysis was performed with RevMan 5.3. Results: A total of 1,073 patients from 11 clinical trials were analysed, with 582 patients in the concurrent chemoradiotherapy group and 491 patients in the radiotherapy group. Hazard ratios (HR) of 0.47 (95% CI 0.31-0.72) and 0.50 (95% CI 0.35-0.72) were observed for overall survival and progression-free survival, indicating a benefit from the additional use of concurrent chemotherapy. Subgroup analyses demonstrated that cervical cancer with high risk factors significantly benefitted from concurrent chemotherapy when examining overall survival (HR 0.44, 95% CI 0.28-0.67) and progression-free survival (HR 0.48, 95% CI 0.33-0.70), but patients with intermediate risk factors showed no benefit from concurrent chemotherapy in overall survival (HR 1.72, 95% CI 0.28-10.41) and progression-free survival (HR 1.09, 95% CI 0.19-6.14). No significant differences were observed for grade 3-4 anaemia (risk ratio (RR) 3.87, 95% CI 0.69-21.84), grade 3-4 thrombocytopenia (RR 3.04, 95% CI 0.88-10.58), grade 3-4 vomiting or nausea (RR 1.71, 95% CI 0.27-10.96), or grade 3-4 diarrhoea (RR 1.40, 95% CI 0.69-2.83). Significant differences were observed for grade 3-4 neutropenia in favour of the radiotherapy group (RR 7.23, 95% CI 3.94-13.26). Conclusions: In conclusion, concurrent chemoradiotherapy improves survival in postoperative cervical cancer with high risk factors but not in those with intermediate risk factors.

Associations of ERCC4 rs1800067 Polymorphism with Cancer Risk: an Updated Meta-analysis

  • Background: Results from previous studies concerning the association of ERCC4 rs1800067 polymorphism with risk of cancer were inconsistent. To explore the exact relation with susceptibility, we conducted the present meta-analysis. Materials and Methods: Literature of electronic databases including PubMed, Web of Science, EMBASE, Wanfang and Chinese National Knowledge Infrastructure (CNKI) were systematically searched. ORs and their 95%CIs were used to assess the strength of associations between ERCC4 polymorphism and cancer risk. Results: There was no significant association between ERCC4 rs1800067 AA or AG genotypes and overall risk of cancer (AA vs. GG: OR=0.998, 95%CI=0.670-1.486, P=0.992; AG vs. GG: OR=0.970, 95%CI=0.888-1.061, P=0.508). A dominant genetic model also did not demonstrate significant association of (AA+AG) genotype carriers with altered risk of overall cancer (OR=0.985, 95%CI=0.909-1.068, P=0.719). In addition, no significant association was observed between A allele of ERCC4 rs1800067 A/G polymorphism and altered cancer risk compared with G allele (OR=0.952, 95%CI=0.851-1.063, P=0.381). Subgroup analysis suggested that AA genotype carriers were significantly associated with decreased risk of glioma compared with wild-type GG genotype individuals (OR=0.523, 95%CI=0.275-0.993, P=0.048). For subgroup of lung cancer, A allele of ERCC4 rs1800067 A/G polymorphism was significantly associated with decreased risk of lung cancer compared with G allele (OR=0.806, 95%CI=0.697-0.931, P=0.003). Conclusions: This meta-analysis indicated that ERCC4 rs1800067 A/G polymorphism might not be associated with risk of overall cancer. However, individuals with the AA genotype were associated with significantly reduced risk of glioma compared with wild-type GG genotype; The A allele was associated with significantly reduced risk of lung cancer compared with G allele. Future large-scale studies performed in multiple populations are warranted to confirm our results.

  • A growing interest in health has been leading to more interest in function of food rather than its taste and nutrition. Usually we think chemical-free or oriental food to be good for health. Yaksun is a food with herbal stuff, which reflects our desire for health and longevity and China's splendid food culture. It is based on the traditional medical thought of the Orient that both medicine and food have the same origin. Yaksun is a traditional functional or nourishing food with both nutritive and medicinal elements, which therefore provides such effects as epicurean pleasure, prevention of diseases and improvement of health. It is recorded that in China there was a dietitian in the royal court from the period of Seoju(B.C. 11∼7), who was responsible for supervising and controlling the health, nutrition and disease of an emperor. Therefore, herbal food has a very long history. Currently, there are many Yaksun stores in Japan and Taiwan as well as China, which are one of popular tourist destinations. Basically Yaksun follows the theory of the Oriental medicine. Yaksun is categorized into four(cold, cool, hot and warm) according to its temperature and into five basic tastes(bitter, sweet, pungent, salty and sour). Yaksun has the functions such as preventing diseases and aging improving internal organs, and healing diseases. In China many colleges of medicine have a department of Yaksun studies through which systematic researches have been being made since a long time ago. For Korea, the discipline of Yaksun studies is still at the beginning stage. To respond to a growing interest in health and prevent chronic adult diseases, it is required to develop a functional food by establishing a systematic theory of Yaksun and making more researches into it.

A Literature Study on Treatment of Infantile Anorexia Based on Chinese Medical Journals (소아 거식증 (Infantile Anorexia)의 한의학(韓醫學)적 치료에 관한 고찰 - 중의학(中醫學) 저널을 중심으로 -)

  • Objectives The purpose of this study is to find a literature study of infantile anorexia clinical cases with Traditional Chinese Medicine (TCM) to utilize with Korean medicine treatment methods. Methods 5 TCM journals with the highest impact factors were searched and selected among the journals listed in www.cnki.net. The search keywords were, "Children's Anorexia", "Infantile Apositia", "Infantile Anorexia", "Anorexia Nervosa", and "child of anorexia". he search was limited to the publication date from 2000 to 2012. Results Total 20 studies were selected: Acupuncture (5), Herbal medicine (9), Complex Treatment (3), External Application (2) and Moxibustion (1). Among the Acupuncture related studies, 1 study tested the effect of Sifeng (EX-UE10, 四縫), 3 studies tested the effect of Sifeng (EX-UE10, 四縫) and other acupoints treatment together, and 1 study tested the effect of combination of other acupoints. In all these studies, the study groups showed better therapeutic effects compared to the control groups. The Herbal medicine related studies showed the improvement in appetite and weight as a result of Herbal Medicines treatment. Among the Complex Treatment related studies, 2 studies tested the effect of Acupuncture and Chiropractic co-treatment, 1 study tested the effect of Moxibustion and Herbal medicine co-treatment. In all these studies, symptoms of Infantile anorexia showed significantly improved. Other papers related to External Application and Moxibustion treatment were reported improving in appetite and had other the clinical effects as well. Conclusions Clinical studies testing the effect of TCM for the treatment of Infantile Anorexia have been conducted in small scales, and all the studies showed a certain level of symptom improvement of the patients in the study groups. These results implicate that the methods in Korean medicine can be highly potential treatment options for the treatment of Infantile Anorexia. Accurate and well-controlled studies in large scale would be required to prove the effect of Korean medicine for the treatment of patients with Infantile Anorexia.

  • Maek Kyoung(Mai Jing, 脈經) is the first chinese specialized book of diagnostics written by Wang Hee(Wang xi, 王熙) from Seo Jin(xi jin, 西晉). He assorted the contents with pulse and medical examination from Nae Kyoung(nei jing, 內經), Nan Kyoung(nan jing, 難經), Sang Han Ron(shang han lun, 傷寒論) and Jung Jang Kyoung(zhong cang jing, 中藏經). And united with his own research, he explains medical examination and the way of talking the pulse by classifying into entrance and class. Maek Kyoung(Mai Jing, 脈經) was imported Korea by Ji Chong(zhi cong, 知聰) AD 561, and he passed through Ko Ku Ryeo(gao gou li, 高句麗) with Nae Wei Jeon(nei wai dian, 內外典), Yak Seo(yao shu, 藥書), Myung Dang Do(ming tang tu, 明堂圖) and Maek Kyoung(Mai Jing, 脈經) to be naturalized in Japan. This treatise make a special study of the second volume of the Maek Kyoung. It consists of the four chapters: Pyoung Sam Kwan Eum Yang E Sip Sa Ki Maek Je II(平三關陰陽二十四氣脈第一), Pyoung In Young Sin Mun Ki Ku Jeon Hu Maek Je E(平人迎神門氣口前後脈第二), Pyoung Sam Kwan Byoung Hu Byoung Chi Eui Je Sam(平三關病侯幷治宜第三), and Pyoung Gi Kyoung Pal Maek Byoung Je Sa(平寄經八脈病第四). Bi Geup Choun Geum Yo Bang(備急千金要方) and Eui Hak Ip Mun(醫學入門) quoted from the contents in the second volume of Maek Kyoung, and Bin Ho Maek Hak(瀕湖脈學), Gi Kyoung Pal Maek Go(奇經八脈攷) and Maek Eo(脈語) extracted from contents in the second volume of the Maek Kyoung and requoted from this contents. Contents in the second volume of the Maek Kyoung have very valuable data like that, but the literature on this subject in the form of a treatise has not been yet in Korea. So I hope this study will be useful to develope Diagnostics by correcting translation and interpretation and fixing wrong translation.

A comparative Study on the Combined Oriental and Western Medicine(COWM) in Four Northeast Countries (동북아시아 4개국의 양.한방 의료협진체계 비교)

  • Since 1990s, the use of Complementary and Alternative Medicine(CAM) has been rising rapidly all of the world. In 1983, WHO recommended that the traditional medicine actively be utilized. At the end of 20th century, as chronic and intractable diseases increased in western countries, traditional medicine has attracted considerable attention. COWM shows possibilities of new approaches for these intractable diseases. Thus, we try to show our proper approach of COWM through the international comparative study. In order to fulfill the objectives, we applied the following methodology: 1) Literature review on previous study, 2) Local survey using self-administered questionnaire, and 3) FGI(Focus Group Interview) with local experts. The results were as follows : Three Asian countries, China, Korea and Taiwan, are very active in implementing COWM policy. Japan, however, has independent system of unified medicine. In regards to the combined care policy and system, China has the most advanced COWM system among four countries. In respect to combined care education, it is needed to increase the COWM education contents and the amount of cross educational curriculum. Based on the current COWM system, Chinese, Japanese and Taiwanese doctors can prescribe both oriental and western drugs. But, Korean medical law prohibits western doctors and oriental doctors from prescribing the counterpart´s medicine. So, the revision of current medical law is urgent for COWM in Korea. And when it comes to patient satisfaction, more than fifty percent responded positively in China, Korea and Taiwan. To achieve the goal of COWM ; 1) mutual understanding and recognition of COWM is essential. 2) institutional and legal support system for COWM is desperately urgent. 3) possible international collaboration and cooperation should be sought to untangle these complex cultural dilemmas.

Short-course Versus Long-course Preoperative Radiotherapy plus Delayed Surgery in the Treatment of Rectal Cancer: a Meta-analysis

  • Background: Short-course preoperative radiation (SCRT) with delayed surgery was found to increase pathologic complete response (pCR) rates in several trials. However, there was no clear answer on whether SCRT or long-course chemo-radiotherapy (LCRT) is more effective. Therefore we conducted this meta-analysis to evaluate the safety and efficacy of SCRT versus LCRT, both with delayed surgery, for treatment of rectal cancer. Materials and Methods: The literature was searched from PubMed, EMBASE, Web of Science, Cochrane Library and clinicaltrials.gov up to November, 2014. Quality of the randomized controlled trials (RCTs) was evaluated according to the Cochrane's risk of bias tool of RCT. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was used to rate the level of evidence. Review Manager 5.3 was employed for statistical analysis. Pooled risk ratios (RRs) and 95% confidence intervals (CIs) were calculated. Results: Three RCTs, with a total of 357 rectal cancer patients, were included in this systematic review. Metaanalysis results demonstrated there were no significantly differences in sphincter preservation rate, local recurrence rate, grade 3~4 acute toxicity, R0 resection rate and downstaging rate. Compared with SCRT, LCRT was associated with significant increase in the pCR rate [RR=0.49, 95%CI (0.31, 0.78), P=0.003]. Conclusions: In terms of sphincter preservation rate, local recurrence rate, grade 3~4 acute toxicity, R0 resection rate and downstaging rate, SCRT with delayed surgery is as effective as LCRT with delayed surgery for management of rectal cancer. LCRT significantly increased pCR rate compared with SCRT. Due to risk of bias and imprecision, further multi-center large sample RCTs were needed to confirm this conclusion.

The Literature Study of Yukjin, good long-term storage of the six oriental medicinal herbs (육진량약(六陳良藥)에 관한 문헌고찰)

  • Objectives : The purpose of this research was to examine the oriental medical term 'Yukjin(in chinese, liuchen)' i.e. 'good long-term storage of the six oriental medicinal herbs'. Yukjin has meant the six sorts of drugs which were well known for the older they stored, the better efficacious they were. This research was carried out to obtain more conclusive information about the time and the reason of the giving a naming Yukjin, and to arrange Yukjin's properties, toxicities and processes. Methods : We investigated the documents recorded in the medical classics of Yukjin and did the current issues and recent experimental researches of these medicines. Results : 1. The properties and flavors of Yukjin are pungent and bitter. When they were stored for a long time after gathering, their toxicities and stimuli were growing thinner, regarded as positive clinically with age and they should be used after processing. 2. The efficacies of Yukjin are not always in proportion to storage period. In case, they are stored too long, their efficacies and qualities may be deteriorated though volatility or pungent flavors are enfeebled. Conclusions : Consequently, toxicities and strong flavors of Yukjin can be reduced in various processes. So the traditional theory of long-term preservation is not necessarily retained. It may be desirable to store them for 1 to 3 years after harvest, to safen their toxicities and adverse reactions in the way of processing. If we use them properly according to oriental medicine method, it will be effective on treating and preventing the various diseases.
