A Study on the Application of the Theory of Sameumsamyang(三陰三陽) Gaehapchu(開闔樞) to Clinical Treatment - Focusing on the Explanation of Diseases and Prescriptions (삼음삼양(三陰三陽) 개합추(開闔樞) 이론(理論)의 임상적(臨床的) 활용(活用)에 대한 고찰(考察) - 병증(病證) 및 방약(方藥)에 대한 설명을 중심으로 -)

  • Objectives : In this article, the application of the theory of Sameumsamyang Gaehapchu to clinical treatment in some important medical books of traditional medicine, for example Jienuelunshu(?虐論疏) etc, is investigated. Methods : The literatures containing the application were searched in the Sikuquanshu(四庫全書) database by key words of Sameumsamyang and Gaehapchu, and then analysis and investigation of significant documents has followed. Results : The application of the theory of Sameumsamyang Gaehapchu to clinical treatment has been mainly done with some characteristics that are expansion to the theory of visceral organs, suggestion of various medicines, and definite explanation of the relationship between Gaehapchu. These characteristics made it easy to predict prognosis of diseases and to determine order of treatment. Conclusions : After the age of Huangdineijing, the theory of Sameumsamyang Gaehapchu to clinical treatment has been developed and realized through application of the theory to clinical treatment.

Correlation between Application Rates for Specialized Majors and Physician Income and Non-Benefit Percentage (전문과별 전공의 지원율과 의사소득, 비급여율 간의 상관관계)

  • Purposes: This study aims to analyze the correlation with the current status of the medical resident application rate, physician's income, and non-benefit rates of majors in each specialty subject and to suggest implications. Methodology: First, it analyzes the correlation between the medical resident application rate by specialty subject and the income of physicians. Second, it analyzes the correlation between the income of specialists and the non-benefit rate for each specialty subject at the clinic level. Findings: First, a significant positive correlation was found between the medical resident application rate and the average physician's income for each specialty subject (r=.718, p<.01). Second, a significant positive correlation was observed between physician income at the practitioner level by medical specialty and the non-benefit rate (r=.726, p<.01). Practical Implications: In this study, the correlation between medical resident application rate by specialty subject and physician's income, non-payment and physician's income was confirmed. Choosing a department that is less risky and can earn higher income is a natural phenomenon, but it is necessary to adjust the physicians crowding phenomenon to a specific specialty subject at the government level to maintain the medical system.

Comprehensive Strategies through the Application of Medical Information Management for the Mitigation of Lumbago

  • Back pain is a pain that occurs from the waist to the legs. If back pain continues, the quality of life is mentally and physically degraded. The purpose of this study is to implement comprehensive strategies through the application of medical information management for the mitigation of lumbago. This study was analyzed using interviews and surveys from Januanry 17 to March 22, 2024. It is classified as 43 members of the experimental group and 43 members of the control group. Low back pain symptoms and treatment were measured by a t-test before and after the application of medical information management. The results of this study are as follows. Firstly, in the experimental group, 67.4% of those who sat for more than 10 hours were significantly higher than 32.6% of those who sat for less than 9 hours a day(X2=3.19, p=.00). Secondly, the forward bending has been increased significantly from the average of 30.58 points before the application of medical information management to 46.27 points after the application(t=-1.65, p=.03). Thirdly, lumbago has been shown to decrease continuously since 6 days. Fourthly, stretching continued to rise significantly from 3 days after applying medical information management. The results of this paper will contribute to reducing symptoms in patients with lumbago.

Development of Navigation Program for Cancer Patients using Mobile application (암환자를 위한 모바일 앱용 네비게이션프로그램 개발)

  • Objectives: The objectives of this research were to develop and evaluate a mobile application for navigation program for cancer patients who might experience some difficulties in obtaining and understanding further schedules, directions due to flooding information at a time and scattered educational materials. Methods: A mobile application was developed an educational mobile app for cancer patients based on a systematic instructional design model called ADDIE (Analysis, Design, Development, Implementation, and Evaluation) model. The developed application was evaluated by 76 users through a questionnaire of satisfaction. Results: A mobile app contains educational contents for cancer patients, based on their satisfaction, demand and knowledge about cancer education and information services. It contains management of symptoms, management of my schedule, and information about chemotherapy, FAQ, symptoms dangerous enough to contact the hospital, personal history about how to overcome cancer, hospital convenience facilities and education schedule of cancer center. A result of the evaluation of user's satisfaction showed 59.4% responding 'Satisfied' and 27.4% 'Very satisfied'. Conclusion: The personalized information and education contents for cancer patients by using the mobile application was given to cancer patient and then educational outcomes became more effective. The development of the application which persons can use regardless of time and place enables health care providers to acquire the foundation of the patients-oriented educational system. Education satisfaction and knowledge level was increased, after using mobile application.

Removal of Chromium (VI) by Escherichia coli Cells Expressing Cytoplasmic or Surface-Displayed ChrB: a Comparative Study

  • Various genetically engineered microorganisms have been developed for the removal of heavy metal contaminants. Metal biosorption by whole-cell biosorbents can be enhanced by overproduction of metal-binding proteins/peptides in the cytoplasm or on the cell surface. However, few studies have compared the biosorption capacity of whole cells expressing intracellular or surface-displayed metal-adsorbing proteins. In this study, several constructs were prepared for expressing intracellular and surface-displayed Ochrobactrum tritici 5bvl1 ChrB in Escherichia coli BL21(DE3) cells. E. coli cells expressing surface-displayed ChrB removed more Cr(VI) from aqueous solutions than cells with cytoplasmic ChrB under the same conditions. However, intracellular ChrB was less susceptible to variation in extracellular conditions (pH and ionic strength), and more effectively removed Cr(VI) from industrial wastewater than the surface-displayed ChrB at low pH (<3). An adsorption-desorption experiment demonstrated that compared with intracellular accumulation, cell-surface adsorption is reversible, which allows easy desorption of the adsorbed metal ions and regeneration of the bioadsorbent. In addition, an intrinsic ChrB protein fluorescence assay suggested that pH and salinity may influence the Cr(VI) adsorption capacity of ChrB-expressing E. coli cells by modulating the ChrB protein conformation. Although the characteristics of ChrB may not be universal for all metal-binding proteins, our study provides new insights into different engineering strategies for whole-cell biosorbents for removing heavy metals from industrial effluents.

  • Radionuclide therapy has been an important field in nuclear medicine. In radionuclide therapy, relevant evaluation of Internally absorbed dose is essential for the achievement of efficient and sufficient treatment of incurable disease, and can be accomplish by means of accurate measurement of radioactivity in body and its changes with time. Recently, the advances of nuclear medicine imaging and multi modality imaging processing techniques can provide change of more accurate and easier measurement of the measures commented above, in cooperation of conventional imaging based approaches. in this review, basic concept for internal dosimetry using nuclear medicine imaging is summarized with several check points which should be considered In real practice.

  • Wavelength selection and prediction algorithm for determining hematocrit are investigated. A model based on the difference in optical density induced by the pulsation of heart beat is developed by taking approximation of Twersky's theory on the assumption that the variation of blood vessel size is small during arterial pulsing[1]. A device is constructed with a five-wavelength LED array as light source. The selected wavelengths are two isobestic points and three in compensation for tissue scattering. Data are collected from 549 out-patients who are randomly grouped as calibration and prediction sets. The range of percent hematocrit was 19.3∼51.8. The ratio of the variations of optical density between systole and diastole at two different wavelengths is used as a variable. We selected several such variables that show high reproducibility among all variables. Multiple linear regression analysis is made. The relative percent error is 8% and the standard deviation is 3.67 for the calibration set. The relative % error and standard deviation of the prediction set are 8.2% and 3.69 respectively. We successfully demonstrate the possibility of non-invasive hematocrit measurement, particularly, using the wavelengths below 1000nm.

Introduction to the New Version of PWS Application and It's Use in Medical Practice

  • Today's mobile phones and tablet PCs offer a considerably wider range of functionalities than before. Mobile applications (apps) are increasingly used for managing various daily health tasks. Currently, more than 165,000 health-related apps are offered on all the stores of different platforms. Pf Jin and the Association for Research on MPS and Rare Diseases (AMARD) have helped Prader-Willi syndrome (PWS) families through medical information and family support since 2015. AMARD developed the first mobile application for Korean patients with PWS, which was released to a limited number of patients under the age of 3 and only provided to Android users. The first version of the PWS application focused on growth hormone therapy and the assessment of growth and development by parents in infant and early-childhood PWS patients. The 2016 version of the PWS application has been improved in many different ways. We have expanded the subjects of the application to late childhood and adolescent groups, changed the user interface accordingly, and made the application available for iOS users. We will show the specialized growth curves of older children with PWS. Therefore, patients with PWS over the age of 3 and their parents can assess the patients' growth. Additionally, we have upgraded the growth hormone therapy menu by improving the input system for the growth hormone therapy injection schedule and the daily growth profile (height and weight). We expect that the new version of the PWS application will help many PWS families cope with growth hormone therapy and evaluate the effects of growth hormones in better ways. Additionally, we are making a constant effort to provide more useful information about patients with PWS in many aspects.

The Development of Image Processing System for Medical Robot Remote Application (의료용 로봇 원격 응용을 위한 영상처리 시스템 개발)

  • In this paper, web-base image processing system has been implemented for remote-controlled medical robot applications. The developed software system was hierarchically composed of diverse image processing and remote operation modules, and the hierarchical composition was satisfied the expandability to higher level application and the accessibility over the web. It can also support diverse file formats including DICOM, VRML, and CAD(STL) to display, transmit, store and share the processed images depending on application environment. Message-based data exchange, object-oriented module and open-source based software configuration will enable the dynamic combination associated with diverse remote medical application requirements.

Pilot Study of Measuring Daily Quality of Life in Breast Cancer Patients Through Mobile Application (모바일 응용프로그램을 이용한 유방암 환자의 삶의 질 측정 초기연구)

  • Objective : As treatments for breast cancer require long period of time, the patients need continuous monitoring and management of their health related quality of life. The purpose of this preliminary study is to explore the utility of monitoring quality of life using a mobile phone based application, which was developed to enable the patients to report daily quality of life more efficiently. Methods : Department of psychiatry, breast cancer center and U-health team of Asan Medical Center collaborated in developing a mobile application titled 'DugunDugun'. Through the application, patients with breast cancer receiving neo-adjuvant chemotherapy could report on various questions about their daily quality of life. We extracted and explored the daily basis of the data of sleep duration, number of awake, sleep satisfaction, mood and anxiety from nine patients who reported on the questionnaire for more than a month. Results : The longitudinal data collected using our mobile application well represented the characteristics of individual sleep patterns and daily emotional changes. Most of the patients showed high daily variations in the scores of their quality of life while their averaged scores were similar among patients. The patient with highest daily variations in the subjective sleep quality and daily emotional change had highest score corresponding to moderate depression in Beck Depression Inventory (BDI). Conclusion : The results suggest that mobile application could be a useful tool for monitoring and collecting the daily quality of life in the patients with breast cancer. We expect that mobile application could facilitate early detection and proper intervention for the cancer patients at psychological crisis.
