For the management of a secondary spontaneous pneumothorax, videothoracoscopic surgery may offer the potential therapeutic benefits of a minimally invasive approach. We report on a series of 36 patients(33 men and 3 women) with a mean age of 56.3 years(range, 31 to 80 years) who underwent thoracoscopic surgical procedures for the treatment of secondary spontaneous pneumothorax. Twenty-one patients had emphysema and 20 patients had old pulmonary tuberculosis. Nineteen patients presented a persistent severe air leak more than 3 days preoperatively and 15 patients had more than one recurrence. Bullectomy or exclusion of the lesion was performed in 33 patients. Mechanical pleurodesis was performed in the entire patients, talc was sprayed in 22 and vibramycin in 14. Mild pleural adhesion at the upper lobe was shown in 10 patients and severe pleural adhesion in 7 patients. One patient with persistent air leak died of persistent air leak and respiratory failure. The mean postoperative stay was 7.0 days(range, 2 to 17 days). At a mean follow-up of 15.8 months (range, 5 to 45 months), no pneumothorax had recurred. In comparison with the result of the treatment for 112 patients with primary spontaneous pneumothorax, the operating time was not significantly longer and there were no more primary treatment failures, but the duration of postoperative chest drainage and hospital stay was longer. Videothoracoscopic surgery has proved to be an effective treatment for secondary spontaneous pneumothorax in elderly patients who represent high-risk candidates for thoracotomy.
In effort to determine whether aircraft noise can have health effects such as hearing loss, hypertension and psychological stress, a total of 111 male professors and administrative officers working a college near a military airport in Korea(exposed group) and a total of 168 males and 112 females matched by age groups(control groups) were analyzed. Personal noise exposure and indoor and outdoor sound level of jet aircraft noise were measured at the exposed are3. And pure tone, air conduction test and measurement of blood pressure were given to the exposed(males) and matched control groups (males and females). BEPSI(Brief Encounter Psychological Instrument) and psychological response to aircraft noise were examined for the exposed group. The noise dosimetry results revealed time-weighted averages(TWAs) that ranged from 61 to 68 dBA. However the levels encountered during taking off jet airplanes reached 126 dBA for two half minutes time period. Th, audiometric, test showed that mean values of HTL(hearing threshold level) in exposed group at every frequency(500, 1,000, 2,000, 4,000, and 8,000 Hz were much lower than them of male and female control groups. And in old age groups, interaction of age and noise was observed at 8,000 Hz in both ears(p<0.05). Conclusively, aircraft noise does not appear to induce hearing loss directly in high frequency, but may decreased hearing threshold level by interaction of aging process and noise exposure. However, difference of mean values of exposed and control groups on blood pressure was not significantly. In psychological test, annoyance was the most severe psychological response to noise in exposed group, but mean value of BEPSI was not correlated with job duration in exposed group.

The purpose of this study was to identify and analyze the rate of change of air respirator consumption based on field fire fighting activities, The results of the experiment are as follows. The mean air consumption of the Five subjects was $15.56{\ell}/min$ when standing still, $32.43{\ell}/min$ when walking with normal pace, $43.07{\ell}/min$ when the fire hoses was expanded and arranged, $55.28{\ell}/min$ when climbing stairs, The situation of running up the stairs and hitting the hammer continuously increases to $127.14{\ell}/min$, which means that the consumption of air increases according to the energy consumption. Despite being the oldest, Experimental subject A was $13.23{\ell}/min$ when standing still, $29.33{\ell}/min$ when walking normally, and $41.08{\ell}/min$ when the fire hose was deployed and arranged. This is a result of familiarity with deep and slow breathing methods. The average respiratory use rate of subjects D was $63.58{\ell}/min$. The reason for this is that obesity seems to increase the air consumption, and it is considered that the physical strength is exhausted by the action of the hammer, which has a greater influence on the increase of air consumption. The subject E had significantly lower air respiratory use rate of $49.90{\ell}/min$. The reason for this is that the age of E is the youngest among the subjects, and it is presumed that it possesses strong physical strength.

The aim of this study was to verify the suitability or unsuitability of Korean body types by measuring the opening force criteria of an air pressurized access door to a smoke control zone. The opening force criteria were verified by comparing the NFSC 501A, NFPA 92A and BS-EN 12101-6 based on the body standards information from the Korean Agency for Technology and Standards. When measuring the opening forces, the posture of the body should be standing upright and pushing an access door with the right hand, which is a criterion for designing doors. As a result of analyzing the actual measurement results, the pushing force of men and women in their 30's was the maximum value and the forces in those in their 60's was the minimum value. In addition, the deviations in the pushing forces varied considerably. As a result of comparing the NFSC 501A, the men showed lower values than the criteria in every gender and age variable except for the 20's, 30's and 50's variable. A comparison of the criteria of NFPA 92A showed that the mean of the measured values from every gender and age was also lower than the criteria. In addition, when comparing the criteria of BS-EN 12101-6, it was found that the men in every age variable were higher than the criteria. On the other hand, the women in every age variable were lower than the criteria. Therefore, considering the Korean body type against the Western body type, it was decided that the opening force of an access door to a smoke control area to make a downward adjustment should be 110 N in the local criteria. Furthermore, the criteria should consider the characteristics of buildings and users because an optional application of the international standard is not necessarily suitable for local situations.
This study aimed to investigate the difference of X-ray exposure by comparing and analyzing entrance surface dose and absorbed dose according to the frame change in coronary angiography using an X-ray machine. Moreover, appropriate frame selection measures for examination, including the effect of frame change on the image quality, were sought by measuring and analyzing the SNR and CNR of the image through image J. The study was conducted on 30 patients (19 males and 11 females) who underwent CAG at this hospital from June 2017 to October 2017. In regard to the patients, their age range was 49-82 years (mean of $65{\pm}9$ years), body weight was 45-91 kg (mean of $67{\pm}8.9kg$), height was 150-179cm (mean of $165.1{\pm}8.9kg$), and BMI was 19.5-30.5(mean of $24.5{\pm}2.9$). For the entrance surface dose and absorbed dose, air kerma value and DAP were obtained and analyzed retrospectively. The SNR and CNR were measured and analyzed through imageJ, and the result values were derived by applying the values to the formula. As for the statistical analyses, the correlations between the entrance surface dose and absorbed dose, and between the SNR and CNR were analyzed by using the SPSS statistical program. The relationship between the entrance surface dose and absorbed dose was not statistically significant for both 10 f/s and 15 f/s (p>0.05). In terms of the relationship between the SNR and CNR, the SNR ($3.374{\pm}2.1297$) and CNR ($0.234{\pm}0.2249$) in 10 f/s were $1.43{\pm}0.4861$ and $0.132{\pm}0.0555$ lower, respectively, than the SNR ($4.929{\pm}2.8532$) and CNR ($0.391{\pm}0.3025$) in 15 f/s, which were not statistically significant (p>0.05). In the correlation analysis, statistically significant results were obtained among the BMI, air kerma, and DAP; between air kerma and DAP; and between SNR and CNR (p<0.001, p<0.001). In conclusion, there was no significant difference between the entrance surface dose and absorbed dose even when the images were taken by changing the frame from 10 f/s to 15 f/s at the time of the coronary angiography. SNR and CNR increased at 15 f/s than at 10 f/s, but they were not statistically significant. Therefore, this study suggests that the concern of the patient and practitioner regarding image quality degradation, as well as the problem of X-ray exposure caused by imaging at 10 f/s and 15 f/s, may be reduced.
In order to prepare the fundamental data for the health promotion by assessing the exposure level of styrene, the author determined the concentration of mandelic acid and phenylglyoxylic acid in urine of 42 workers who were exposed to styrene by high performance liquid chromatography and surveyed 16 symptoms, by questionnaire and also tested neurobehavioral test(digit symbol, benton visual retention) in 2 FRP plants of Kyung Nam area from July to September, 1995. Control was sampled by age sex matching method. The concentration of styrene in air was determined by gas chromatography. The results were as follows; 1. Geometric mean concentration of styrene in air was 17.4ppm, geometric mean concentration of mandelic acid(MA) in urine were 404.3mg/g creatinine for exposure group, 46.4mg/g creatinine for control group, geometric mean concentration of phenylglyoxylic acid(PGA) in urine were 57.5mg/g creatinine for exposure group, 9.5mg/g creatinine for control group. Mean concentration of MA and PGA showed statistically significant difference between exposure group and control group(p<0.01). 2. Number of symptom were 2.9 for exposure group, 3.3 for control group, number of digit symbol were 24.1 for exposure group, 32.5 for control group, number of Benton visual retention test were 6.1 for exposure group, 6.0 for control group, respectively. As result of adjusting the education year, number of Benton visual retention test showed statistically significant difference between exposure group and control group(p<0.05). 3. Excellent correlation were observed between environmental styrene exposure and urinary MA(r=0.80), PGA(r=0.73), and MA+PGA(r=0.81).

Background: With its developed port and related industries, the concentration of fine dust is high in Busan compared to other cities in South Korea. Many studies have reported the health effects of fine dust, but there has been a lack of information regarding concentrations of volatile organic compounds among those who exposed to high levels of fine dust. Objectives: This study aimed to define an area with high concentrations of particulate matter and perform biomonitoring surveys among the residents of the area. Methods: Air quality data was collected and the mean level of each district in Busan was derived. We then defined the area with the highest concentrations of PM10 as a target site. Urine samples were collected from the 400 participants and analyzed for VOCs metabolites - trans,trans-Muconic Acid (t,t-MA) and N-AcetylS-(benzyl)-L-cysteine (BMA). Interviews were conducted by trained investigators to examine demographic information. The levels of t,t-MA and BMA were compared with representative South Korean population data (Korean National Environmental Health Survey). The association of the VOC metabolites and fine dust were analyzed by general linear regression analysis. Results: The mean of PM10 in the target site was 42.50 ㎍/m3 from 2018 to 2020. Among the 400 participants in the target site, 74.8% were female and the average age of the participants was 66 years. The geometric mean of t,t-MA was 71.15 ㎍/g creatinine and the BMA was 7.00 ㎍/g creatinine among the residents. The levels were higher than the geometric mean from the 4th KoNEHS. The levels of t,t-MA showed significance in BMI, smoking status, and household income. BMA showed significance in gender and age. Conclusions: Compared to the general population of South Korea, the target site's residents had higher biomonitoring levels. Based on this study, continuous screening for high risk areas, including the target site, and biomonitoring of the residents are required.
To evaluate farmers' workload during harvesting grapes in summer, this study investigated farmers' physiological, psychological responses, work postures and thermal environment around in the field. This field study was conducted in the Anseong County of Kyonggi Province at the end of August. Five career farmers (1 male, 4 females) volunteered as subjects. Three of them were over their sixties. During harvesting grapes in the field, physiological responses were monitored continuously. 1. Air temperature (T/sub a/), air humidity(H/sub a/), black globe temperature(T/sub g/), air velocity and WBGT around the grape field were 26.9℃, 77.7%RH, 32.8℃, 0.08㎧ and 26.3℃, respectively. Because farmers started the harvesting task in early morning, thermal environments weren't conditions to give farmers severe heat strain. 2. The percentage of the work postures was larger in order of standing, walking, and bending one's back posture. Particularly, the percentage of standing posture with raising both arms above shoulder of two farmers was up to 29% and 61% of the total work duration. 3. Rectal temperature (T/sub re/), mean skin temperature (T/sub sk/), clothing microclimate temperature (T/sub cl/) on the chest and the back, heart rate (HR) and energy expenditure (EE) were 37.2℃, 33.1℃, 32.0℃, 32.4℃, 88bpm and 1.3 Kca1/㎡/min respectively. In the point of these physiological results, we evaluated that the harvesting task was a moderate work. 4. All farmers expressed‘hard, hot, humid and slightly uncomfortable’ at the end of works for each subjective questionnaire. The grape harvesting tasks were not evaluated as a very hard work in the point of physiological work standards. But we considered 1) inappropriate work posture (standing posture with raising both arms above shoulder) and 2) farmers' age as burden factors. These findings suggest that adding adequate protective clothing/equipments for farmers may contribute to maintain their body temperature within the normal range, stabilize HR and decrease psychological strain.

Personal exposure relies on characteristics of time activity patterns of the population of concern as human activities impact the timing, location, and level of personal exposure. The information about time spent in microenvironments plays a critical role for personal exposure to air pollutants. It is useful to determine the precise times of the day that the subjects are in certain locations or engaging in specific activities because exposure to some air pollutants can depend on temporal trends. This study investigated time activity pattern for workers of Korean population over 19 years old with 8,778 workers in weekday. The residential indoor times were 12 hours. Time activity was different by gender and the mean times stayed at home in weekday were 12.9 hours in female and 11.42 hours in male, respectively. The major factors on residential indoor time and workplace time were age, monthly income, occupation and industry type, work position, education, and gender. Considering shorter than those in other countries, Korean workers spent less time at home after the working hours. Determinants of time activity pattern need to be taken into account in exposure assessment, epidemiological analyses, exposure simulations, as well as in the development of preventive strategies. Since there are substantial difference of Korean worker activity pattern, this information can be critical for exposure assessment in Korea.

Purpose : The purpose of this study was to determine the prevalence, radiographic appearance, and characteristics of patients with zygomatic air cell defect (ZACD), and to give recommendations concerning radiographic evaluation and surgery. Materials and Methods: Routine panoramic radiographs of 1,400 patients admitted to the Kyungpook National University Hospital Dental Clinic, were retrospectively examined for the clinical and radiographic features of ZACD. Results: ZACD was found in 31 cases, representing a prevalence of 2.2%. Patients with ZACD had a mean age of 27.5 years and a range of 9-52 years. Most ZACD cases were in their thirties. ZACD showed a strong male prediliction, 22 of the 31 subjects were males and 9 were females. Twenty-four cases of ZACD (77.4%) were unilateral, with the half occurring on the right side. In seven cases (22.6%), ZACD was bilateral. Twenty-six (68.4%) of the defects were of unilocular, while twelve (31.6%) of the defects were multilocular. Conclusion: Knowledge of ZACD may be helpful in interpreting images, including panoramic radiographs, in planning surgical treatment of the TMJ and in understanding the spread of pathological processes into the joint.
