A Single Measure of Cancer Burden Combining Incidence with Mortality Rates for Worldwide Application

  • We attempted to develop an indicator combining incidence with mortality rates (single measure of cancer burden, SMCB) and to compare the magnitudes of cancer burden by world region. The SMCB was used to measure the size of cancer burden summarizing the incidence and mortality. The incidence and mortality were divided in equivalent forms and were split. The criteria dividing the size of cancer burden were used as the maximum incidence and mortality by men and women according to the world database, and the value corresponding to 10% of each maximum was set as the cut-off value. In SMCB, the size of cancer burden was highest for men with lung cancer (SMCB=18) and for women with breast cancer (SMCB=14) in MDR (more developed regions) compared to the size of burden in LDR (lower developed regions) (lung, SMCB=11, breast, SMCB=8). For men, the size of cancer burden by region was highest in EURO (SMCB=18, lung), followed by WPRO (SMCB=16, lung), PAHO (SMCB=14, prostate), AFRO (SMCB=8, prostate) and SEARO (SMCB=7, lung). Moreover, for women, the size of cancer burden was greatest in EURO (SMCB=14, breast), followed by PAHO (SMCB=13, breast), AFRO (SMCB=11, cervix uteri), EMRO (SMCB=9, breast) or SEARO (SMCB=8, cervix uteri) and WPRO (SMCB=7, lung). The summary indicator will help to provide a priority setting for reducing cancer burden in health policy.

The Burden and Social Support of Mothers of Nephrotic Syndrome Patients (신증후군 환아 어머니의 부담감과 사회적지지)

  • This study was conducted to identity the degree of burden and social support perceived by mothers of nephrotic syndrome patients. Also, relations between burden and social support were investigated to provide basis data for their family health and nursing intervention. The study subjects were mothers of nephrotic syndrome patients whose children were hospitalized in 2 Pediatric wards of University Hospital in Seoul and 1 in Pusan from Jun. 1, 1998 to Jun. 30, 1998. Burden measurement Instrument for this study was designed by the researcher and its basis in one developed by Montgomery et al.(1985) and the reliability was .78. Also. P.R.Q. Part I, II by Brandt and Weinert(1981) was used as social support measurement instrument and the reliability .71. The data analysis was done by SPSS, t-test, ANOVA Pearson correlation. The result were as follows. 1. Burden felt by mothers shows an average value of 60.82 (Maximum 86, Standard deviation 1.244). 2. Of the mother characteristics, the score of burden was high in case of no religion and low income. Of the patient characteristics, the score burden ranked as high MCNS, doing oral therapy, injection therapy at the same time and negative perceived patients condition. 3. The mean score of support was 77.54(Maximum 96, Standard deviation 1.096). 4. The main supporters were husband (the highest), brother, sister, health speciality and the subject expressed the highest satisfaction toward supporters in chronic disease. 5. Of the patient characteristics, the higher age group and the elder group showed high support. Also, positive perceived patient's condition, high support. 6. The relationship between burden and social support is not significant. In conclusion to the above study, the researcher suggests. 1. The Qualitative research to investigate influential factors on burden of family of nephrotic syndrome patients is needed.

The Study on Burden of Mothers of Children with Nephrotic Syndrome (신증후군 환아 어머니의 부담감에 대한 연구)

  • This study was designed to identity the degree of burden felt by mothers of children with nephrotic syndrome. Also, relations between the subject characteristics and burden were investigated to provide basis data for their family health and nursing intervention. The study subjects were composed of 70 mothers of nephrotic syndrome patients whose children were hospitalized in 2 Pediatric wards of University Hospital in Seoul and 1 in Pusan from Mar. 2nd, 1998 to May. 30th in the same year The questionnaires were used which dealt with burden of mothers. the questionnaires for this study designed and used by researcher placed their basis in Burden Measurement Instrument developed by Montgomery et. al (1985) and the reliability of the used instrument was .78. The data analysis was done by SPSS, t-test, ANOVA and Stepwise Multiple Regression. The results of were as follows. 1. Mean score of burden of subjects was 60.82(Maximum 86, standard deviation 1.244). 2. Of the mothers characteristics, the score of burden was high in case of no religion and low income. 3. Of the patients characteristics, the score of burden ranked as high in MCNS, doing oral therapy and injection therapy at the same time, and negative perceived patients' condition. 4. The degree of burden felt by mothers of children with nephrotic syndrome was significantly predicted by the level of pt's diagnosis(11%), pt's condition(8%), economic state (6%) and pt's sex (5%), respectively. In conclusion to above study, the researcher suggests that the development of instrument for measurement of burden is in much need. Relations between burden and social support should be studied to lessen burden of mothers of children with nephrotic syndrome.

A Maximum Likelihood Decoding Scheme Based on Breadth-First Searching for Multi-Input Multi-Output Systems (여러 입력 여러 출력 시스템에 알맞도록 너비를 먼저 탐색하는 가장 비슷함 복호 방식)

  • The sphere decoder (SD) has recently been proposed to perform maximum likelihood (ML) decoding for multi-input multi-output systems. Employing a 'breadth-first' searching algorithm for closet points in a lattice, we propose a novel ML decoding scheme for multi-input multi-output systems. Simulation results show that the proposed scheme has the same bit error rate performance as the conventional ML decoders while allowing significantly lower computational burden than the SD.

Home Energy Cost and Housing Cost Burden of Urban Monthly Renter Households in Korea and the United States (한국과 미국 도시 월세가구의 가정 에너지 비용과 주거비 부담)

  • The purpose of this study was to examine influence of home energy cost on housing cost burden of Korean and U.S. monthly renter households in urban areas and to explore influences of household and housing characteristics on their home energy cost burden. Microdata for this research was extracted from the 2011 Korean Household Budget Survey and 2009 American Housing Survey. Monthly renter households in urban areas were initially grouped based on household income, followed by a detailed analysis of housing and home energy cost. Findings are as follows: (1) The maximum ratio of home energy cost to household house hold income in Korea was 49% compared to 83% in the US; (2) Energy cost to income ratio were found to have significant influences on housing cost burden and lower income households' housing cost burden was found more vulnerable to their energy cost; (3) In general, the energy cost burden of low-income renter households in Korea tended to be influenced by household size, the number of household members staying at home during daytime hours and housing unit size. The energy cost burden of low-income renter households in the U.S. tended to be influenced by home structure type, size and age, the householder's age, race, educational attainment, the household size, number of wage earners per household, income, and the number of household members between 7 and 17 years of age.

Impact of Adjusted Out-of-Pocket Maximum Rules of Long-stay Admissions in Long-Term Care Hospitals (요양병원 장기입원에 대한 본인부담상한제 개편 영향 분석)

  • Purpose: This study aimed to determine the effect of adjusted out-of-pocket maximum rules in the 'differential co-payment ceiling', which means having a higher burden of co-payment, that expanded to the entire ceiling level in long-stay admission patients in long-term care hospitals(LTCH). Methodology: We used health insurance claim data between January 1, 2022, and December 31, 2022 received from the National Health Insurance Service. The study populations were inpatients in long-term care hospitals more than 1 days during the study period. We performed the difference in characteristics of the LTCH patient of the differential and general ceiling by the chi-square test. We estimated the change of the population, cost, and co-payments per person under the assumption of restructuring. Finding: Based on adjusted out-of-pocket maximum rules in 2023, it was expected that the number of benefits decreases at the high-income level while increasing at the low-income level. The burden of health expenditure after reimbursement of co-payment ceiling, is expected to increase by 65.1% in the highest medical necessity, whereas the low medical necessity would decreases compared to 2022. Practical Implications: The results demonstrate that the current out-of-pocket maximum rules do not reflect the needs of medical necessity. This study suggested the need to reflect the medical necessity in LTCH on the out-of-pocket maximum rules in the future.

  • Parameters of Swedish bench blast design was analyzed by robust design method. Orthogonal array which is adopted in this study was $L_9(3^4)$ and the parameters were hole diameter, explosive type, hole inclination and rock factor of 3 levels. Result of analysis showed that maximum and minimum burden are most affected by hole diameter, followed by explosive type, rock type and inclination of hole. Parameters affecting specific charge are in the order of rock type, explosive type and to specific drilling are hole diameter and explosive type. Cost analysis showed that robust design is capable of parameter optimization.

  • Recently, there has been a constant barrage of worms over the Internet. Besides threatening network security, these worms create an enormous economic burden in terms of loss of productivity not only for the victim hosts, but also for other hosts, as these worms create unnecessary network traffic. Further, measures taken to filter these worms at the router level incur additional network delays because of the extra burden placed on the routers. To develop appropriate tools for thwarting the quick spread of worms, researchers are trying to understand the behavior of worm propagation with the aid of epidemiological models. In this study, we present an optimization model that takes into account infection and treatment costs. Using this model we can determine the level of treatment to be applied for a given rate of infection spread.

Influence Analysis of the Common Mean Problem

  • Two influence diagnostic methods for the common mean model are proposed. First, an investigation of the influence of observations according to minor perturbations of the common mean model is made by adapting the local influence method which is based on the likelihood displacement. It is well known that the maximum likelihood estimates are in general sensitive to influential observations. Case-deletions can be a candidate for detecting influential observations. However, the maximum likelihood estimators are iteratively computed and therefore case-deletions involve an enormous amount of computations. An approximation by Newton's method to the maximum likelihood estimator obtained after a single observation was deleted can reduce much of computational burden, which will be treated in this work. A numerical example is given for illustration and it shows that the proposed diagnostic methods can be useful tools.

  • This paper proposes maximum torque control of IPMSM drive using optimal current. This control method is applicable over the entire speed range which considered the limits of the inverter's current and voltage rated value. For each control mode, a condition that determines the optimal d-axis current $i_d$ for maximum torque operation is derived. This paper considers the design and implementation of novel technique of high performance speed control for IPMSM using multi-MFC and ANN controller. Also, this paper proposes maximum control of IPMSM drive using approximation method. This method is decreased the burden of digital signal process(DSP) in calculation of optimal current. This paper proposes the analysis results to verify the effectiveness of the MFC and ANN controller. Also it verifies the validity of maximum torque control of IPMSM drive with optimal current.
