근긴장도검사기(Myotonometer)를 이용한 상완이두근과 대퇴사두근 검사의 신뢰도 연구 (Intra-Rater and Inter-Rater Reliability of the Myotonometer in the Assessment of Biceps Brachii and Quadriceps)

  • Muscle tone (stiffness/hardness) or muscle compliance changes during muscle contraction. The purposes of this study were to assess the intrarater and interrater reliabilities of the Myotonometer$^{(R)}$, electronic device that quantifies muscle tone. Two raters used the Myotonometer to assess the right bicep brachia and quadriceps muscles of 30 voluntary persons without any orthopedic or neurological problems (age range, 18~21 yrs). Muscles were measured in a relaxed state and during brief sustained voluntary maximal isometric contraction. Intrarater correlation coefficients were calculated for each muscle and for each condition (relaxed and contracted). Intrarater reliabilities (intraclass correlation coefficients, ICCs) ranged from .778 to .954, relaxed, biceps brachia), .926 to .963 (contracted, biceps brachia), .935 to .990 (relaxed, quadriceps) and .679 to .952(contracted, quadriceps). Interrater reliabilities ranged from .652 to .790 (relaxed, biceps brachii), .813 to .907 (contracted, biceps brachii), .831 to .950 (relaxed, quadriceps) and .849 to .937 (contracted, quadriceps). Myotonometer measurements had high to very high intrarater and interrater reliability for measurements of the biceps brachia and quadriceps muscles.

만성요통환자의 한발서기 시 체간 안정화 근육의 활성화 변화 (Trunk Stabilization Muscle Activity in Chronic Low Back Pain Patients during One Leg Stance)

  • Purpose : The purpose of this study was to analyze and compare the surface electromyography(EMG) activity of trunk region muscle between normal subjects and chronic low back pain(CLBP) patients during one leg stance. Methods : The subjects were 27 people, were consisted of 12 subjects who don't have low back pain and 15 subjects who have low back pain from 19 to 28 year of age(mean age 22.22). We used surface EMG to evaluate the activity of the Rectus abdominis, External abdominal oblique, Quadratus lumborum, Gluteus medius muscle. We used independent two samples t-test for statistical data. Results : The result of this study showed that the maximal voluntary isometric contraction(%MVIC) ratio of the trunk muscles was higher in patients with CLBP than in normal subject. But there were no statistically significant differences. There were statistically significant differences of the activity of the Quadratus lumborum, Gluteus medius muscle(p<0.05).

뇌졸중 후 편마비 환자의 Global Synkinesis 수준이 보행능력에 미치는 영향 (The Effects of Global Synkinesis Level on Gait Ability in Post-Stroke Hemiplegic Patients)

  • Purpose: We determined the effect of global synkinesis(GS) on gait ability, muscle contraction, and central neuron action potentials in post-stroke hemiplegic subjects. Methods: Thirty hemiplegia patients were evaluated for walking ability, muscle contraction, central neuron action potential, and comparing differences between the H-GS(high-global synkinesis) group and L-GS(low-global synkinesis) group. To obtain the GS level, surface electromyography(EMG) data were digitized and processed to root mean square(RMS). Walking ability was tested with a modified motor assessment scale(MMAS), a 10 m walking test, timed up and go(TUG) test, and a Fugl-Meyer assessment(FMA). Muscle contraction ability was measured as maximal isometric contraction(MIC) peak, MIC slope, and MIC ramp up using mechanomyography(MMG). Central neuron action potential was measured as the H/Mmax ratio or V/Mmax ratio using EMG. The data were analyzed with t-tests to determine the statistical significance. Results: MMAS(p<0.01), 10 m walking velocity(p<0.01), TUG(p<0.01), FMA-HKA(Hip, Knee, Ankle)(p<0.05), FMA-coordination(p<0.05), MIC peak (p<0.05), MIC slope(p<0.01), and MIC ramp up(p<0.05) were significantly different between H-GS and L-GS, as was the V/Mmax ratio(p<0.05), but H/Mmax was not. Conclusion: Lower GS levels indicated better walking ability and motor function. Therefore, intervention programs should consider GS levels in gait training of chronic hemiplegia.

비 절단부에 적용한 PNF 상지 패턴과 견갑골 움직임에 의한 방산이 절단부 주변 근육 활성도에 미치는 영향 -단일 사례연구- (Effects of Irradiation on the Muscle Activity Around an Amputation Site During Proprioceptive Neuromuscular Facilitation Pattern Exercise for Upper Extremity and Scapular Exercise on the Non-Amputated Part -A Case Study-)

  • Purpose: This study verifies the muscle activity around the amputation site during proprioceptive neuromuscular facilitation (PNF) pattern exercise for the upper extremities on the non-amputated part in upper extremity amputees and provides basic data on effective exercise around an amputation site. Methods: Manual resistance was applied to the PNF upper extremity pattern of the non-amputated part to generate muscle activity around the amputation site. The resistance was adjusted to an intensity that could cause maximal isometric contraction. The muscle activity of the amputation site and the non-amputated part was measured using a surface electromyogram for the upper trapezius, middle trapezius, infraspinatus, serratus anterior, and pectoralis major. Results: During the scapular exercise in the painless range, the amputated side showed significantly lower muscle activity and a lower muscle contraction ratio compared with the non-amputated side. During the PNF pattern exercise in the painless range, the amputated side showed lower muscle activity and a lower muscle contraction ratio compared with the non-amputated side. When the direct scapular exercise of the amputated side was compared with the PNF pattern exercise of the non-amputated side, their muscle contraction ratios were similar. Conclusion: This study confirmed the effectiveness of the PNF pattern exercise of the non-amputated part as a way to indirectly train the injured site with no pain for rehabilitation of patients with serious body injuries, such as amputation. It is necessary to develop effective exercise programs for the rehabilitation of the amputation site based on the results of this study.

수준별 힘 안정성에 대한 EMG 변인 및 근육 특성의 관여 (Involvement of EMG Variables and Muscle Characteristics in Force Steadiness by Level)

  • The present study was designed to evaluate changes in neuromuscular properties and the structural and qualitative characteristics of muscles during submaximal isometric contractions at low-to-relatively vigorous target forces and to determine their influence on force steadiness (FS). Thirteen young adult males performed submaximal isometric knee extensions at 10, 20, 50, and 70% of their maximal voluntary isometric contraction using their non-dominant legs. During submaximal contractions, we recorded force, EMG signals from vastus medialis (VM), vastus lateralis (VL), and rectus femoris (RF), and ultrasound images from the distal RF (dRF). Force and EMG standard deviation (SD) and coefficient of variation (CV) values were used to measure FS and EMG steadiness, respectively. Muscle thickness (MT), pennation angle (PA), echo intensity (EI), and texture features were calculated from ultrasound images to assess the structural and qualitative characteristics of the muscle. FS, neuromuscular properties, and texture features showed significant differences across different force levels. Additionally, there were significant differences in EMG_CV among the quadriceps at the 50% and 70% force levels. The results of correlation analysis revealed that FS had a significant relationship with EMG_CV in VM, VL, and RF, as well as with the texture features of dRF. This study's findings demonstrate that EMG steadiness and texture features are influenced by the magnitude of the target force and are closely related to FS, indicating their potential contribution to force output control.

시간경과에 따른 지연성근통증 내측 비복근의 초음파 영상 (Medial Gastrocnemius Ultrasound Imaging of Delayed Onset Muscle Soreness over time)

  • 본 연구의 목적은 지연성근통증에 대한 내측 비복근 초음파 영상의 측정도구로서의 가능성에 대해 알아보고자 함에 있다. 본 연구는 2011년 4월 21일부터 4월30일까지 실시되었다. 평소 규칙적인 운동을 하지 않으며, 최근 외상의 경험, 근골격계 질환, 심혈관계 질환이 없고 약물을 복용하지 않는 35명의 건강한 성인이 참여하였다. 대상자들은 5시간의 등반을 통해 지연성근통증을 유발하였으며, 지연성근통증 전과 지연성근통증 후 24, 48, 72시에 통증(visual analogue scale :VAS), 혈중 크레아틴 키나아제(creatine kinase: CK) 활성도, 족저 굴곡근의 최대근수축력(maximal voluntary isometric contraction: MVlC)을 측정하였고, 이 측정값과 초음파로 측정한 내측 비복근의 우상각을 비교하였다. 연구의 결과는 다음과 같다. 통증지표, 혈중 크레아틴 키나아제 활성도, 족저 굴곡근 최대등척성근수축력에 측정시기에 따른 유의한 차이가 발생하였으며(p<0.05), 내측 비복근 우상각에서도 측정시기에 따른 유의한 차이가 발생하였다(p<0.05). 또한 변수 간 측정시기에 따른 변화의 흐름이 일치하는 것을 확인하였다. 본 연구를 통해 초음파 검사를 통한 내측 비복근 우상각의 시간경과에 따른 변화 측정이 지연성근통증을 감지하는 새로운 측정방법으로 사용될 수 있을 것이라 생각한다.

무릎넙다리통증증후군 환자에서 무릎관절 각도별 스쿼트 융합운동이 넙다리네갈래근의 근력에 미치는 효과 (A Effect of the Squat Convergence Exercise Among Knee Joint Angle on Quadricpes Strength in the Patients With Patellofemoral Pain Syndrome)

  • 본 연구의 목적은 넙다리네갈래근 중 안쪽빗넓은근과 가쪽넓은근의 강화를 위해 효과적인 스쿼트 운동자세를 제시하는 데 있다. 연구대상자는 무릎넙다리통증증후군이 있는 20명의 환자로 본 연구에 자발적으로 참여하였다. 대상자들은 무릎관절 굽힘 각도 $45^{\circ}$, $60^{\circ}$, $90^{\circ}$ 자세에서 정적인 스쿼트 융합운동을 30초 동안 총 5회 실시하였다. 측정변수는 넙다리네갈래근의 최대 수의적 등척성 수축력(MVIC), 넙다리네갈래근 각(Q 각)과 넙다리 둘레이며, 무릎관절 자세에 따라 스쿼트 운동 전후로 측정되었으며 그 변화률을 통계분석에 사용하였다. 연구결과 넙다리네갈래근의 무릎관절 각도별 MVIC 변화율은 $90^{\circ}$ 무릎관절 굽힘에서 유의하게 증가하였으나(p<.05), Q 각과 넙다리 둘레의 변화율엔 유의한 차이가 없었다(p>.05). 따라서 무릎통증증후군 환자의 넙다리네갈래근의 강화를 위해서 무릎관절 $90^{\circ}$ 굽힘된 스쿼트 자세가 효과적임을 제시한다.

운동 후 극저온 냉각치료 적용이 운동유발성 근육 손상의 최대등척성근력과 관절가동범위에 미치는 효과 (The effect of whole-body cryotherapy intervention after an exercise on MVIC and ROM of EIMD)

  • Background: The purpose of this study was to investigate the effects of WBC (whole-body cryotherapy)on the MVIC (maximal voluntary isometric contraction)and active ROM (range of motion) recovery of after EIMD (exercise-induced muscle injury). Design: Randomized Controlled Trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I(n=10), and experiment group II (n=10). The subjects in the experimental group II were intervened by WBC (-130℃, 3 minutes) before induced EIMD, the experimental group were intervened by WBC (-130℃, 3 minutes) after induced EIMD, and the control group weren't by any intervened after induced EIMD. Results: First, In the comparison of the MVIC, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the MVIC of experimental group II was significantly larger than those of other groups (p<.001). Second, In the comparison of the active extension angle, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active extension angle of experimental group II was significantly smaller than those of other groups (p<.001). Third, In the comparison of the active flexion angle, there were significant variations with the lapse the time in three groups (p<.01) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active flexion angle of experimental group II was significantly larger than those of other groups (p<.05). Conclusion: The above results revealed that the WBC intervention after an exercise had a positive effect of muscle function after EIMD. Therefore we can consider the WBC as a considerable intervention method to prevent or reduce an exercise injury.

신경근전기자극 적용양식에 따른 대퇴사두근의 중앙주파수 변화 (Changes in Median Frequency of Quadriceps Muscle According to Application Modes of Neuromuscular Electrical Stimulation)

  • This study assigns each 8 of 24 normal persons to control group(Group I), strength increase group(Group II) and endurance increase group(Group III) to analyze differences in changes of strength and endurance with surface electromyography and kinetics according to application modes of neuromuscular electrical stimulation(NMES). Group I had not any treatment, group II performed 15 repeated contraction with 60% intensity of maximal voluntary isometric contraction(MVIC) by setting 10-sec on time and 50-sec off time and group III conducted 30 repeated contraction with 30% intensity of MVIC by setting 10-sec on time and 20-sec off time. For neuromuscular electrical stimulation, 2,500 Hz of Russian current, 35 pps of pulse rate and 200 of pulse width. Neuromuscular electrical stimulation was conducted by five times for total 4 weeks. Before and after experimentmotor unit action potential of vastus medialis, rectus femoris and vastus lateralis were measured with sEMG, median frequency(MDF) was analyzed, and thus the following results were obtained. There was significant difference in the period of measuring vastus medialis and rectus femoris in change of MDF and interaction among groups with analysis of surface electromyography before and after neuromuscular electrical stimulation(p<.001) and in particular, there was a remarkable change among groups according to the period of measurement. In conclusion, NMES influenced changes of strength and endurance according to its application modes and in particular, it was found that strength increment application had a significant influence on strength increment in applying short-time NMES.

고령자의 족관절 근육 양측성 수축시의 전운동 반응시간 (Premotor-time of the Ankle Muscle during Bilateral Contraction in the Elderly)

  • The purpose of this study is to investigate the effects of sex and age and their interactions in premotor-time (PMT) of ankle muscle. Forty-eight elderly subjects (aged 65-90 years) and thirty young subjects (aged 19-27 years) participated in this study. Subject were instructed to perform maximal, voluntary, isometric, bilateral contraction of ankle muscle in reaction to auditory stimulus to determine PMT. As analysis variables, PMT, intrasubject variability of PMT and asymmetry of PMT between dominant and nondominant legs were used. As statistical analysis, two-way ANOVA was performed to assess the main effects of age group and sex and also their interactions. All variables showed significant age effects (p<0.01). However, no sex effect and interaction existed in all variables in both dominant and nondomiant legs. Theses results suggest that the PMT of ankle muscle is related to the age-related deterioration in postural control, however, not related to the sex-difference of fall incidence in the elderly population.
