This study was designed to help to be given esthetics in construction of denture and prosthodontic rehabilitation of anterior tooth region. The author took the facial straight photograph of 100 old aged people(male 50, female 50) above 55 years of age, who have natural teeth in anterior tooth region, in a resting and a smiling position. And the author measured and analyzed the lip shape, the relation between the lip and the teeth and the change of lip length and height when they were smiling. The results of this study were as follows : 1. In the shape of the upper lip, when the upper lip curved downward, it was 53%, straight was 28% and curved upward was 9%. 2. In the relation between the upper lip and the teeth, average smile was 53% , high smile was 26% and low smile was 21%. 3. In the parallel relation between the lower lip and maxillary anterior incisal curvature, the group of straight was 54%, the group of parallel was 40% and the group of reverse was 6%. 4. In the relationship between maxillary anterior incisor and lower lip, the group of not-touching was 92%, the group of touching was 5% and the group of the maxillary anterior incisor were slightly covered by the lower lip was 3%. 5. In the teeth displayed in a smile, displayed to second premolar was 50%, displayed to first molar was 34%, displayed to first premolar was 12%, and displayed to canine and second molar were 2%. 6. At smiling, the width of the mouth was 0.94 times of the interpupillary distance and 0.45 times of the full face width. 7. At smiling, the length of the upper lip was 0.73 times and lower lip was 0.98 times of the length in a resting postion and the width of the mouth corner was 1.19 times of the resting position.
PURPOSE. The midline fracture of maxillary complete dentures is a frequently encountered complication. The purpose of this study was to assess the effect of frenulum height on midline strains of maxillary complete dentures. MATERIALS AND METHODS. A removable maxillary complete denture was fabricated and duplicated seven times. Four different labial frenulum heights were tested for stresses occurring on the palatal cameo surface. The strains were measured with strain gauges placed on 5 different locations and the stresses were calculated. To mimic occlusal forces bilaterally 100 N of load was applied from the premolar and molar region. RESULTS. A statistically significant association between the height of the labial frenulum and the calculated stresses and strains was shown (P<.05) predominantly on the midline and especially on the incisive papilla. The results showed that stress on the anterior midline of the maxillary complete denture increases with a higher labial frenulum. CONCLUSION. Within the limitations of this in vitro study, it can be concluded that the stress on the anterior midline of the maxillary complete denture increases with a higher labial frenulum. Surgical or mechanical precautions should be taken to prevent short-term failure of maxillary complete dentures due to stress concentration and low cycle fatigue tendency at the labial frenulum region.
This study was designed to investigate the criteria of the perfect smile that was necessary to improve the esthetic problem in oral and maxillo-facial region and treating the anterior tooth region. The author took the facial straight photograph of 240 university students(male : 129, female : 111) in a resting and a smiling position, measured and analized the lip pattern and the relation between the lip and the teeth when they were smiling. Besides, 10 members of committee for appraisal (dentist : 5 persons, professor of the college of fine arts : 5 persons) estimated the smiling pattern. After that the author have compared and analyzed the obtained results. The results obtained were as follows : 1. In the shape of the upper lip, when the upper lip curved downward, it was 42.92% , straight was 45.00% and curved upward was 12.08%. The group in which the upper lip curved upward was the most esthetic. 2. In the relation between the upper lip and the teeth, high smile was 29.17%, average smile was 55.83% and low smile was 15.00%. The group of average smile was the most esthetic. 3. In the parallel relation between the lower lip and maxillary anterior incisal curvature, the group of paralled was 60.42%, the group of straight was 34.17% and the group of reverse was 5.41%. The group of parallel was the most esthetic. 4. In the relationship between maxillary anterior incisor and lower lip, the group of the maxillary anterior incisor were slightly covered by the lower lip was 10.42%, the group of the maxillary anterior teeth touched to the lower lip was 35.83%, and the no-touching was 53.75%. The group of the maxillary anterior teeth touched to the lower lip was the most esthetic. 5. In the teeth displayed in a smile, displayed to the canine was 0.84%, displayed to the first premolar was 19.17%, displayed to the second premolar was 57.92%, displayed to the first molar was 20.00% and displayed to the second molar was 2.08%. The group of displayed to the first molar was the most esthetic. 6. At smiles, the width of the mouth corner was 0.46 times of the full face width, 0.95 times of the interpupillary distance, and 1.23 times of the resting position. 7. At smiles, the lengh of the upper lip was 0.71 times and lower lip was 0.93 times of the length in the resting position.
A radicular cyst is a slowly-growing epithelial sac lining a pathologic cavity in the bone at the apex of a tooth. Large radicular cyst revealed by radiographic examination almost always appear to involved several teeth. The author observed a 31 year old woman with a labial swelling and palatal mucosal expansion of the maxillary anterior region. Radiographic examination indicated that the lesion extended from the left maxillary lateral incisor to the right lateral incisor. Cyst consisted of tan to gray yellow slightly firm to partly hard plaque like tissue measuring 3.4 x 2.2 x 0.3㎝. The author has treated the cyst by means of enucleation with apicoecomies of the involved teeth except the right central incisor. Recovery proceed satisfactorily and there were no clinical or radiologic signs of recurrence 8 months later.
Purpose: The aim of this study was to evaluate the effects of pterygomaxillary separation on dimensional changes of dental arch following surgically-assisted rapid maxillary expansion (SARME). Patients and Methods: Eighteen adults who had been treated by SARME for transverse maxillary deficiency from May 2000 to August 2005 were evaluated. Thirteen patients (Group 1) were treated with subtotal Le Fort I osteotomy including pterygomaxillary separation and anterior midpalatal osteotomy. The same operation was performed in five patients (Group 2) except pterygomaxillary separation. Dental study casts were taken before operation and after removal of expansion device. And then, skeletal and dental parameters were measured pre- and post-operatively. Results: 1. Changes of mean interdental width 1) In group 1, mean maxillary interdental width was increased 70%($47{\sim}99%$), 95%($84{\sim}115%$), and 77%($57{\sim}94%$) of total expansion on canine, first premolar, and first molar region, respectively after retention. 2) In group 2, mean maxillary interdental width was increased 77%($59{\sim}100%$), 78%($45{\sim}107%$), and 86%($57{\sim}116%$) of total expansion on canine, first premolar, and first molar region, respectively after retention. 3) There was a statistical difference between the change of interdental width of group 1 and group 2 at first premolar(p<0.05). 2. Changes of mean interalveolar width 1) In group 1, mean maxillary alveolar bone width was increased 66%($42{\sim}84%$), 74%($42{\sim}104%$), and 57%($31{\sim}78%$) of total expansion on canine, first premolar, and first molar region, respectively after retention. 2) In Group 2, mean maxillary alveolar bone width was increased 73%($55{\sim}98%$), 67%($36{\sim}89%$), and 59%($48{\sim}73%$) of total expansion on canine, first premolar, and first molar region, respectively after retention. 3) There were no statistical differences between group 1 and group 2 at each teeth area. Conclusion: These results suggest that SARME without pterygomaxillary separation may allow the relatively equal expansion at both anterior and posterior teeth area and most amounts of maxillary interdental expansions were acquired with the expansion of the maxilla by SARME.

In children, large odontogenic cysts affect adjacent anatomical structures as well as displace developing permanent teeth. Odontogenic cysts are treated via enucleation or marsupialization. This case reports a 5-year-old boy and a 10-year-old boy who have not only displaced 3 or more permanent teeth but also elevated the maxillary sinus floor due to the large size of the cyst in the maxillary anterior region. In both cases, marsupialization was selected to minimize complications. After marsupialization, a customized acrylic obturator, window opening, and orthodontic traction for eruption guidance were gradually attempted, and it showed a good prognosis, so we report these cases.

Mandibular anterior region has high implant survival rates and better accessibility compared with any other region. Even if there are possibilities for perforation on lingual cortical bone due to improper drilling and bleeding caused by lingual artery damage, mandibular anterior region is a safe region because less amount of major anatomical structures exist compared with other regions. However, because of narrow bucco-lingual width of alveolar ridge, it is challengeable to obtain esthetic implant prosthesis. Although patients are less sensitive subjectively, mandibular anterior region is as difficult as maxillary anterior region in that implant placement location plays a critical role on the prognosis of implant prosthesis. One-piece implant is a very useful option for mandibular anterior region. Considering the narrow roots and thin alveolar bone of mandible, it is clinically difficult for implant diameter to be greater than 3mm In this case, we could approach the esthetic restoration in mandibular anterior region with one-piece implant and immediate loading.

Supernumerary teeth are teeth added to the normal complement of teeth. They are most often found in the maxillary anterior region. Most supernumerary teeth in the maxillary anterior region have complications, such as impaction or malposition of permanent teeth, formation of diastema, cysts, and eruption into the nasal cavity or into the oral cavity. These complications have influence on deciding the treatment and its prognosis is depends on how these complications are treated. In clinical studies, it has been found that the removal of supernumerary teeth before the age of 5 years resulted in significant fewer eruption problems of the permanent tooth compared to removal at 7 years of age or later. However, such an early removal may leads to serious consequences of tooth development and behavioral management of patients. Therefore clinicians must consider complications of supernumerary tooth when deciding the time of removal. These cases report five-type of complication associated with supernumerary tooth such as diastema, delayed eruption, external root resorption, migration into nasal cavity and cyst formation.
PURPOSE. The aim of this study was to compare the fracture of implant component behavior of external and internal type of implants to suggest directions for successful implant treatment. MATERIALS AND METHODS. Data were collected from the clinical records of all patients who received WARANTEC implants at Seoul National University Dental Hospital from February 2002 to January 2014 for 12 years. Total number of implants was 1,289 and an average of 3.2 implants was installed per patient. Information about abutment connection type, implant locations, platform sizes was collected with presence of implant component fractures and their managements. SPSS statistics software (version 24.0, IBM) was used for the statistical analysis. RESULTS. Overall fracture was significantly more frequent in internal type. The most frequently fractured component was abutment in internal type implants, and screw fracture occurred most frequently in external type. Analyzing by fractured components, screw fracture was the most frequent in the maxillary anterior region and the most abutment fracture occurred in the maxillary posterior region and screw fractures occurred more frequently in NP (narrow platform) and abutment fractures occurred more frequently in RP (regular platform). CONCLUSION. In external type, screw fracture occurred most frequently, especially in the maxillary anterior region, and in internal type, abutment fracture occurred frequently in the posterior region. placement of an external type implant rather than an internal type is recommended for the posterior region where abutment fractures frequently occur.
Alveolar distraction osteogenesis (ADO) has been regarded as an acceptable treatment for the alveolar bone deficiency. For ADO at anterior maxillary area, the vector should be oriented to forward and down-ward direction to get an adequate occlusion with mandibular teeth and to increase bone length and width for implant placement. However, the conventional commercial distraction devices for ADO are designed to allow mainly downward movement of alveolar segment, even though a forward movement can be obtained a little by controlling of inclination of device. To make ADO with controllable bidirectional vector possible, we used customized devices using self-manufactured ABDUL (Alveolar Bone Distractor Using Lag screw principle) and commercial orthodontic palatal expansion device ($Hyrex^{(R)}$). In all cases (n = 4), ADO could be performed successfully and dental implants were able to placed with adequate occlusion. We report the procedures, advantages and disadvantages of these methods.
