Distance of insertion points in a mattress suture from the wound margin for ideal primary closure in alveolar mucosa: an in vitro experimental study

  • Purpose: This study was conducted to determine how the distance of the near insertion points in a vertical mattress suture from the wound margin influences the pattern of primary closure in an in vitro experimental model. Methods: Pairs of 180 porcine gingival and alveolar mucosa samples were harvested from 90 pig jaws and fixed to a specially designed model. A vertical mattress suture was performed with the near insertion point at 3 different distances from the wound margin (1-, 3-, and 5-mm) on both the gingival and mucosal samples (6 groups; n=30 for each group). The margin discrepancy and the presence of epithelium between the wound margins were measured on histologic slides. Results: The margin discrepancy decreased significantly as the near insertion point became closer to the wound margin both in mucosal tissue (0.241±0.169 mm, 0.945±0.497 mm, and 1.306±0.773 mm for the 1-, 3-, and 5-mm groups, respectively) and in gingival tissue (0.373±0.304 mm, 0.698±0.431 mm, and 0.713±0.691 mm, respectively). The frequency of complications of wound margin adaptation reduced as the distance of the near insertion point from the wound margin decreased both in the mucosal and gingival tissues. Conclusions: Placing the near insertion point close to the wound margin enhances the precision of wound margin approximation/adaptation using a vertical mattress suture.

Effectiveness of chest compression by bystander on the home bed mattress

  • 본 연구는 가정용 침대 위에서 발생한 심정지 환자에게 가슴압박을 수행할 경우 환자의 체중과 구조자의 체중에 따른 가슴압박 정확도의 차이를 확인하고자 하였다. 2023년 01월 07일부터 01월 19일까지 대한심폐소생협회의 KBLS 교육과정을 이수한 N대학교와 S대학교의 응급구조(학)과 학생 36명을 대상으로 대상자의 무릎 위치에서 매트리스가 눌리는 깊이, 마네킹의 무게에 의해 매트리스가 눌리는 깊이를 측정하였고, 2분간 연속된 가슴압박을 수행하는 동안 가슴압박 깊이, 속도, 압박 위치 불량, 가슴압박 깊이 50 mm 미만 횟수, 모든 변수를 고려한 정확도를 측정하였다. 연구결과 가정에서 소아 심정지를 발견한 경우 환자를 바닥으로 이동시켜 가슴압박을 진행하고, 성인 심정지를 발견한 최초목격자는 환자를 무리하게 침대 밑으로 내려 가슴압박을 진행하기보다는 침대 위에서 진행하는 것을 권장한다.

In large hospitals, and dependent products are sold at high prices. On the other hand, improvement of living standards of our country and to the advancement of residential culture of the demand for luxury products is increasing. The study of elderly Edition mattresses have been developed mainly in Europe to maximize the use of sleep can increase the efficiency of products has been studied. Due to the increase in elderly population Elderly Hospital, which is installed in the elderly silver town Edition requires the dissemination of the mattress in the bedroom several times a day resting and sleeping, eating, etc. Designing a variety of actions are needed. Edition of the elderly in Korea developed the design of bed with bed design and the manufacturer and the Institute for Ergonomics, welfare agencies and to build organic cooperation system under the direction of a professional designer to develop and you need to configure the space and the results of such actions by the distribution of cheap materials, ergonomic mattress for optimal development of behavior analysis to the need to promote the convenience of the user.

심폐소생술에서 두 개의 가속도 센서를 활용한 흉부 압박 깊이 추정 (Estimation of Chest Compression Depth using two Accelerometers during CPR)

  • During the cardiopulmonary resuscitation (CPR), the correct chest compression depth and period are very important to increase the resuscitation possibility. For the feedback of chest compression depth, the depth monitoring device based on the accelerometer is developed and widely used. But this method tends to overestimate the compression depth on the bed. To overcome this limitation, the chest compression depth estimation method using two accelerometers is suggested With the additional accelerometer between the patient and mattress on the bed, the compression of the mattress is also measured and it is used to compensate the overestimation error. The experimental results show that the single accelerometer estimates as 61.4mm for the actual compression depth of 43.6mm on the mattress. The depth estimation with the dual accelerometer was 44.6mm which is close to the actual depth. With the automatic zeroing in every single compression, the integration error for the depth can be reduced. The dual accelerometer method is effective to increase the accuracy of the chest compression depth estimation.

Usefulness of an Additional Mattress Suture for the Extracranial Drainage Catheter

  • In most intracranial surgery cases, a drain catheter is inserted to prevent the collection of the wound hematoma or seroma. A drain catheter is also inserted to drain the hematoma or the cerebrospinal fluid. The drain catheter itself does not cause complications; but many complications occur during its removal, such as hematoma, seroma, air collection and pseudomeningocele formation. To prevent these complications, neurosurgeons perform a suture on the catheter to remove the site. In this study, an additional horizontal mattress suture and an anchoring suture to the drainage catheter are proposed. This method maintains negative pressure in the catheter insertion site during the catheter removal, compresses the catheter tunnel site and attaches the external wounds strongly. The technique is easy and safe to perform, and does not require an additional suture to remove the catheter.

  • 보행보조차와 욕창방지 매트리스를 포함하는 고령친화제품의 사용빈도는 고령자 인구의 증가에 의하여 높아지고 있다. 고령자의 경우, 인간공학적으로 잘못 디자인된 제품을 사용하는 경우 사용성 측면에서의 문제뿐만 아니라 안전성 측면에서도 심각한 문제를 야기할 수 있다. 고령자 사고의 상당부분은 고령친화제품을 사용하는 과정에서 발생한다. 본 연구에서는 고령친화제품을 사용할 때의 고령자의 사용성에 대하여 산학협력을 통하여 연구하였다. 사용성 평가 프로그램이 고령친화제품을 개발하거나 제조하는 기업을 지원하기 위하여 개발되었고, 보행보조차와 욕창방지 매트리스에 대한 사용성이 평가되었으며 그 결과가 기업에 제공되었다.

침대 사용시의 제반 현상에 대한 실태 조사 (Survey on the Use of Bed for Comfortable Sleeping)

  • The use of bed in Korea has increased steadily, however, intensive survey on the general use and consumer's evaluation of their beds have not been performed sufficiently, yet. Thus, the purpose of this study is to investigate the actual use of the consumer and their degree of evaluation for the component of the bed as well as their feeling of discomfort on the various parts of body. More than one hundred users of commercial beds were responded to the survey. The results obtained were as follows: (1) Complaint about the depression of the mattress and the noise from the bed was apparent in the current use of beds. (2) Important physical variable related to the comfortable sensation of the sleeping was the degree of support in maintaining the natural shape of the spine and waist. Durability and the degree of hardness of the mattress were also important physical parameters of bed perceived by consumer. (3) The degree of discomfort on the back and waist during sleep was closely related to the comfort evaluation of the bed.

Reconstruction of a Traumatic Cleft Earlobe Using a Combination of the Inverted V-Shaped Excision Technique and Vertical Mattress Suture Method

  • Traumatic cleft earlobes are a common problem encountered by plastic and reconstructive surgeons. Various techniques have been reported for the repair of traumatic cleft earlobes. Usually, the techniques of split earlobe repair are divided into two categories, namely straight- and broken-line repairs. Straight-line repair is simple and easy, but scar contracture frequently results in notching of the inferior border of the lobule. It can be avoided by the broken-line repair such as Z-plasty, L-plasty, or a V-shaped flap. Between April 2016 and February 2017, six patients who presented with traumatic cleft earlobe underwent surgical correction using a combination of the inverted V-shaped excision technique and vertical mattress suture method. All the patients were female and had a unilateral complete cleft earlobe. No postoperative notching of the inferior border the lobule occurred during 6-16 months of follow-up. Without the use of a broken-line repair, both the patients and the operators attained aesthetically satisfactory results. Therefore, the combination of the inverted V-shaped excision technique and vertical mattress suture method is considered useful in the treatment of traumatic cleft earlobes.

다가구 주택의 매트리스 화재 시 차동식 스포트형 감지기의 온도상승 및 동작시간 분석 (Analysis of Temperature Rise and Operation Time of Differential Spot Type Detector in Case of Mattress Fire in Multi-family House)

  • This study developed a scenario to understand the reaction rate and operational time according to RTI value of rate of rise detector in each type in case of fire mattress. In the results of analyzing the reaction rate and operational time of detector in each scenario, in case when installing a single detector, the elevated temperature per minute was raised to 8℃/min ~ 9℃/min. In case when installing two detectors, it was raised to 9℃/min ~ 10℃/min. In case when installing three detectors, it was raised to 10℃/min. The horizontal distance between detector and mattress was 1.8m~2.5m. Whenever the number of detectors was increased, the horizontal distance was decreased. The operational time of detector was within maximum 540 seconds and minimum 420 seconds. As the research tasks in the future, there should be the researches on the effects of reaction rate of detector on the evacuation in case of fire through the result value of RSET by setting up the latency until the detector operates, and the researches on the safety by understanding if the operational time of detector is suitable for the evaluation standard of performance-centered design.

소규모 주거 공간 실물화재실험을 통한 적재 가연물의 연소특성 분석 (An Analysis of Combustion Characteristics of Residential Facilities through Real Fire Tests in a Small Residential Space)

  • In this study, real fire tests were performed on representative combustibles of residential facilities to analyze the fire behavior and combustion characteristics in a small residential space. The considered combustibles were a sofa, a combination of a desk and a chair, and a combination of a mattress and an electric mat. A compartment space fire test was performed using the room corner test equipment prescribed in the KS F ISO 9705 specification. Three real fire tests were conducted by placing the combustible material inside a small compartment with insulation and finishing materials and by igniting the combustible material. Results showed that the peak heat release rate and peak smoke production rate occurred in the combination of the mattress and electric mat. Furthermore, from the result of the fire rate analysis, it was estimated that the fire risk of the mattress and electric mat combination was the highest, followed by the sofa and thedesk and chair combination.
