Pediatric Dehydration Assessment at Triage: Prospective Study on Refilling Time

  • Purpose: Dehydration is a paediatric medical emergency but there is no single standard parameter to evaluate it at the emergency department. Our aim was to evaluate the reliability and validity of capillary refilling time as a triage parameter to assess dehydration in children. Methods: This was a prospective pilot cohort study of children who presented to two paediatric emergency departments in Italy, with symptoms of dehydration. Reliability was assessed by comparing the triage nurse's measurements with those obtained by the physician. Validity was demonstrated by using 6 parameters suggestive of dehydration. Comparison between refilling time (RT) and a validated Clinical Dehydration Score (CDS) was also considered. The scale's discriminative ability was evaluated for the outcome of starting intravenous rehydration therapy by using a receiver operating characteristic (ROC) curve. Results: Participants were 242 children. All nurses found easy to elicit the RT after being trained. Interobserver reliability was fair, with a Cohen's kappa of 0.56 (95% confidence interval [CI], 0.41 to 0.70). There was a significant correlation between RT and weight loss percentage (r-squared=-0.27; 95% CI, -0.47 to -0.04). The scale's discriminative ability yielded an area under the ROC curve (AUC) of 0.65 (95% CI, 0.57 to 0.73). We found a similarity between RT AUC and CDS-scale AUC matching the two ROC curves. Conclusion: The study showed that RT represents a fast and handy tool to recognize dehydrated children who need a prompt rehydration and may be introduced in the triage line-up.

Thrombocytopenia in Moderate- to High-Risk Sutureless Aortic Valve Replacement

  • Background: This study aimed to compare preliminary data on the outcomes of sutureless aortic valve replacement (SU-AVR) with those of aortic valve replacement (AVR). Methods: We conducted a retrospective study of SU-AVR in moderate- to high-risk patients from 2013 to 2016. Matching was performed at a 1:1 ratio using the Society of Thoracic Surgeons predicted risk of mortality score with sex and age. The primary outcome was 30-day mortality. The secondary outcomes were operative outcomes and complications. Results: A total of 277 patients were studied. Ten patients (50% males; median age, 81.5 years) underwent SU-AVR. Postoperative echocardiography showed impressive outcomes in the SU-AVR group. The 30-day mortality was 10% in both groups. In our study, the patients in the SU-AVR group developed postoperative thrombocytopenia. Platelet counts decreased from $225{\times}10^3/{\mu}L$ preoperatively to 94.5, 54.5, and $50.1{\times}10^3/{\mu}L$ on postoperative days 1, 2, and 3, respectively, showing significant differences compared with the AVR group (p=0.04, p=0.16, and p=0.20, respectively). The median amount of platelet transfusion was higher in the AVR group (12.5 vs. 0 units, p=0.052). Conclusion: There was no difference in the 30-day mortality of moderate-to high-risk patients depending on whether they underwent SU-AVR or AVR. Although SU-AVR is associated with favorable cardiopulmonary bypass and cross-clamp times, it may be associated with postoperative thrombocytopenia.

An Efficient Method to Extract the Micro-Motion Parameter of the Missile Using the Time-Frequency Image (시간-주파수 영상을 이용한 효과적인 미사일 미세운동 변수 추출 방법)

  • It is very difficult to intercept the missiles because of the small radar cross-section and the high maneuverability. In addition, due to the decoy with the similar motion parameters, additional features other than those of the translation motion parameters need to be developed. In this paper, for the successful recognition of missiles, we propose an efficient method to extract micro-motion parameters and scatterers of the missile engaged in the micro motion. The proposed method extracts motion parameters and scatterers by using the matching score between the modeled micro-Doppler function and the time-frequency binary image as a cost function. Simulation results using a target composed of the point scatterer show the parameters and the scatterers were accurately extracted.

Do Long Term Cancer Survivors Have Better Health-Promoting Behavior than Non-Cancer Populations?: Case-Control Study in Korea

  • Background: We compared the health-promoting behavior of long-term cancer survivors with those of the general population to identify necessary behavioral interventions to reduce the health risk among cancer patients. Materials and Methods: We used data from the 2007 and 2012 Korea National Health and Nutrition Examination Surveys (KNHANES IV [2007~2009] and KNHANES V [2010~2012]) on smoking status, alcohol use, physical exercise, and disease screening. We compared long-term cancer survivors with members of the general population; the controls were matched by propensity score matching. A multiple logistic regression model was used to investigate the association between cancer status and health-promoting behavior. Results: Long-term cancer survivors had a lower risk of smoking than the general population controls (OR: 0.42, 95%CI: 0.25-0.71). In addition, the long-term cancer survivors had a lower risk of alcohol use than the general population controls (OR: 0.70, 95%CI: 0.50-0.98). However, in terms of physical exercise and disease screening, no statistically significant differences were detected (physical exercise OR: 1.01, 95%CI: 0.75-1.35; disease screening OR: 1.27, 95%CI: 0.93-1.74). All covariates were adjusted. Conclusions: The long-term cancer survivors had a much lower risk of smoking and alcohol use than the general population controls. However, almost no differences in physical exercise and screening for cancer recurrence or secondary disease were detected between the long-term cancer survivors and general population controls. To reduce the health risks and challenges facing long-term cancer survivors, interventions to encourage physical exercise and screening for cancer recurrence and secondary disease should be implemented.

The Effectiveness of the Error Reporting Promoting Program on the Nursing Error Incidence Rate in Korean Operating Rooms

  • Purpose. The purpose of this study was to develop and evaluate an error reporting promoting program(ERPP) to systematically reduce the incidence rate of nursing errors in operating room. Methods. A non-equivalent control group non-synchronized design was used. Twenty-six operating room nurses who were in one university hospital in Busan participated in this study. They were stratified into four groups according to their operating room experience and were allocated to the experimental and control groups using a matching method. Mann-Whitney U Test was used to analyze the differences pre and post incidence rates of nursing errors between the two groups. Results. The incidence rate of nursing errors decreased significantly in the experimental group compared to the pre-test score from 28.4% to 15.7%. The incidence rate by domains, it decreased significantly in the 3 domains-"compliance of aseptic technique", "management of document", "environmental management" in the experimental group while it decreased in the control group which was applied ordinary error-reporting method. Conclusion. Error-reporting system can make possible to hold the errors in common and to learn from them. ERPP was effective to reduce the errors of recognition-related nursing activities. For the wake of more effective error-prevention, we will be better to apply effort of risk management along the whole health care system with this program.

An Empirical Analysis of Management and Innovative Performances by the Characteristics of the Industrial Park Tenancy (산업단지 입주특성에 따른 기업 경영 및 혁신성과 분석)

  • This study applies regression analysis and propensity score matching to analyze the agglomeration economies which the characteristics of tenancy affect the performance of the manufacturing industry based on industrial parks. The estimation main data are from Kis-Value, KIPRIS and FEMIS. The results show that the industrial park tenancy tends to work positively on the management performances. But there is no evidence that on-Park firms in the metropolitan areas(Gyeonggi-do) have higher management and innovative performances than the comparable firms. The firms that have lived for a long time in the industrial parks, are good in total sales, however, they have no significant efficiency in net profit, operating profit, and patents. The firms, having several branches of the industrial parks, have lower operating profit than others. Long-term and multiple tenant firms do not learn over time nor do they establish better linkages and networks.

Comparison of Radial Artery and Saphenous Vein Composite Y Grafts during Off-pump Coronary Artery Bypass

  • Background: The safety and efficacy of arterial composite grafts for total arterial revascularization have been demonstrated. The saphenous vein (SV) is a widely used graft because of its accessibility, sufficient length, and ease of manipulation. Our aim was to compare mid-term outcomes of saphenous vein Y-grafts with radial artery Y-grafts joined by anastomosis to the left internal thoracic artery. Materials and Methods: Records of off-pump coronary artery bypass grafting with composite Y-grafts based on the left internal thoracic artery technique in 552 patients were analyzed retrospectively. After propensity score matching, 79 radial arterial (RA) composite grafts (RA group) and 79 saphenous vein composite grafts (SV group) were compared. The duration of mean follow-up was $24.6{\pm}14.6$ months (range, 1 to 55 months). Results: There were no differences in surgical mortality, all-cause mortality, or morbidity among the groups. Rates of 4-year survival were 91.7% and 96.3% in the RA and SV groups, respectively (p=0.519). The coronary reintervention-free survival rate and freedom from major adverse cardiovascular or cerebrovascular events were similar in the two groups (p=0.685, p=0.564). Conclusion: Construction of composite Y-grafts using the radial artery or saphenous vein showed similar mid-term results. Long-term follow-up and randomized trials will be needed to confirm our present conclusions.

A study of Teacher's Perception on Selecting Mathematics Textbook (수학 교과서 선정 기준에 관한 교사들의 인식 조사)

  • The purpose of the research is to examine the teachers' cognition about the math textbook selecting and to provide the implications about the rational process of the textbook selection and criteria based on the research. In terms of the research result about the inner and outer criteria of the actual process of the textbook selecting, they utilized the standard selection criteria distributed by Office of Education, and they considered 'Organization of learning contents' and 'Learning evaluation' as the most important factors. The selection method was that the answer 'After scoring the grade with matching standards, selecting by adding the total score' was the highest. In case of the actual selection, the most considerable inner criteria appeared 'Learning quantity and propriety of the difficulty level'. The outer criteria showed 'Awareness', and the difficulty in the selection process was 'in sufficient time for reviewing'. Based on this research result, we draw the implication that needs to be improved in the process of the textbook selection and the criteria.

Recognition of Hmm Facial Expressions using Optical Flow of Feature Regions (얼굴 특징영역상의 광류를 이용한 표정 인식)

  • Facial expression recognition technology that has potentialities for applying various fields is appling on the man-machine interface development, human identification test, and restoration of facial expression by virtual model etc. Using sequential facial images, this study proposes a simpler method for detecting human facial expressions such as happiness, anger, surprise, and sadness. Moreover the proposed method can detect the facial expressions in the conditions of the sequential facial images which is not rigid motion. We identify the determinant face and elements of facial expressions and then estimates the feature regions of the elements by using information about color, size, and position. In the next step, the direction patterns of feature regions of each element are determined by using optical flows estimated gradient methods. Using the direction model proposed by this study, we match each direction patterns. The method identifies a facial expression based on the least minimum score of combination values between direction model and pattern matching for presenting each facial expression. In the experiments, this study verifies the validity of the Proposed methods.

An Analysis of the Effect of Government Support on Automation and Smart Factory (자동화 및 스마트 공장 구축에 대한 정부 지원사업의 효과 분석)

  • The purpose of this paper is to figures out the impact on the business performance based on the case studies about the establishment of automated smart factories supported by the government. In this study, the effectiveness of supporting business is verified by comparing supported company with non-supported ones using methods such as T-test and ANOVA. The PSM method was used to solve the selection bias issue between the experimental group and the controlled group. The research results have shown that the effect of the supporting business to the automated system was tenuous, and the amount of sales and research and development costs was increased after a certain schedule passed in case of the supporting project to the smart factory. There is some time lag to appear the effect of the government supporting businesses and the supporting business to the automated system leads to long term sales increase by increasing parameters like research and development costs rather than direct influence. Therefore, this research will be useful information for the process of establishing useful basic data and policies which helps to secure new budget Government Supporting Businesses and find ways improve the business.
