중증 폐혈전색전증 환자에서 Urokinase와 Alteplase의 효과 및 출혈 합병증 비교 (The Therapeutic Efficacy and the Bleeding Complications of Urokinase and Alteplase in Patients with Massive Pulmonary Thromboembolism)

  • 연구배경: 폐혈전색전증 환자에서 혈전용해제의 효과는 차이가 없는 것으로 보고되어 있으나 출혈 등의 부작용에 대해서는 잘 알려져 있지 않다. 본 연구는 한 병원에서 중증 폐혈전색전증환자들에 사용한 alteplase와 urokinase의 치료효과 및 출혈 유발 정도를 비교하였다. 방법: 2000년 1월부터 2008년 7월까지 대상 환자 40명을 urokinase군(16명)과 alteplase군(24명)으로 나누어 의무기록을 후향적으로 분석하였다. 결과: 혈전용해제 사용한 이유는 혈역학적 불안정 23명, 중증 우심실부전 7명 그리고 헤파린 사용에도 불구하고 진행하는 저산소증 10명이었다. 두 군간 나이, APACHE II, SOFA score, 혈전용해제에서 유의한 차이는 없었다. 혈전용해제를 사용 후 승압제의 사용기간에서 aletplase군이 urokinase군보다 더 짧았으나, 인공호흡기 사용 기간과 중환자실 입원기간에서는 양 군간 유의한 차이가 없었다. 입원기간 동안 사망한 환자수는 13명(30%)으로 urokinase와 alteplase군 5명, 8명으로 유의한 차이는 없었고, 폐혈전색전증과 연관된 사망한 환자수에서도 urokinase와 alteplase군에서 1명, 4명으로 유의한 차이는 없었다. 혈전용해제 사용 후 수술적 치료가 필요한 환자 수도 urokinase군 2명, alteplase군 1명으로 차이가 없었다. 혈전용해제 사용 후 발생한 출혈은 14명(35%)에서 관찰되었고 urokinase군 3명(7.5%), alteplase군 11명(27.5%)으로 urokinase군에서 적은 경향을 보였다. 중증 출혈은 2명으로 모두 alteplase군에서 발생하였으며 1명은 뇌출혈로 사망하였다. 결론: 중증 폐색전혈전증환자에서 urokinase 사용은 alteplase에 비하여 치료효과는 차이가 없으면서 출혈의 위험은 낮을 것으로 사료된다.

  • The object of this study is to compare properties of massive fly-ash concrete with plain concrete. Two concrete mixtures comprising two batch each $1.0m^3$ in volume, were made from ready mixed concrete batch plant. The water-to-cementitious materials ratio was kept constant at 51.4%. Therefore, massive concrete specimen($W800{\times}D800{\times}H800mm$) was cast from ready mixed concrete to analyze history of temperature and core strength properties. Bleeding, time of slump loss and time of setting of the fresh concrete were measured. In order to estimate the properties of massive fly-ash concrete in hardened concrete, non-destructive tests such as rebound hardness, ultrasonic pulse velocity and maturity were performed and analyzed.

폐결핵의 외과적 요법에 대한 임상적 고찰: 3,566예의 분석 (Surgical Management of Pulmonary Tuberculosis - A Review of 3,566 Cases -)

  • Between October 1964 and August 1979, 2,537 patients underwent operation for pulmonary disease including tuberculosis at the National Kongju Hospital. Ages at operation ranged from 6.6 to 67.6 years with a mean age of 30.8 years and the male to female ratio was 3.07: l. Indications for operation were, respectively, as follows; destroyed lobe or segment with /without cavity[68.6%], ipsilateral total destroyed lung[14.8%], empyema with/wit-hout fistula[6.0%], atelectasis with bronchostenosis[2.6%], bronchiectasis[2.4%], tuberculoma[1.6%], lung abscess[1.7%], etc. There were 1,416 thoracoplasties[most common], 915 lobectomies, 591 pneumonectomies, etc. 1,073 patients with hemoptysis were found, streaking[23.8%], mild[42.3%], moderate[26.6%], severe[3.4%] and massive[3.9%] as the severity. Tuberculosis was the most common cause of severe and massive hem-optysis. Postoperative complications were developed 16.% of the operated cases, bleeding was the most frequent complication found in these cases. The mortality of operation was 8%, the most common cause of death was bleeding also.

Clinical Outcomes of Endoscopic Hemostasis for Bleeding in Patients with Unresectable Advanced Gastric Cancer

  • Purpose: Bleeding is one of the most serious complications of advanced gastric cancer (AGC) and is associated with a poor prognosis. This study aimed to evaluate the clinical outcomes of endoscopic hemostasis for bleeding in patients with unresectable AGC. Materials and Methods: This study included 106 patients with bleeding associated with gastric cancer who had undergone endoscopic hemostasis between January 2010 and December 2013. Clinical characteristics, treatment outcomes, including rates of successful endoscopic hemostasis and rebleeding, risk factors for rebleeding, and overall survival (OS) were investigated. Results: Successful initial hemostasis was achieved in 83% of patients. Rebleeding occurred in 28.3% of patients within 30 days. The median OS after initial hemostasis was lower in patients with rebleeding than in those without rebleeding (2.7 and 3.9 months, respectively, P=0.02). There were no significant differences in disease status and rebleeding rates among patients with partial response or stable disease (n=4), progressive disease (n=64), and first diagnosis of disease (n=38). Univariate and multivariate analyses (P=0.038 and 0.034, respectively) revealed that transfusion of ${\geq}5$ units of RBCs was a significant risk factor for rebleeding. Conclusions: Despite favorable success rates of endoscopic hemostasis for bleeding associated with gastric cancer, the 30-day rebleeding rate was 28.3% and the median OS was significantly lower in patients with rebleeding than in those without rebleeding. Massive transfusion (${\geq}5$ units of RBCs) was the only significant risk factor for rebleeding. Patients with bleeding associated with AGC who have undergone massive transfusion should be observed closely following endoscopic hemostasis. Further research on approaches to reduce rebleeding rate and prevent death is needed.

Massive Hemorrhage Facial Fracture Patient Treated by Embolization

  • Major maxillofacial bone injury itself can be life threatening from both cardiovascular point of view, as well as airway obstruction. Significant hemorrhage from facial fracture is an uncommon occurrence, and there is little in the literature to guide the management of these patients. We report a 73-year-old male driver who was transported to our hospital after a motor vehicle collision. The patient was hypotensive and tachycardic at presentation and required active fluid resuscitation and transfusion. The patient was intubated to protect the airway. All external attempts to control the bleeding, from packing to fracture reduction, were unsuccessful. Emergency angiogram revealed the bleeding to originate from terminal branches of the sphenopalatine artery, which were embolized. This was associated with cessation of bleeding and stabilization of vital signs. Despite the age and severity of injury, the patient recovered well and was discharged home at 3 months with full employment. In facial trauma patients with intractable bleeding, transcatheter arterial embolization should be considered early in the course of management to decrease mortality rate.

  • Trauma is an important risk factor for a pulmonary thromboembolism, and anticoagulation is essential to prevent deep vein thrombosis (DVT) in patients with trauma. Low-molecular-weight heparin (LMWH) is excreted in the kidney; therefore, using LMWH in patients with renal insufficiency may increase the risk of bleeding complication. The following case describes a 55-year-old traffic accident victim who had massive bleeding and underwent a laparotomy for bleeding control. The patient had acute renal failure, and enoxaparin was administered for the prophylaxis of DVT. Although the patient suffered from serious complications such as pericardial hematoma, the patient recovered without sequellae and was discharged at day 84.

Spinal Hemangiopericytoma Which Needed Intraoperative Embolization due to Unexpected Bleeding

  • Spinal intradural hemangiopericytoma is a very rare tumor and can be characterized by massive bleeding during surgeries, frequent recurrence, and metastasis. However, definite radiologic differential points of hemangiopericytoma are not known. We describe an unexpected hemangiopericytoma case with large bleeding and management of the tumor. A 21-year-old man visited complaining of progressive neck pain and tingling sensation in both hands. Magnetic resonance imaging of his spine revealed C1-2 ventral intradural mass. When the dura was opened, the intradural tumor was placed behind spinal accessary nerves. The tumor was partially exposed only after some accessary nerves had been cut. When internal debulking was performing, unexpected bleeding was noted and it was difficult to control because of narrow surgical field and hypervascularity. Intraoperative spinal angiography and embolization were performed. The tumor was completely removed after embolization. Pathological diagnosis was consistent with hemangiopericytoma. When surgeons meet a flesh-red tumor that bleeds unexpectedly during surgery, hemangiopericytoma may be considered. When feeder control is hard due to reciprocal location of spinal cord, the tumor, and feeders, intraoperative angiography and embolization may be a possible option.

  • Klippel-Trenaunay syndrome (KTS) is a rare disorder characterized by a triad of abnormal bone and soft tissue growth, the presence of a port-wine stain, and venous malformations. Gastrointestinal (GI) manifestations of KTS are relatively common and generally do not cause significant problems. However, persistence can lead to chronic GI blood loss or even massive bleeding in rare cases. The majority of the severe GI manifestations associated with KTS present as vascular malformations around the GI tract and exposed vessels can lead to serious bleeding into the GI tract. Herein, we report a case of a 16-year-old boy with severe iron deficiency anemia who was previously misdiagnosed as hemorrhoid due to small amount of chronic bleeding. The actual cause of chronic GI bleeding was from an uncommon GI manifestation of KTS as rectal polyposis.

술 중 대량 출혈을 동반한 거대 갑상선유두상암종 절제술 1례 (A Case of Giant Papillary Thyroid Carcinoma Resection with Massive Intraoperative Bleeding)

  • A 62-year-old female patient had goiter for twenty years. She visited out-patient clinic with a hoarse voice and intermittent breathing difficulties. About protruding 15cm sized mass located the anterior neck and right vocal cord paralysis was observed. Preoperative CT scan was strongly suspected of thyroid gland cancer and cervical lymph node metastasis. Therefore, fine needle aspiration test was performed and surgical treatment was planned with the histopathologic results (papillary thyroid carcinoma). Surgery was performed with total thyroidectomy, bilateral cervical lymph node dissection, and right selective nodal lymph node dissection (level II-V). During operation right thyroid seemed to be adherent to surrounding tissue and the blood vessels were extremely engorged. There was hypotensive crisis because of intraoperative excessive bleeding. However it was managed by repetitive transfusion. The operation was completed without abnormalities. She underwent 4 times of bleeding control operation due to postoperative bleeding. After complications were improved, we are currently undergoing out-patient follow up without morbidity.

Embolization for treating posttraumatic pseudoaneurysm of the sphenopalatine artery

  • Posttraumatic pseudoaneurysms of the sphenopalatine artery are rare. Only a few cases have been reported. We report two cases of hemorrhage due to pseudoaneurysm of the sphenopalatine artery. The hemorrhage was uncontrollable. It required embolization. Two patients visited our hospital for treatment of zygomaticomaxillary complex fracture. At the emergency room, patients presented with massive nasal bleeding which ceased shortly. After reduction of the fracture, patients presented persistent nasopharyngeal bleeding. Under suspicion of intracranial vessel injury, we performed angiography. Angiograms revealed pseudoaneurysms of the sphenopalatine artery. Endovascular embolization was performed, leading to successful hemostasis in both patients. Due to close proximity to pterygoid plates, zygomaticomaxillary complex fracture involving pterygoid plates may cause injury of the sphenopalatine artery. The only presentation of sphenopalatine artery injury is nasopharyngeal bleeding which is common. Based on our clinical experience, although pseudoaneurysm of maxillary artery branch after maxillofacial trauma has a low incidence, suspicion of injury involving deeply located arteries and early imaging via angiogram are recommended to manage recurrent bleeding after facial trauma or surgery.
