Implications of Emphysema and Lung Function for the Development of Pneumonia in Patients with Chronic Obstructive Pulmonary Disease

  • Background: Chronic obstructive pulmonary disease (COPD) is sometimes complicated with pneumonia, but little is known about the risk factors that promote the development of pneumonia in COPD. These risk factors were evaluated in the present study. Methods: The data of 324 patients with COPD from a prospective multi-center observational cohort with obstructive lung disease were evaluated retrospectively. To identify risk factors for the development of pneumonia in COPD, the clinical and radiological data at enrollment and the time to the first episode of pneumonia were analyzed by Cox proportional hazard analysis. Results: The median follow-up time was 1,099 days and 28 patients (8.6%) developed pneumonia. The Cox analysis showed that post-bronchodilator forced expiratory volume in one second ($FEV_1$, % of predicted) and the computed tomography (CT) emphysema extent (inspiratory V950) were independent risk factors for the development of pneumonia (post-bronchodilator $FEV_1$: hazard ratio [HR], 0.97; 95% confidence interval [CI], 0.94-1.00; p=0.048 and inspiratory V950: HR, 1.04; 95% CI, 1.01-1.07; p=0.01). Conclusion: Emphysema severity measured by CT and post-bronchodilator $FEV_1$ are important risk factors for the development of pneumonia in COPD.

Assessment of airborne bioaerosols among different areas in the hospitals (일부 종합병원 내 영역별 공기 중 미생물 평가)

  • Three major hospitals with over 500 beds located in and near Seoul were surveyed for airbone microorganisms from February 1, 1998 to February 18, 1998. The purpose of the study was to identify and quantify microbiological organisms circulating in the air of three different areas in the hospitals. For the study, a RCS air sampler was utilized equipped with two different collection media, the agar strip GK-A for bacteria and the agar strip HS for fungi. The areas investigated were the intensive care unit (ICU) in the Department of internal medicine, the Newborns room in the Department of Obstetrics, and the microbiology laboratory. The results were as follows; 1. The average numbers of general microbiological particles collected on the agar strip GK-A media were $205CFU/m^3$, $232CFU/m^3$, and $128CFU/m^3$ in each hospitals. The highest concentration of $387CFU/m^3$ was found in the ICU of A hospital at 15:00 during the day. Further analysis of the collected bioaerosols by gram staining, revealed that there were gram positive cocci (89.5%), gram positive bacilli (7.2%), gram negative bacilli (2.8%), and fungi (0.5%), in descending order of frequency. 2. Ten different genes were identified from the agar strip GK-A. The most frequently identified organisms were: the Coagulase negative staphylococcus (55.0%), Micrococcus (21.4%), Enterococcus species(10.4%), and Bacillus species (7.2%). A series of antibiotics susceptibility test were conducted against the aforementioned four(4) organisms. Ninety percent of coagulase negative stapylococcus were sensitive to Penicillins. Pathogenic microbes isolated include: Staphylococcus aureus, Acinetobacter species, Klebsiella pneumonia, Klebsiella oxytoca, and Pseudomonas aeroginosa. 3 Although 56.8% of the microorganisms grown on the strip HS media for fungi could not be identified, some of them were successfully identified. The most frequently found fungi were Aspergillus (35.3%), Yeast or Molds (6.2%), and Penicillium (0.7%). Based on the results obtained from the study, it was concluded that some areas in the hospitals had abnormally high bioaerosol concentrations which could be attributed to human activity. Therefore, it is recommended that periodic assessments of indoor bioaerosols aiming to identify the possible sources should be conducted in order to maintain clean indoor environment in the hospitals.

  • For biomaterials for skin regeneration with minimized inflammatory response, high bioactivity and biocompatibility are highly required. Also, it should have a porous microstructure to improve cell adhesion and growth. In this study, we extracted a new collagen source from duck's feet which is by-product, and made the shape of sponges from duck's feet collagen (DC) to compare with DBP and SIS. To analyze physical and chemical property of the scaffold, SEM and FTIR were used. MTT assay was used to measure the attachment and proliferation of NIH/3T3 in the scaffolds. RTPCR was used to evaluate the expression of proinflammatory cytokine. Also, 1,1-diphenyl-2-picrylhydrazyl (DPPH) was used to measure the ability of antioxidant activity. Overall, this study shows that DC scaffold is biocompatible and has good physical property. Additionally, DC scaffold shows the potential as wound healing biomaterials.

Effects of Korean Red Ginseng on White Matter Microstructure and Cognitive Functions : A Focus on Intrusion Errors (고려 홍삼이 대뇌 백질 미세구조 및 인지기능에 미치는 효과 : 침입 오류를 중심으로)

  • Objectives Although ginseng has been reported to protect neuronal cells and improve various cognitive functions, relationship between ginseng supplementation and response inhibition, one of the important cognitive domains has not been explored. In addition, effects of ginseng on in vivo human brain have not been investigated using the diffusion tensor imaging (DTI). The purpose of the current study is to investigate changes in intrusion errors and white matter microstructure after Korean Red Ginseng supplementation using standardized neuropsychological tests and DTI. Methods Fifty-one healthy participants were randomly allocated to the Korean Red Ginseng (n = 26) or placebo (n = 25) groups for 8 weeks. The California Verbal Learning Test was used to assess the number of intrusion errors. Intelligence quotient (IQ) was measured with the Korean Wechsler Adult Intelligence Scale. Depressive and anxiety symptoms were evaluated using Hamilton Depression Rating Scale, Hamilton Anxiety Rating Scale, and Hopkins Symptom Checklist-25. The fractional anisotropy (FA) was measured from the brain DTI data. Results After the 8-week intervention, Korean Red Ginseng supplementation significantly reduced intrusion errors after adjusting age, sex, IQ, and baseline score of the intrusion errors (p for interaction = 0.005). Change in FA values in the left anterior corona radiata was greater in the Korean Red Ginseng group compared to the placebo group (t = 4.29, p = 0.04). Conclusions Korean Red Ginseng supplementation may be efficacious for improving response inhibition and white matter microstructure integrity in the prefrontal cortex.

  • Purpose: This study was to evaluate the accuracy of how to use the Mechanical Torque-Limiting Devices. Materials and methods: Three different implant manufacturers (Dentium, Osstem, Shinhung) were prepared for each two implant torque controllers. Divided into two groups depending on the method used for removal torque was measured. Repeated measures of ANOVA test (${\alpha}=.05$) was used as statistics to evaluate the effect of repeated loading number on the removal torque. Independent t-test was used to evaluate the difference in removal torque of two groups. Results: The removal torque significantly decreased as the number of loading repetition increased (P<.05). There was significant difference between two groups. Conclusion: Loosen the handle part of the implant torque controllers spring resilience to recover one group compared to the group that did not, showed a little more closed to the reference value.

  • Although gastric cancer is quite common in Korea, the treatment outcome is relatively favorable compared to those in western countries. However, there are currently no Korean multidisciplinary guidelines for gastric cancer. Experts from related societies developed guidelines de novo to meet Korean circumstances and requirements, including 23 recommendation statements for diagnosis (n=9) and treatment (n=14) based on relevant key questions. The quality of the evidence was rated according to the GRADE evidence evaluation framework: the evidence levels were based on a systematic review of the literature, and the recommendation grades were classified as either strong or weak. The applicability of the guidelines was considered to meet patients' view and preferences in the context of Korea. The topics of the guidelines cover diagnostic modalities (endoscopy, endoscopic ultrasound, and radiologic diagnosis), treatment modalities (surgery, therapeutic endoscopy, chemotherapy, and radiotherapy), and pathologic evaluation. An external review of the guidelines was conducted during the finalization phase.

  • Purpose : The purpose of this study is to make and use monoclonal Ab which reacts with CMV major immediate early(${\alpha}$) protein(p72). Methods : Normal human fibroblast(Foreskin derived) was cultured in Eagle's minimal essential medium(MEM) containing 10% cowfetus serum and mouse chondroblast was cultured in P3X63 Ag8.653(ATCC. Maryland USA) to maintain $5{\times}10^5/ml$ cell counts. CMV(KJHJ90) from congenital CMV infected infant's urine was multiplied and used for Ab making. CMV Ag was injected 4 times, 1 week interval into the peritoneal space of 6~8 weeks old mice. And then lymphocyles and fibroblasts taken from spleen were obtained and conjugated. After the conjugated cell cultured, we chose the cell that had high Ab titer using indirect immunofluerescent method. Results : Among the 28 monoclonal antibodies obtained LPC12 and LPC23 reacted highly with nucleus of AD169 infected cell. Purified AD169 after SDS-PAGE, molecular weight of Ag, which reacted with purified monoclonal Ab, was obtained using Western blotting. Monoclonal Ab of LPC12 and LPC23 clone reacted most highly with 72 kd Ag. Conclusion : LPC12 and LPC23 clonal Ab with AD 169(P63-27) is useful on early diagnosis of CMV infection.

  • The aim of this study was to understand home hospice care status and problem in Korea, and ultimately to develop the home hospice standard. This study was conducted as a part of a study on the institutionalization of the home hospice in Korea. A focus group interview with representatives of seven home hospice agency where have provided home hospice service for years was conducted. All of the participants agreed to the essential components for home hospice service including 24 hour on call service, multidisciplinary team visiting, and periodical team meeting. Visiting frequency was 1-3 times per week mostly by nurses. And they agreed requisitely to fulfill an office for home visiting nurses, storage space, and home visiting bags. The obstacles of providing home hospice were 1) no reimbursement system, 2) difficulties to change medication at home, 3) lack of inpatient beds for symptom control. Standardization of home hospice is critical to improve service quality and to develop reimbursement system. The findings of this study could be used as a basic data to develop home hospice standards and guidelines.

The Teaching Effectiveness of Preceptorship and Satisfaction of Student for Clinical practice on Nursing Education (간호학생 임상실습 교육에서 실습 지도자 활용(preceptorship)의 교수 효율성과 임상실습 만족도)

  • To identify the effects of preceptorship on teaching effectiveness at clinical nursing education, a group of nursing students who participate in program under the instruction of the preceptors was compared with the other group of nursing students trained by head nurses.The data were collected from 14 Mary. 2001 to 8 June 2001. The subjects of this study were 85 nursing students trained by the preceptors in the G. medical college hospital in Inchon, and 122 nursing students trained by head nurses at several hospitals in Inchon and Puchon, Korea. The scale for measuring the teaching effectiveness and satisfaction of student for clinical practice developed by Kim, Miae and Lee, Sookja were employed as the tool for the study. The data collected were analysed with SPSS on the teaching effectiveness items, then mean value and SD were evaluated. The t-test were also employed to compare the teaching effectiveness and satisfaction of student for clinical practice of the both groups. The relation of teaching effectives and satisfaction of student for clinical practice were analysed with Pearson's correlation coefficient. The following findings were carried out from this study. 1) The group trained by preceptors showed significant higher teaching effectiveness statistically than the group trained by head nurses, scored mean value of 3.46 and 3.04 respectively in the item of teaching effectiveness. 2) The respective item showing high score of teaching effectiveness in both groups was 'smooth and reasonable dealing on the issues occurred in the wards'. But the items of 'help nursing students to familiarize with other staffs quickly'and 'help always at near' showed low scores. 3) For 5 causal factors classified on the teaching effectiveness, the factor of 'professional knowledge and capability' got the highest score, but 'availability as a supporter' showed the lowest score in both groups. 4) The group trained by preceptors showed significant higher students' satisfaction of clinical practice than the group trained by head nurses, scored mean value of 3.54 and 3.17. 5) Total teaching effectives were posive correlation with students' satisfaction of clinical practice significantly (r=0.603)

  • This study was aimed to be able to dedicate to the quality promotion of nursing of the nurses and the development of hospital organization by investigating interrelations between job satisfaction degree and organizational commitment of the clinical nurses. The objects of this study have been 507 nurses working in university hospital in Seoul and the materials have been collected through structured questionnaires for 14 days from Feb. 23. 1998 through March 8. The materials have been analyzed and computerized statistically with SPSS. General characteristics have been analyzed by practical number and percentage. and job satisfaction degree and organizational commitment have been analyzed by average evaluation marks and standard deviation and the relation between general characteristics and job satisfaction degree and the relation between general characteristics and organizational commitment have been analyzed by t-test or F-test(ANOVA) according to the characteristics of variable. Correlations between job satisfaction degree and organizational commitment have been analyzed by Pearson's Correlation Coefficient. The followings are the summaries of the results of the study: 1. The degree of job satisfaction of the object was 3.26 in average on the basic of 5 marks and worth area showed highest by 3.78 while wage and welfare 2.70 the lowest when analyzed 11 areas. 2. Organizational commitment of the object showed 3.24 in average on the basic of 5 marks. 3. According to the age. marital status. educational background. employment experience. experience in current department. position. types of work. department of work. wage, satisfaction about the current department, whether they wish to be transferred to the other department and whether they are transferred or not. there were significant differences in the characters of job satisfaction degree when general characteristics and job satisfaction degree have been examined. 4. According to the age. marital status. religion. educational background. employment history. position and experience in current department. types of work. department of work. wage. satisfaction about the department they are assigned. whether they wish to be transferred. Whether they are transferred or not and number of being transferred to the other department. there have been significant differences in the characters of organizational commitment when general characteristics and organizational commitment have been examined relatively. 5. Correlations between the degree of job satisfaction and organizational commitment has shown the degree of organizational commitment higher respectively when the degree of job satisfaction was higher. the degree of sanitation factor was higher and the degree of motive factor was higher (r=.73799. r=.6826. r=.6570). 6. The result of the analysis of correlations between organizational commitment and the turnover intension and job related action has shown comparatively high reverse correlations (r=.6375) between organizational commitment and turnover intension and low reverse correlations(r=.3253) between organizational commitment and job related action. Based on the above results. the study of the ways of increasing the degree of satisfaction about wage. welfare. position and stability and the supervision of the senior which showed the low degree of job satisfaction should be conducted and through the management of the degree of job satisfaction which affects to the organizational commitment, I would like to propose that the ways of increasing the sense of commitment to the hospital organization of the nurses should be studied.
