A Study on the Improvement of Search and Rescue Coordination for Effective Response to Marine Casualties (해양사고의 효율적 대응을 위한 수색구조조정 개선방안 연구)

  • KCG, responsible Agency of maritime SAR operation has been known to address the marine casualties well, however, it was criticized because KCG failed to take prompt and effective response to the recent Ferry disaster at the very early stage. In this regards, the authors analyze the characteristics of marine incident with historical data, and also review and examine the national SAR system focusing on SAR coordination and human resources to make recommendation and suggestion in order to minimize the loss of lives to future marine incidents. As a result, KCG shall augment its capabilities, inter alia, fast libeboats and rescue helicopters for prompt response to marine incidents at the coastal waters, and KCG shall re-adjust the establishment of Rescue Coordination Center to improve operational problems of current system in accordance with IMO guidance, KCG is required to introduce mandatory training, qualification and certificate system to enhance the professionalism of SAR personnel.

  • Marine casualties and environmental pollution have increased recently. Especially, the rate of incident of Coastal tankers is higher, but the assessment tool for safety management system is lack in this fields. However Oil Major Vetting System being professional assessment tool for tanker is widely applied in ocean going tanker by the worldwide Oil Majors. According to the analysis of the marine tankers' incidents which applied Oil Major Vetting System, the incidents were reduced rapidly for recent about 5 years. Using Oil Major Vetting System is helped to improve safety management and to prevent marine incidents. Therefore if applying a parts of the Oil Major Vetting Systems to the coastal tankers' Safety Management System, the Coastal tankers incidents would be reduced and improved gradually.

  • The results of investigations into marine incidents have become an important basis in determining not only possible causes, but also the extent of negligence between the perpetrator and victim. However, marine incidents occur under special circumstances i.e. the marine environment, and this leads to difficulties in identifying causes due to problems in scene preservation, reenactment and acquisition of witnesses. Given the aforementioned characteristic of marine incidents, the International Convention for the Safety of Life at Sea (SOLAS) has adopted mandatory regulations on the carriage of Voyage Data Recorders (VDRs) and Automatic Identification Systems (AIS) for ships of a certain gross tonnage and upwards, so as to reflect recent developments in radio communication and marine technology. Adopted to provide an international standard for investigations and to promote cooperation, the Code of the International Standards and Recommended Practices for a Safety Investigation into a Marine Casualty or Marine Incident (Casualty Investigation Code) recommends member states to build capacity for analysis of VDR data. Against this backdrop, this paper presents methods for efficient investigations into the causes behind marine incidents based on data analysis of VDR, which serves as the black box of ships, as well as digital forensic techniques.

A Comparative Study on the Investigation Manuals of Marine Accidents (해양사고 조사매뉴얼의 비교연구)

  • The investigation of marine accidents in Korea can not handle the international codes and guidelines and is being followed the conventional methods with a big gab to the international criterion and procedure. Also there is no proper investigation manual which is cope with the new era and surrounding circumstances. This study analyzed the investigation manual of other countries, and proposed new investigation manual according to the analysis and international standards and procedure as followings; (1) Develop own investigation manual separating from the field manual which is mixed with judge, (2) Correct and improve the error and hazy words in the existing field manual which is not compatible a current situation, (3) The contents of new manual should be changed to cope with the international rules, criterion and procedures. The list of contents and index is added, an abundant explanation and examples are given, and charts and illustrations are inserted to make the users understand easily. Also the inquiry techniques according to the classification of each accidents are included.

  • Marine casualties of dangerous cargo carriers lead to not only the loss of human life and properties, but also marine pollution caused by spilled oil and hazardous cargo. So the prevention of marine casualties and emergence response to minimize pollution are very important problems to be settled. But until now, most of salvage assistance jobs have been done based on rule of thumb without any technical support. It is not easy to cope with the incidents safely and rapidly under the current situation. The purpose of this paper is to develop a systematic and engineering salvage assistance tool available in the casualty sites. This paper contains the construction of salvage system, classification of technical items, and prototype system for the safety assessment. The developed system enable a salvage engineer in his/her salvaging jobs to perform safely and promptly.

  • In 2010 Manila Conference, the revised STCW (International Convention on Standards of Training, Certification and Watch-keeping for Seafarers) of IMO recalled that a large percentage of maritime casualties and pollution incidents are caused by human error. Therefore, education and training improvement will play important role to reduce maritime casualties. From this, we focus on improving safety during sailing through upgrading the education and training for Korean cadets by updating maritime education and training program. In terms of a maritime education program from some of the maritime universities in the world such as United States Merchant Maritime Academy(USMMA) Philippine Merchant Marine Academy(PMMA), are compared to the Maritime University in Korea. As a result, we suggest an effective education system.

Human Error Analysis Technique and Its Application to Marine Accidents

  • The management of safety at sea is based on a set of internationally accepted regulations and codes, governing or guiding the design and operation of ships. The regulations most directly concerned with human safety and protection of the environment are, in general, agreed internationally through the International Maritime Organization(IMO). IMO has continuously dealt with safety problems and, recognized that the human element is a key factor in both safety and pollution prevention issues(IMO, 2010). This paper proposes a human error analysis methodology which is based on the human error taxonomy and theories (SHELL model, GEMS model and etc.) that were discussed in the IMO guidelines for the investigation of human factors in marine casualties and incidents. In this paper, a cognitive process model, a human error analysis technique and a marine accident causal chains focused on human factors are discussed, and towing vessel collision accidents are analyzed as a case study in order to examine the applicability of the human error analysis technique to marine accidents. Also human errors related to those towing vessel collision accidents and their underlying factors are discussed in detail.

  • 선박산업에서의 국내외 해양사고 통계에 의하면, 모든 해양사고의 80% 정도가 직접적 또는 간접적으로 관련된 인적요인(Human Factors)에 의하여 발생한다고 보고되고 있다. 이러한 해양사고의 원인을 규명함으로써 유사 해양사고의 발생을 방지하고, 또한, 해양사고 조사에 있어서 공통적인 접근방법의 이용과 국가 간의 협력을 증진시키기 위한 목적으로, IMO(International Maritime Organization)에서는 해양사고 인적요인 조사지침(Guidelines for the Investigation of Human Factors in Marine Casualties and Incidents)을 포함한 해양사고조사코드(Casualty Investigation Code: Code of the International Standards and Recommended Practices for a Safety Investigation into a Marine Casualty or Marine Incident)를 채택하였으며, 동 코드는 2010년 1월 1일 발효되었다. 이에 따라, 해양안전심판원에서는, 3단계 절차로 구성된 인적요인 조사 및 분석 방법론을 제안하고, "해양사고 인적요인 분석 매뉴얼"을 마련하였다. 본 논문에서는, 해양안전심판원에서 제안한 인적요인 조사 및 분석 방법론을 실제 해양사고에 적용하여 인적요인 조사분석 모델의 적용사례를 마련하고, 분석 모델을 검토 및 보완하고자 한다.

Problems and Ways of Improving on Personal Organization of Maritime Safety Tribunal (해양안전심판원의 인적구성의 문제점 및 개선방안)

  • The purpose of Maritime Safety Tribunal is that experts about ship operation investigate and reveal causes of marine disasters and suggest plans to prevent recurrence in Korean Maritime Safety Tribunal which is a kind of administrative court. Despite the fact that 72% of marine disasters is fishing vessel accidents and 75% of collision accidents between vessels which most occur is related to fishing boats, it is the very serious problem that there is no any person with licenses of marine technicians for fishing vessels in judges and investigators of Korean Maritime Safety Tribunal. The operational characteristics of fishing vessels, their sizes, and shift patterns of duties in wheelhouses are completely and incommensurably different from those of merchant vessels. By the way, if the investigators and judges who just consist of merchant vessel experts investigate and judge marine accidents about fishing vessels, there could be errors in establishment of policies to investigate and reveal their causes and prevent recurrence of accidents. Especially, in case of collision accidents between fishing vessels and merchant vessels, it is thought that the marine accident interested provides causes which can be doubtful about fairness of judges. Therefore, it is thought that the ratio of judges and investigators is most desirable to compose it to be similar to the occurrence frequency of marine accidents. For this, the following solution plans are suggested. First, qualification for appointment requires first class marine technicians. But there is the only one vessel which needs the first class in fishing vessels. Therefore, it is thought that the provisory clause should be added so that the second class marine technicians can be used instead of the first class ones. Second, the marine accidents of fishing vessels reach 72% but the fact that there is no any judge and investigator with licenses of marine technicians for fishing vessels is thought to go against the purpose of establishment of Korean Maritime Safety Tribunal. Therefore, it is thought that there is, at least, one more judges and investigators in the central and local Korean Maritime Safety Tribunals. The same method should be applied to judge assistants and investigation assistants.

A Study on the Revision of the Notification Form and Procedures of Marine Incident (준해양사고 통보서식 및 절차 개정에 관한 연구)

  • Accident prevention is more important than follow-up, which is based on Heinrich's law. The marine incident system is a very meaningful system that can prevent similar accidents, and was introduced in 2010 in Korea in accordance with the enforcement of the Code for the Investigation of Marine Casualties and Incidents (CI Code). Based on the CI Code, ship owners or ship operators are required to notify the Central Chief Inspector using the designated notification form in the event of a marine incident, but the number of voluntary notifications is still small. In this regard, this study intends to provide a direction for improvement by conducting an in-depth analysis focusing on the lack of notification procedures and forms of the marine incident system. To this end, we analyzed related regulations, cases of excellent overseas shipping countries such as the United Kingdom and Singapore, cases of similar domestic transportation systems such as aviation and railways, and marine incident notification procedures and forms of leading shipping companies. Major improvements in the notification process include the transition of the marine incidents to voluntary reporting, the expansion of the reporting subjects, and the identification of the security of the informer's identity. The main contents of the notification form revision include the use of the term "reporting" instead of "notification," the content of the identity guarantee in the notification form, and the increase in statistical value through the expansion of optional entries.
