The purpose of this study was to determined the effects of reciprocal inhibition on spatial-temporal gait parameters in spastic hemiplegic patients through GaitRite system. The subjects were consisted 45 patients who had spastic hemiplegia due to stroke. All subjects randomly assigned to 3 group : manual reciprocal inhibition program group(manual group), neuromuscular electrical stimulation group(NMES group) and control group. The manual group received voluntary isometric contraction of pre-tibia muscle. The NMES group received neuromuscular electrical stimulation on tibialis anterior. The control group was not recieved any therapeutic intervention. Before and after experiments, spatial-temporal gait parameters and functional ambulatory profile was measure in all patients. The data of 30 patients who complete experimental course were statistically analysed. The results of this study were as following : 1. The percentage of change of functional ambulatory profile were markedly increased in manual group but statistically non significant(p>.01). 2. The percentage of change of gait velocity and cadence were markedly increased in manual group but statistically non significant(p>.01). 3. Asymmetry ratio of gait elements were more improved in manual group but statistically non significant(p>.01). 4. There were no statistical difference between pre-test and post-test with functional ambulatory profile, gait velocity, cadence and asymmetry ratios in NMES group(p>.01). 5. There were no statistical difference between pre-test and post-test with unctional ambulatory profile, gait velocity, cadence and asymmetry ratios in control group(p>.01). In conclusion, the present results revealed that reciprocal inhibition which produced by voluntary isometric contraction of pre-tibia muscle can be improved spatial-temporal gait parameters including functional ambulatory profile in hemiplegic patients. Therefore, reciprocal inhibition is useful to improve functional activities in hemiplegic patient. Further study should be done to analyze the effects of intervention duration of reciprocal inhibition, appropriate muscle contraction, optimal time to apply the reciprocal inhibition in more long period.
Objective : The purpose of this study was conducted to find correct head and neck patterns, manual contact. verbal commands with proprioceptive neuromuscular facilitation(PNF). Method : This is a literature study with books, seminar note and book for PNF international course. Result : Keep the information of the biomechanics and neural science in head and neck patterns and emphasize that manual contact, verbal commands and visual stimulus. Manual contacting for movement guide and stability of the $C_0/C_1$ verbal command and visual stimulus for correcting of the $C_0/C_1$ movements. Conclusion : In reminder for PNF learning, begin with head and neck and upper trunk patterns. In that time, Knott and Voss(1968) had not enough information about biomechanic movement components and neural science movement components. But Knott and Voss(1968) emphasized that head and neck patterns relate with trunk, upper extremities and lower extremities directly. Alar ligaments are relaxed with the head in neutral and taut in flexion. Axial rotation of the head and neck tightens both alar ligaments. The right upper and left lower portions of the alar ligament limit left lateral flexion of the head and neck. Therefore, head and neck patterns has to be modify. When head moving, eye and vestibular stimulus will be change. During head and neck patterns, must be consider about stimulus of eye system and vestibular system also.

Parkinson's disease(PD) is a progressive neurodegenerative disease that affects the functioning of the basal ganglia, a brain area that contributes to the control of movement. The disease is caused by the death of nerve cells in the brain that produce dopamine, a chemical messenger. The cells affected usually produce a neurotransmitter(a chemical that transmits nerver impulses) called dopamine, which acts with acetylcholine, another neurotransmitter, to fine-tune muscle control. In Parkinson's disease, the level of dopamine relative to acetylcholine is reduced, adversely affecting muscle control. When the supply of dopamine is depleted, the function of the basal ganglia is disrupted and its ability to control movement deteriorates. The result is that PD patients experience moderate rigidity, difficulty in initiating movements and slowness in executing them, and a rhythmical tremor at rest. Although the cause of Parkinson's disease is not known, genetic factors may be involved. About 3 in 10 people with the disorder have an affected family member. About 1 in 100 people over the age of 60 in the US have Parkinson's disease. And Parkinson's disease is slightly more common in men. The course of the disease is variable, but drugs may be the best effective in treating the symptoms and improving quality of life. But, The doctor may arrange physical therapy to help with physical mobility problems. It is important to continue to exercise and take care of your general health. Try to take a walk each day. Stretching exercises can help you maintain your strength and mobility. So, This papers will serve about the information of PD for clinical physical therapist. Finally, The aim of review is increasing approach method and technique for PD patients by the view of physical therapy.

Background: This study aimed to investigate the effect of modified constraint-induced movement therapy (mCIMT) and resistive exercise using elastic band with pressure belt on improving upper extremity function in stroke patients. Methods: Sixteen patients with stroke were randomly assigned to a control group that received mCIMT and resistive exercise using elastic band (n=8) and an experimental group that received mCIMT and resistive exercise using elastic band with pressure belt (n=8). Over the course of four weeks, mCIMT were conducted 60 minute three times per week and resistive exercise using elastic band (with pressure belt) were conducted twice daily, three times per week. The function of the upper extremities were evaluated before, after 2 weeks and 4 weeks using the grip strength test (GST), the box and block test (BBT), and motor activity log (MAL). Results: The values for the GST, the BBT, and MAL increased in both groups as the treatment period progressed. The values for the GST (p<.01), the BBT (p<.001), and MAL (p<.001) were significantly higher in the experimental group than in the control group at 4 weeks after initiating the treatment. Conclusion: We found that mCIMT and wearing a pressure belt during resistive exercise was very useful in improving the function of the upper extremities in patients with stroke.

Purpose: The purpose of this study was to evaluate the effects of exercise on functional status in stroke patients. The understanding of the course of recovery after stroke and factors affecting outcomes are important in planning and evaluation of stroke rehabilitation. Methods: To predict the outcom of stroke patient, we measured in the beginnig of rehabilitation, on 36 patients. The variables were gender, age, duration of rehabilitation treatment, motor assessment scale(MAS). Results: The patient age in 50-59 years was 41.7%. The stroke left side motor weakness were found 55.6%. The duration of rehabilitation treatment in 2-3 months was 50.0%. In the difference 24.63 for motor assessment scale after a rehabilitation treatment had significantly higher than 10.86 for motor assessment scale at initial. Gender by motor assessment scale after a rehabilitation treatment was 25.7 in male and 23.2 in female(P<0.05). Past history by motor assessment scale after a rehabilitation treatment was 26.7 in hypertension, 24.6 in cardiac disorder and 21.8 in diabetes mellitus(P<0.05). Conclusion: The subjective symptoms of motor assessment scale after a rehabilitation treatment was significantly associated with gender.

Background: The purpose of this study is to identify the elements for the construction of the Korean academy of orthopedic manipulative physical therapy's (KAOMPT's) non-face-to-face lecture system using the Delphi method. Methods: The Delphi method was applied to 50 expert panel members of the Central Committee and the Provincial Branch of the KAOMPT. The Delphi survey was conducted in two rounds, and the first Delphi survey collected opinions on 40 questions on 12 topics. The second Delphi survey was collected into 25 questions on 4 topics. As a result of the survey, the content validity ratio (CVR), consensus and convergence were measured. Referring to the number of expert panels and previous studies were determine a CVR of at least 2.29, a consensus of at least .75 and a convergence of 0 to .5. Result: In the first Delphi result, out of the total 40 items, 20 items with high content validity ratio were found, and 10 items found double agreement. In the second Delphi result, 13 out of the total 25 items had a content validity ratio higher than 2.29, and 5 items found a double agreement. Conclusion: This study derived items on the role of central and municipal councils, lecture support and lecture room construction, non-face regular course and special lecture operation and personnel for the establishment of non-face-to-face lecture system. Based on this content, it is expected that it will help establish a non-face-to-face lecture system in 2021 through a pilot non-face-to-face lecture that will be implemented in the future.

Thoracic outlet syndrome is actually a collection of syndromes brought about by abnormal compression of the neurovascular bundle by bony, ligamentous or muscular obstacles between the cervical spine and the lower border of the axilla. First of all a syndrome is defined as a group of signs and symptoms that collectively characterize or indicate a particular disease or abnormal condition. The neurovascular bundle which can suffer compression consists of the brachial plexus plus the C8 and T1 nerve roots and the subclavian artery and vein. The brachial plexus is the network of motor and sensory nerves which innervate the arm, the hand, and the region of the shoulder girdle. The vascular component of the bundle, the subclavian artery and vein transport blood to and from the arm. the hand. the shoulder girdle and the regions of the neck and head. The bony, ligamentous, and muscular obstacles all define the cervicoaxillary canal or the thoracic outlet and its course from the base of the neck to the axilla or arm pit. Look at the scheme of this region and it all becomes more easily understood. Compression occurs when the size and shape of the thoracic outlet is altered. The outlet can be altered by exercise, trauma, pregnancy, a congenital anomaly, an exostosis, postural weakness or changes. Thoracic outlet syndrome has been described as occurring in a diverse population. It is most often the result of poor or strenuous posture but can also result from trauma or constant muscle tension in the shoulder girdle. The first step to beginning any treatment begins with a trip to the doctor. Make a list of all of the symptoms which seem to be present even if the sensations are vague. Make a note of what activities and positions produce or alleviate the symptoms and the time of day when symptoms are worst. Also, note when the symptoms first appeared. This list is important and should also include any questions one may have.
Objectives : The purpose of this study was to observe the effect of Bong (a type of stick which is called bong) Chuna manual therapy (CMT) on the correction of posterior inferior (PI) ilium deviation, and evaluate changes of pain intensity and Oswestry Disability Index (ODI) for the treatment of low back pain. Methods : We measured leg length inequality (LLI), as an indicator of correction of PI ilium, before and after the Bong CMT twice immediately, and also checked the change of LLI in the course of 10 times of treatment. Ten times of Bong CMT with oriental medical treatment, such as acupuncture and bee venom pharmacopuncture, were performed not only to correct the PI ilium deviation but also to alleviate pain. Visual Analog Scale (VAS) and Oswetry Low-back Pain Disability Index (ODI) score were scored to evaluate the pain and function of low back. Results : In this study, LLI was decreased in general while we performed correction of PI ilium deviation immediately, also effectively decreased comparing between before and after 10 times of Bong CMT, Bong CMT with oriental medical treatment improved pain and function of low back. Conclusions : These results suggest that Bong CMT may be an effective method for the correction of PI ilium deviation. However more rigorous study should be followed because of small cases report.
Background: This study had been carried out with 20 elderly subjects as its object for about one month from November 3, 2008 to December 14, 2008 in order to observe the effect of Underwater Treadmill on the elderly's walking and balance ability. Methods: Subjects were assigned either experimental group (n=10) or the control group (n=10), experimental group received Underwater Treadmill program (30 min per course, 3days a week for 6weeks). Subjects were assessed for muscle power (Nicolas Manual Muscle Test), balance (Functional Reach Test), gait ability (Time Up and Go, 10M walking test) before experiments and after experiments. Results: The results of this study were as follows; 1. After underwater treadmill exercise, the change of isometric contraction indicated a beneficial increase on lower extremity muscle power of experimental group and functional test of balance function; FRT, TUG indicated beneficial difference between groups. 2. beneficial difference between groups in walking speed of hourly index change of walking function. 3. between muscle power and balance, gait ability, we could find out there's high correlation ship between lower extremity muscle power increasing and balance and gait ability of the elderly. Conclusion: Aerobic exercise using underwater treadmill effects on muscle power strengthening of the elderly, and because of this, increase of lower extremity muscle power is very helpful not only to improvement of balance ability, but also to improvement of gait ability, so it will be used as a physical therapy program on clinic and used as an exercise program for protecting the elderly from falling down very well.
Like acupuncture, magnetic therapy has been known to yield effectiveness when it is applied to relieve from fatigue, musculoskelectal diseases, sore sites, rheumatic arthritis and chronic pain syndromes. However, combined application of acupuncture and magnet has not yet been studied. This study is designed to investigate effectiveness of acupuncture therapy when in the magnetic field for the pain relief. Magnetic field was made by magnetic ring ($7{\psi}{\times}2.3{\psi}{\times}1.5mm$). Twenty-one male swimmers with latent muscular pain at the GB21 area in the university course of physical education in Daegu were chosen and divided into three groups; 1) acupuncture treatment group (n=7), 2) acupuncture treatment with iron ring group (n=7), 3) acupuncture treatment with magnetic ring group (n=7). Manual Acupuncture was given to the GB21 point for 20 minutes. The degree of pressure pain threshold (PPT, $kg/cm^2$) in GB21 was measured with algometer. Before acupuncture treatment, the PPT values were $6.08{\pm}1.69$, $6.39{\pm}1.72$ and $5.59{\pm}1.11$ in acupuncture treatment group, acupuncture treatment with iron ring group, acupuncture treatment with magnetic ring group, respectively. After acupuncture treatment, the PPT values were $6.48{\pm}2.33$, $6.31{\pm}1.31$ and $6.59{\pm}1.80$, respectively. Pressure threshold was significantly increased in the acupuncture treatment with magnetic ring group compared to the other groups. Based on these results, acupuncture treatment with magnetic ring produced better effects on pain threshold, and these effects can be considered to be associated with the currents or voltages induced by the acupuncture needle and magnetic ring at present.
