사무직 근로자의 만성요통에 대한 복부 드로잉-인 기법이 요부 기능장애와 균형 능력에 미치는 효과 (The effect of Abdominal Drawing-in Maneuver on Chronic Low Back Pin in Office Workers on Lumbar Dysfunction and Balance Ability)

  • Objective: To compare the effects of lumbar dysfunction and balance when office workers with chronic back pain performed the abdominal drawing-in maneuver. Methods: A total of 16 office workers with chronic low back pain were included in this study. The participants were randomly divided into two groups: the general and abdominal drawing-in maneuver groups. The intervention was applied for 6 weeks, 3 times a week, 70 min per day. Participants in the general exercise group(n=8) performed the general physical therapy and lumbar flexibility exercise, whereas those in the abdominal drawing-in maneuver group(n=8) performed the general physical therapy and lumbar stabilization exercise using abdominal drawing-in maneuver(3 times/week for 6 weeks). All tests, were the Korean Oswestry disability index (KODI) and balance ability, were completed pre and post-intervention. Results: Significant improvements in the KODI and balance ability test were observed in the abdominal drawing-in exercise group (p<.05), whereas no significant changes (p>.05) were observed in the general exercise group. Conclusion: This study revealed that abdominal drawing-in maneuver can effectively improve the lumbar dysfunction and balance ability of office workers with chronic back pain.

견갑골 익상을 동반한 어깨 통증 환자에 대한 자가 운동 중심의 관리: 사례 보고 (Self-Exercise Oriented Management for Shoulder Pain Patient with Winging Scapular : Case Report)

  • Background: Scapular winging is a rare disorder that is commonly caused by nerve damage of the dorsal scapular nerve, spinal accessory nerve, or the long thoracic nerve. This affects the scapulohumeral rhythm which may cause abnormal kinetic motion of the shoulder. The purpose of this case report is to describe a self-exercise oriented management incorporating shoulder strengthening to reduce symptoms in a shoulder pain patient with winging scapular. Methods: A 45 year old male patient complained of pain in his both shoulders without any trauma. Shoulders were treated with steroid injections for supraspinatus tendonitis, but although pain improved to some extent, pain and disability continued for 3 months. Both shoulders had pain, decreased active range of motions, muscles weakness, and scapular winging. The patient underwent 9 interventional sessions over 3 months and was managed mainly by self-exercise. The intervention method involved push up plus, sling, muscle strengthening, and stabilization exercises. Loads were increased as symptoms improved. Results: Clinical outcomes were measured at every session. Pain in both shoulders reduced to 0 on a numerical pain rate scale by the 4th session, and the active range of motion was fully recovered. During the 9th session, the strength of the serratus anterior had improved from grade P to G on the right side and grade G to N grade on the left. Conclusion: In this case study, the self-exercise program was effective in reducing pain, increasing active range of motion, and improving muscle strength in subjects with scapular winging.

턱관절 질환 치료 시 Sodium Hyaluronate의 효과 (EFFECT OF SODIUM HYALURONATE IN TREATING TEMPOROMANDIBULAR JOINT DISORDERS)

  • The term temporomandibular disorders is used to describe a group of conditions that involve the temporomandibular joint, masticatory muscles, and associated structures. Many modalities have been proposed for treating temporomandibular disorders, including medication, physical therapy, occlusal stabilization splints with or without manual repositioning, surgery, and arthrocentesis. Temporomandibular disorders are treated in a step-wise manner. Initially, conservative treatment is used. Depending on the response, more aggressive interventions may be necessary. This usually takes the form of arthrocentesis. Arthrocentesis is used in the treatment of not only acute, closed, and locked TMJs but also various other temporomandibular disorders. Recently, the intra-articular injection of sodium hyaluronate after arthrocentesis was shown to have long-term palliative effects on TMJ symptoms. Synovial fluid consists of plasma and glycosaminoglycan, including hyaluronic acid derived from synovial cells. Sodium hyaluronate, the sodium salt of hyaluronic acid, is a high-molecular-weight polysaccharide and a major component of synovial fluid. This highly viscous substance has analgesic properties, lubricant effects, and anti-inflammatory actions; it causes cartilage formation and plays a role in the nutrition of avascular parts of the disc and condylar cartilage. We conclude that the intra-articular injection of sodium hyaluronate is effective for treating temporomandibular disorders.

CLOSED LOCK증례에 대한 악관절 세정술의 임상적 연구 (CLINICAL STUDY OF ARTHROCENTESIS-CASES OF CLOSED LOCK)

  • Purpose : To evaluate the efficacy of arthrocentesis for treatment of closed lock. Material : 42 patients were diagnosed to closed lock by physical, radiographic examriation and undergone arthrocentesis. All patients have a pain and mouth opening limitation on affected site. Method : Arthrocentesis was done under conscious sedation and local infiltration anesthesia, normal saline and some steroid was injected on upper compartment of tempormandibular joint. After pumping and lavage, manual reduction procedure of anterially displaced disc was done. All the patients wear an anterior repositioning splint just after arthrocentesis. The result of arthrocentesis was assessed by pain and difference of mouth opening. Result : Difference of mouth opening after arthrocentesis was improved to 18.85mm and pain was gradually decreased. All patients were worn stabilization type splint after mean 1.84 months. 6 patients had relapse of mouth opening limitation so done arthrocentesis again and delivered good results.

견관절의 각도에 따른 악력변화 (Changes of the Hand Grip Strength accodring to Shoulder joint Angle)

  • The shoulder joint permit ate greatest mobility of any joint area carries out the importment function of stabilization for hand use. So handgrip activity is important to evaluate while assessing shoulder load in manual work. There was an association between static handgrip and shoulder muscle activity. The purpose of this study was to find out the changes of the hand grip strength according to shouter an81e. One hundred (50 female, 50 male) college adult volunteers with no known shoulder dysfunction participated subject in three positions with elbow extension: (1) shoulder $0^{\circ}$ flexion (2) shoulder $90^{\circ}C$ flexion (3)shoulder $180^{\circ}C$ flexion. The paired t-test was used to determine the different in grip strength between right and left hand at shouter position change. All, there was significiant for all three position by right and left shoulder (p<0.001). In mon, the ANOVA results revealed not a significiant F-ratio fer all three position by right and left hand. In woman, revealed significiant (p<0.05).

  • Active and therapeutic sport recovery is becoming a popular and important component in approving performance for pro and recreational athletes alike. It is also helping in the realm of injury prevention. In the search of finding modalities that are widely effective, natural, and safe, acupuncture is a viable and cost-effective treatment for helping athletes achieve this goal. More direct related research is needed, but testimonials from pro athletes and the body of research that currently exists provides powerful evidence on acupunctures ability to help with enhancing recovery. Specializing in acupuncture and exercise science, Chris integrates acupuncture into musculoskeletal rehabilitation therapy or fitness training for pain modulation, speedy recovery, and enhanced performance. Clients can choose to focus on one-on-one corrective exercise therapy, manual and massage therapy, or acupuncture. However, for best results, Chris recommends all three. Effect of postural change on shoulder joint internal and external rotation range of motion in healthy adults in their 20s

  • Objective: We aimed to investigate differences of range of motion in measuring shoulder internal rotation (IR) and external rotation (ER) resulting from posture change in manual scapular stabilization in prone, hook-lying, sitting, and standing positions in healthy young adults. Design: Cross-sectional study. Methods: This study included healthy young adults who agreed to participate after a thorough explanation about the study purpose and methods. A clinometer was used to measure shoulder rotation. Measurements of shoulder rotation according to postural change were performed in prone, hook-lying, sitting, and standing positions. The repeated measures analysis of variance was used to compare between-group differences in postural change. Results: The lower the posture, the greater the average value of IR angle. In contrast, the higher the posture, the greater the average value of the external rotation angle. In active and passive IR with posture change, there was difference in average value but with no statistical significance. In active and passive ER, there was a statistically significant difference between prone and sitting, prone and standing, hook-lying and sitting, hook-lying and standing, and sitting and standing position (p<0.05). Conclusions: Our findings suggest that postural change should be considered in order to increase the strength or range of motion of the internal and external rotation of the patient's shoulder joint.

Isolated Spinal Accessory Nerve Palsy from Volleyball Injury

  • Spinal accessory nerve (SAN) palsy is typically a result of posterior triangle surgery and can present with partial or complete paralysis of the trapezius muscle and severe shoulder dysfunction. We share an atypical case of a patient who presented with SAN palsy following an injury sustained playing competitive volleyball. A 19-year-old right hand dominant competitive volleyball player presented with right shoulder weakness, dyskinesia, and pain. She injured the right shoulder during a volleyball game 2 years prior when diving routinely for a ball. On physical examination she had weakness of shoulder shrug and a pronounced shift of the scapula when abducting or forward flexing her shoulder greater than 90 degrees. Manual stabilization of the scapula eliminated this shift, so we performed scapulopexy to stabilize the inferior angle of the scapula. At 6 months postoperative, she had full active range of motion of the shoulder. SAN palsy can occur following what would seem to be a routine volleyball maneuver. This could be due to a combination of muscle hypertrophy from intensive volleyball training and stretch sustained while diving for a ball. Despite delayed presentation and complete atrophy of the trapezius, a satisfactory outcome was achieved with scapulopexy.

요통 환자를 대상으로 후중둔근 도수근력검사의 일치도 및 휴대용 근력계 측정 방법의 신뢰도 검사 (Agreement of Manual Muscle Testing and Test-Retest Reliability of Hand Held Dynamometer for the Posterior Gluteus Medius Muscle for Patients With Low Back Pain)

  • The purpose of this study was to assess the agreement of manual muscle testing (MMT) and test-retest reliability of a hand held dynamometer for the posterior gluteus medius muscle, with and without lumbar stabilization, using a pressure biofeedback unit for patients with low back pain. The pressure biofeedback unit was used to minimize the substitute motion of the lumbopelvic region during hip abduction in patients lying on their side. Fifteen patients with low back pain participated in this study. A tester determined the MMT grades of the posterior gluteus medius with and without the pressure biofeedback unit. Active hip abduction range of motion with an inclinometer and the strength of their posterior gluteus medius using a hand held dynamometer were measured with and without the pressure biofeedback unit in the MMT position. The agreement of the grade of muscle strength in the MMT, and intra-rater reliability of both the active hip abduction range of motion and the strength of posterior gluteus medius were analyzed using the weighted kappa and intraclass correlation coefficient (ICC), respectively. The agreement of MMT with the pressure biofeedback unit (weighted kappa=.92) was higher than the MMT (weighted kappa=.34)(p<.05). The inclinometer with pressure biofeedback unit measurement of the active hip abduction range of motion had an excellent intra-rater reliability (ICC=.90). Also, the hand held dynamometer with pressure biofeedback unit measure of strength of the posterior gluteus medius had a good intra-rater reliability (ICC=.85). Therefore, the test for muscle strength with pressure biofeedback unit will be a reliable method for the determination of the MMT grades or amount of posterior gluteus medius muscle strength and the measurement of the range of motion for hip abduction in patients with low back pain.

견관절부 근막동통증후군의 근육내 전기자극치료시 신장운동과 안정화운동의 효과 비교 (The Comparison of Effective of Stretching Exercise and Stabilizing Exercise for The Patients with Myofascial Pain Syndrome at The Shoulder Girdle)

  • Summary of Background Data: Myofascial pain syndrome (MPS) is a common painful muscle disorder caused by trigger points occurring in myofascial. MPS is a major cause of chronical pain and is the subject of further clinical examination. Purpose: To uncover effective intramuscular stimulation therapy (IMS), the patients' actively participation stretching exercise and stabilizing around shoulder girdle where trigger point provoking myofascial pain syndrome in usually occurred. Methods: 45 myofascial pain syndrome patients were randomly chosen and divided into 3 groups. The first group (G1) received only IMS therapy, The second group (G2) had both IMS and active stretching exercises administered and the final group (G3) was given IMS therapy and stabilizing exercises. Therapy intervention was given for 3 weeks, 3 times a week and then only stretching exercise for the second group and stabilizing exercise for the third group was given for another 3 weeks. The visual analogue scale was dine before the experiment, 3 weeks after the experiment and 6weeks after the experiment to measure subjective degrees of pain and pressure pain threshold to measure sensitivity improvement of trigger point and functional ability questionnaire to measure daily life performance. Results: There were no significant changes after 3 weeks but after 6 weeks, between GI and G2 and between G1 and G3 showed significant change of pain, pressure pain threshold and daily life performance. There were significant improvement of the measurement of degrees of pain, pressure pain threshold and improvement of daily life performance at different times for G1 showed change 3 weeks after the experiment, but there were no changes 6 weeks after the experiment. There were significant improvement of the measurement at different times for G2 and G3 showed change 3 weeks after the experiment and 6 weeks after the experiment. Conclusions: IMS therapy proved to be effective in inactivation of trigger points of the myofascial pain syndrome patients and stretching and stabilizing exercises beside it keep remedial value longer by improvement of pain and dysfunction that occurred by the trigger point.
