메이트란드 도수치료가 만성 요통환자의 즉각적인 동적 균형능력에 미치는 영향 (Effect of Immediate Dynamic Balance Ability of Chronic Low Back Pain on Maitland's Manual Therapy)

  • 만성 요통 환자들에 대한 다양한 치료적 중재 방법이 있다. 그러나 대부분의 치료들이 통증에 대한 치료들이다. 만성 요통 환자들은 요추관절들의 구조적 고착들을 가지고 있다, 이러한 구조적 고착들은 관절가동범위을 저하시키고 나아가 동적 균형능력의 저하를 발생시킨다. 동적 균형능력의 저하는 손상을 가중시키고 위험상황에서의 대처능력을 감소시킨다. 따라서 본 연구에서는 만성요통환자들에 대한 즉각적인 동적 균형능력의 회복을 위하여 메이트란드의 도수치료의 방법들을 이용하였다. 만성 요통 환자 64명을 대상으로 메이트란드의 도수치료를 적용한 실험군 32명, 이학적 도구(온열팩, 저주파치료기, 초음파)를 사용한 대조군 32명을 대상으로 하였다. 각 군의 치료 전, 후의 동적 균형능력을 비교하기위하여 신뢰도가 높은 스타 익스커션 밸런스 테스트(0.81-0.96)를 이용하였다. 실험군에서 치료 후 동적 균형능력이 전방, 오른쪽, 왼쪽, 후방모두에서 증가하였고 대조군에서는 변화가 없었다. 따라서 메이트란드의 도수치료가 만성요통환자들의 요추 관절을 유연하게 해서 즉각적인 동적 균형능력을 증가시키는데 영향을 주었다고 생각된다.

허리관절가동기법이 만성요통환자의 균형과 요통 기능장애 지수에 미치는 영향 (The Effect of Spine Mobilization Technique on Balance and the Low Back Pain Disability Index of Patients with Chronic Back Pain)

  • Purpose : The purpose of this study was to examine the effects of waist mobilization technique intervention for patients with chronic back pain on balance and the low back pain disability index. Methods : The subjects were 30 patients with chronic back pain. They were sampled and divided into a manual therapy group and a spinal decompression group. 15 subjects were randomly assigned to each group. Each training in this study participated for 6 weeks, 5 times a week, once a day, 15 minutes a day. Balance ability was measured with a balance analyzer, and the low back pain disability index was measured using the Oswestry Disability Index (ODI). Results : In the balance abilities, there was a significant difference in the manual therapy group compared to the spinal decompression group. In the low back pain disability index, there was a significant difference in the manual therapy group compared to the spinal decompression group. Conclusion : The analysis results of the effect of 6 weeks of waist mobilization technique intervention on the balance and low back pain disability index for patients with chronic back pain revealed that the manual therapy is more effective for static and dynamic balance ability and the low back pain disability index. In the future, we can promote independent life skills and expect a rapid recovery of patients with chronic back pain. Based on this study, further studies are needed on the effects of balance, the mechanical properties of muscle, and the low back pain disability index depending on various manual therapy techniques.

지지면 변화에 따른 교각운동이 복횡근 두께와 균형능력에 미치는 영향에 관한 연구 (The Effects of Transverse Abdominis Ratio and Balance Ability during Bridge Exercises on Different Support Surface)

  • Purpose: The purpose of this study was to investigate the effect of transverse abdominis ratio and balance ability during bridge exercises on different support surface. Methods: The subjects were 20 students at G University. Ultrasound equipment was used to measure the transverse abdominisratio and the computerized balance measurement equipment was used to measure the balance ability before and after bridge exercises on different support surface. Results: Transverse abdominis ratio change between the groups was a significant difference only in the unstable support surface training group (p<.05). Normal standing eye close balance ability change between the groups was significantly different only in the unstable support surface training group (p<.05). The changes in the transverse abdominis ratio after 7 days of exercise and 14 days after exercise were significantly different between the groups (p<.05). Normal standing eye close balance ability showed a significant difference at 7 days after exercise and 14 days after exercise (p<.05). Conclusions: It is more effective to balance ability and ratio of the transverse abdominis when bridge exercises on an unstable support surface than to perform bridge exercises on a stable support surface. Therefore, we propose an effective program for patients with instability of the lumbar in clinical practice. bridge exercises on different support surface.

  • Background: Instability due to ankle sprains will be accompanied by a problem of balance and pain change. Balance trainer is used to improve the ankle strength and balance ability. The purpose of this study was to evaluate the change of pain and postural balance ability in ankle joint after balance trainer application in patients with ankle sprain and instability. Methods: Twenty patients in K hospital in Incheon were enrolled. Balance trainer was applied to 10 subjects in the experimental group and 10 subjects in the Balance cushion under the same conditions as the experimental group to compare the pain and balance ability. Results: In the experimental group, there was a significant difference in the change of the pain variation. In the postural balance ability comparison, there was a significant difference in total and post - posterior comparison compared to the control, but there was no significant difference in the postural balance ability comparison. Conclusion: Pain and postural balance ability of patients with instability due to ankle sprain improved the pain and balance ability of the Balance trainer group compared to the Balance cushion training group.

지지면에 따른 과제 지향적 훈련이 발목 불안정성 환자의 통증과 기능 수준, 균형능력, 그리고 근 활성도 및 근 두께에 미치는 영향 (Effect of Task-Oriented Training According to the Support Surface on Pain, Function, Balance Ability, Muscle Activity and Muscle Thickness in Patients with Ankle Instability)

  • Background: Task-oriented training on an unstable support surface is an effective intervention for improving the ankle joint stability and muscle strength in patients with ankle instability. This study examined the effects of balance training on an unstable support surface in patients with ankle sprains with ankle instability. Methods: Forty-four patients with ankle sprains participated in this study. Screening tests were performed and assigned to an experimental group, who performed task-oriented training on an unstable support surface (n=22), and a control group, who performed task-oriented training on a support surface (n=22) using a randomization program. All interventions were applied 3 times per week for 4 weeks. The numeric rating scale (NRS), cumberland ankle instability tool (CAIT), balance ability, muscle activity, and muscle thickness were compared to evaluate the effects of the intervention. Results: Both groups showed significant differences in the NRS, CAIT, balance ability, and muscle activity between before and after the intervention (p<.05). In addition, there were significant differences in balance ability, muscle activity, and muscle thickness between the experimental and control groups (p<.05). Conclusion: Task-oriented training on an unstable support surface is an effective intervention for improving the balance ability, muscle activity, and muscle thickness during contraction.

천장관절에 적용한 움직임을 동반한 관절가동술이 뇌졸중 환자의 보행에 미치는 효과 (The Effect of Mobilization With Movements Applied Sacro-Iliac Joint on Gait of Stroke Patient)

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

허리 불안정성을 가진 성인에게 적용한 복합운동프로그램이 정적균형능력에 미치는 영향 (The Effect of the Combined Exercise Program on the Static Balance Ability of the Lumbar Instability of Adults)

  • Background: The purpose of this study is to investigate the effect of the combined exercise program on the static balance ability of the lumbar instability of adults. Methods: The experimental group was divided into two groups and randomly. The experimental group 1 (Exp 1) applied the combined exercise program for the lumbar stabilization exercise and strengthening exercises and the experimental group 2 (Exp 2) applied the combined exercises program for the lumbar stabilization exercises and active stretching exercises. Each experiment group performed exercise program for 4 weeks, 3 times a week, 30 minutes a day. Static balance ability was measured using GOOD BALANCE system. Results: As the comparing results of static balance ability, normal standing eye open was Ant-Post and Med-Lat showed significant differences in Exp 1 (p<.05). One leg left eye open was velocity moment showed significant differences in Exp 1 (p<.05). and one leg right eye open was Med-Lat showed significant differences in Exp 1 (p<.05). Conclusion: These results show that thoracic self mobilization is immediate effective on balance and gait ability. Thus, thoracic self mobilization will help recovery of balance and gait ability in acute stroke patients.

전방머리자세를 가진 초등학생에게 복합운동프로그램이 자세변화와 보행, 균형능력에 미치는 영향: 사례연구 (The Effects of Complex Exercise Program on Postural Change, Gait and Balance Ability in Elementary School Students with Forward Head Posture - Case Study)

  • Background: The purpose of this case study was to investigate into the effect of complex exercise program on the postural change, gait and balance ability in elementary school students with forward head posture. Methods: Four patients with forward head posture were recruited. They were evaluated pre-treatment, and after 6weeks, using neck disability index (NDI), numeric pain rating scale (NPRS), balance ability, foot pressure (fore foot/rear foot peak pressure ratio, F/R ratio), gait ability (cadence, toe out angle, stance phase). Results: First, the angle of forward head posture (craniovertebral angle; CVA and cranialrotation angle; CRA) was decreased in all subjects. The NPRS and NDI were decreased in all subjects. Also, The cadence, toe out angle and F/R ratio were increased in all subjects. The stance phase of gait cycle was positively change in all subjects. Lastly, the static balance ability improved in all subjects. Conclusion: According to the results above, the complex exercise program for students with forward head posture can help improve the postural change, gait and balance ability. Also, the complex exercise program was able to select interventions depending on the patient's condition and the desired goal.

무릎관절 전치환술 환자에게 적용한 무릎 안정화 운동이 균형과 보행능력에 미치는 영향 (Effect of Knee Stabilization Exercise on Balance and Walking Ability in Patients with Total Knee Replacement)

  • Background: Degenerative arthritis accounts for a large portion of the elderly, causing a lot of inconvenience in daily life. Total knee replacement (TKR) are performed to relieve pain in the knee joint. The purpose of this study was to determine whether knee joint stabilization exercises are effective in improving balance and walking ability in degenerative arthritis patients who have undergone TKR. Methods: A total of 30 TKR patients participated in this study. They were assigned to two groups. The experimental group, the group that underwent knee stabilization exercises, joint mobilization and general physical therapy. On the other hand, and the controlled group, the group that underwent joint mobilization and general physical therapy. All exercises were conducted thrice a week, for four weeks. The main balance outcomes were evaluated using the posture balance training system, while walking ability was assessed using a wireless 3-axis accelerometer. Results: The experimental group had significantly higher scores in postural stability testing (PST), limits stability testing, and waling ability. The post-intervention evaluation, there were significant differences in all variables between the two groups. However, no significant difference was noted in the foam eyes closed test in PST. Conclusion: This study suggests that knee joint stabilization exercises effectively improve the balance and walking ability of TKR patients. Thus, it could be presented as useful in clinical practice.

무릎 각도의 차이에 따른 불안정 지지면에서의 스쿼트훈련이 건강한 성인의 균형 능력에 미치는 영향 (The Effect of Squat Training on an Unstable Support Surface According to the Angle Different Knee Flexion Angles on Healthy Adult's Balance Ability)

  • Background: The aim of this study was to compare the effects of squat training on an unstable support surface with different knee flexion angles on the balance ability of normal adults balance ability. Methods: 41 university students in their 20s attending N University in Cheonan, Chungcheongnam-do were divided into a 45-degree knee-bending squat training group and a 90-degree knee-bending squat training group. The groups trained on an unstable support surface 20 minutes per day, 3 times a week, for a total of 4 weeks. Results: Changes in static balance ability were not significant within and between the groups for both the sway distance and sway area in the eyes open and eyes closed states (p<.05). The changes in dynamic balance ability were significant in the forward, leftward, and rightward angles in both groups at the limit of stability (p<.05), but not significant in the backward angle (p>.05), and the comparison between groups was not significant (p>.05). Conclusion: No significant difference between static balance-related variables within and between the groups was found. Significant changes in dynamic balance-related variables within the groups were found but not between the groups. Therefore, in future studies, it is considered necessary to study various ages and differentiated intervention periods, such as young adults and the population of elderly people, with sufficient intervention periods to affect balance ability.
