Acupuncture for Subacute and Chronic Post-thoracotomy Pain in Patients with Traumatic Multiple Rib Fractures: A Study Protocol for a Randomised-controlled, Two-arm, Parallel Design, Pilot Trial

  • Background: The aim of this study is to assess the feasibility of acupuncture treatment for the management of subacute and chronic post-thoracotomy pain in patients with traumatic multiple rib fractures. Methods: A total of 30 participants who have undergone thoracotomy after traumatic multiple rib fractures will be recruited. Participants will be invited and equally randomised into acupuncture plus usual care and usual care alone groups. A computer-generated random number sequence will be used and concealed using opaque, sealed, sequentially numbered envelopes. Twelve sessions of manual and electrical acupuncture performed by Korean medicine doctors will be provided over a span of 3 months to participants allocated to the acupuncture group. Participants in the usual care group will continue pain medication, exercise and physical therapy as required. Study feasibility will be measured based on the proportion of patients who complete the measurement of pain at 12 or 24 weeks after baseline. The clinical outcomes will include; the average pain intensity over the recent week at rest, movement and cough, quality of life, patient's global assessment of recovery, respiratory function measured by the pulmonary function test and use of pain medication at 4, 8, 12 and 24 weeks after enrolment. Adverse events will be recorded for all participants. Written informed consent will be obtained from all participants. The local ethics committee has approved the study. This pilot trial will inform further studies investigating the potential role of acupuncture for subacute and chronic post-thoracotomy pain in patients with traumatic multiple rib fractures.

Post Sender Recognition using SIFT (SIFT를 이용한 우편영상의 송신자 인식)

  • Previous post sender recognition study was focused on recognizing the address of receiver. Relatively, there was lack of study to recognize the information of sender's address. Post sender recognition study is necessary for the service and application using sender information such as returning. This paper did the experiment and suggested how to recognize post sender using SIFT. Although SIFT shows great recognition rate, SIFT had problems with time and mis-recognition. One is increased time to match keypoints in proportion as the number of registered model. The other is mis-recognition of many similar keypoints even though they are all different models due to the nature of post sender. To solve the problem, this paper suggested SIFT adding distance function and did the experiment to compare time and function. In addition, it is suggested how to register and classify models automatically without the manual process of registering models.

Method of extracting context from media data by using video sharing site

  • Recently, a lot of research that applies data acquired from devices such as cameras and RFIDs to context aware services is being performed in the field on Life-Log and the sensor network. A variety of analytical techniques has been proposed to recognize various information from the raw data because video and audio data include a larger volume of information than other sensor data. However, manually watching a huge amount of media data again has been necessary to create supervised data for the update of a class or the addition of a new class because these techniques generally use supervised learning. Therefore, the problem was that applications were able to use only recognition function based on fixed supervised data in most cases. Then, we proposed a method of acquiring supervised data from a video sharing site where users give comments on any video scene because those sites are remarkably popular and, therefore, many comments are generated. In the first step of this method, words with a high utility value are extracted by filtering the comment about the video. Second, the set of feature data in the time series is calculated by applying functions, which extract various feature data, to media data. Finally, our learning system calculates the correlation coefficient by using the above-mentioned two kinds of data, and the correlation coefficient is stored in the DB of the system. Various other applications contain a recognition function that is used to generate collective intelligence based on Web comments, by applying this correlation coefficient to new media data. In addition, flexible recognition that adjusts to a new object becomes possible by regularly acquiring and learning both media data and comments from a video sharing site while reducing work by manual operation. As a result, recognition of not only the name of the seen object but also indirect information, e.g. the impression or the action toward the object, was enabled.

Effect of Underwater Treadmill on the Elderly's Walking and Balance Ability (수중 트레드밀 운동이 노인의 보행 및 균형능력에 미치는 영향)

  • Background: This study had been carried out with 20 elderly subjects as its object for about one month from November 3, 2008 to December 14, 2008 in order to observe the effect of Underwater Treadmill on the elderly's walking and balance ability. Methods: Subjects were assigned either experimental group (n=10) or the control group (n=10), experimental group received Underwater Treadmill program (30 min per course, 3days a week for 6weeks). Subjects were assessed for muscle power (Nicolas Manual Muscle Test), balance (Functional Reach Test), gait ability (Time Up and Go, 10M walking test) before experiments and after experiments. Results: The results of this study were as follows; 1. After underwater treadmill exercise, the change of isometric contraction indicated a beneficial increase on lower extremity muscle power of experimental group and functional test of balance function; FRT, TUG indicated beneficial difference between groups. 2. beneficial difference between groups in walking speed of hourly index change of walking function. 3. between muscle power and balance, gait ability, we could find out there's high correlation ship between lower extremity muscle power increasing and balance and gait ability of the elderly. Conclusion: Aerobic exercise using underwater treadmill effects on muscle power strengthening of the elderly, and because of this, increase of lower extremity muscle power is very helpful not only to improvement of balance ability, but also to improvement of gait ability, so it will be used as a physical therapy program on clinic and used as an exercise program for protecting the elderly from falling down very well.

A Case Report on Posterior Circulation Cerebral Infarction with Early Neurological Deterioration in an Elderly Patient with Chronic Kidney Disease (고령의 만성콩팥병 환자에서 발생한 초기 신경학적 악화가 동반된 후방순환뇌경색 치험 1례)

  • Objectives: This study investigated the effect of Korean medicine on an elderly patient with posterior circulation cerebral infarction, chronic kidney disease (CKD), and early neurological deterioration (END). Methods: The patient, who already had CKD, was treated with Korean medicine, comprising herbal medicine, acupuncture, moxa, and cupping combined with Western medicine (antiplatelet, diabetes) and physical therapy. A manual muscle test (MMT) and a modified Barthel index (MBI) were used to observe the treatment effects, and blood tests were performed to check estimated glomerular filtration rate (eGFR), creatinine and blood urea nitrogen (BUN), which represent renal function. Results: After the treatment, MMT, MBI, and renal function scores had increased. Conclusions: This study suggests that Korean medicine can effectively treat posterior circulation cerebral infarction with END in CKD, but further studies should be conducted.

  • Goal of this study is to introduce newly developed Oral Motor Facilitation Technique(OMFT), to identify effect of oral motor therapy on oral praxis and oral function of Down syndrome child. OMFT is comprehensive oral motor therapy for improving sensory adaptation, oral sensori-motor function, oral motor coordination of oro-facial structure by therapist's direct manual stroking. Subject was 10years old down syndrome boy. Treatment was practiced 8 times, 15minutes per time, from May to July, 2020. Oral praxis, drooling, quality of chewing ability were tested before and after treatments. Every single items of Oral Praxis Test was increased. Severity and frequency of drooling were decreased. Quality of chewing ability is improved. Through this case study, we can find the positive effect of OMFT on oral praxis, drooling, chewing ability of Down syndrome child.

Robust Object Tracking based on Weight Control in Particle Swarm Optimization (파티클 스웜 최적화에서의 가중치 조절에 기반한 강인한 객체 추적 알고리즘)

  • This paper proposes an enhanced object tracking algorithm to compensate the lack of temporal information in existing particle swarm optimization based object trackers using the trajectory of the target object. The proposed scheme also enables the tracking and documentation of the location of an online updated set of distractions. Based on the trajectories information and the distraction set, a rule based approach with adaptive parameters is utilized for occlusion detection and determination of the target position. Compare to existing algorithms, the proposed approach provides more comprehensive use of available information and does not require manual adjustment of threshold values. Moreover, an effective weight adjustment function is proposed to alleviate the diversity loss and pre-mature convergence problem in particle swarm optimization. The proposed weight function ensures particles to search thoroughly in the frame before convergence to an optimum solution. In the existence of multiple objects with similar feature composition, this algorithm is tested to significantly reduce convergence to nearby distractions compared to the other existing swarm intelligence based object trackers.

Study on Earthquake Hazard Response Process by 'Pohang Earthquake' Case Analysis (포항지진사례 분석을 통한 지진재난 대응 프로세스에 관한 연구)

  • The 2017 Pohang earthquake left us with issues related to long-term repair and restoration from massive earthquake damage. The existing Earthquake response manual was insufficient to consider the flow of earthquake disaster work and the characteristics of long-lasting earthquake disaster. Accordingly, It is important to analyze and record how to earthquake response work was carried out during the Pohang earthquake. The functions that require the most work and manpower in the event of an earthquake disaster were emergency life stabilization support, facility emergency recovery, and energy functional restoration. As a result of analyzing the difficulties and problems of disaster response by function, it was found that the prevention and preparation for damage in advance was insufficient for each function. In conclusion, we subdivided the response step applied with the concept of time and presented the overall work flow process for thirteen collaboration functions. It is expected that this result will help disaster managers to work effectively in the event of a large scale earthquake.

Clinical Benefit of Computerized Cognitive Therapy in Patients with Brain Disease : A Systematic Review and Meta-Analysis (뇌질환 환자에서 전산화 인지치료의 임상적 유용성 : 체계적 고찰 및 메타분석)

  • Purpose : This study aims to confirm the clinical usefulness of computerized cognitive therapy program for patients with brain diseases in Korea and to present basic data that can confirm the effectiveness of computerized cognitive therapy program to experts related to rehabilitation in the future. Methods : A systematic review and meta-analysis research method was used to confirm the clinical effectiveness of computerized cognitive therapy applied to patients with brain diseases in Korea. Based on the national institute of health and medical colleges association (NECA) systematic literature manual and preferred reporting items for systematic reviews and meta-analysis (PRISMA) guidelines for conducting research through systematic review and meta-analysis methods. Systematic review was PICOST-SD was used to research and select papers. For meta-analysis, the data was input and analyzed separately by literature result using the revman manager 5.3 program and the results were presented visually through tables and forest, funnel plot figure. Results : As a result of comprehensive analysis of the contents of the finally selected literature, it as found that studies on stroke and dementia were mainly conducted. In addition, through the evaluation of the quality of the literature, most of the existing computerized cognitive therapy studies were identified as high-quality studies, but the results were insufficient for randomization and blinding. And through meta-analysis, the clinical effectiveness of computerized cognitive therapy on cognitive function, activities of daily living, and visual perception function of patients with brain disease was found. Conclusion : The research literature that applied computerized cognitive therapy to brain disease subjects was comprehensively analyzed and identified. It is thought that this could be used as basic data on the characteristics of cognitive therapy that should be applied to patients with brain diseases in the future and the usefulness of computerized cognitive therapy program in actual clinical practice.

Acupuncture for Symptomatic Rotator Cuff Disease: A Systematic Review and Meta-Analysis

  • The objective was to evaluate the effectiveness and safety of acupuncture for patients with rotator cuff diseases. There were 12 electronic databases and 3 trial registries searched up to November 30th, 2019. All randomized trials were eligible, regardless of language, date of publication, or settings. The primary outcomes were pain, shoulder function, and proportion of improved participants assessed within 12 weeks of randomization of the trial. The Cochrane risk of bias for the studies was assessed. Effects sizes were presented as a risk ratio, mean difference, or standardized mean difference with a 95% confidence intervals. Grading of Recommendations Assessment, Development and Evaluation approach was adopted to rate certainty of evidence. Of the 3,686 records screened, 28 randomized trials (2,216 participants) were included in this review. The types of acupuncture included manual acupuncture, dry needling, electroacupuncture, acupotomy, warm needle acupuncture, and fire needle acupuncture. All of the studies had an unclear or high risk of bias related to more than 1 domain. Significant benefits of acupuncture in terms of pain and shoulder function were observed in all comparisons, however, the proportion of improved participants was not described in 2 comparisons. There was substantial heterogeneity among meta-analyzed trials. No serious harm was observed. For primary outcomes, the overall certainty of evidence was very low. There was very low certainty of evidence for the benefits of acupuncture for patients with rotator cuff diseases. The safety of acupuncture remains unclear due to the incompleteness of reporting. Future well-designed randomized trials with transparent reporting are required.
