A torque-measuring micromotor provides operator independent measurements marking four different density areas in maxillae

  • PURPOSE. Bone density at implant placement site is a key factor to obtain the primary stability of the fixture, which, in turn, is a prognostic factor for osseointegration and long-term success of an implant supported rehabilitation. Recently, an implant motor with a bone density measurement probe has been introduced. The aim of the present study was to test the objectiveness of the bone densities registered by the implant motor regardless of the operator performing them. MATERIALS AND METHODS. A total of 3704 bone density measurements, performed by means of the implant motor, were registered by 39 operators at different implant sites during routine activity. Bone density measurements were grouped according to their distribution across the jaws. Specifically, four different areas were distinguished: a pre-antral (between teeth from first right maxillary premolar to first left maxillary premolar) and a sub-antral (more distally) zone in the maxilla, and an interforaminal (between and including teeth from first left mandibular premolar to first right mandibular premolar) and a retroforaminal (more distally) zone in the lower one. A statistical comparison was performed to check the inter-operators variability of the collected data. RESULTS. The device produced consistent and operator-independent bone density values at each tooth position, showing a reliable bone-density measurement. CONCLUSION. The implant motor demonstrated to be a helpful tool to properly plan implant placement and loading irrespective of the operator using it.

중증 치주염에 의해 발거된 치아의 수평부착상실에 대한 연구 (Horizontal attachment loss in extracted teeth due to severe periodontitis)

  • Purpose: The attachment level is strongly associated with tooth loss and provides useful information on patterns of destruction of the periodontium. The presence of horizontal attachment loss would not be detected in clinical measurement. Therefore, the purpose of the present study was to estimate the patterns of periodontal destruction based on the attachment area and horizontal attachment loss in extracted teeth due to severe periodontitis. Materials and Methods: 307 teeth satisfied the criteria for assessment. An indirect method, based on digital images obtained from a digital camera and an image analysis program, was used to calculate the area of root surface and attachment loss and the extent of horizontal attachment loss. The data were analysed using SPSS. Results: No statistically significant differences among root surfaces were observed in anterior teeth on the loss of attachment area. However, in posterior teeth statistically significant differences in palatal surfaces of maxillary and mandibular premolar and molar surfaces compared with buccal surfaces were observed. Horizontal attachment loss was observed in 21.5% of the teeth examined. Frequency of horizontal attachment loss was highest in the maxillary first premolar (34.8%), followed by the maxillary second premolar (27.3%) and maxillary canine (25%). The mean length of horizontal attachment loss was 1.5mm. Conclusion: More meticulous examination will be needed of the palatal surfaces of maxillary and mandibular premolar and molar teeth. The percentage of teeth with horizontal attachment loss greater than 2.1 mm was 5.2%. Considering the length of curette blades, about 5.2% of teeth were not properly debrided. Therefore, Additional supportive therapy such as local drug delivery has to be considered in treatment of the first maxillary, second premolar and canine due to the high prevalence of horizontal attachment loss.

소구치 치주인대의 탄성계수에 대한 유한요소 해석 (Finite Element Analysis for Elastic Modulus of the Periodontal Ligament in Premolar Regions)

  • In this study, two dimensional and three dimensional finite element models of lower first premolar were analyzed. The mandibular specimen including a premolar was obtained from a cadaver and scanned with micro-CT. Finite element method models were reconstructed from CT images at mid-sagittal plane of the tooth. Most studies have used a wide range of value(0.07${\~}$1000MPa) for elastic modulus of periodontal ligament. The elastic modulus of the periodontal ligament was analyzed by finite element method and compared with that of experiment model. This study indicated that the model without pulp was more suitable than that with pulp in two dimensional finite element analysis.

성장기 환자에서 하악의 차등적 소구치 발치 (MANDIBULAR DIFFERENTIAL PREMOLAR EXTRACTI0N IN GROWING PATIENTS)

  • 성장기 환자에서 치아의 배열이나 입술의 외형을 개선하거나 경미한 악골 부조화에 의한 부정교합을 절충(camouflage)하기 위하여 발치가 적용될 수 있는데, 하악 치열궁에 총생의 양이 미약하거나 전치부 분절을 과도하게 후방견인 시킬 필요가 없는 경우 하악 제 2소구치 발치를 계획할 수 있다. 그러나, 성장기 환자에서 이러한 제 2소구치 발치의 기준과 치료에 의한 상, 하악 치아의 이동양상에 대한 연구는 부족한 실정이다. 본 연구는 상, 하악 소구치 발치를 통한 포괄적 교정치료를 시행한 성장기 환자를 대상으로 상악 제 1소구치와 하악 제 1 소구치를 발치한 군(4/4발치군, 14명)과 상악 제 1 소구치, 하악 제 2 소구치를 발치한 군(4/5발치군, 15명)의 치료 전, 후의 측모두부규격방사선사진을 비교하여 하악 제 2소구치 발치의 적응증에 대한 진단기준을 평가하고, 안정골 구조물을 참고한 중첩법을 이용하여 치아이동에 어떠한 차이가 있는지를 평가하여 다음과 같은 결론을 얻었다. 1. 4/4발치군과 4/5발치군은 치료 전 계측치에서 교합평면에 대한 상악전치의 치축각도, 구치부 교합관계, IMPA 그리고 interincisal angle에서 차이를 나타내었다. 2. 하악 제 1 대구치의 전방 이동량은 4/5 발치군에서 더 전방이동 되는 경향을 나타내었다(p<0.05). 3. 치료 전, 후의 상악 치아의 이동양상은 4/4 발치군과 4/5 발치군 사이에 차이가 없었다(p<0.05).

Evaluation of the alignment efficiency of nickel-titanium and copper-nickel-titanium archwires in patients undergoing orthodontic treatment over a 12-week period: A single-center, randomized controlled clinical trial

  • Objective: The aim of this trial was to compare the alignment efficiency and intermaxillary arch dimension changes of nickel-titanium (NiTi) or copper-nickel-titanium (CuNiTi) round archwires with increasing diameters applied sequentially to the mandibular arch. Methods: The initial alignment phase of fixed orthodontic treatment with NiTi or CuNiTi round archwires was studied in a randomly allocated sample of 66 patients. The NiTi group comprised 26 women, 10 men, and the CuNiTi ($27^{\circ}C$) group comprised 20 women, 10 men. The eligibility criteria were as follows: anterior mandibular crowding of minimum 6 mm according to Little's Irregularity Index (LII), treatment requiring no extraction of premolars, 12 to 18 years of age, permanent dentition, skeletal and dental Class I malocclusion. The main outcome measure was the alignment of the mandibular anterior dentition; the secondary outcome measure was the change in mandibular dental arch dimensions during 12 weeks. Simple randomization (allocation ratio 1:1) was used in this single-blind study. LII and mandibular arch dimensions were measured on three-dimensional digital dental models at 2-week intervals. Results: No statistically significant difference was observed between NiTi and CuNiTi according to LII (p > 0.05). Intercanine and intermolar arch perimeters increased in the CuNiTi group (p < 0.001). Inter-first premolar width showed a statistically significant interaction in week ${\times}$ diameter ${\times}$ application (p < 0.05). Conclusions: The effects of NiTi and CuNiTi round archwires were similar in terms of their alignment efficiency. However, the intercanine and intermolar arch perimeters, and the inter-first premolar width changes differed between groups.

한국인 아동에 있어서 완골과 치령에 대한 비교연구 (INTERRELATION IN THE CARPAL BONE INDEX AND ORTHOPANTOMOGRAPHIC DENTAL AGES ON THE KOREAN CHILDREN)

  • The authors have taken the x-ray films of carpal bone and orthopantomographs in Korean children to research the degree of ossification of carpal bone, that of calcification of tooth in jaw bone, the eruption rate of tooth, and the completion rate of apical foramen, and have compared the bone age and tooth age. We have gained a series of intersting conclusions to dare to report. 1. The ossification of carpal bone and the growth of tooth had processed proportionally to each other. 2. Both the bone age and tooth age were earlier in the female than in the male. 3. The completion of tooth crown formation and that of tooth root formation had processed proportionally to each other, and the general tendency of process was the earliest in the mandibular first molar and the latest in the second molar. 4. The eruption of tooth was the earliest in the mandibular first molar. The other teeth had erupted in the following orders: the central incisor, the lateral incisor, the first premolar, the second premolar, the canine and the second molar.

하학 제 1 소구치의 3 차원 CT 영상 분할 및 정합 연구 (A Study on 3D CT Image Segmentation and Registration of Mandibular First Premolar)

  • The aim of the 3D medical imaging is to facilitate the creation of clinically usable image-based algorithm. Clinically usable imaging algorithm for image analysis requires a high degree of interaction to verify and correct results from registration algorithms, such as the Insight Toolkit (ITK) and the Visualization Toolkit (VTK) which are the class libraries. ITK provides segmentation algorithms and VTK has powerful 3D visualization. However, to apply those libraries to the medical images such as Computerized Tomography (CT), the algorithm based on the interactive construction and modification of data objects are necessary. In this paper we showed the 3D registration about mandibular premolar of human teeth acquired by micro-CT scanner. Also, we used the ITK to find the contour of pulp layer of premolar, furthermore, the 3D imaging was visualized with VTK designed to create one kind of view on the data of 3D visualization. Finally, we evaluated that the volume model of pulp layer would be useful for the tooth morphology in dental medicine.

A Retrospective Clinical Investigation of Delayed Eruption of Premolars in the Mandible

  • This study aimed to investigate the treatment options for the delayed eruption of mandibular premolars and identify the predictors of spontaneous eruption using panoramic radiography. The prevalence of delayed mandibular premolar eruption in this retrospective analysis, comprising 254 patients (aged 9 - 15 years), was 5.19%, with no significant difference based on gender. The mandibular second premolars were most affected (4.39%) compared to the first premolars (0.76%). No significant difference in prevalence was observed between the left and right sides. Among the treated mandibular premolars, primary molar-related lesions were identified as the leading cause (7.85%) of delayed tooth eruption. The treatment duration varied based on the Nolla stage, eruption stage, and treatment method. Teeth with Nolla stage 7 or lower had a treatment duration of 22.89 ± 11.96 months, whereas those with stage 8 or higher had a 15.02 ± 6.34 month duration. The deeper the tooth was located in the bone, the longer the treatment period became. The treatment duration varied depending on the treatment method, and statistically, there was no significant difference. The treatment durations for affected mandibular premolars increased with the depth of impaction angle of inclination. In this study, the treatment duration for delayed eruptions varied depending on the Nolla stage, eruption stage, and treatment method. Variations in the impaction depth and inclination angle across various treatment approaches, as explored in this study, might offer valuable insights into the selection of the most suitable management options for delayed tooth eruptions.
