Comparision of Osseointegration Depending on Surface Treatment (임플란트 표면처리에 따른 골융합의 차이)

  • The present study was performed to evaluate histomorphological difference in various surface-treated implants in beagle. Implants($Implantium^{(R)}$, Dentium Co. Korea) with pure titanium machined surface, acid treated surface, and $Al_2O_3(50{\sim}100\;{\mu}m)$blasted with acid treated surface were used in this study. All mandibular premolars of $1.5{\sim}2$ year old male beagle dogs were extracted. At 3 months after extraction, the implants(${\Phi}$ 4mm, l 6mm) were installed. The beagle were sacrificed at 1, 3 months after installation and then tissues including implants were prepared for non-decalcified specimens. These specimens were analyzed comparatively under light microscope. The results of this study were as follow 1. Higher rate of osseointegration were showed in the $Al_2O_3(50{\sim}100\;{\mu}m)$blasted with acid-treated surface. 2. Increased osseointegration were showed in the $Al_2O_3(50{\sim}100\;{\mu}m)$blasted with acidtreated surface with time. 3. Higher maturation of integration were showed in the $Al_2O_3(50{\sim}100\;{\mu}m)$blasted with acid-treated surface. In conclusion, surface treatment of $Al_2O_3blasted$ with acid might be considered to shorten healing time and improve success rate as increasing contact of implant and bone.

Three-dimensional finite element analysis of the stress distribution and displacement in different fixation methods of bilateral sagittal split ramus osteotomy

  • Objectives: This study evaluated a range of fixation methods to determine which is best for the postoperative stabilization of a mandibular osteotomy using three-dimensional finite element analysis of the stress distribution on the plate, screw and surrounding bone and displacement of the lower incisors. Materials and Methods: The model was generated using the synthetic skull scan data, and the surface model was changed to a solid model using software. Bilateral sagittal split ramus osteotomy was performed using the program, and 8 different types of fixation methods were evaluated. A vertical load of 10 N was applied to the occlusal surface of the first molar. Results: In the case of bicortical screws, von-Mises stress on the screws and screw hole and deflection of the lower central incisor were minimal in type 2 (inverted L pattern with 3 bicortical repositioning screws). In the case of plates, von-Mises stress was minimal in type 8 (fixation 5 mm above the inferior border of the mandible with 1 metal plate and 4 monocortical screws), and deflection of the lower central incisor was minimal in types 6 (fixation 5 mm below the superior border of the mandible with 1 metal plate and 4 monocortical screws) and 7 (fixation 12 mm below the superior border of the mandible with 1 metal plate and 4 monocortical screws). Conclusion: Types 2 and 6 fixation methods provide better stability than the others.

Leukocyte- and platelet-rich fibrin as an adjuvant to the surgical approach for osteoradionecrosis: a case report

  • We present a case of osteoradionecrosis treated with leukocyte- and platelet-rich fibrin (LPRF) and surgery and followed up with clinical and tomographic investigations. A 65-year-old woman presented with pain in the posterior region of the right palate. Her medical history included cardiovascular disease and squamous cell carcinoma in the anterior region of the floor of the mouth that had been treated with intensity-modulated radiation therapy. Measurements of isodose curves showed a full dosage of 6,462.6 cGy in the anterior mandibular region, whereas that in the posterior region on the right side of the maxilla reached 5,708.1 cGy. Osteotomy was performed using rotary instruments, and debridement and placement of two LPRF membranes were also carried out. New gum tissue with no bone exposure was noted 14 days postoperatively. Tissue repair was complete, and the patient had no further complaints. During a 39-month follow-up period, the oral mucosa remained intact, and the patient was rehabilitated with a new upper denture. Since there is no consensus regarding the best protocol to treat osteoradionecrosis, LPRF might be an interesting adjuvant to a surgical approach. The use of LPRF is simple and reduces operational costs, time of handling, probability of technical failure, and associated morbidities for patients with osteoradionecrosis.

EFFECT OF A MOUTHGUARD ON STRESS DISTRIBUTION IN TEETH, MAXILLA AND MANDIBLE FOR MAXILLA AND MANDIBULAR IMPACT USING FINITE ELEMENT ANALYSIS (악안면부 충격시 치아와 악골의 응력 분포에 대한 구강보호장치의 역할에 관한 유한요소분석 - 상악 중절치에 가해진 충격에 대하여)

  • Statement of problem : The use of mouthguard is important as the leisure life is popular today. Purpose: The purpose of this study is to investigate the effect of a mouthguard on stress distribution in teeth, maxilla and mandible for maxilla impact. Material and methods: The 3-dimensional finite element model was based on a CT scan film of an average korean adult when the subject is using a customized mouthguard which was made with the Signature Mouthguard system of Dreve. The load was applied to the upper central incisor cervical area parellel impact force for 0.1sec(L1). The Von-mises stress analysis with a mouthguard and without a mouthguard was compared. Results: The results of this study were as follows: 1. Without the mouthguard, stress was concentrated on teeth and alveolar bone in all load conditions. 2. With the mouthguard. maximum stress value was decreased and stress was dispersed in all load conditions. 3. Stress extinction with the mouthguard was faster than without the mouthguard in all load conditions. Conclusion: We acknowledged that the mouthguard has a stress buffer effect as the maximum stress value was decreased and stress was dispersed when impact force was applied.

Mandibular implant overdenture using milled bar attachment on implants with unfavorable fixture angulation: A case report (불량한 식립 방향을 갖는 임플란트를 이용한 하악 Milled Bar 임플란트 피개의치수복 증례)

  • Implant overdenture exhibits higher retention, masticatory function, and patient's satisfaction compared to conventional dentures. Particularly, in treatment of severe alveolar bone resorption, implant overdenture can be considered as the first treatment option. The types of attachments used for implant overdenture can be classified into solitary type, which implants are not connected to each other, and bar type in which implants are connected. In the case of solitary type commonly used in clinical practice, parallel relationship is important. When it is not established, there is a higher risk of attachment wear, retention loss, and frequent visits for maintenance. In this case, satisfactory results have been obtained with implant overdentures using milled bar on two unparallel implants placed on the mandible.

LANGERHANS CELL HISTIOCYTOSIS IN MANDIBLE : CASE REPORT (하악골에 발생한 Langerhans Cell Histiocytosis 환아의 증례보고)

  • Langerhans Cell Histiocytosis (LCH) is characterized by proliferation of Langerhans cells. The clinical manifestation varies from solitary bone lesion to multi-system, life threatening disorder. The younger the patient is and the more system is involved, the worse the prognosis is. The jaw is involved $10\sim20$ percent of all LCH and it is involved usually in early stage of LCH. In this case the patient is three years old girl who suffered from pain of whole mandibular body and histological examination confirmed the diagnosis LCH. She is referred to pediatrics and managed with combined chemotherapy. Due to the possibility of recurrence, we follow up the girl and she need orthodontic and prosthodontic treatment in the future because of the loss of lower left 2nd premolar. We report this case because early recognized LCH in dental hospital result in good prognosis.

A PHOTOELASTIC STUDY ON THE STRESS ANALYSIS UNDER MADIBULAR DISTAL-EXTENSION REMOVABLE PARTIAL DENTURE WITH DIFFERENT DESIGN OF THE MAJOR CONNECTOR (주 연결장치의 설계변화에 따른 하악 유리단 국소의치의 광탄성 응력 분석에 관한 연구)

  • The purpose of this study was to analyze the magnitude and distribution of stress using a photoelastic model from a distal extension removable partial dentures with three kinds of mandibular major connectors, that is, lingual bar, linguoplate, and swing-lock attachment. A photoelastic model was made of the epoxy resin(PC-1) and hardener(PCH-1) and coated with plastic cement-1 (PC-1) at the lingual surface of the epoxy model and set with three kinds of chrome-cobalt removable partial dentures. A bilateral vertical load of 15kg to the middle portion of the metal bar crossing both the first molars of the right and the left, and a unilateral vertical load of 12.5kg to the right first molar were applied with the use of specially designed loading device and the reflective circular polariscope was used to analyze the photoelastic model under each condition. The following results were obtained : 1. When the bilateral vertical load was applied, the magnitude and distribution of the stress concentration of the edentulous area and the terminal abutment or adjacent teeth was in the order of lingual bar, linguoplate, swing-lock attachment. 2. When the unilateral vertical load was applied, the magnitude and distribution of the stress concentration of the edentulous area and the terminal abutment or adjacent teeth was in the order of lingual bar, linguoplate, swing-lock attachment. 3. When the unilateral vertical load was applied, the magnitude and distribution of the stress concentration of the termial abutment or adjacent teeth on the non-loaded side showed the least stress distribution in case of swing-lock attachment. 4. When the bilateral vertical load and the unilateral vertical load were applied the swing-lock attachment showed the mildest uniform stress distribution on the edentulous area and the alveolar bone around the abutment teeth.

Association between dental implants in the posterior region and traumatic occlusion in the adjacent premolars: a long-term follow-up clinical and radiographic analysis

  • Purpose: The aim of this retrospective study was to determine the association between dental implants in the posterior region and traumatic occlusion in the adjacent premolars, using data collected during from 2002 to 2015. Methods: Traumatic occlusion in the adjacent premolars was assessed by examining clinical parameters (bleeding on probing, probing pocket depth, fremitus, and tooth mobility) and radiographic parameters (loss of supporting bone and widening of the periodontal ligament space) over a mean follow-up of 5 years. Clinical factors (gender, age, implant type, maxillary or mandibular position, opposing teeth, and duration of functional loading) were evaluated statistically in order to characterize the relationship between implants in the posterior region and traumatic occlusion in the adjacent premolars. Results: The study inclusion criteria were met by 283 patients, who had received 347 implants in the posterior region. The incidence of traumatic occlusion in the adjacent premolars was significantly higher for splinted implants (P=0.004), implants in the maxillary region (P<0.001), and when implants were present in the opposing teeth (P<0.001). The other clinical factors of gender, age, and duration of functional loading were not significantly associated with traumatic occlusion. Conclusions: This study found that the risk of traumatic occlusion in the adjacent premolars increased when splinted implants were placed in the maxillary molar region and when the teeth opposing an implant also contained implants.

TREATMENT OF CLASS Ⅲ MALOCCLUSION WITH HORSESHOE APPLIANCE : CASE REPORT (Horseshoe Appliance를 이용한 Ⅲ급 부정교합의 치험례)

  • In mixed dentition there exists many empty spaces in the arch due to eruption of permanent teeth and exfoliation of primary teeth. The empty spaces makes it difficult to apply fixed orthodontic appliances. Horseshoe Appliance can be used effectively at this stage, holding the whole dentition in one piece. It covers every surface of erupted teeth and prevents extrusion and rotation of single tooth. By using intermaxillary elastic force, remodeling of the alveolar bone is opposite in each arch. In patients who were treated with horseshoe appliance, forward growth of maxilla, labioversion of maxillary incisors and linguoversion of mandibular incisors were obtained. Minimum downward and clockwise rotation of mandible was shown, so increasing anterior facial height was minimized.

A Case Report of Maxillary Complete Edentulous Patient with a Class III Jaw Relations (III급 악골 관계를 가진 상악 편악 무치악 환자의 수복)

  • In a case of class III skeletal patients with severe alveolar bone resorption, it must be a complete denture treatment plan provided stable and durable occlusion. Despite a markedly increased tooth mobility and unfavorable crown-to-root ration due to periodontal tissue breakdown, if the inflammatory process is controlled and an adequate oral hygiene performed, fixed splints will be considered. Patient's discomfort will be eliminated by achievement better clinical tooth mobility using fixed splints. So it can be a cost and time effective treatment option. In this case, it used a T-Scan System for confirmation a bilateral balanced occlusion and a occlusal force distribution reflected a patient's functional mandibular movement.
