Bone Density Relationship of Mandible and Cervical Vertebrae in Panoramic Radiography

  • Purpose: Upper cervical vertebrae are commonly imaged together with the jaw bones in panoramic radiography. There have been many studies investigating the possible role of mandible as an indicator of osteoporosis. But the result doesn't show unanimity. This study measured bone densities of mandible and second and third cervical vertebrae to find out any relationship between these two areas. These results may contribute in panorama being used as a screening method in detecting possible osteoporotic patient. Materials and Methods: Randomly selected 226 digitized panoramic images with cervical vertebrae shadows from 156 dental patients between 5 to 80 years of age were used. And the bone densities of second and third cervical vertebrae, apical areas of first and second mandibular molars and interdental areas were measured. The bone density measurements were restricted to the cancellous bone and the average and standard deviations and paired t-tests were done to each measurements. Results: All the measurements were statistically significantly related. The best relationship was found between the third cervical vertebrae and first and second mandibular apical areas. The average and standard deviations of the measured bone density ratios of these areas were 1.20±0.45 and 1.34±0.48 each. Conclusion: Patients whose panoramic bone density of the third cervical vertebrae are much below those of mandibular first or second molar apical areas may have osteoporosis.

  • Osteoma is a slow growing benign osteogenic lesion characterized by proliferation of compact or cancellous bone. Osteomas may be classified as peripheral, central, or extraskeletal. Central osteoma arise from the endosteum, pheripheral osteoma from the periosteum, and extraskeletal soft tissue osteoma within a muscle. Peripheral osteoma of the mandible is uncommon. They manifest as asymptomatic, fixed tumors of bony-hard consistency that may be sessile or pedunculated. Radiographically, a well circumscribed round or oval radiopaque mass is seen. Here, we report a case of a huge solitary peripheral osteoma of the buccal posterior mandible in a 40-year-old woman who was otherwise in good health. The patient visited at the dental clinic because the lesion makes esthetic problem. Treatment was performed by surgical esthetic recontouring and histological examination. The patient remains free of recurrence after surgical esthetic recontouring operation.

  • The population were increased by industrialization and urbanization of the modern society and social activities of the person were rapid increased too. Subsequently the number of motor vehicle accident, sports accident and industrial accident were increased, resulting in the number of oral and maxillofacial trauma were increased. Because of the mandible relatively protruded among the facial bone, the most frequent associated oral and maxillofacial injuries was mandible fracture in the trauma center setting.   • This study was undertaken to evaluate the cephalometric changes of the soft tissue and skeletal profile subsequent 10 the rapid palatal expansion in 25 Angle's Class III cases, ranging in age from six to fifteen years, with cross-bite of the anterior teeth, underdevelopment of maxilla and facial disharmony Following results were obtained: 1. ANS moved downward, Point A presented forward & downward movement increasing SNA and Point B presented backward & downward movement decreasing SNB. 2. Mandible was rotated to backward & forward and maxilla moved forward & downward with the bite opening and improvement of anterior teeth cross-bite. 3. Soft tissue on mandible was rotated to backward & forward following hard tissue changes causing the decrease of facial convexity angle and backward & downward rotation of Point B', Pog'. 4. Response of the upper lip was more significant in downward than forward direction, and correlated with the upper central incisor and mandible rotation. 5. Response of the lower lip was more significant in downward than backward direction, and correlated with the mandible rotation. 6. There was a rather high degree of correlation between skeletal profile and soft-tissue profile, 1 : LS, $\bar{1}$:Pog', Pog:LS, Pog:LI, Pog:Pog' in horizontal measurements and $\bar{1}$:Pog', Pog:LI, Pog:Pog' in vertical measurements.

  • To corroborate that the width of attached gingiva should be changed according to ages, and what relationships between the changes and the results of Glickman's clinical tension test would be, The author measured the width of attached gingiva of 85 Korean children in male, 94 Korean children in female from 8 to 11 ages and performed clinical tension test. The results were as followings; 1) At midline region of each evaluated teeth, Width of attached gingiva was the narrowest at midline region of deciduous canine, and nearly same at midline region of central incisor and lateral incisor. 2) At interproximal region of each evaluated teeth, Width of attached gingiva between left and right central incisors was the narrowest, that of between deciduous canine and lateral incisor, and between lateral incisor and cental incisor were the widest at maxilla and All were nearly same at mandible. 3) In general, width of attached gingiva of interproximal region was wider than that of midline region. 4) In this study, width of attached gingiva tended to be increasing according to ages both at maxilla and at mandible. 5) Compared maxilla with mandible, Width of attached gingiva of maxilla was wider than that of mandible. 6) The results of tension test were it that Over-all incidence was the highest in 8 year old children who had the narrowest width of attached gingiva at frenum attached region and tended to be decreasing according to ages from 8 to 11 years.

Treatment of a Huge Odontogenic Myxoma in the Mandible with Surgical Resection and Reconstruction Using a Vascularized Fibular Free Flap: Case Report (하악골에 발생한 거대한 치성 점액종의 절제 및 비골 혈관화 유리 피판을 이용한 재건: 증례보고)

  • The odontogenic myxoma is a relatively rare, benign tumor that occurs on the jaw. A 41 year-old man visited Seoul National University Dental Hospital because of swelling of the mandible. Clinical and radiographic evaluation showed a huge mass invading most of the mandible. After biopsy, he was diagnosed with odontogenic myxoma. For resection of the lesion, partial mandibulectomy and reconstruction with a vascularized fibular free flap was done. The result showed successful removal of the lesion. Reconstruction resulted in satisfactory functional and esthetic outcomes. We conclude that huge benign neoplasms such as odontogenic myxomas can be successfully treated by using a wide margin of resection followed by vascularized fibular free flap reconstruction.

  • Odontogenic myxoma is one of rare tumors in oral and maxillofacial region and it is thought to be mesenchymal or ectomesenchymal origin. Its characteristics are benign and non-metastatic but it has the potential of local invasion and high recurrence rate. It originally occurs in atrium of heart and in central case, my xoma is located mainly in the maxilla and mandible. Most odontogenic myxoma develops in 2nd or 3rd decades of life and rarely occurs in child or older persons over fifty. The distribution of reported cases between the sexes is similar and the maxilla and mandible are equally affected or slightly higher in mandible. Clinically it is usually asymptomatic, however it can cause pain and paresthesia is complained in the advanced stages. Displacement and mobility of teeth have also been reported. Odontogenic myxoma is not a frequent tumor, but in case of slow and painless growing tumor it must be considered as a differential diagnosis.

  • Homogenization technique is employed to investigate the series of stress analyses of mandible for three different types of dental implants. This technique helps to make proper material model of bone and analyze such a non homogeneous structure at the level of individual microstructural unit. The stress analyses with homogenization technique show much higher stress level in the sponge bone, compared to those of conventional FEM. It also manifested that even a minor lateral force results in crucial stresses in the dental implant system and that the macroscale model should take the shape and size after real mandible to produce reasonable solution in the analyses of dental implant systems. The shapes of dental implants simulated in this study are rectangular-cross-sectioned type, hemi-sphere rooted type, and wedge type implant. The stress states of mandible with hemisphere rooted type implant and wedge type implant show similar levels, while those with sectioned rectangular implant results in higher stresses. It is suggested that the distance between the implant tip and cortical bone be kept far enough to prevent stress concentrations in the mandible.

  • Occlusal splints have been widely used in dentistry for treatment and diagnosis of signs and symptoms related to dental occlusion. The autor studyed the effect of occlusal splint on masticatory movement of young adult men (mean age 23.8 yrs.). Who had worn. occlusal splint for 1 week. The result were as follows: 1. Lateral movement of the mandible during mastication could be increased after wearing occlusal splint for 1 week. 2. Vertical movement of the mandible during mastication was increased after wearing occlusal splint for 1 week. 3. Maximum opening velocity of the mandible during mastication was increased after wearing occlusal splint for 1 week. 4. Mandibular movement during mastication was more affected by chewing of hard foods than of soft foods following occlusal changes. 5. Steepness of the anterior guidance affected several aspects of masticatory movement. 6. Masticatory movement of the mandible became more regular after wearing occlusal splint for 1 week.

  • The ameloblastic carcinoma is an extremely rare, aggressive odontogenic neoplasm of the jaws. It is described as an ameloblastoma in which there is histologic evidence of malignancy in the primary or recurrent tumors, regardless of whether it has metastasized. We report an aggressive case of ameloblastic carcinoma of the mandible. A 68-year-old man with the complaint of the left facial swelling and intermittent pain was referred to our hospital. Serial images of panoramic radiograph, computed tomograph, and magnetic resonance imaging showed an ill-defined destructive radiolucent lesion of the left mandible. The lesion had typically aggressive behavior with extensive local destruction of bone and extended to the adjacent soft tissues. Bone scan revealed increased uptakes in the left mandibular body and ramus regions. Histological features were generally resembled with those of an ameloblastoma but with cytologic features of epithelial malignancy.
