  • 최근 몇 년간 방사선 의학진단과 관련된 연구가 한층 높아진 가운데 유방암은 여성의 암 중에서 1위를 차지하고 조기에 진단하고 치료하기 위한 국가적인 노력이 필요한 시점이다. 이렇듯 여성들의 유방암 발생빈도수가 급증하면서 대두 되고 있는 것이 조기 진단방법인 Mammography와 초음파 진단이며 그로인하여 발생하는 오진률 역시 많은 연구가 진행 되고 있다. 먼저 Mammography 및 초음파 진단의 문제점 보면 첫째 촬영과정에서의 오차, 둘째 영상의 선명도 ,셋째 전문의의 판독에 대한오차, 넷째 의사의 경험으로 진단함으로 표준화가 존재하지 않는다는 공통적인 문제점을 가지고 있다. 본 연구에서는 CAD 시스템의 프레임웍 및 요소 기술을 제시하여 의사의 진단을 보조적 수행이 보다 수월하도록 하고자 한다. 본 연구에서는 CAD시스템의 기능은 Detection기능(Image enhancement, Morphology, segment detection)과 Diagnosis기능( Neural Natwork등을 이용하여 증상을 판단)이다. 또한 과거 자료를 이용한 변이 및 변화를 예측함으로써 향후 있을 위험요소에 대비가 가능한 모듈과 전문의사가 대화형으로 빠르게 진단지식을 구축할 수 있는 지능형, 대화형 온라인 진단기능을 추가함으로써 외국의 CAD시스템과는 많은 차이가 있다고 볼 수 있다.

  • 최근 국내 여성의 유방암 발생율은 1위를 차지하며 그 비율 또한 나날이 늘어가고 있는 추세이다. 하지만 유방암은 다른 암에 비해 5년간 관찰 생존율이 약 76%로 갑상선에 이어 두 번째의 생존율을 보이며, 이는 조기발견의 중요성을 다시 한번 상기시키게 한다. 하지만 국내에서 사용되는 유방암 조기검진 방법에는 Mammography(유방촬영술)와 초음파 진단 두 가지가 주를 이루고 있으나 촬영과정 및 장비에 따른 오차로 인한 객관화된 정보생성 부족 및 전달의 부족으로 문제점이 대두되고 있다. 본 연구에서는 Mamography 및 초음파 유방 진단술을 이용하여 전문의의 의사결정에 도움을 줄 수 있는 CAD(Computer Aided Diagnosis) 시스템의 유방암 진단의 특징을 이용, 전문의 관점의 모델링을 기술해보고자 한다.

  • It is very important to evaluate how the functions of products, facilities, and spaces affect human life. The evaluation of these actions has legal regulations such as certification, inspection, and diagnosis, and the degree of quality, safety, and excellence of the results is announced. This study sought to secure safety through efficient system operation by reviewing safety-related certification systems established and implemented by each government department in Korea and analyzing the characteristics of each system and similarities between systems. There was also an aspect of cross-checking safety through the certification and diagnosis system implemented by each ministry, but it was confirmed that the efficiency of the system should also be considered in terms of overlapping application. Therefore, when securing safety is confirmed based on each safety-related law, active alternatives such as exemption, substitution, delegation, etc. should be presented.

  • With regard to the indolent clinical characteristics of prostate cancer (PCa), the more selective detection of clinically significant PCa (CSC) has been emphasized in its diagnosis and management. Magnetic resonance imaging (MRI) has advanced technically, and recent international cooperation has provided a standardized imaging and reporting system for prostate MRI. Accordingly, prostate MRI has recently been investigated and utilized as a triage tool before biopsy to guide tissue sampling to increase the detection rate of CSC beyond the staging tool for patients in whom PCa was already confirmed on conventional systematic biopsy. Radiologists must understand the current paradigm shift for better PCa diagnosis and management. This article reviewed the recent literature, demonstrating the diagnostic value of pre-biopsy prostate MRI with targeted biopsy and discussed unsolved issues regarding the paradigm shift in the diagnosis of PCa.

  • An internet and network based software and related interface have been developed, which can remotely control and manage an on-site operating system. Developed software modules were composed of two parts: monitoring/management modules and control/diagnosis modules were developed for the network status, warehouse, production and selling status. Modules of control with diagnosis were developed for the on-site operating system and interface. Each module was integrated and the whole modules have been tested with an automatic mushroom grading/sorting system which was built in a laboratory. Developed software modules worked successfully without any uncommon situations such as system down caused by the software or data transfer error. Each software module was developed independently in order to apply easily to other existing on-site systems such as rice processing centers, fruit and vegetable sorting, packaging and distribution centers scattered over the country.

Structural Dashboard Design for Monitoring Job Performance of Internet Web Security Diagnosis Team: An Empirical Study of an IT Security Service Provider

  • Company A's core competency is IT internet security services. The Web diagnosis team analyzes the vulnerability of customer's internet web servers and provides remedy reports. Traditionally, Company A management has utilized a simple table format report for resource planning. But these reports do not notify the timing of human resource commitment. So, upper management asked its team leader to organize a task team and design a visual dashboard for decision making with the help of outside professional. The Task team selected the web security diagnosis practice process as a pilot and designed a dashboard for performance evaluation. A structural design process was implemented during the heuristic working process. Some KPI (key performance indicators) for checking the productivity of internet web security vulnerability reporting are recommended with the calculation logics. This paper will contribute for security service management to plan and address KPI design policy, target process selection, and KPI calculation logics with actual sample data.

Customer Loyalty to Health Services According to Hospital Type (병원 규모별 의료소비자의 고객충성도 형성요인)

  • Objectives : This research used an exploratory approach to identify factors affecting business strategies due to changes in the healthcare market and customer loyalty factors. Methods : The research model was formulated using antecedents divided into diagnosis quality, employee attitudes, and servicescape. Moreover, differences in the structured model were analyzed according to hospital size. The data were gathered through surveys on clients, who has received care at participating hospitals. From the 200 that were distributed, 150 questionnaires were analyzed, to facilitate analysis of the research model. Results : The effects of diagnosis quality, employee attitudes, and servicescape, on customer loyalty were mediated by trust. We also found the differences between small and large hospitals. Conclusions : Customer loyalty in small hospitals was affected by servicescape, whereas that in large hospitals was affected by diagnosis quality and employee attitudes. The research results could be used to develop strategies to improve customer loyalty.

Understanding and managing patients with adult rare diseases

  • Despite advances in the diagnosis and management of rare diseases (RDs), there remains a tendency to overlook adult RD patients. In addition to the considerable number of adult-onset RDs, advances in the diagnosis and management of pediatric RDs have led to an increase in the survival of these patients into adulthood. Adult RDs exhibit distinct features from pediatric counterparts, necessitating careful consideration during medical assessments. Given the extended life expectancy of adult RD patients, precise diagnosis and management strategies can significantly enhance patient outcomes. This review aims to provide an in-depth exploration of the characteristics unique to adult RDs. Special emphasis will be placed on the importance of cascade screening and prenatal genetic testing in the context of adult RDs, highlighting the need for a comprehensive understanding of these aspects in clinical practice.

  • Telematics system includes the "vehicle remote diagnosis technology", "driving pattern analysis technology" which are commercially attractive in the real life. To implement those technologies, we need vehicle signal interface, vehicle diagnosis interface, accelerometer/yaw-rate sensor interface, GPS data processing, driving pattern analysis, and CDMA data processing technique. Based on these technologies, we analyze the error existence by diagnosing the EMS(Engine Management System), TMS(Transmission Management System), ABS/TCS, A/BAG in real time. And we are checking about a driving pattern and management of the vehicle, which are sent to the information center through the wireless communication. These database results will make the efficient vehicle and driver management possible. We show the effectiveness of our results by field driving test after completing the H/W & S/W design and implementation for vehicle remote diagnosis and driving pattern analysis.

Second Opinion Diagnoses of Cytologic Specimens on Consultation : Asan Medical Center Experience (세포 병리 자문 재판독의 진단 불일치에 관한 연구: 서울아산병원의 경험)

  • Background : Second opinion diagnosis of outside pathology slides is a common practice for efficient and proper patient management. We analyzed cytology slides from outside hospitals submitted for a second opinion diagnosis to determine whether the second opinion diagnosis had any influence on patient care. Methods : We reviewed 1,153 outside cytology slides referred to Asan Medical Center for second opinions from January, 2007, to December, 2007. All cases were categorized into three groups; no diagnostic discrepancy, minor diagnostic discrepancies (no impact on the management), and major diagnostic discrepancies (significant impact on the management and subsequent follow-up). Results : The thyroid was the most common organ system (933 cases, 80.9%), Forty cases (3.6%) belonged to the major diagnostic discrepancy group and 149 cases (12.8%) to the minor discrepancy group. For validation of second opinion diagnoses in major discrepancy cases, subsequent biopsy or surgical resection specimens and clinical information were reviewed, which were available in 29 cases. The second opinion diagnoses resulted in alteration of clinical management in 21 of 29 cases. Conclusion : For all referred patients, second opinion diagnosis is important and mandatory for appropriate patient care.
