2022 KLCA-NCC Korea Practice Guidelines for the Management of Hepatocellular Carcinoma

  • Hepatocellular carcinoma (HCC) is the fifth most common cancer worldwide and the fourth most common cancer among men in South Korea, where the prevalence of chronic hepatitis B infection is high in middle and old age. The current practice guidelines will provide useful and sensible advice for the clinical management of patients with HCC. A total of 49 experts in the fields of hepatology, oncology, surgery, radiology, and radiation oncology from the Korean Liver Cancer Association-National Cancer Center Korea Practice Guideline Revision Committee revised the 2018 Korean guidelines and developed new recommendations that integrate the most up-to-date research findings and expert opinions. These guidelines provide useful information and direction for all clinicians, trainees, and researchers in the diagnosis and treatment of HCC.

  • Riverfront plays an important role as accessible and inclusive public space for residents, and a riverfront development and management has become a social and environmental issue. The purpose of this study is to analyze Korean legislation and guidelines applied in a riverfront assessment process to identify any legislative problems. To this end, major laws, regulations, national river management plans, and various guidelines related to river management were reviewed. The followings are the suggestions proposed for future improvement. First, clear and consistent definitions on riverfront areas and its spatial range are required across the laws. Also, recreational activities and facilities in riverfront should be categorized and listed for possible development and maintenance. Second, it is necessary to develop an objective and unified riverfront assessment system. Also, guidelines for surveying and evaluating the conditions and potentials of riverfront should be developed. Third, efforts should be made to revise related laws and guidelines to enable comprehensive and systematic approach in design, planning, implementation, maintenance of riverfront. Improving the related legislations and streamlining an riverfront assessment process can help create environmentally-friendly riverfront spaces and mange them efficiently.

RAND 방법으로 합의한 임상진료지침의 정의와 질 평가 기준 (Consensus on definition and quality standard of clinical practice guideline using RAND method)

  • Background : Clinical practice guidelines are an increasingly familiar part of clinical practice. Moreover, rigorously developed evidence based guidelines has been widely used. However, in Korea, some of published documents as clinical practice guidelines have shown considerable disparity in structure, contents and quality. This is mainly because there is no consensus on the definition and quality standard of clinical practice guidelines. The purpose of this study was to draw consensus on the definition and the quality standard about clinical practice guidelines. Method : We developed a questionnaire about the definition of clinical practice guidelines with inclusion criteria(23 items) and the quality standard(30 items). We selected 9 experts who had prior experience in developing and implementing guidelines. Rating methods for appropriateness of items were adopted from the RAND method. Consensus was drawn in three rounds. Results : Of the 47 items agreed, 40 items were determined to be appropriate. Clinical practice guidelines were defined as "scientifically and systematically developed statements to assist practitioners and patients on making decisions about appropriate health care for specific clinical circumstances." Narrative reviews, systematic reviews or health technology assessment without recommendations, translation of foreign guidelines, guidelines for patients only and training manuals were not considered as clinical practice guidelines. For the quality standard of clinical practice guidelines, 27 items were deemed necessary. Conclusions : The consensus on the definition with inclusion criteria and the quality standard of clinical practice guidelines carries an important meaning as the first attempt to draw a general agreement in our society. The unique achievement of the consensus reflects the current status of clinical practice guidelines that there has been a high tendency to adapt foreign guidelines. We hope efforts of this kind will continue to bring improvement in clinical practice guidelines.

Clinical Characteristics and Current Managements for Patients with Chronic Subdural Hematoma : A Retrospective Multicenter Pilot Study in the Republic of Korea

  • Objective : Chronic subdural hematoma (CSDH) is a common disease in neurosurgical departments, but optimal perioperative management guidelines have not yet been established. We aimed to assess the current clinical management and outcomes for CSDH patients and identify prognostic factors for CSDH recurrence. Methods : We enrolled a total of 293 consecutive patients with CSDH who underwent burr hole craniostomy at seven institutions in 2018. Clinical and surgery-related characteristics and surgical outcomes were analyzed. The cohort included 208 men and 85 women. Results : The median patient age was 75 years. Antithrombotic agents were prescribed to 105 patients. History of head trauma was identified in 59% of patients. Two hundred twenty-seven of 293 patients (77.5%) had unilateral hematoma and 46.1% had a homogenous hematoma type. About 70% of patients underwent surgery under general anesthesia, and 74.7% underwent a single burr hole craniostomy surgery. Recurrence requiring surgery was observed in 17 of 293 patients (5.8%), with a median of 32 days to recurrence. The postoperative complication rate was 4.1%. In multivariate analysis, factors associated with CSDH recurrence were separated hematoma type (odds ratio, 3.906; p=0.017) and patient who underwent surgery under general anesthesia had less recurrence (odds ratio, 0.277; p=0.017). Conclusion : This is the first retrospective multicenter generalized cohort pilot study in the Republic of Korea as a first step towards the development of Korean clinical practice guidelines for CSDH. The type of hematoma and anesthesia was associated with CSDH recurrence. Although the detailed surgical method differs depending on the institution, the surgical treatment of CSDH was effective. Further studies may establish appropriate management guidelines to minimize CSDH recurrence.

싱가포르 도시계획체계와 디자인가이드라인의 특징에 관한 연구 (Study on Characteristics of Urban Planning System and Design Guidelines in Singapore)

  • Even if Korea has established various legal guidelines as well as plans for the purpose of systematic management of urban citycapes, various problems have been pointed out, such as a decrease in operational efficiency because the legal framework is not clearly defined and the guidelines do not fully reflect regional characteristics. In the midst of this, Singapore is being mentioned as a representative example of enhancing international competitiveness by forming an outstanding urban cityscape through successful urban planning and operating system. Therefore, this study aims to suggest a direction for the improvement of Korea's urban planning and design guidelines by investigating and analyzing the characteristics of Singapore's urban planning system and design guidelines. As a result of the research and analysis, it was concluded that Singapore's urban planning system was based on the unified planning and management system. In particular, the Singapore Urban Design Guidelines provide differentiated guidelines to maintain and strengthen regional characteristics based on linkage with higher-level plans as detailed practical guidelines, and were operated as flexible guidelines reflecting the changes of the times and the needs of members of society. It also provides a direction for the improvement of the Korean urban design system, such as to increase the executionablity of the plan and the efficiency of operation by utilizing various guidance and support policies, rational and simplified deliberation procedures, and allow a platform that enhances the accessibility and convenience of related information.

Inverse association of improved adherence to dietary guidelines with metabolic syndrome: the Seoul Metabolic Syndrome Management program

  • BACKGROUND/OBJECTIVES: The Seoul Metabolic Syndrome Management (SMESY) program is a 1-yr lifestyle modification program targeting metabolic syndrome (MetS) in Seoul residents. This study investigated the associations between adherence to dietary guidelines and MetS among the SMESY program participants. SUBJECTS/METHODS: Data of 54,385 participants aged 20-64 yrs who completed the SMESY program in 2015, had information on adherence to dietary guidelines, and were not medicated for diabetes, hypertension, or dyslipidemia were analyzed. Participants underwent MetS screening and completed a lifestyle questionnaire including adherence to 10 dietary guidelines before and after participation. Participants were classified according to the number of MetS risk factors at baseline (MetS group, ≥ 3; risk group, 1-2; healthy group, none). Adherence to dietary guidelines was determined from the number of "yes" responses regarding the fulfillment of each guideline on ≥ 5 days/week. Multiple logistic regression was used to evaluate associations between newly diagnosed MetS and changes in adherence to dietary guidelines. RESULTS: In the MetS group, MetS prevalence decreased after the SMESY program (men, -41.9%p; women, -48.7%p), and all risk factors were significantly improved (P < 0.0001). All groups exhibited improved adherence to all dietary guidelines after participation (P < 0.0001). In the MetS group with positively changed adherence scores, the MetS prevalence decreased by -44.1%p for men and -49.5%p for women, whereas the prevalence in those with negative changes decreased by -38.1%p for men and -48.6%p for women. In the risk group, those with positively changed adherence scores had significantly decreased odds ratios (ORs) for newly diagnosed MetS compared with those with negative changes (OR, 0.70; 95% confidence interval [CI], 0.61-0.80 for men; OR, 0.88; 95% CI, 0.79-0.99 for women). CONCLUSIONS: The SMESY program may effectively reduce the risk of MetS among adults with risk factors by improving adherence to dietary guidelines.

귀촌형 공동체마을 지속성을 위한 생활 및 관리 가이드라인 개발 (Developing Living and Managing Guidelines for the Sustainability of Back-to-Farm Community Village)

  • The purpose of this study was to develop guidelines for sustainability of back-to-farm community village in 4 aspects; social environment, maintenance and management, regional environment, and economic environment. These 4 aspects, which count for sustainability, based on the comprehensive comparative analysis about items that field survey and literature suggested. Conclusions of guideline are of the following. The guidelines on social environment are developed by 3 items; resident participation and activity, village leader, decision making in village and person in charge of operation. The guidelines on maintenance and management are developed by 5 items; village operation system, village regulation, transparency of management, village operation expenses, and management type. The guidelines on regional environment are developed by 2 items; support by central and local government, and area projects and connection to regional resources. The guideline on economic environment is not developed due to low demands of based production facilities and income increase facilities.

환자감시가 간호사의 말초혈관 카테터 관리지침 이행에 미치는 영향 (The Effects of Patient Surveillance on Nurses' Adherence to Peripheral Catheter Management Guidelines)

  • Purpose: The purpose of this study was to examine the effect of patient surveillance on nurses' adherence to the guidelines on peripheral vascular catheter management. Method: A one group time series design was used. Seventy six nurses participated in the study. The data were collected from October 1 to November 4, 2008. Result: The effects of patient surveillance were significant on nurses' average adherence rates to the guidelines estimated by observers and by themselves, and the levels of nurses self recognition on the importance of the guidelines. In addition, overall occurrence rates of phlebitis tend to decrease after patients surveillance. Even though 33 nurses didn't follow the guidelines perfectly, most patients did not provide feedbacks to the nurses' non-adherence. Among the nurse participants, 36.8% answered that patient surveillance was acceptable. Conclusion: This study found that patients with peripheral catheters could positively influence nurses' adherence to the catheter management guidelines. Therefore, inviting patients in nursing activities could benefit to the quality of patients care. However, it seems essential to create an environment fostering nurses' acceptance of the patients' surveillance. Further research warrants the potential benefits of the patient's participations in nursing activities.

Management Strategy of Sediment-Related Disasters for Adaptation to Climate Change

  • To establish a management strategy of sediment-related disasters for adaptation to climate change, it is necessary to 1) understand the specific details and problems about the present status, 2) systematize related technologies by using exact numerical values obtained from physically-based analysis, and 3) ensure the basic guidelines are applied to field elastically. To achieve these successfully, detailed guidelines are required by scientifically considering the utilization and impact of related technology on the field. Here, detailed guidelines should include 1) the development of a basic plan, 2) enhancement of relevant technical instructions, 3) establishment of survey and inspection methods, 4) procedure of erosion control works in urban living sphere, and 5) proactive countermeasures against sediment-related disaster caused by earthquakes.

소방 119구급대원에 의해 시행된 전문기도관리 현황 및 법적 고찰 (The current status and legal review of advanced airway management implemented by 119 EMTs)

  • Purpose: This study aimed to analyze the data from public information disclosure about pre-hospital advanced airway management and identify the problem by considering domestic laws and guidelines. Methods: Data were collected between 2017 and 2018 and analyzed using SPSS 25.0. Then, the problems of the analysis results based on the relevant laws and practical guidelines were reviewed. Results: The review of domestic laws and practice guidelines revealed that ambulance nurses can implement supraglottic airway device only under the following three conditions: ① smart advanced life support pilot project area, ② trained to insert I-Gel, and ③ member of a special ambulance. In total, 21,574 cases of advanced airway management (endotracheal intubation: 2,428, I-Gel: 18,502, LMA : 499, KING AIRWAY: 144) were reported. In many cases, advanced airway management was performed by ambulance nurses who did not meet the above conditions, which was in violation of laws and guidelines. In addition, the prognosis of intubated patients was not followed up. Conclusion: The Korea National Fire Agency must stop all unlicensed medical practice by untrained, uneducated, and uncertified nurses and demand quality control programs for intubated patients.
