Malignant Proliferating Trichilemmal Tumor - A Case Report - (악성 증식성 모낭 종양 - 증례 보고 -)

  • Proliferating trichilemmal tumor(PTT) is a rare neoplasm derived from the outer root sheath of the hair follicle. Malignant transformation is rare, but abrupt enlargement of size, infiltrative growth, and non-scalp location are suggestive of malignant PTT. Histopathologic characteristics of malignant PTT are nuclear atypia, pleomorphism, and mitotic figures. Recurrence and metastasis have been documented in malignant PTT. Wide resection of the tumor with the normal tissue and accurate follow-up is the treatment of choice of the both malignant and benign PTT. We report a 51-year-old man with the mass on right 5th finger distal phalanx for 7 years. The tumor was excised with middle phalangiectomy, and based on the histopathologic findings of the tumor, this case was diagnosed as mailignant PTT. We cannot detect recurrence and metastasis because the patient was lost to follow-up.

Effect of Caffeine on Transformation Induced by Adenovirus type 12 (Adenovirus type 12에 의해 유발된 Transformation에 미치는 Caffeine의 영향)

  • Adenovirus group consists of over 100 related viruses which have been isolated from respiratory or gastro-intestinal tract of primate, cattle, dog and mice. Approximately 40 serologic types of adenovirus producing a variety of human respiratory and conjunctival infections were identified. Adenoviruses are icosahedral virions containing double-stranded linea DNA. They are 70nm to 90nm in diameter and each of capsid is composed of 252 capsomeres. Several numbers of this group, including types commonly associated with respiratory disease in man, are capable of producing malignant tumors in young hamsters and a few types have been shown to be on-cogenic in young rat. Previous report involving effect of Hormone on replication of adenovirus(9) has been carried out. The present report represents a continuation of previous study. To obtain evidence concerning the effect of caffeine on the transformation, investigation of adenovirus type 12 of this group was undertaken. For practical consideration it was desirable to investigation of the effect of caffeine on the adenovirus type 12-induced transformation in L cell. Results were as follows; 1. Adenovirus type 12-induced transformation was inhibited in the presence of caffeine. 2. Yields of adeoovirus type 12 in L cell were slightly inhibited by treatment of caffeine.

Loss of Expression of Cyclin D2 by Aberrant DNA Methylation: a Potential Biomarker in Vietnamese Breast Cancer Patients

  • DNA methylation of tumor suppressor gene promoters is the most frequent phenomenon leading to inactivation of function, consequently driving malignant cell transformation. Cyclin D2 is implicated in tumor suppression. In our study, we carried out the MSP assay to evaluation the methylation status at CpG islands in the cyclin D2 promoter in breast cancer cases from the Vietnamese population. The results showed that the frequency of methylation reached 62.1% (59 of 95 breast cancer tumors), but was low in non-cancer specimens at 10% (2 of 20 non-cancer specimens). Additionally, with an RR (relative risk) and OR (odd ratios) of 6.21 and 14.8, DNA hypermethylation of cyclin D2 increased the possibility of malignant transformation. Our results confirmed the cyclin D2 hypermethylation could be used as the potential biomarker which could be applied in prognosis and early diagnosis of Vietnamese breast cancer patients.

Malignant Transformation of an Epidermoid Cyst in the Cerebellopontine Angle

  • Intracranial squamous cell carcinoma is extremely rare, with most of the cases arising from malignant transformation of an epidermoid or a dermoid cyst. The patient presented with facial weakness. Initial magnetic resonance imaging revealed a mass in the right cerebellopontine angle. A subtotal resection was performed via right retrosigmoid suboccipital approach. Histopathological findings were consistent with an epidermoid tumor. Five months later, the patient underwent gamma knife radiosurgery due to highly probable recurrent epidermoid tumor. Two years after, the patient's neurological deficit had been newly developed, and follow-up magnetic resonance imaging demonstrated a large contrast-enhancing tumor in the left cerebellopontine angle, which compressed the brainstem. After resection of the tumor, histopathological examinations revealed a squamous cell carcinoma probably arising from an underlying epidermoid cyst. We report a case of an epidermoid tumor in the cerebellopontine angle that transformed into a squamous cell carcinoma.

p53 Expression in a Malignant Mesothelioma Patient during Seven-Year Follow-up

  • Malignant mesothelioma (MM) is the aggressive tumor of serosal surfaces. There are crude pathogenetic results regarding the biology of MM. Coordinated upregulations of p53 gene expression are shown in malignancies. We believed that there are changes in the p53 expression with transformation from reactive hyperplasia to MM. A 65-year-old male was admitted the hospital because of left pleuritic chest pains in 2004. Chest computed tomography (CT) results showed left pleural effusions with loculation and pleural thickening. Pathologic findings revealed reactive mesothelial hyperplasia. In 2008, the patient again felt left pleuritic chest pains. Chest CT showed progressive thickening of the left pleura. Pathologic diagnosis was atypical mesothelial hyperplasia. In 2011, chest CT showed progressive thickening of his left pleura. He was diagnosed with well-differentiated papillary mesothelioma. Serial change was analyzed by immunohistochemical staining for p53 of pleural tissues. There were no remarkable changes in p53 expressions during the transformation to MM.

A Case of Giant Congenital Melanocytic Nevi in a Neonate (신생아에서 발생한 거대 선천성 멜라닌세포 모반 1례)

  • Giant congenital melanocytic nevi are very rare, with an estimated incidence of 1 in 20,000 live births. They have a high risk of malignant melanoma transformation and neurological deficits such as neurocutaneous melanocytosis and epilepsy. Early evaluation, surgical intervention and careful long term follow up are recommended to monitor for malignant transformation. We report one case of giant congenital melanocytic nevi diagnosed at birth with the related literatures.

Basal cell carcinoma misdiagnosed as trichoepithelioma

  • Trichoepithelioma is a benign hair follicle tumor that can undergo malignant transformation into basal cell carcinoma in rare cases. Due to the similar clinical and histological features of trichoepithelioma and basal cell carcinoma, distinguishing between these types of tumors can be a diagnostic challenge. Punch biopsy obtains only a small sample of the entire lesion, and thus inherently involves a risk of misdiagnosis between histologically similar diseases. Therefore, if the possibility of misdiagnosis can reasonably be suspected, clinicians should conduct an excisional biopsy or immunohistochemical staining (e.g., CD10 and Bcl-2) to ensure an exact diagnosis. Although trichoepithelioma is benign, the surgical excision of solitary trichoepithelioma should be considered in order to avoid the possibility of malignant transformation, which has occasionally been documented for multiple familial trichoepitheliomas. Herein, we report a case that was initially misdiagnosed as trichoepithelioma before ultimately being diagnosed as basal cell carcinoma through excision and immunohistochemical staining.

  • Intraductal papillary neoplasm of the bile duct (IPN-B) is a rare disease characterized by multicentric proliferation of adenomatous epithelium within the bile ducts. It has better prognosis because of slow disease progression and high probability of early diagnosis, but also has potential for malignant transformation. Therefore early treatment such as surgical resection has been recommended. There are few reports about nature course of IPN-B without treatment. We report a case of papillary adenoma of intrahepatic duct that transformed into intrahepatic cholangiocarcinoma with 8 years of follow up.

Malignant Hidroacanthoma Simplex: A Case Report (악성 한선 극세포종 증례보고)

  • Purpose: Hidroacanthoma simplex (HAS) is a rare benign neoplasm of the skin and known as intraepidermal poroma. Malignant transformation of HAS has been reported in the literature, but it is very rare. We experienced a case of malignant hidroacanthoma simplex (MHA) and performed the surgery of the neoplasm. Methods: A 73-year-old patient visited our department for evaluation and treatment of the skin lesion ($3.2{\times}3.0\;cm^2$) that occurred two years before on the right ankle area. The lesion was characterized by dark-brown, hyperkeratotic and protruding patch. The patient had no symptom, and any palpable lymph node was not found on physical examination. The result of laboratory was also nonspecific. The lesion was excised completely with a resection margin of 5 mm including subcutaneous tissue partially, followed by full-thickness skin graft for reconstruction. Results: The result of biopsy was reported as malignant hidroacanthoma simplex. Histologically, the epidermis showed discrete aggregates of tumor cells which had round, hyperchromatic nuclei and pale or clear abundant cytoplasm. Cystic spaces were found within the tumor and mitosis were seen, frequently. No recurrence was found one month after surgery. Conclusion: The case of malignant hidroacanthoma simplex was treated with full-thickness skin graft following wide excision of the tumor.
