Design and Construction of Cylindrical Multi-Loop Z-gradient Coil for Linearity Improvement in MRI (핵자기공명영상에서 선형성의 증강을 위한 원통형 Multi-Loop Z-경사자기장 코일의 설계와 제작)

  • 본 논문에서는 핵자기공명영상의 고해상도를 이루고, 영상의 신뢰성을 높이기 위해서 선형성이 증가된 경사자기장 코일의 설계와 제작에 목적을 두었다. Maxwell pair를 이용하여 Z-경사자계의 선형성을 증강시키기 위해 코일의 기하학적인 형태의 이론적인 계산과 몇 가지 형태의 코일로부터 예상되는 자기장 및 경사자기장의 분포를 구현했다. 즉, 코일 축 방향의 자기장을 구면좌표계에서 전개하는 방법으로 Maxwell pair의 크기와 위치를 계산하고, 유한요소법을 이용하여 자기장 및 경사자기장의 2차원 분포를 그렸다. 더불어 이론적인 계산 결과와 함께 자기장의 2차원 분포를 토대로 실제 경사자기장 코일을 제작하였고 이를 0.15 Τ 핵자기공명영상기에 적용하여, 영상을 획득하였다. 기존의 방법에 따른 Maxwell pair 형태를 이용한 경우에 코일지름의 40% DSV(diameter spherical volume)내에서 DSV의 5% 이내의 뒤틀림(distortion)을 갖는 선형성이 유지되었고, 새롭게 시도된 방법에 의한 경사자기장 코일의 경우는 코일지름의 70% DSV 내에서 DSV의 5% 이내의 뒤틀림을 갖는 선형성을 유지하였으며, 설계 제작된 경사자기장 코일과 RF-코일을 이용하여 이를 확인하였다. 본 연구에서 제작된 경사자기장 코일의 선형성은 Maxwell pair 코일보다 향상된 결과를 보았으며, 본 논문에서 제시하는 방법은 자기공명영상의 해상도 향상에 기여할 것으로 판단된다.

  • 최근 의료 장비들이 발전하고 진단 및 연구에 다양하게 이용되면서 이로부터 얻은 3차원 의료 영상들을 자동으로 처리해주는 기술의 수요가 늘고 있다. 자동 뼈 영역화 기법은 이러한 기술들 중 하나로써 골다공증이나 뼈 골절, 골격질환 등의 진단의 효율성을 크게 높여줄 것으로 기대되고 있다. 그러나 현재까지 이를 위한 다양한 연구들이 진행되었음에도 2차원 영상과는 달리 높은 데이터양과 주변 조직과의 모호한 경계들이 많다는 어려움 때문에 실제 진단에는 사용되지 못하고 있다. 이에 따라 본 논문에서는 다중 해상도를 기반으로 하여 영역화와 정합기법을 반복적으로 수행함으로써 3차원 의료 영상 내에서 자동으로 뼈를 영역화 해내는 기법을 제안한다. 낮은 해상도 단계에서 학습된 집합의 뼈 정보들을 이용하여 대략적인 뼈 위치를 검출하고, 이후 해상도를 높여가면서 정합 과정과 영역화 과정을 반복적으로 수행한다. 성능을 확인하기 위해 무릎 자기공명영상(magnetic resonance image)내에서 대퇴골(femur)과 경골(tibia)을 영역화 하는 실험을 진행하였으며 60개의 학습 데이터들을 바탕으로 40개 영상에서의 뼈들을 영역화 하였다.

Endovascular Treatment of a Ruptured Posterior Inferior Cerebellar Artery Aneurysm during Pregnancy

  • Aneurysmal subarachnoid hemorrhage (SAH) during pregnancy is quite rare, however it has a high maternal mortality rate. A pregnant woman in the 16th gestational week was admitted to our hospital with a drowsy level of consciousness. A brain magnetic resonance (MR) image showed hemorrhage on the prepontine cistern, and both sylvian fissures, and MR angiography and cerebral digital subtraction angiography demonstrated an aneurysm at the left posterior inferior cerebellar artery (PICA). We performed endovascular coil embolization attempting to minimize radiation exposure. She was discharged with no neurologic deficit and delivered a healthy baby by cesarean section at the 38th week of gestation. This case study reported the shortest gestational period and this is the first report on an aneurysmal rupture arising from PICA which was treated using an endovascular method. Using an appropriate technique for reduced radiation exposure to the fetus and limited alterations in maternal-fetal physiology, endovascular coil embolization could guarantee good results in treatment of aneurysmal SAH in pregnant women.

Spontaneous Regression of a Radiculopathic Cervical Herniated Disc following Non-surgical Treatment -3 case reports- (비수술적 치료 후 자연소실된 경추추간판탈출증 -증례보고-)

  • The spontaneous regression of herniated cervical discs is not a well established phenomenon. However, we encountered the 3 cases of spontaneous regression of severe radiculopathic herniated cervical discs that were treated using a non-surgical method. Each of the patients were treated with a combination of manipulation, dry needling and analgesics. In each case, the symptoms improved within 12 months of treatment and magnetic resonance imaging (MRI) conducted at that time revealed marked regression of the herniated disc in all cases. These cases provide additional examples of spontaneous regression of herniated cervical discs documented by MRI following non-surgical treatment.

  • In an examination of the temporomandibular joint disc, MRI(Magnetic Resonance Imaging) is a useful method, and it is necessary to conduct an examination with one's mouth open for a long time to observe the accurate position change of the disc. Thus, this study would produce a TMJ device, using the 3-D printing technology, which would maintain the state of opening the mouth and would evaluate its usefulness as compared to the existing fixed device. As compared to the image using the existing TMJ device, the image taken with the self-produced TMJ device with a 3-D printer showed a somewhat lower SNR, but there was no defect for a clinical use. It is judged that benefits to costs would increase, since it can be customized for the individual patient and can contribute to the production of similar tools by utilizing the 3-D printing technology.

  • We report a case of non communicating hydrocephalus due to membranous obstruction of Magendie's foramen. A 37-year-old woman presented with intracranial hypertension symptoms caused by the occlusion of Magendie's foramen by a membrane probably due to arachnoiditis. As far as the patient's past medical history is concerned, an Epstein-Barr virus infectious mononucleosis was described. Fundoscopic examination revealed bilateral papilledema. Brain magnetic resonance imaging demonstrated a significant ventricular dilatation of all ventricles and turbulent flow of cerebelospinal fluid (CSF) in the fourth ventricle as well as back flow of CSF through the Monro's foramen to the lateral ventricles. The patient underwent a suboccipital craniotomy with C1 laminectomy. An occlusion of Magendie's foramen by a thickened membrane was recognized and it was incised and removed. We confirm the existence of hydrocephalus caused by fourth ventricle outflow obstruction by a membrane. The nature of this rare entity is difficult to demonstrate because of the complex morphology of the fourth ventricle. Treatment with surgical exploration and incision of the thickened membrane proved to be a reliable method of treatment without the necessity of endoscopic third ventriculostomy or catheter placement.

  • Various surgical procedures for the treatment of post-traumatic syringomyelia have been introduced recently, but most surgical strategies have been unreliable. We introduce the concept and technique of a new shunting procedure, syringo-subarachnoid-peritoneal shunt. A 54-year-old patient presented to our hospital with a progressive impairment of motion and position sense on the right side. Sixteen years before this admission, he had been treated by decompressive laminectomy for a burst fracture of L1. On his recent admission, magnetic resonance (MR) imaging studies of the whole spine revealed the presence of a huge syrinx extending from the medulla to the L1 vertebral level. We performed a syringo-subarachnoid-peritoneal shunt, including insertion of a T-tube into the syrinx, subarachnoid space and peritoneal cavity. Clinical manifestations and radiological findings improved after the operation. The syringo-subarachnoid-peritoneal shunt has several advantages. First, fluid can communicate freely between the syrinx, the subarachnoid space, and the peritoneal cavity. Secondly, we can prevent shunt catheter from migrating because dural anchoring of the T-tube is easy. Finally, we can perform shunt revision easily, because only one arm of the T-tube is inserted into the intraspinal syringx cavity. We think that this procedure is the most beneficial method among the various shunting procedures.

In Silico Study of the Ion Channel Formed by Tolaasin I Produced by Pseudomonas tolaasii

  • A toxin produced by Pseudomonas tolaasii, tolaasin, causes brown blotch disease in mushrooms. Tolaasin forms pores on the cellular membrane and destroys cell structure. Inhibiting the ability of tolaasin to form ion channels may be an effective method to protect against attack by tolaasin. However, it is first necessary to elucidate the three-dimensional structure of the ion channels formed by tolaasin. In this study, the structure of the tolaasin ion channel was determined in silico based on data obtained from nuclear magnetic resonance experiments.

  • Over the past decade, researchers were able to solve complex medical problems as well as acquire deeper understanding of entire issue due to the availability of machine learning techniques, particularly predictive algorithms and automatic recognition of patterns in medical imaging. In this study, a technique called transfer learning has been utilized to classify Magnetic Resonance (MR) images by a pre-trained Convolutional Neural Network (CNN). Rather than training an entire model from scratch, transfer learning approach uses the CNN model by fine-tuning them, to classify MR images into Alzheimer's disease (AD), mild cognitive impairment (MCI) and normal control (NC). The performance of this method has been evaluated over Alzheimer's Disease Neuroimaging (ADNI) dataset by changing the learning rate of the model. Moreover, in this study, in order to demonstrate the transfer learning approach we utilize different pre-trained deep learning models such as GoogLeNet, VGG-16, AlexNet and ResNet-18, and compare their efficiency to classify AD. The overall classification accuracy resulted by GoogLeNet for training and testing was 99.84% and 98.25% respectively, which was exceptionally more than other models training and testing accuracies.

Synthesis and Characterization of MPEG-b-PDPA Amphiphilic Block Copolymer via Atom Transfer Radical Polymerization and Its pH-Dependent Micellar Behavior

  • Block copolymer micelles are generally formed via the self-assembly of amphiphilic block copolymers in an aqueous medium. The hydrophilic and hydrophobic blocks form shell and core micelles, respectively. The block copolymers of methoxy poly(ethylene glycol) (MPEG)-b-poly(2-diisopropylamino)ethyl methacrylate (PDPA) were synthesized via atom transfer radical polymerization, with the macro initiator synthesized by the coupling of 2-bromoisobutyryl bromide with MPEG in the presence of a triethyl amine base catalyst. The atom transfer radical polymerization of 2-diisopropylamino)ethyl methacrylate was performed in conjunction with an N,N,N',N",N"-pentamethyl-diethylenetriamine/copper bromide catalyst system, in DMF, at $70^{\circ}C$. The pH induced micellization/demicellization was studied using fluorescence, with a pyrene probe. Furthermore, the pH dependent micellization was confirmed using the microviscosity method, with a dipyme fluorescence probe. The pH dependant micelle size distribution was studied using dynamic light scattering. The characterization of the synthesized polymers was established using gel permeation chromatography and from the $^1H-nuclear$ magnetic resonance spectroscopy.
